Big Mountain No 16 Coal

Employee was installing roof bolts and as he was removing the drill steel from the bolt hole the drill steel struck his right middle finger.

While tightening bolt on railroad switch, finger caught between bolt and rail when impact wrench applied. Crushing injury, right hand, middle finger. Employee missed no work, was placed on restricted duty until 1/24/12 when reevaluated by doctor.

Check top and rib before taking coal section. EE noticed rock coal shell chunk started coming off rib and EE pushed IE out of the way. Chunk of coal hit IE first, then a piece of coal fell on EE's ankle.

Employee stated that he stepped in a water hole while dismounting the coal hauler he was operating and twisted his left leg.

Employee stated that he slipped on a roof bolt strap as he was walking to the rear of the roof bolting machine to get roof bolts and fell forward and twisted his lower back as he tried to catch himself.

Employee was installing roof bolts and as he reached up to turn the roof bolt plate as it was spinning and being pushed upward, his hand came in contact with the plate and it caused a laceration to his left middle finger.

Diagnosis of Occupational Hearing Loss, received on 10-04-2011. Employee is off work as a result of an injury at the time of the hearing loss diagnosis, therefore he is not off work related to his hearing loss.

Employee was assisting the loader operator in cleaning up supply yard. As the loader operator was scooping up mud and debris the bucket struck the end of a timber causing it to swing around and strike the injured employee in the legs causing him to fall. As he fell he extended his right arm out to break his fall causing his shoulder to dislocate.

Employee was helping build cribs and some small wood was needed to cap and wedge the last crib. Employee was using a block hammer as a chisel and a pick hammer to drive the block hammer in between nailed wood of a pallet and as the employee struck the block hammer with the pick hammer, a piece of metal flew off of the pick hammer and struck and embedded in his left arm.

Employee stated that he slipped while stepping from roof bolting machine operator platform to the machine platform and fell and struck his back on machine.

Employee was lifting top belt conveyor structure to put on top rail and felt pain in his left shoulder.

Employee was performing roof bolting duties and turning a 16 ft. metal roof strap and the edge of the strap cut through his glove resulting in a laceration to his right hand that required 5 sutures.

Employee was installing roof bolts and T-3 channel and the channel slid off the ATRS and struck him on the right shoulder. Note: Employee did not start missing work as a result of this injury until 12/8/2011.

Employee had his left hand resting on the roof bolting machine divider between the roof bolt tray and glue tray and as he removed the bolt wrench from the drill pot and was placing it onto the roof bolt tray he struck his left middle finger with the bolt wrench resulting in a laceration.

Employee was assisting in the repair of the roof bolting machine in the LOB in No. 2 entry when a piece of rock fell from the mine roof from between roof bolts and struck him on the left shoulder resulting in a fractured clavicle.

Employee slipped and fell at the surface shop and sprained his right ankle and bruised his right elbow.

Employee was walking behind a coalhauler and the rear of the coalhauler bottomed-out and struck a 4" x 4" x 37" piece of wood that was in the roadway and causing it to flip up and strike the employee on his right leg.

Employee was installing roof bolts and stated that he placed his left hand on the roof bolt plate to align it and applied the up-pressure at the same time. His left middle finger was caught between the plate and mine roof resulting in the amputation of the tip of his finger.

Employee was roof bolting in the No.4 entry and had drilled the outside bolt hole when unsupported mine roof approximately 1' to 2' thick fell inby the roof bolter. A piece of the rock fell across the ATRS arm and broke and fell on the outby side of the ATRS and struck the employee on the left shoulder blade resulting in a laceration requiring sutures.

Employee was cleaning rock and coal off top of MRS shields when a coal rib fell and struck the employee's right hand.

Employee was installing top belt conveyor structure and as he was lifting the structure he started to fall and grabbed the belt conveyor rail and felt pain in his upper left arm. NOTE: Employee continued to work until 07-11-11 and started missing work as a result of his injury.

Employee was building a stopping and stepped off of cinder blocks and twisted his left knee and fell when he stepped on the edge of a pallet.

Employee was operating a scoop and was bounced upward and struck his head against the scoop canopy, resulting in pain to his neck.

Employee was using a 16 lb hammer to straighten the bed of a coalhauler. As he swung the hammer his footing slipped causing him to miss the coalhauler bed and causing pain in his right forearm.

Employee was assigned the task of breaking ice cycles in the intake roadway entry and as he was walking back to his mantrip he slipped on ice and fell to the mine floor and injured his right leg and ankle.

On 01/04/2011 a pillar fall occurred on the MMU 023-0 section. Water came from under the pillar gob and after two days accumulated in low areas and resulted in the secondary escapeway being blocked. MSHA determined that this was an "inundation". This report is being filed to avoid any further enforcement action.

Employee was operating a shuttle car when he hit a hole in the mine floor causing him to bounce up into the canopy of the shuttle car.

Occupational Hearing Loss

A roof fall was found at approximately 9:15 a.m. on 11/23/2010 in the No. 2 Entry of B-Intake. The fall was approximately 20 ft.long x 18 ft. wide x 5 1/2 ft. high. Five foot fully grouted roof bolts were in use in the area.

EE was bolting on the right side of the 463 Roof Bolt Machine. He reached toward the roof bolt machine tray to grab a half header and slipped off the platform, bruising his right shoulder, left elbow and left hand. EE finished his shift and started missing work as a result of this incident on 11/12/2010.

Employee was moving the section waterline manifold and when he placed the manifold down and against the mine rib, the manifold slipped and mashed the end of his right middle finger between the manifold and the mine rib.

EE was hanging the continuous miner cable when he felt pain in his left shoulder.

Employee was carrying a driveshaft through muddy water and stepped into a hole and twisted his left knee as foot slipped off the ledge. He continued to work and went to the doctor on 09-30-10 and started missing work his 10-01-10 shift.

Employee was installing roof bolts and as he was reaching to remove the drill steel, a rock (approximately 18" x 14" x 4") fell from the mine roof and mashed his right index finger against the roof bolter drill pot, resulting in a laceration and crushing injury. His right index finger was amputated at the middle phalange at the hospital as a result of his injury.

Employee was installing roof bolts, when the drill steel got stuck in the bolt hole in the mine roof. As the employee was reaching to pull the drill steel out of the bolt hole the drill steel fell out of the bolt hole and struck his left middle finger which was resting on the roof bolter drill pot, resulting in a fracture to his finger.

EE was trying to bend a belt conveyor guard with a slate bar when the bar slipped, causing his hand to strike the guard, resulting in a laceration to his left little finger that required 5 sutures.

Occupational Pneumoconiosis Award. Received notification 07/15/2010.

Occupational Pneumoconiosis - Received diagnosis 07-15-2010.

EE was assisting setting breaker timbers in No. 4 entry when a piece of rock fell from between roof bolts and struck him in the back right side of his ribs. EE finished his shift and had medical evaluation on 06/21/2010 and diagnosed with a fracture to his rib and taken off work.

Employee was operating the continuous miner when a piece of rock fell from the mine roof from between roof bolts and struck him on the hard hat.

EE was repairing belt structure at No. 7 belt take-up when a fire suppression spray broke and sprayed water in his face and as he was trying to move away, he tripped and fell striking his left knee on the mine floor.

Employee was pulling the continuous miner waterline while walking backward and tripped on a rock and fell and landed on his right hand and injured his right shoulder.

EE was unloading a roof bolting machine canopy jack from the maintenance truck with a chain and hoist at the surface shop. As the jack was being raised, and EE placed his hand on the jack to guide it, the jack slid through the chain and mashed his right index finger between the jack flange and the hoist hook.

Employee was preparing to cut strings off belt conveyor rollers at F-Belt Take-up, when a water hose in the area burst and the pressure pump turned on causing the hose to strike EE on his legs and causing him to fall and strike his head on the mine floor.

Employee was walking beside the section feeder breaker when he was struck on his left arm and hand by a piece of coal from the mine rib.

Occupational Hearing Loss Diagnosis - Received 03-16-2010

Walking to face in No. 6 entry to repair line curtain and slipped and fell on her right arm and shoulder.

EE was closing a walk through mandoor when he lost control of the door due to the air pressure and his left hand was caught between the door and the door frame.

Unintentional Roof Fall found in air course during weekly exam. 18' wide x 25' long x 4 1/2' to 5' thick.

Diagnosis of Pneumoconiosis. Received 01/22/2010.

Employee stated that on 01-09-10 he lifted a crib block on to his shoulder and turned and stepped off a ledge in the mine floor near the section feeder and twisted his knee and causing him to fall. Employee finished his shift and did not start missing work as a result of this incident until 01-12-10.

Diagnosis - Carpal Tunnel Syndrome

Employee was performing the duties of a continuous miner helper, when a piece of rock fell from the mine roof and struck him on the top of his right foot, resulting in a laceration.

Employee had checked the pressure on the mobile roof support and was walking back out of the place when a rib fell and struck him and knocked him to the mine floor.

Employee was getting out of the scoop and slipped and fell striking his right elbow against the frame of the scoop deck.

Employee stated that he and another employee were pulling on the continuous miner water line when he slipped and fell striking his back on a crib block. EE continued to work and started to miss work on 12/23/2009 and I was notified today by ee that he was off work as a result of his incident on 09/14/09.

Employee was standing by No. 4 belt at belt switch and was turning the belt off to spot the belt, when a rib rolled and struck him on his right shoulder.

Employee was standing on a wooden block examining an MRS when he slipped and fell to the mine floor injuring his left shoulder.

Employee stated he bent over and picked up a 7" x 7" x 12' timber and felt pain in his right shoulder and chest.

EE was lifting a coalhauler battery lid in order to place it on another battery, when he felt pain in his abdomen. NOTE: EE did not miss work as a result of this incident until 09-29-2009.

An unintentional roof fall was found at approximately 7:25 p.m. in H-Return aircourse, No. 2 entry from Crosscut 29 (Spad 112540) to Crosscut 30 (Spad 11254). The fall was approximately 20' wide x 100' long x 8' high.

A rock fell from the mine roof approx 35' inby survey station No 13356 as the 6-left crosscut was being mined on the 020-0 MMU, East section. The rock measured approx 4'6" to 5' high x 15'6" long x 5'6" wide. 6' roof bolts were being used as primary roof support in the No 6 entry. This report is being submitted to prevent any additional citations.

Employee was tramming the roof bolting machine. When the machine went through the check curtains, the drill steel got caught on the curtain, causing the drill steel to strike the employee on his left neck and shoulder.

Employee was operating roof bolter machine. He went to place the roof bolt wrench in the drill pot, the drill steel fell from the mine roof and pinched his left index finger between the drill steel and the wrench, resulting in a laceration.

Unintentional roof fall: d-intake, No 8 entry, intersection at No. 15, crosscut, 80 ft inby survey station No. 10013. The fall measured approx. 20ft long, 20 ft wide and 6 ft thick. Five foot fully grouted resin roof bolts were in use as primary roof support.

Unintentional roof fall: No 1 Belt Entry, Intersection, Crosscut No. 11. The fall measured approx 20' long, 20' wide and 4' thick. Four foot conventional roof bolts were in use as primary roof support.

EE was replacing a hydraulic hose to the conveyor chain motor on the feeder breaker. EE was standing on a 6" crib block to reach the hose fitting that was 6' above the mine floor and approx 20" from the frame of the feeder. As he was loosening the hose fitting with a wrench, he felt a sharp pain in his left adbomen area.

EE was diagnosed with arthritis and degenerative changes in his left kee after he experienced pain, redness and swelling two days after working on his knees changing a drive shaft in a shuttle car.

Employee stated that he went to C-Belt Hole to get a solid steel bar to hang 1000 ft. of conveyor belt. While lifting the bar he felt pain in his lower back.

Unintentional roof fall: d-return, No. 1 entry, crosscut no. 32, survey station no. 10183. The fall measured approx. 40 ft long , 20 ft wide and 5 ft thick. Five foot 3/4" ida. fully grouted resin roof bolts were in use as primary roof support.

Occupational illness: Employee stated he was removing a front drive shaft fron the 851 diesel mantrip when he struck his left elbow in the drag link. on 4-30-2009, Employee was diagnosed with bursitis of his left elbow.

East section, No. 5 Entry (roadway) 80 ft. inby survey station 12968. The fall measured approx. 80ft long x 20ft wide x 5 ft high. Five ft 3/4" dia. Fully grouted resin roof bolts were in use as primary support.

The employee stated he was standing at the inby corner of break 6 to 7, 100' inby spad 12591. The continuous miner was finishing a lift and the employee was getting ready to operate the MRS. He stated that a piece of coal fell from the rib and struck him on the right side of his head.

The EE stated he was walking outby in the No. 7 entry and as he came through the back-up curtains, he slipped on a ledge in the bottom and fell to the mine floor.

An unintentional roof fall occurred on the retreat mining South Section. The fall was approx. 20ft wide x 14ft long x 9ft high. The fall was the result of a slip formation in the mine roof that occurred just outby mobile roof supports. The area was supported with 6 ft point anchor roof bolts.

Employee stated that he had lifted a piece of rock off the line curtain when another rock fell from the mine roof striking him on the top of his right foot.

Employee stated that he was walking and throwing miner cable out of the roadway when he stepped onto the ledge of a hole in the mine floor causing him to hyperextend his right knee.

The employee stated he was walking outby the No. 5 entry and as he passed a scoop that was parked in the entry his foot slipped in a hole in the mine floor that was covered in mud and water causing his foot to twist as he fell to the bottom injuring his left foot and ankle.

An unplanned roof fall (highwall failure) occurred at the mine fan portal area. The fall was found at approx 9:18 PM and it was determined that ventilation was impaired at approx 10:35 PM.

Carpal tunnel syndrome.

The employee stated that he had walked over between the roof bolting machine and the mine rib and was bending over to pick up the line curtain to hand it to the other roof bolter operator, when a piece of rock fell from between the roof bolt and the rib and struck him on his left shoulder resulting in a contusion.

The empoyee stated he was assisting in the hanging of back-up fly curtains and was standing against the mine rib when a scoop ran over a lump of coal causing a piece of coal to shoot out from under the scoop tire and strike him on the left knee.

The employee was moving No. 4 MRS after mining the second lift from the bottom end of the pillar block. He was standing at the crosscut to the right of the intersection when a piece of rock fell from the top striking the employee on top of the hard hat.

The employee was helping make a belt splice at #1 belt drive between the take-up and drive. He & other ee were hammering in splice nails. During the swing the hammer caught an over hanging metal fire supression line. When the hammer caught the line he strained his left shoulder due to the abrupt stop of his swing. The area was confined & had limited space to make a splice.

An unintentional roof fall occurred in No.4 entry, 100 ft inby spad 12371, H3 belt, approximately 20 ft wide x 45 ft long x 5 1/2' to 7 ft thick. Roof support in the area is 5 foot fully grouted roof bolts.

Employee stated that he walked over to talk to the CM helper and was reaching down to move the CM water line out of the roadway when he was struck in the back by a rib roll from the barrier block behind him. He was knocked to the mine floor and suffered bruises and abrasions to his back, left hip and left knee.

An unintentional roof fall was found in the No 2 Entry, at Crosscut No 5. The fall is approx 18' long X 17' wide X 4' thick. Area is supported with 4' conventional roof bolts. The fall was found at approx 8:45 am and the individual who found the fall had to walk outside to a phone and called immediately to have the fall reported at approx 9:20 am.

Employee stated that he was standing on a rock nailing the line curtain to the rib in preparation to scoop and rockdust the place, when he slipped off the rock and fell striking his right knee on the mine floor.

Occupational Pneumoconiosis.

EE was installing 6 ft torque tension roof bolts when the roof bolt wrench became misaligned causing it to come out of the roof bolting machine drill pot and strike his in the hard hat & forehead resulting in a laceration requiring 8 sutures. He returned to work his next shift.

EE was trying to loosen a 6inch pipe nipple from a 90 degree elbow with a 42inch pipe wrench, when the wrench slipped it caused EE to fall and twist his right ankle.

An unintentional roof fall, approximately 20' wide x 30' long x 7' high, was found at approx. 1:20 pm on an idle shift 6-15-08 in No. 5 entry at crosscut 12. The roof was supported with 6' point anchor torque tension roof bolts.

An unintentional roof fall, approx 20' wide x 20' long x 5' thick, was found at approx 7:40am on 6/12/08 in No. 1 Belt Entry at crosscut No. 9. The fall was a continuation of an old fall in this area. The roof was supported with 4' conventional roof bolts.

EE was reaching to get a roof bolt out of the tray of the roof bolting machine, when the bolt became stuck on other bolts. EE pulled on the bolt to free it and felt a pain in his right shoulder.

EE stated he was walking to mantrip, when he stepped on a piece of coal, causing him to twist his right knee and fall to bottom/mine floor. EE started missing work as a result of this incident on 9/25/08.

Unintentiona roof fall: an unintentional roof fall was found at approx. 7:55 am on 3-10-08 in the No. 1 Return, No. 2 entry, Crosscut No. 12, approx. 80 ft. outby spad No. 49. The fall is approx. 4 1/2 ft high, 20 ft long and 25 ft wide. The roof was supported with 4 dft conventional roof bolts.

Employee stated that he was hanging backup fly pads in No.7 entry while standing on the last rung to the top of a four foot step ladder when his foot slipped causing him to fall to the mine floor, injuring his left hand.

An unintentional roof fall occured on the East Pillar Section, (MMU 020) in the No. 6 entry, approximately 50' inby Survey Station 11685. The fall extended from the first lift in the left hand block of the No. 6 entry. The fall was approximately 4' high, 25' wide and 30' long.

The employee stated he was pouring a five gallon power tran tractor hydraulic fluid in the tank when he lost his footing. The employee stated that he felt a sharp pain in his lower back as his foot slipped and stepped into an opening between the frame and battery.

Employee stated he was inserting rib bolt in rib when top of rib (coal) fell striking him on left thumb against drill wrench. Employee was wearing his bolt gloves (metacarpal).

EE was posititioned outby the boom of the cont. miner, moving the cable. The cont. miner had completed the left side lift. A piece of rock 4' x 2' x 6" fell out from between roof bolts. EE saw the rock falling & put his hands up to block it from striking him. It struck his hands, & pushed him backwards. As he fell backward, the rock struck his upper lt leg knocking him down.

An unintentional roof fall occurred on the East pillar section, in No. 1 entry, approx 30' inby survey station 11783. The fall extened from the second lift in the left hand barrier into the entry and fell on the No.3 and No.4 MRS's. The fall was approx 5' high, 10' wide and 20' long.

Unintentional Roof Fall - An unintentional roof fall was found in the 1-Northmains, No. 2 entry, No. 29 crosscut, 80 ft. outby survey station No. 228. This was a continuation of a fall that had been present in No. 3 and No. 1 entries and now has extended through the crosscuts into No. 2 entry. The new fall area is approximately 5 ft. high, 20 ft. wide and 60 ft. long.

The apparent failure of a tail roller bearing resulted in the misalignment of the No.6 belt conveyor and caused belt conveyor material to wrap around the shaft of the tail roller. This caused a belt fire accident at the tail roller of No. 6 belt conveyor in the draw-off tunnel on the surface.

Employee stated that while walking through No.4 crosscut left into No.3 entry, he stepped through the fly pads and stepped off a 16" ledge, twisting his left knee. He stated that he looked up at the mine roof because a piece of rock fell on other side of flies and he did not notice the ledge.

Employee stated he was turning 2 xcut left. The top started to vibrate as the miner started to cut. A piece of rock fell out of the top, striking his hat and then struck his little finger on his left hand. The rock was 7 1/2 long, 2 1/2 wide and 0 to 1 1/2 thick. this occurred 200' from #1 entry 100' outby spad 11846, 2 break left on the east section.

The miner operator backed the continuous miner out of the face of #5 entry and told injured EE to hook up his slack rope. When injured EE hooked up the rope, a coal rib fell, striking injured EE in the middle of his back and legs. EE started missing work as a result of this injury on 11/8/07.

Unintentional roof fall - East section - MMU 020-0 10 ft inby spad 11985 - No. 1 entry. Fall approximately 100' long, 18 - 20' wide & 7 ' high.

Employee was trimming a piece of skirt rubber in the feeder breaker on the South section. After cutting the rubber he laid the utility knife to the side with the blade out and pointed up. As he was getting up from a kneeling position, he inadvertently laid his hand down on the knife, resulting in a laceration of the right little finger requiring 3 sutures.

Employee stated that he was just completing the first lift to the left in No. 5 entry, when the right rib and rock from the mine roof fell striking him on his right shoulder knocking him to the mine floor and another part of the rib/roof then struck his left foot.

Occupational Hearing Loss

Making a belt move, pulling the conveyor belt through the tail piece, the clavis that attaches the wire rope to the belt puller became fouled up on the crossbrace of the v-scraper. He placed his foot on the conveyor belt to dislodge the stuck clevis. The clevis released and the belt puller and conveyor belt pulled through and caught his right foot between the cross-brace and belt.

EE help move continuous miner was backed down #4 entry to drop off some cable, & he was walking to #5 to help hang continuous miner cable when & slipped on uneven bottom in the mine floor & twisted his right ankle, was a hole in the mine floor approx. 6" deep and 2' in diameter & wearing rubber metatarsal boots at the time.

EE was hanging fly (back-up) pads in NO. 4 entry. He was driving a nail with his hammer; he felt pain in his right wrist. He did not hit his hand or wrist or strike it against anything but that it just started hurting. His wrist continued to bother him and he was seen by a physician on August 9th and was diagnosed with a sprained right wrist and a cast was applied to immobilize it.

2 EEs were assigned the task of hanging a solid curtain in the x-cut between No. 5 & 6 entries. EE was working off a 6 ft step ladder & was descending ladder when he slipped to the mine floor as he was stepping from the 3rd rung of the ladder to the 2nd rung. He complained of pain in back but finished his shift. The area was wet & muddy and there was mud on the ladder rungs.

Employee was operating the No. 187 continuous miner in the No. 9 entry and as the continuous miner was backing up, a loop of water line folded under the tail of the continuous miner. Employee stated that he stopped the continuous miner and reached down and was pulling the water line out of the way, when he felt a tearing feeling in his right shoulder.

EE was trouble shooting a power problem on No.221 roof bolting machine. He was walking from No.1 entry to the tool car in No.5 entry, when he slipped on slick bottom in No.4 entry just inby the feeder breaker and fell striking his left elbow on the mine floor.

An unintentional roof fall occurred in No. 3 belt entry at crosscut No. 50 at spad No. 466. The fall was approximately 6 ft. high x 20 ft. wide x 60 ft. long. This area was developed in 1991 and supported with 48 in. conventional roof bolts.

EE stated he was dragging a section of line curtain when he felt a burning sensation in his left groin area. The incident investigation revealed that the line curtain was wet & muddy causing the curtain to be heavier than normal. EE was diagnosed with a hernia but continued to work until 8/27/07 when he started missing work as a result of this injury.

Employee was firebossing F-2 belt when he stepped on a rock in the walkway and twisted his right ankle causing him to fall to the mine floor. NOTE: EMPLOYEE DID NOT START MISSING WORK AS A RESULT OF THIS INJURY UNTIL 07-18-2007.

EE was installing a new terminal end on contactor battery cable lead on the maintenance mantrip using a knocker type crimping tool & chipping hammer. While striking the crimping tool, a piece of metal flew off striking EE in the left hand between thumb & forefinger. Result: puncture & a piece of metal in his hand. Metal was surgically removed the next day & required 4 sutures.

EE was installing roof bolts, drill steel became lodged in the hole. EE was reaching for drill steel to pull out of hole, when the drill steel fell from hole and struck his left middle finger against the roofbolter drill pot. EE was wearing metacarpal gloves at the time of accident. Went to Health Plus and diagnosed with a simple fracture and sutures to close wound.

Carpal tunnel Syndrome.

Employee stated that he was pulling up a 16' metal roof strap, lengthwise, and he was trying to hold the strap with his right hand and trying to get the resin glue into the drill hole with his left hand when he pulled something in his neck or left shoulder.

The investigation indicated that the employee was pulling on the continuous miner cable when he felt pain and burning in his right side. The employee started missing work as a result of this injury on 5-16-07.

Unintentional Roof Fall: Intake air course, 35' inby Whites Branch Main Fan. Approx 30' long X 20' wide X 15' high. 4540' to nearest section (north). Area of fall was developed in Sept 2000 and supported with 5' resin bolts with supplemental support with 6' torque tension bolts.

Unintentional roof fall 20 ft. long, 20 ft. wide, 5 ft. high in No. 3 Return, No. 1 Entry, Crosscut No. 49. This is a continuation of an old fall in adjacent entry.

Untentional roof fall-120 ft inby spad #11235 No.5 entry 7 sourthwest mains.

Unintentional roof fall. Approx 80'l x 20'w x 7-10'thick, 40' inby Spad #11,083 at No 6 entry, H intake, at 7 southwest.

Occupational pneumoconiosis - Date notified 01-19-2007.

Employee stated while walking up No.5 entry towards the face area, he stopped to get some spad nails off of #25 scoop to use to ventilate. After retrieving the nails, he preceeded to walk towards the face area when he stepped off a ledge in the mine floor causing his to fall & twist his lt. knee. The mine floor in the area of the incident was damp & slightly muddy.

Roof material fell on No.4 MRS that was located on the East section in No.8 entry, No.50 & 51 pillar blocks. The roof material caused the MRS to become fouled for a short period of time.

Mining in No.5 entry No.31 and No.32 pillar block, when slate rock fell on No.3 MRS jack.

The employee was standing on 213 coal hauler pulling on a piece of belt with a pair of channel locks. The channel locks slipped off the belt causing the employee to fall from the hauler and strike his left knee on the coal hauler.

The ee stated he was moving phone line up from a spool of wire. He was holding the spool of phone wire with his hands and while rolling the sire off the spool a sharp end of the wire cut his index finger and the top of his left hand.

EE stated that he was leaning over his tool box looking for a splice kit for the continuous miner. EE then proceeded to move a piece of metal out of the way, when he felt a burning sensation in his lower back and left leg.

On the morning of 9/25/06 EE reported at the end of his shift that he was having pain in his lower back. When ask if he had done anything to hurt his back. He stated that he had not, that the problem had been coming on for some time. When ask if he knew of any specific event that he was aware of to injure his back, he stated, that he did not know of anything.

Employee stated that while he was positioning a shield over the feeder breaker the shield slipped & caught his right hand between the shield and the frame.

The employee was in the process of cleaning up around a coal hauler that he was preparing to change a tire on when he tripped on half a crib block that was on the floor. He fell to the shop floor that resulted in twisting his left knee.(He was wearing knee pads)

Employee stated that he bent over at No 2 MRS to pull the No 1 MRS cable out of the way, when ee suddenly pressurized the No 2 MRS against the mine roof causing a piece of rock measuring 43" x 30" x 8" thick to fall, striking him on the upper back.

Employee stated that he was helping set breaker timbers in no. 6 entry when he felt pain in his right side and back spasm in the lower back. Note: employee first started missing work as a result of this injury on 09-08-2006.

Employee stated at 1:30am he was hanging MRS cable between No.2 & No.3 entry. While he was standing on a lump of coal to hang the cable, the lump of coal rolled out from under him causing him to fall.While getting up, he felt pain in his lower right abdomen area.Note: EE started missing work as a result of injury 10/16/06.

EE stated he was helping a co-worker put a bearing on No.5 belt take-up carriage roller shaft. He was straddling belt roller & holding the bearing as the co-worker was knocking bearing onto roller shaft with chisel & hammer. Several pieces of metal flew off bearing striking him in lt arm & leg. Pieces of metal became lodged in arm & were removed by a dr. & sutures applied.

Carpal Tunnel Syndrome (Received Meical Information on 8-3-06

Employee stated while traveling on No.4 roadway, at crosscut 12, the No.20 mantrip canopy struck a roof bolt plate causing the mantrip to come to a sudden stop causing him to hit the left side of his head on the front of mantrip.

Employee stated that he & 6 co-workers were loading 48" conveyor belt into a coal hauler on the supply yard. The diesel mini-trac was pulling belt in the coal hauler from back, while 966 loader was holding roll of belt. EE was standing approx 10' in front of loader. As the mini-trac was pulling on the belt, he stepped on the belt, lost his balance, struck his head on the ground.

Employee states that while fire bossing on E-belt, he crossed under the belt to check the pressure pump. While doing so, he jammed his head into the four inch waterline when he raised up on the other side of the belt. Note: Employee did not start missing work as a result of this injury until 6-13-2006 and returned to work on 6/16/06.

EE stated that he broke loose the main 4" water line at E2 head & removed on section. He stated that as he pulled the waterline out of the way, he felt a pop in his rt shoulder. He then helped remove add'l 4" waterline from a scoop & place it along E2 belt. He stated that as he was pulling a section of this waterline that he felt another pop in rt shoulder & pain rt shoulder blade.

Employee was operating the continuous miner in No. 3 entry mining the first lift left from the pillar when a pillar bump occurred, causing a rock brow measuring 19" long x 8" thick x 10" high to fall from the mine roof and rib. The rock struck his right shoulder and neck resulting in contusions to this area. The mine roof was supported with 5' resin bolts, plates & mats.

Employee was in process of cleaning up shop work area, when he picked up a five gallon bucket of oil that was located beside the coal hauler. When carrying the bucket of oil out of the way, he tripped over a jack stand, when his metatarsal boot caught a yellow tie hanger that was attached around the jack stand causing him to trip and fall.

Employee stated that he was installing the inside bolt in the 3rd row of bolts in 6 crosscut left. He had placed the drill steel in the drill pod & raised the drill steel to the mine roof & when the drill steel came in contact with the mine roof, a piece of slate rock measuring 47"L x 22"W x 3" thick fell striking his left forearm & left wrist causing a laceration to his forearm.

EMPLOYEE STATED WHILE BOLTING 3XL CROSSCUT, ON 4/15/06, A PIECE OF SLATE ROCK FELL STRIKING EE ON THE RIGHT SHOULDER. EE WORKED ON 4/17/06, AND TURNED IN FLOATING VACATION DATYS FOR 4/18 & 4/19. BASED ON MEDICAL INFORMATION RECEIVED ON 5/25/06 IT HAS BEEN DETERMINED THAT EE SUFFERED A OCCUPATIONAL INJURY.

EMPLOYEE STATED HE WAS HELPING MOVE THE CONTINUOUS MINER CABLE OUT OF THE HAULWAY, WHEN HE TRIPPED OVER A RIB ROLL & FELL STRIKING HIS LEFT LEG JUST BELOW THE KNEE ON A LUMP OF COAL ON THE MINE FLOOR. EMPLOYEE WENT TO THE E.R. AFTER WORK TO HAVE HIS LEG CHECKED & RETURNED TO WORK ON HIS NEXT SHIFT. EE CONTINUED TO WORK UNTIL 4/22/06 WHEN HE RETURNED TO THE DR.

Occupational illness: Chronic back pain. X-ray examination indicates degenerative changes. EE has previous and ongoing medical treatment for this condition.

EE STATED THAT HE HAD PARKED THE NO. 8 COAL HAULER THAT HE WAS OPERATING TO HELP A COWORKER CHANGE THE BATTERIES ON HIS COAL HAULER. WHILE WALKING BACK TO HIS COAL HAULER, EE STEPPED ON A FLY PAD THAT WAS LYING IN THE WALKWAY & SLIPPPED & FELL AGAINST THE BATTERY OF THE COAL HAULER. HE HIT HIS UPPER CHEST ON THE CORNER OF THE BATTERY. DID NOT START MISSING WORK UNTIL 3/17/06.

Had belt jacked up. Come a long broke and belt came down on right foot.

Employee stated that he was drilling the outside roof bolt hole in the No. 7 Room, when a piece of slate rock fell and struck the operator canopy and a piece measuring 2' x 2' x 1" struck him on the lower right side of his back and knocked him forward into the controls. He stated that he did not observe any loose roof material when he had installed the inside roof bolt.

EE stated he was helping set a breaker timber approx 10 1/2'L & 20" dia. The timber had been cut to length & 2 co-workers lowered 1 end to the ground from the scoop & EE & another co-worker were holding other end of timber. W/out telling EE, other co-worker lowered the timber to rest on his knee, shifting the weight to EE who experienced pain in lower back.

According to employee, he was pulling on Red Devils comealong, and had shortness of breath & back & neck hurt.

Occupational Illness - Degenerative disc disease. Employee stated that he was sitting on a bench in the change/lamp room prior to his shift and had removed his street shoes and was reaching for his work boots when he felt pain in his lower back.

Walking down no.5 entry toward mantrip, EE slipped and fell to the mine floor

CHRONIC BACK PAIN, X-RAY EXAMINATION INDICATES SPURING OF THE SPINE. PREVIOUS & ONGOING MEDICAL TREATMENT FOR THIS CONDITION.

EE was attempting to install an oxygen tank guard tube measuring 11" o d pipe w/1/2" thick walls x 34" long on the #5 mainten. truck. EE had it supported w/a 10" long piece of 6" channel, channel iron slipped out from under the pipe before the ee could tack weld it in place causing the channel iron to strike him on the rgt thumb & index finger and hyper extended his lft thumb.

EMPLOYEE WAS CUTTING ACROSS THE BELT & THE UTILITY KNIFE SLIPPED & STRUCK HIS LEFT LEG, BELOW THE KNEE.

EE stated that he locked & tagged out the power to #94 Coal Hauler Charger & then replaced the receptacle on #3 Battery & receptacle on #94 Charger. He then connected jumper cable to battery & charger. He stated he then was attempting to bolt a fuse to a shunt when an electrical arc caused electric burns to both hands.Result of grounded lead & power from battery

EE stated that he and other ee were loading 48" belt conveyor structure (rollers) into the back of a mantrip. His left hand was caught between the belt conveyor structure and the canopy of the mantrip, as Other ee soved the belt structure into the mantrip. EE suffered a fracture and laceration to his left middle finger. The laceration required 4 sutures to close.

Occupational illness: ignuinal hernia.

UNINTENTIONAL ROOF FALL APPROXIMATELY 10' HIGH X 20' WIDE X 25-30' LONG AT NORTHWEST, #1 ENTRY, 70' INBY SPAD 9878

EMPLOYEE STATED THAT HE WAS WALKING ACROSS THE LAST ROW OF CROSSCUTS WHEN HE SLIPPED AND FELL IN AN UNEVEN PLACE IN THE MINE FLOOR. HE STATED THAT THERE WAS WATER IN THE AREA WHERE HE FELL. HE STATED THAT HE FELL ON HIS RIGHT KNEE WHICH CAUSED A TWISTING MOTION. HE REPORTED THAT HE EXPERIENCED PAIN TO THE RIGHT KNEE AREA.

EE STATED THAT HE OBSERVED THAT THE CONTINUOUS MINER WATER LINE HAD BEEN WRAPPED AROUND A TIMBER AFTER THE MINER CABLE SLACK HAD BEEN MOVED TO A LOCATION CLOSER TO THE LOB. EE STATED THAT AS HE ATTEMPTED TO UNTANGLE THE WATERLINE FROM TIMBER, HE LOST HIS FOOTING ON A LEDGE IN THE MINE FLOOR. EE STATED THAT HE STARTED TO FALL, CAUGHT HIMSELF & TWISTED HIS BACK.

Employee stated that he was installing a roof bolt in No.2 entry, when he bent the roof bolt he was attempting to install. EE was trying to retrieve the roof bolt back out of the hole by using his rotation on the roof bolter. When doing so, the roof bolt turned while his right hand was on the roof bolt jerking his right hand and causing pain to right wrist.

EE WAS HELPING ON THE CONTINUOUS MINER AND AS HE ATTEMPTED TO HOOK THE CABLE ROPE ONTO THE CABLE POST/HOOK, HE PLACED HIS RIGHT HAND BETWEEN THE STOP BLOCK OF THE MINER HEAD AND THE PAN OF THE MINER. AT THE SAME TIME THE MINER OPERATOR LOWERED THE MINER HEAD, RESULTING IN EE'S RIGHT LITTLE FINGER BEING CAUGHT BETWEEN THE STOP BLOCK AND THE PAN OF THE MINER.

EE STATED THAT HE WAS WALKING DOWN #6 ENTRY TO GET A PIECE OF LINE CURTAIN. HE SAID THAT HE STEPPED IN A HOLE IN THE MINE FLOOR AT THE INTERSECTION AND SPRAINED HIS RIGHT ANKLE. EE STARTED MISSING WORK AS A RESULT OF THIS INJURY ON 4/11/05.

Occupational illness - Alleged Bilateral Carpal Tunnel Syndrome.

Employee stated that he and another employee had made a belt conveyor splice in no. 4 belt and were using chisels and hammers to spread the belt clamp apart to release the tension from the belt conveyor, when the belt clamp came loose and struck him in the upper right arm.

OCCUPATIONAL ILLNESS: PERITENDINITIS AND TENDINOPATHY OF THE ROTATOR CUFF AND DEGENERATION ANTERIOR FIBROCARTILAGINOUS LABRUM AND CYSTIC CHANGE IN THE POSTERIOR HUMERAL HEAD. MRI RESULTS ATTACHED.

Employee stated he was sliding a continuous miner cat idler out of the back of a supply truck onto the forkds of the warehouse forklift when the idler sled unexpectedly and pinched the index finger of his right hand between the forks of the forklift and the cat idler.

OCCUPATIONAL ILLNESS. SCOLIOSIS AND DEGENERATIVE CHANGES AT C5-C6. EE FIRST MISSED WORK AS A RESULT OF THIS ILLNESS ON 12/13/2004.

An unintentional Roof Fall approx. 18' wide x 35' long x 4" thick ocured int he 1-north mains. No.7 entry, one crosscut outby spad no. 132.

FINAL PUSH OUT HAD JUST BEEN COMPLETED IN NO.3 ENTRY & THE CONTINUOUS MINER BACKED OUT OF THE PLACE. INJURED & HIS SUPERVISOR WERE STANDING IN THE INTERSECTION IN THE LAST OPEN BREAK IN NO.3 ENTRY PREPARING TO MOVE THE MRS'S WHEN A PIECE OF ROCK (APPROX 34"X20"X 3"-7" THICK) FELL FROM THE MINE ROOF FROM BETWEEN ROOF BOLTS STRIKING HIM ON HIS BACK AND LOWER RIGHT LEG.

DEGENERATIVE DISC DISEASE. X-RAY REPORT FINDINGS ARE; SPONDYITIS AT L2-3 AND L4 LEVELS, DISC SPACE HEIGHT LOSS AT L2-3 AND OSTEOPHYTOSIS AT L5-S1 LEVEL. NO FRACTURE OR MALALIGNMENT.

Ee stated that he was operating No. 23 scoop with the float lever in the locked position. He stated that the float lever, which is spring loaded, popped out of the locked position catching the index, middle and ring fingers of his right hand between the float lever and the steering lever.

Unintentional roof fall, 15' x 20' x 5'. 1-southwest mains, break 45, no. 7 entry. Inactive area of mine. In intake air course developed 05-31-93 and supported with 5' fully grouted resin roof bolts.

UNINTENTIONAL ROOF FALL. LOCATION: 6 WEST SECTION NO.4 ENTRY/120'INBY SPAD 9438. SIZE: APPROX. 20' WIDE X 35'LONG X 15'HIGH. ROOF SUPPORTS USED IN AREA: 6-8' POINT ANCHOR TENSION REBAR BOLTS.

EMPLOYEE STATED THAT HE WAS AT THE NO.1 BATTERY CHARGING STATION CHANGING THE BATTERY ON THE NO.1 COALHAULER. AS HE STEPPED OVER THE BATTERY LIFT ARM TO DISCONNECT THE JUMPER CABLE AND CONNECT THE COALHAULER CABLE TO PLUG TO THE FRESH BATTERY, HIS FOOT SLIPPED AND HE FELT PAIN IN HIS LEFT KNEE.

OCCUPATIONAL PNEUMOCONIOSIS AWARD. (NOTIFICATION RECEIVED ON 8/5/04).

Ee was in the process of installing roof bolts in face of #4 entry. Drill steel that he was using to drill roof bolt hole hung on top of drill pot as he was lowering the steel out of hole. He stated he was holding steel w/rt hand & was moving drill pot back & forth to free the steel. When steel came loose it caught little finger of rt hand between pot & steel.

EMPLOYEE STATED THAT HE HAD TAKEN THE DRILL STEEL OUT OF THE DRILL POT & STOOD IT ON THE ROOF BOLTER PLATFORM. AFTER INSTALLING THE ROOF BOLT HE WAS REMOVING THE WRENCH AS HE WAS LOWERING THE DRILL POT & AT THE SAME TIME THE ROOF BOLTER MAST CAUGHT THE DRILL STEEL & BENT IT LIKE A BOW PINCHING HIS RT.INDEX FINGER ON TOP OF THE DRILL WRENCH. FRACTURED & LACERATED RT. INDEX FINGER.

EMPLOYEE STATED THAT A 10' TIMBER HAD FALLED OUT OF THE SCOOP BUCKET AND HE AND A CO-WORKER LIFTED THE TIMBER TO PUT IT BACK INTO THE SCOOP BUCKET AND AS HE LIFTED THE TIMBER HE FELT PAIN IN HIS LOWER BACK.

EMPLOYEE STATED THAT HE HAD USED A ROOF BOLT WRENCH TO FINISH PUSHING THE DRILL STEEL INTO THE HOLE AS HE WAS DRILLING A ROOF BOLT HOLE. WHEN HE COMPLETED DRILLING THE ROOF BOLT HOLE HE ATTEMPTED TO DROP THE WRENCH AND DRILL STEEL OUT OF THE HOLE. THE DRILL STEEL FELL OUT OF THE HOLE CATCHING THE LITTLE FINGER OF HIS RIGHT HAND.

OCCUPATIONAL HEARING LOSS

OCCUPATIONAL PNEUMOCONIOSIS.

(DEGENERATIVE DISC DISEASE). EE STATED HE HAS CHRONIC BACK PAIN & MEDICAL INFO (X-RAY REPORT) INDICATES DEGENERATIVE DISC DISEASE SEEN EVIDENT BY ANTERIOR OSTEOPHYTES.

Employee stated that he was hanging a fly pad using a four foot step ladder. He stated that when he stepped backwards off the ladder his right knee was hyperextended causing pain to his right knee area.

WHILE INSTALLING BELT HANGERS HE CAUGHT HIS LEFT THUMB BETWEEN THE BOTTOM OF THE "T" BAR ON THE ATRS SYSTEM AND THE TOP OF THE DRILL POT. HE WAS PUSHING A ROOF BOLT INTO A DRILLED HOLE USING A 12" ROOF BOLT WRENCH WHEN THE INCIDENT OCCURRED
