Employee stated that EE used a torch to heat up a hand rail on dozer so as to bend it back into place. As EE was getting down, EE accidentally put hand on the hot metal hand rail causing burn to right hand. Got checked on 7/21/16 and had some skin debrided and bandaged. Was put on restricted duty on 7/26/16 and returned on regular duty on 8/2/16.

Employee stated that EE was operating a dozer creating drill bench area. The rock EE was working on caused the dozer to jar EE around during the shift. This caused pain to back, neck, and hip area. Continued working. To ER on 4/1 to check. Continued working till 4/11/16 when doctor took EE off work thus lost time starting 4/11.

Employee stated that EE was operating loader in coal pit. The jarring of the loader caused pain in the employee's lower back area. Finished that work shift. Started work the next day but ended up going to doctor that day to have checked. Injury is reportable due to missing the shift on 2/5/2016.

Employee stated that he was tightening the bolts on the fan belt pulley on a hauler. As he was pulling on the ratchet, he stated that he felt pain in his left side area. The pain eased up later but never went away and was worse on Tuesday morning. Missed work and went to doctor. Returned on 09/16/15 on restrictions.

Employee stated that he was operating a dozer. He stopped the dozer so as to check the hard bar oil. When he opened the panel door to check the oil level, wind blew a foreign body into his right eye. He could not get it flushed out. Went to ER to get removed. Given eye medication and missed one shift, thus reportable.

Employee stated that he was using a come along to repair the hand rail on a dozer. As he was pulling on the come along, it slipped causing him to strike the hand rail with his left elbow. Went to doctor on 8/11/15 to have checked due to swelling and pain. No lost time, some restrictions, and removed fluid from his elbow.

Employee stated that he handed another mechanic a step ladder to put into his truck. As the mechanic was putting the ladder into the truck, it hit the tailgate causing the end of the ladder to strike the injured employee's nose resulting in a laceration to the nose. Reportable due to requiring two sutures.

Employee stated that he was prying on rock with the dozer blade. The blade slipped off of the rock causing the dozer to jar the employee resulting in low back pain. He decided to go to doctor on 3/2/15 due to persistent low back pain and missed the 3/3/15 work shift. He is currently on restricted duty.

Employee stated that he was standing beside parts truck while another employee was dislodging a stuck piece of steel plate in the parts truck bed with a sledge hammer. When the employee was swinging the hammer against the steel plate, he missed and the hammer slipped out of his hands striking the injured employee in the mouth area causing a laceration requiring sutures.

Employee was re-installing a metal plate on a Komatsu WA900 when it slipped catching his left pinky finger between the plate and a hydraulic tank

Employee stated that he was removing the bottom pin from the steering cylinder on the loader. As he was coming down off of the step ladder onto the ground, he stepped onto uneven ground causing him to sprain his right ankle. Missed two shifts of work.

Employee stated that he was dumping material out of hauler when the front end came off of the ground and then sat back down. This jarred the employee causing him to hit his left arm on the door panel and up into his left shoulder. Went to ER to check and missed 5/7/14 work shift and is on restrictive duties.

Employee stated that they had chained up the push beam on a dozer so as to move it out of the shot. The employee was pulling on the push beam while another employee was removing the chain which was stuck. He said he felt a pop in his right shoulder. Went to doctor, missed work shift on 1/20/14 and on restrictions until follow-up appointment.

Employee stated that he was going to the loader when he stepped onto uneven ground. As he was climbing onto the loader, he felt a pop in his right knee. His knee started hurting and swelling later. Continued to work but went to ER to get it checked later that evening. Missed the next work shift.

Employee stated that he was removing radiator cap to check anti-freeze in a light plant. As he reached in to remove cap, a small non-venomous snake bit him on the arms. Went to ER to get checked. Given tetanus shot and antibiotic ointment to put on bite area. No lost time incurred and no work restrictions.

Employee stated that he was back dragging the road from the parking lot to the pit area with a dozer. The road was muddy and rough. The dozer jarred the employee and he said that he felt pain from his waist down through his legs. He went to the ER doctor. Has a herniated disc and is off of work.

Employee stated that he was servicing a dozer. He was climbing on and off the dozer repeatedly. He stated that he hurt his right foot during this process. He went to doctor on 11/19/12 and missed work starting 11/20/12.

Employee stated that he was pushing trees down with dozer. A tree dented engine compartment door. He was going to lay door off of dozer onto ground. Standing on tracks, pried on door, door came loose, employee lost balance and jumped onto ground. Hit left hand on push beam fracturing left ring finger.

Out on a shot loading holes, something went into right eye.

Employee stated that he had started up a step ladder to work on a front end loader but stepped back off to get a tool. As he stepped off, his right foot stepped onto uneven ground causing him to sprain his right ankle. No lost time incurred but reportable due to restricted work duties.

Employee stated that he was fueling the dozer. He pulled the fuel hose from the reel, turned and felt something pull in back. He went to his doctor, filed on insurance and disability in Oct. 2010. He did not tell the company to file as a workers comp injury. Was off again in Feb. 2011 for same injury but called on 2/22/11 & said to turn in as comp injury & would go to comp doctor.

Employee stated that he was getting onto a dozer. He had one foot on the step of the push beam and the other foot on top of the push beam. His right foot slipped off of the step causing him to fall down and strike his right rib cage against the top of the push beam resulting in a fractured right rib.

Employee stated that he was laying on the frame of a 992G loader in an awkward position trying to change out two starters. He felt a pull/strain in right hip area. Did not bother him until the next night when he experienced pain in hip area. He went to doctor on 11/24/10 and incurred lost work shift on that day.

Employee stated that he was working on the pilot control valve on a D11R dozer. He hit his left knee against the hydraulic control valve, causing bruising and swelling of left knee. Reportable when he went to doctor on 10/21/2010 and was put on restrictions until follow-up appointment.

Putting on downspout bucket on end of auger, truck operator moved auger too quickly, pinched end of ring finger.

Employee stated that he was operating a dozer. The muffler stack fell off. He stopped and got out of the dozer to pick it up, when something was blown into his left eye.

Employee stated that he was stepping off of dozer. As he stepped onto the ground, his right foot landed on a rock and caused him to turn his ankle toward the outside, resulting in pulled ligament.

Employee stated that he was getting out of the dozer. He stepped on a loose bolt on the deck causing him to slip. He grabbed hold of a safety rail to keep from falling. As he did, he strained his shoulder. Taken off work on 4-9-09 to have minor shoulder surgery to repair rotator cuff tear.

Employee stated that he was operating a dozer pushing out shot rock/material. He backed the dozer up over a rock ledge. When the rear of the dozer sat down, it jostled the employee causing his left side strike the arm rest on the seat, resulting in a fractured left rib.

Employee stated he was getting onto dozer when left foot slipped off step. He had hold of grab iron which prevented fall but it strained right shoulder. Hurting but seemed ok. Went to doctor later. Found small tear in shoulder. Opted to keep working. Finally decided on minor surgery to repair tear. Did so and lost time starting on 11/17/08, thus reportable.

Employee stated that he was operating loader in coal pit. He noticed his right hand itching with red spot developing. Swelling became more pronounced at end of shift. To ER next day when right hand and arm was swollen. Probable spider bite but was not felt by employee.

Employee stated that he was obtaining oil samples while servicing the loader. He was in the transmission area of the loader, stooped down getting the samples when the overhead compartment door fell and struck his head causing a laceration.

Employee stated that he was using an impact wrench to tighten the bolts on the cutting edges which he had replaced on a D11R dozer. He said he felt a burning sensation in right shoulder area and some numbness in his right hand toward the end of the work day. He did not report this as a work related injury until 8-11-06 when his doctor said it was work related.

Employee stated that he loosened radiator cap on dozer to check antifreeze level. As he did, hot steam/liquid spewed out and struck employee's left hand, arm and side, causing burns to these areas.

Employee stated that he was removing the right side top bucket pin from the loader bucket. There was pressure on the pin and when he struck the pin with a hammer, the pin jumped out, fell and landed on top of the employee's right foot.

TRUCK CAME DOWN A HILL AND TRIED TO DOWN SHIFT. HE GOT TRUCK OUT OF GEAR, ENGINE BRAKE KILLED THE MOTOR AND HE COULDN'T GET THE TRUCK CRANKED BACK, WOULDN'T STEER, AND TRUCK TURNED UP ON ITS SIDE IN A DITCH.

EE stated that he was in the process of removing the right side rear tires off of a hauler. The hauler bed was raised and pinned. A small rock fell over the rock guard from the cab shield of the bed and struck ee on the top of his head. No lost time incurred but reportable due to laceration requiring sutures.

EE WAS OPERATING A HAULER. HIS RIGHT LEG STARTED HURTING. AT QUITTING TIME HIS RIGHT FOOT WAS NIMB. HE WOKE THE NEXT MORNING WITH BACK PAIN. HE WENT TO THE DOCTOR, HAD BACK SURGERY & NOTIFIED HIS EMPLOYER OF THE PROBLEM ON 2/27/03. THE 1ST LOST WORKDAY OCCURRED ON 2/21/03.

SYNOPSIS: TWO CONTACT MECHANICS NOTICED TWO INDIVIDUALS ON ATVS NEAR WHERE A BULK TRUCK WAS PARKED. UPON INVESTIGATION THEY FOUND THE TRUCK ON FIRE. IT HAD TO BE EXTINGUISHED BY THE FIRE DEPT. IT APPEARED THE TRUCK WAS VANDALIZED & THE FIRE INTENTIONALLY SET. THE FRONT OF THE TRUCK INCLUDING THE CAB HAD COMPLETELY BURNED.

EE WAS OPERATING A HAULER. AS HE WAS BEING LOADED WITH SHOT ROCK, A ROCK SLIPPED OFF OF THE LOADER BUCKET, FELL AND STRUCK THE SIDE RAIL ON THE HAULER BED JARRING THE EE. THIS RESULTED IN RE I NJURING THE BACK STRAIN FROM A PREVIOUS INJURY. THIS BECAME REPORTABLE DUE TO FIRST LAST WORK DAY ON 2-20-02.

EE STATED THAT HE WAS CLIMBING ONTO A DOZER TO ADD HYDRUALIC OIL TO THE DOZER. HE HAD THE OIL HOSE IN HIS L. HAND W/ HIS R. HAND ON THE HAND-RAIL USING IT TO PULL HIMSELF UPON THE DOZER. DUR ING THIS PROCESS, HE FELT A MUSCLE PULL IN HIS LOWER RIGHT SIDE. INJURY NOT REPORTABLE UNTIL INCURRING LOST TIME ON 11-29-01.

EE WAS DESCENDING THE LADDER ON THE BACK OF THE OIL TRUCK AFTER HE HAD FILLED IT WITH OIL. HIS FOOT SLIPPED OFF THE BOTTOM STEP OF THE LADDER & HE CREELED HIS ANKLE ON THE GROUND.

EMPLOYEE STATED THAT HE WAS OPERATING A FRONT END LOADER IN SHOT MATERIAL. AS HE WAS LOADING A HAULER, HE BACKED INTO A HOLE CAUSING THE LOADER TO TILT AND JAR HIM. HE STATED THAT PAIN IN THE LOWER BACK AREA BEGAN WITH IT RADIATING DOWNHIS LEFT LEG. WENT TO DR. ON 11-09-01 AND INCURRED LOST WORK DAYS.

EE STATED THAT HE WAS CHANGING OUT A TOOTH ON THE FRON END LOADER BUCKET. HE HAD KNOCKED THE PIN OUT WHICH HOLDS THE TOOTH ONTO THE BUCKET. THE TOOTH FELL OFF AND STRUCK THE EE'S LEFT FOOT BEH IND THE STEEL TOE AREA CAUSING AN INJURY (CONTUSION) TO HIS FOOT.

EE STATED THAT HE WAS CLIMBING UP THE HAULER STEPS TO CHECK THE OIL LEVEL AND TO START UP THE HAULER TO WARM IT UP. AS HE WAS CLIMBING THE STEPSHE MISSED THE HAND RAIL AND FELL BACKEWARDS AND ONTO THE GROUND(APPROX FALL OF 7 TO 8 FT.) CAUSING A COMPRESSION FRACTURE TO THE T12 VERTEBRAE.

EE STATED HE WAS OPERATING A DOZER ON 2ND SHIFT HE STOPPED THE DOZER TO CHECK AN OUTSIDE LITE WHICH HAD STOPPED WORKING AS HE WAS STANDING ON THE DOZER TRACK TO CHECK THE LIGHT, HE FELL OFF OF THE DOZER IN BETWEEN THE PUSH BEAM & ROLLER FRAME OF THE DOZER. MISSED 1ST WORK SHIFT ON 10/19/00

EE STATED THAT HE WAS OPERATRING A FRT END LDR IN SHOT MATERIAL & WAS PRYING ON ROCK WITH THE LOADER BUCKET THE BUCKET SLIPPED OFF THE ROCK CAUSING THE LOADER TO BOUNCE & JAR EE CAUSING PAIN TO HIS NECK. WENT TO DR 10/14/00 & BECAME REPORTABLE WHEN MISSED 1ST WORK SHIFT ON 10/16/00

EE STATED THAT HE WAS OPERATING A COAL LOADER AND WAS CLEANING COAL IN THE PIT AREA. AS HE WAS CLEANING ALONG THE HIGHWALL, SOME ROCKS FELL OUT OF THE HIGHWALL AND ONTO THE TOP FRONT OF THE LO ADER CAB/ROPS BENDING THE CAB DOWNWARD AND BREAKING OUT FRONT GLASSES. EE SUSTAINED VERY MINOR CUTS TO RIGHT ARM AND A CERVICAL STRIAN FR5OM HITTING HIS HEAD AGAINST AN OVERHEAD OBJECT.

EE STATED THAT HE WAS CHECKING A HYDRAULIC OIL LEAK IN AND AROUND THE PIVOT AREA OF THE LOADER. HE RAISED HIS HEAD UP AND HIT THE TOP OF HIS HEAD ON A GUARD UNDERNEATH THE CAB AREA OF THE LOAD ER. LACERATION REQUIRED SUTURES. THIS BEING REPORTABLE BUT NO LOST TIME WAS INCURRED.

EMPLOYEE STATED THAT HE WAS OPERATING A HAULER IN THE STRIP PIT AND WAS BEING LOADED WITH SHOT MATERIAL. A LARGE ROCK ROLLED OUT OF THE SPOIL SLOPE AND STRUCK THE REAR HAULER'S WHEELS CAUSING THE HAULER TO JAR THE EMPLOYEE INSIDE RESULTING IN A WHIPLASH EFFECT TO THE NECK AREA.

EE STATED THAT HE WAS OPERATING A DOZER IN SHOT ROCK. AS HE WAS BACKING UP, THE DOZER BACKED UP ONTO A ROCK WITH ITS LEFT TRACK. THE TRACK SLID OFF OF THE ROCK AND TWISTED THE DOZER TO THE RIG HT. THIS SUDDEN MOTION JARRED THE EE CAUSING HIMTO TWIST HIS BACK REUSLTING IN LOW BACK PAIN. WENT TO DR. NO LOST TIME BUT 2 DAYS LIGHT DUTY.

EMPLOYEE STATED THAT HE WAS HANDING BAGS OF SLURRY UP TO THE BLASTER. HIS FOOT SLIPPED ON LOOSE ROCKS CAUSING HIM TO LOSE HIS BALANCE. WHILE TRYING TO REGAIN HIS BALANCE, HE TWISTED HIS BACK C AUSING PAIN DOWN HIS RIGHT LEG. WENT TO DOCTOR BUT INJURY WAS NOT REPORTABLE UNTIL 3-19-98 WHEN HE INCURRED LOST TIME DUE TO PERSISTENT PAIN AND FURTHER MEDICAL DIAGNOSIS OF PINCHED NERVE.

WHILE WELDING CRACK IN BUCKET, EE RECEIVED FLASH BURNS TO BOTH EYES. TOTAL OF TWO WELDERS WERE WELDING AT SAME TIME. BOTH WELDERS WERE WEARING DARK SAFETYU GLASSES IN ADDITION TO WELDING HELME TS.

WHILE WELDING CRACK IN BUCKET, EE RECEIVED FLASH BURNS TO BOTH EYES. TOTAL OF TWO WELDERS WERE WELDING AT SAME TIME. BOTH WELDERS WERE WEARING DARK SAFETY GLASSES IN ADDITION TO WELDING HELMET S.

EE STATED THAT HE WAS DIGGING OUT SHOT ROCK WITHA FRONT END LOADER WHEN THE LOADER BUCKET SLIPPED OUT FROM UNDER THE ROCK THIS CAUSED THE LOADERTO JAR THE EE RESULTING IN A CERVICAL STRAIN.

EE STATED THAT HE WAS OPERATING A DOZER IN THE SHOT ROCK WHEN HE BACKED OVER A LEDGE AND D0ZER SLIPPED SIDEWAYS AND DROPPED 1 TO 2 FT ONTO GROUND. THIS CAUSED THE DOZER TO JAR THE EE. EE WENT TO DR FOR KIDNEY STONE BUT DR TOLD HIM THATINJURY WAS A TRUAMA TO RIGHT KIDNEY AND WAS CASUDD BY HIS WORK. REPORTABLE WHEN INJURY REPORTED BY EE ON 10/31/97

EE STATED THAT HE WAS GETTING OUT OF THE KENWORTH BULK TRUCK. AS HE STEPPED DOWN ONTO TO STEP, HE GRABBED HOLD OF THE HAND RAIL ON THE TRUCK. THE STEP BROKE OFF CAUSING THE EE TO FALL WITH HIS HAND STILL GRASPING THE HAND RAIL. THIS CAUSED A RIGHT SHOULDER STRAIN. NO LOST TIME BUT 3 DAYS LIGHT DUTY.

EE STATED THAT HE WAS OPERATING A FRONT END LOADER DIGGING UP SOME TIGHT SHOT ROCK. THE LOADER BUCKET SLIPPED OFF OF THE ROOF AND CAUSED THELOADER TO JAR THE EE SEVERELY. THIS CAUSED BACKTO HI S BACK AREA. WENT TO DR ON 4-4-97 AND HAS INCURRED LOST WORK DAYS BEGINING ON 4-4-97.

EMPLOYEE STATED THAT HE WAS HELPING LOAD A SHOT IN THE OLD PIT AREA. A ROCK FELLINTO AMUD PUDDLE NEAR HIM AND SPLASHED MUD INTO HIS LEFT EYE. HE FLUSHED HIS EYE OUT AND IT FELT OKAY. THAT E VENING IT BEGAN TO HURT AND HE WENT TO DR TO GET IT CHECKED. DIAGNOSED AS CORNEAL ABRASION BUT REORTABLE DUE TO MEDICATION PUT INTO HIS EYE.

EMPLOYEE STATED THAT HE WAS OPERATING A FRONT END LOADER IN SHOT MATERIAL LOADING HAULERS. AFTER HE HAD PICKED UPA BUCKE TLOADOF SHOT MATERIAL AND STARTED TOWARD THE HAULER, THE LOADER START ED ROCKING BACK AND FORTH DUE TO UNBALANCED LOAD. THIS CAUSED THE EMPLOYEYE TO STRIKE THE TOP OF HIS HEAD AGAINST THE AIR CONDITIONER CONTROL KNOBS. NO LOST TIME.

EMPLOYEE STATED THAT HE WAS OPERATING A DOZER IN SHOT ROCK AS HE WAS WORKING IN THE SHOT MATERIAL HE BACKED INTO & OVER ROCKS WHICH CAUSED THE DOZER TO JAR HIM RESULTING IN LOWER BACK PAIN WEN T TO DOCTOR ON 08-19-95 & BECAME REPORTABLE ON 08-25-95 AS A RESULT OF FIRST LOST WORK DAY

EE STATED THAT HE WAS RAISING THE BELLY PAN OF A D10N DOZER WITH A CHAIN HOOKED ON ONE SIDE OF THE PAN SO AS TO PUT A PINUNDERNEATH THE PAN FOR SUPPORT UNTIL THE CHAIN COULD BE REHOOKED. CHAIN SLIPPED OFF AND EE GOT HIS RIGHT INDEX FINGER CAUGHT BETWEEN THE PIN AND BELLY PAN CAUSING A CRUSHING INJURY FRACTURE TO THE RIGHT INDEX FINGER REPORTABLE WITH NO LOST TIME BUT RESTRICTED

EE STATED THAT HE WAS BACKING DOZER OVER SHOT MATERIAL WHEN THE DOZER TRACK RAN OVER A SMALL ROCK OUTCROP. THIS CAUSED THE DOZER TO JAR AND IN TURN JERKED THEEE. THIS CAUSED A WHIPLASH EFFECT ON THE EE'S NECK RESULTING IN A SEVERE CERVICAL STRAIN.

EE STATED THAT WHILE BENDING OVER REPETITIVELY TO HELP WIRE UP A BLAST ON THE JOB SITE HE EXPERIENCED PAIN IN THE LOWER BACK AREA HE CONTINUED TO WORK AND FINALLY WENT TO A DR ON 8-9-93 DUE TH E PAIN HE NEVER REPORTED THE INJURY TO HIS EE UNTIL 8-10-93. HE INCURRED LOST WORK TIME BEGINNING 8-9-93.

EMPLOYEE WAS LOADING HA ULERS WI TH SHOT ROCK MA TERIAL A ROCK ROLLED OUT OF THE SHOT MATERIAL & SDTRUCK THE LOADER BUCKET CAUSING THE LOADER TO JAR THE EMPLOYEE OPERATOR THIS MOTION DFAUSED THE EMPLOYEES HAND TO STRIKE THE CONTROL LEVERS & DISLOCATE HIS RT THUMB THIS WAS A NO LOST TIME ACCIDENT

EMPLOYEE WAS IN HAULER (CAT777) BEING LOADED WITH SHOT MATERIAL. A LARGE ROCK ROLLED OUT OF THE SHOT MATERIAL, HIT THE HAULER BED AND TIRE CAUSING THE HAULER TO JAR VIOLENTLY. THIS CAUSED A WH IP LASH EFFECT ON THE EMPLOYEE RESULTING IN LOWER BACK STRAIN AND POSSIBLE KIDNEY BRUISE.

EMPLOYEE WAS USING A PRY BAR TO PULL BACK A METAL STEP AS IT WAS BEING HEATED BY A TORCH. T HE METAL PRY BAR SLIPPED OF AND STRUCK EMPLOYEE IN MID FOREHEAD AREA, CAUSING A LACERATION TO THE FO REHEAD. THIS IS A NO LOST TIEM ACCIDENT BUT IS REPORTABLE BECAUSE OF THE LACERATION REQUIRED SUTURES.

EMPLOYEE WAS GETTING DOWN OFF OF HAULER ONTO THE GROUND. EMPLOYEE JUMPED FROM THE BUMPER LEVEL DOWN ONTO THE GROUND LANDING ON HIS LEFT FOOT CAUSING TORN LIGAMENTS IN LEFT FOOT.

EMPLOYEE WAS GETTING ONTO A 777B HAULER SO AS TO START HIS WORK SHIFT WHEN THE WIND BLEW SOME FOREIGN MATTER INTO HIS RIGHT EYE.WENT TO DOCTOR AND WAS GIVEN PRESCRIPTION FOR EYE DROPS AND PAIN PILLS.NO LOST TIME WORK DAYS INVOLVED BUT PRESCRIPTION FOR EYE-THUS REPORTABLE.

EMPLOYEE WENT TO THE DOCTOR ON 5-7-90 AS A RESULT OF AN INJURY REPORTED ON 4-27-90 WHEN THE LOADER HE WAS OPERATING HAD A ROCK FALL FROM THE SHOT MATERIAL HE WAS LOADING AND HIT THE BUCKET TEE TH CAUSING THE LOADER TO SEVERELY JAR THE EMPLOYEE RESULTING IN PAIN IN HIS LOWER BACK/MIDSECTION AREA. DOCTOR DIAGNOSED AS A POSSIBLE BRUISED KIDNEY.

THE INJURE IS THE SAME INJURY THAT WAS REPORTED ON 10-19-89 ON THE SAME MINE SITE. THE EE REFUNED TO WORK ON 12-5-89 AND WORKED UNTIL 1-15-90. ON THIS DATE HE MISSED WORK WENT TO DR DUE TO BAC K PAIN AND WAS PLACED OFF WORK AS A RESULT OF REINJURY OF PREVIOUS BACK INJURY.

STEPPING DOWN FCORM LADDER ON CAT BOTTOM STEP MISSING AND FELL ON TO GROUND PAIN IN LOWER BACKFIRST LOST DAY 10-25-89
