Employee was putting lime in the lime feeder and felt something pull in his lower back.

EE reported on 3/4/10 that while lifting a 50lb bag of lime he felt something pull in his stomach.

Employee was setting on the floor and was using his foot to push a piece of metal. His foot slipped and he pulled a muscle in his calf.

Employee was dropping loaded railroad cars when his foot was caught between the railroad car wheel "Truck" and the rail.(Foot crushed with all toes amputated)

While shaking roll-feed gate the chain support broke a link, causing the bar he was using to slip. Injured fell on right foot.

Was knocking troughing roller back with 16 lb hammer when roller stuck injured on left leg.

A ground bar was tack-welded to screen frame. Injured turned and ran into rod.

Employee was shoveling wet refuse from the plant floor when he felt a pain in his lower back.

Injured was knocking gate in on refuse chute, missed gate & hit his thumb between hammer handle & handrail rod.

EE was washing down fly ash with 1 1/2 water hose Hot ashes blew back onto his left forearm, wrist and hands.

EE was replacing metal screen and strained back muscle.

Employee was climbing down from dozer. His ring jewery caught in the handrail and cut into the finger. He received medical treatment and stitches.

Employee stepped off screen - higher elevation to lower - and sprained his rt. knee.

Employee was working down slope. His feet slipped and he fell on his back.

Employee was pulling water hose. He slipped and fell on left hip.

Employee was going down steps when he slipped and fell, he landed on his right foot.

After tightling the brake injured person was coming back down ladder when he sliped on step and fell to the ground.

EMPLOYEE WAS DROPPING RAIL ROAD CARS, WHEN CARS BUMPED, EE STATED THAT HE FELT PAIN IN HIS NECK.

EE HAD DROPPED 3 EMPTY RAILROAD CARS AND WAS DISMOUNTING THE CARS WHEN A RAILROAD CAR FILLED WITH GRAVEL BEING MOVED BY NORTHFORK/SOUTHERN WAS RELEASED AND STRUCK THE EMPTY CARS THE EE WAS ON. THIS CAUSED THE EE TO BE KNOCKED FROM THE EMPTY CARS.

EE WAS CHANGING A GREASE FITTING ON THE STOKER ABOVE THE FIREBOX AT THE THERMAL DRYER. THE DOOR WAS OPEN ON FIREBOX. HIS "COVERALLS" IGNITED CAUSING BURNS TO HIS LEGS AND FINGERS.

EE STATED THAT WHILE CLIMBING LADDER AND TURNING BRAKE ON RAILROAD CAR HE HURT HIS BACK-EE DID NOT REPORT ACCIDENT UNTIL 11-1-02. EE DID NOT GO TO DR UNTIL 11-14-02.

EMPLOYEE WAS WORKING ON A METAL PIPE WHEN HE GOT A PIECE OF METAL IN HIS EYE. USING CUTTING TORCH.

WHILE EMPLOYEE WAS WALKING NEAR WASHER HE TURNED AROUND AND HIT HIS HEAD ON A BEAM.

EMPLOYEE WAS CHANGING A TAIL ROLLER BEARING- THE BEARING STRUCK EMPLOYEE AND KNOCKED HIM TO FLOOR.

EMPLOYEE WAS WORKING ON THE BLENDING BINS, WHEN EMPLOYEE JUMPED OFF PLATFORM HE HIT ON HIS RIGHT HEEL.

EE WAS ATTEMPTING TO PUT A CHAIN ON A SPROCKET WHEN HE CAUGHT HIS LEFT THUMB. POWER WAS LOCKED AND TAGGED. SPROCKET WAS NOT MOVING.

EE WAS WALKING FROM NO. 5 BIN WHEN HE SLIPPED ON MUD AND PULLED MUSCLES IN HIS RT. LEG AND BACK.
