Employee stated that they were climbing up onto the dozer to work on the engine. As they did, they felt a pop in left knee. They experienced extreme pain in the knee as they attempted to walk on it. Went and got checked. Missed work on 06/24 and returned on regular duty on 06/25. Went to ortho on 06/27. Returned to work on regular duty on 06/30. Lost 2 days work.

Employee stated that he was driving to the job parking lot. He hit a hole in the road causing the seat in his mechanic's truck to bottom out. He stated that this caused immediate pain. He mentioned pain to his back/leg area a few times afterwards. He continued to work and never went to doctor. Left work on 1/12/16, missed starting 1/13/16, and now wants to see doctor.

Employee stated that he was helping set up a 6 inch water pump. He was pulling the discharge hose while walking backwards and felt a sharp pain in his lower back. Did not go to doctor until 03/20/13 and was not reportable until he missed his first shift of work on 4/04/13, thus reportable at that time.

Employee stated that he was on the ground outside the drill. He stepped on a rock and twisted his left ankle. The pain got worse as day progressed. He could not put weight on his left ankle the next day. Went to doctor on 6/14/12 and missed work.

Employee was hauling coal from surface pit to tipple in a Mack DM886SX tandem coal truck. As he was descending a grade on haul road, the truck overturned onto the driver's side entrapping the driver. The driver had a medical condition which led to the accident. The heart attack led to the driver's death.

Employee stated that he was helping mechanic to change the teeth on a loader bucket. Mechanic cut teeth off with cutting torch and employee was knocking teeth off with a hammer. A piece of metal slag flew off, struck and embedded in his upper left arm.

Employee stated that he was operating a dozer scaling the highwall. He was prying on a rock with the dozer blade, when the blade slipped off and hit the highwall. This stopped the dozer and caused a whiplash effect. Went to doctor on 5/29/09 and on 6/9/09 missing two shifts work, thus reportable.

EE STATED THAT HE WAS ON A LADDER CUTTING THE TIRE GUARD OFF OF THE HAULER BED. THE GUARD BROKE OFF SUDDENLY. IT PARTIALLY STRUCK HIS FACE AREA CAUSING HIM TO LOSE HIS BALANCE. HE TURNED AND JUMPED ONTO THE GROUND. HE LANDED ON HIS LEFT LEG AND THE IMPACT CAUSED A FRACTURE TO HIS LEFT LEG WITH LACERATIONS TO HIS FACE.

ACCORDING TO THE EE, HE WAS OPERATING A HAULER & HE WAS DUMPING THE LOAD ON A STEEP GRADE. THE LOAD DUMPED OUT QUICKLY CAUSING THE HAULER BED TO MOVE UP AND THEN BACK RATHER QUICKLY. THIS CAUSED THE HAULER TO JAR THE EE'S NECK.

AS HE KICKED LOOSE THE DISCHARGE HOSE FROM THE PUMP HOUSING, STEAM SPEW OUT FROM THE PUMP CHAMGER AND ONTO BOTH OF THE EE'S LEGS CAUSING BURNS ON BOTH LEGS.

HE TRIED TO REMOVE IT BUT COULD NOT AND WENT TO DOCTOR ON 8-13-98 TO HAVE FOREIGN BODY REMOVED FROM HIS EYE. NO LOST TIME INCURRED BUT REPORTABLE DUE TO OBJECT EMBEDDED IN THE EYE.
