Removing rust & dirt from springs to install on a truck. Springs on ground, bent down with grinder. 8" electric wire wheel on grinder kicked back on edge of spring & struck right knee.

The C3 Coal Pile showed signs of heat and possible fire.

Employee stated that he was stacking tires when as he was rolling a tire, it rolled on a bearing and began to fall. As he tried to catch it, grabbing it with his left arm, he felt something pop in his arm. Employee began missing work on 01/12/2011.

Employee was repairing chains on pad beside shop beside a fire built in a barrel. Wind blew flames to his backside catching clothing on fire.

Employee was using a hammer & chisel to spread apart an equipment part when the chisel flew from part and hit employee, knocking off safety glasses; resulting in injury to left eye.

A coal seam fire started due to blasting in the area of job #4 K cut reclaim. The fire is not close to anything that can be damaged and poses no threat to miners.

Employee was driving off-road coal truck and had stopped; before proceeding after stop, the truck was'rear-ended' by an approaching truck with minimal damage to both trucks. Both trucks had just dumped in loading bin before incident occurred.

Employee alleges that he sustained multiple physical injuries from minor motor vehicl accident. Employee was driving truck and had left the truck dump station. He was traveling approximately 5-mph going down hill and 'rear-ended' another truck that had stopped. SUBMISSION BE FILED UNDER PROTEST

Employee states he was tightening the wheel and the wrench slipped, causing him to fall with right leg under him. "REPORT BEING FILED UNDER PROTEST"

Employee states he was pulling pins out of rear suspension using an OTC ram when he felt his back pop.

Employee stated he was cleaning windows on truck - stepped on lower step mounted to the fuel tank causing it to break. He alleges his right leg hit the ground while his left foot remained on step.

Employee was working on truck when the crib block became dislodged,causing the tailgate to fall on him.

EE was performing maintenance on a hyd shovel. Wind was gusting at times. Around 3:00 a piece of grit was felt in the eye. It was washed out with water and felt OK. That night the grit was felt again. All attempts to remove didn't work. Went to hospital and had it taken out. Eye protection being worn.

Lceration from chain saw on arm.

EE was handling a clevis for 2" steel cable when the clevis dropped catching the tip of his ring finger on the right hand between the clevis and a rock resulting in a fracture to the tip of the finger.

Employee fell while exiting from truck, striking his right thumb on a pod cap.

The bulldozer slid out of the road. Employee put blade down to stop. The bulldozer rolled 3 times down hill. He fractured neck in two places and had 9 staples in his head.

Injury states he was driving his coal truck on the haul road when he started sliding backwards, even though he had chains on. He slid into a ditch.

He says he was walking to truck and slipped and fell. We had a witness that says he saw him walk to truck and get in and didn't fall. WE ARE FILING UNDER PROTEST

Employee was helping a mechanic stand up a barrel and got his finger caught between two barrels and fractured his finger.

He said he was checking his tires at the end of his shift to make sure they was not flat. He did this by kicking them He was kicking with right foot when his left foot slipped. He tried to prevent fall by bringing his right foot down.

He says he thinks it happened while he was tightening up a lug nut on a WS Tamdem wheel when he pulled a muscle in his right knee.

A brier got caught in chain saw. The chain saw cut strap on chaps and rolled chaps up. The chain saw jerked back and cut his leg.

EE stated that he raised the hood to check the fluids, when hsi shoulder popped, resulting in a "fractured" collar bone. This hood weighed approx 25's maximum.

Employee states that he was walking to a truck to perform scheduled maintenance when he twisted his ankle and fell on his knee.

While depressing clutch pedal to engage clutch, his left knee apparently became immoblile. "REPORT FILED WITH PORTEST"

Employee was installing input shaft in rotary head, using hammer to install bearing on pinion shaft. A piece of metal chipped from bearing and went into the left eye. He was taken by ambulance to medical center, then to hospital for surgery. He has been instructed to remain out of work for a least 14 days to recover, unsure of prognosis from that point.

Medical report of noise-induced hearing loss award for compensation for 2.57%.

Employee slipped from truck while exiting truck.

While exiting truck, he slipped on middle step. He stated that he grabbed handrail, swung around and hit his left shoulder on bed of truck, then fell to the ground.

As the employee went to the fill to dump Cat 785 rock truck company number unit #334, the truck went through the berm and over the fill. The employee received a laceration to back of the head, which required 9 staples in addition has contusions to waist and left leg. Medical reportable only - no lost work days.

Employee stated that he was picking up a crib block when he hurt his back. Stated that it was an old injury from a non-occupational motor vehicle accident.

EE was exiting 'cut A' coal pit. Because other adjacent vehicles/equipment, he drove on left side of roadway which is inclined; vehicle turned over onto its drivers side.

Employee stated that he was driving up hill from 'Y' cut when he missed a gear. The truck rolled back against the berm and turned over onto it's driver side.

EE stated that he was backing truck onto the stockpile ramp to dump coal, when the hoist nest broke, causing the bed to shift, laying truck partically on its side.

Employee stated that he was traveling in a curve when the power steering hose blew. This apparently caused the steering wheel to center & jerk his right arm.

EE alleges an incident/injury on 3/31807. There are no known witnesses. The company became knowledgeable on 7/26/07, when he called and reported the incident. The company does not ackowledge this claim.

Doing service work on Dozer. Oil filter on extremely tight. Was using 1/2" ratchet. Would not budge. Hit ratchet with 4 lb. hammer. On the 5th swing, he missed the handle and hit his thumb.

While opening the door on a rail car, used slate bar to open door. Stuck slate bar in door release to turn open and release, skipped a tooth causing employee to slip down to knees and bar hitting track rail with right thumb between rail and bar mashing right thumb.

Fire occurred in the area of #1 engine on RH340 excavator; the machine is a total loss. No injuries occurred from the fire.

Operating drill, steel got stuck. Asking for assistance, employee was using the wrench as co-worker had controls. Wrench grabbed, forcing his hand between wrench and steel.

EE was operating fuel truck when he reached over to get material from seat lost control and drove into a ditch. The truck turned slightly onto its side. The EE was not wearing seat belt and struck top of his head, resulting in a laceration requiring seven staples.

Employee was operating front end loader in pit when rock fell from the highwall striking the loader and bending portion of the cab entrapping the operator's right foot. It took approximately 45 to 60 minutes to free the operator. Employee was released from hospital the same day; slight contusion, no fractures or other injuries.

Employee stated on 8/03/06 that he had pulled a muscle in his lower back while tightening lug nuts on a truck tire. He notified company of the alleged incident after receiving pay minus a previous pay advance which the company paid because he was considered a voluntary quit. His 1st Dr release for "illness" from 7/25 thru 7/29; 2nd release from 08/03 thru 8/07 "no remarks".

Employee was removing lip saver from bucket of 340 excavator with sledge hammer. A small piece of metal struck his lower right leg. The piece of metal was removed and four stitches were required. The employee returned to regular job at full capacity the same day.

EE was removing material from highwall miner with boom truck. The hook slipped from material being removed & struck the EE on the wrist. He continued to work until 6/6/06 at which time it was determined to be fractured & a pin was put in the wrist 6/7/06. The employee is a highwall miner supertindent and has continued his regular each scheduled day with restricted sctivity

Putting up belt pan and wind blew dirt in eye. Received accident report today 5-31-06.

Burst disk on truck 639 blew and sprayed emulsion from head to toe and got into his eyes.

A possible unplanned ignition of explosives was reported due to smoke observed. Called in immediately per regulations. Futher investigation revealed smoke observed however no evidence of an explosion was found(noise,disturbed material). Our opinion: the shovel uncovered unignited ammonium nitrate that was burning in a hole that could not be observed until it was uncovered.

Employee stated that after lifting tire & wheel with forklift, he was holding tire up when the wheel slid out of the tire and hit him on top of the foot.

Employee was checking condensor blower motor that was inoperative when his right hand came in contact with a fan adjacent to the one he was checking. The result was lacerations to his right index and middle finger requiring stitches. The employee did not miss any days worked - medical reportable only.

EE STATED THAT WHILE HAULING COAL HE HIT A HOLE IN THE ROAD, CAUSING HIS SEAT TO BOTTOM OUT, HURTING HIS BACK.

Employee stated that while attempting to open tailgate, he was using the bullpin out of the bumper to try to pry the tailgate open. The pin slipped and mashed his thumb between the pin and bed.

Employee with another employee fell from push beams on a highwall miner, Employee was in the lanch pad area. He struck the ground and was unconicous for approx. 2 minutes. He return to work his next regular shift at full capacity. Medical reportable only no work days lost.

EMPLOYEE STATED THAT HE WAS ATTEMPTING TO OPEN THE TAILGATE OF TRUCK TO REMOVE GOB UNDER THE TAILGATE. HE SLIPPED, TAILGATE FELL MASHING HIS FINGER

Employee initially did not mention incident to anyone. He later stated that he got dirt in his eye while working on a truck fender, but did not know what time it happened.

Employee alleges the injury occurred on May 26, yet he worked twelve hours that day. On or about May 27/28 his supervisor noticed that the employee's finger was bandaged. The employee stated he had cut his finger at home and did not report an occupational injury; later he filed a WV Worker's Comp claim on Sept. 30. The employer became aware of this on Oct. 13 and is protesting.

EE was installing the bolts in the front of hood to the radiator from a ladder when he fell and fractured his right ankle/leg and foot. He was working at Fola Coal at the time of the accident on a 685 E dresser Haul Truck.

HE NOTIFIED EMPLOYER OF AN ALLEGED SPRAIN/STRAIN INJURY TO BACK OCCURRING ON THE DATE OF INCIDENT WHEN HE BUMPED INTO THE BACK OF ANOTHER TRUCK WHILE ATTEMPTING TO STOP AT BOTTOM OF HILL. HE DID NOT ACKNOWLEDGE AN INJURY OR ANY FORM OF PAIN OR DISCOMFORT. HE HAS SINCE ALLEGED THIS INJURY AFTER BEING TERMINATED FROM EMPLOYMENT.

Employee stated that while lifting a gear he strained his back.

EMPLOYEE WAS HELPING MAKE STRESS CONES ON A HIGH VOLTAGE CABLE. HE WAS TRIMMING THE EDGE WITH A RAZOR KNIFE AND THE KNIFE SLIPPED CUTTING HIS LEFT INDEX FINGER. THE EMPLOYEE RECEIVED 5 SUTURES AND RETURNED TO REGULAR JOB AT FULL CAPACITY.

EE STATED THAT HE WAS GETTING OUT OF TRUCK DURING HIS PRE-SHIPT INSPECTION AND STEPPED ON A ROCK. HE STATED THAT HE TWISTED HIS KNEE AND ANKLE, AND THAT HE FELL ON HIS BACK.

Employee stated that while climbing down from dozer, he slipped off track and sprained his right ankle.

EE WAS WALKING ON PALLETS THAT HAD BEEN PLACED IN DITCH LINE BECAUSE OF EXCESSIVE WATER DRAINAGE TO GOB BIN AREA. EE TWISTED RIGHT KNEE WHILE STEPPING ON PALLET AND ALSO SPRAINED BACK

Employee stated that he was getting out of truck & stepped on a large piece of coal. He fell to the ground.

EE stated that he was taking binder off tailgate when it slammed shut and mashed finger.

EE WAS CLEANING WINDSHIELD ON TRUCK. EE TURNED ANKLE WHILE STEPPING FROM TRUCK TO GROUND

EE WAS OPERATING A D11 DOZER MAKING A BENCH FOR DRILLING ON G2 CUT. EE STATES EE HEARD A "POP" AND NOTED FLAMES ON THE LEFT SIDE OF THE DOZER AROUND THE DOOR. EE IMMEDIATELY UNFASTENED SEAT BELT AND SHUT THE DOZER OFF AND EXITED THE RIGHT DOOR. NO INJURIES OCCURED DUE TO THE FIRE OR EXITING THE DOZER

It started raining at about 8:00 a.m and froze on the ground instantly.

EE WAS DRIVING HAUL TRUCK. IT STARTED RAINING ABOUT 8:00 A.M. AND WHEN IT HIT THE GROUND IT FROZE. EE WAS PULLING A SMALL HILL AND SPUN OUT. EE LOCKED THE TRUCK BRAKES AND THE TRUCK SLID BACK DOWN THE GRADE AND TURNED OVER TO THE RIGHT SIDE. (EE WAS NOT EJECTED.)

Employee was grinding area on drill mast, the ginder kicked back and struck employee in the forehead causing a laceration.

EMPLOYEE WAS CUTTING A PIECE OF RUBBER TUBING WITH RAZOR BLADE KNIFE (UTILITY KNIFE) AND CUT THE END OF HIS LITTLE FINGER ON THE LEFT HAND. THE LACERATION RQUIRED FOUR STITCHES

Employee was loading a tank of oxygen into his service truck and felt a pain in his lower back. The employee continued to work until 12/25/04 at which time he obtained medical attention and started missing days worked, at this time the incident became reportable.

EE WAS HELPING ANOTHER MECHANIC REMOVE PULLEY FROM ALTERNATOR AND THE INPACT SOCKET BEING USED BROKE AND THE IMPACT WRENCH STRUCK THE EMPLOYEE ON THE BACK OF RIGHT HAND, RESULT WAS A LACERATION AND CONTUSION.

EE WAS EXITING WA1200 KOMATSU FRONT END LOARDER FOR A LUNCH BREAK, AS EE WAS WALKING ACROSS DECK EE TWISTED LEFT ANKLE.

Employee stated that he slipped and sprained his ankle while getting out of truck.

EMPLOYEE WAS HELPING INSTALL A NEW METAL CUTTING EDGE ON A GRADER. THE METAL SLID AND FELL STRIKING EMPLOYEE ON THE TOP OF THE LEFT FOOT.

Employee stated that he slipped on oil while checking brakes-sprained ankle.

EE STATED THAT WHILE ATTEMPTING TO SEPARATE A WHEEL FROM A TIRE WITH A SLEDGE HAMMER, THE SLEDGE HAMMER SLIPPED AND EE HIT HIS HAND/FINGER ON THE WHEEL.

Employee stated that he was tightening wheel lugs on truck-wrench slipped, causing him to fall on hand.

EMPLOYEE WAS TIGHTENING BOLT WITH AIR GUN - LATER FELT SOMETHING IN EYE.

EMPLOYEE WAS DISMOUNTING A D11 DOZER & HE WAS STEPPING OFF TRACK TO PUSH ARM STEP, HE SLIPPED SCRAPING HIS RT. ARM & BUMPING HIS RT. KNEE. THE EE CONTINUED TO WORK UNTIL 8/8/04 AT WHICH TIME HE STARTED MISSING WORK DAYS & THE INCIDENT BECAME REPORTABLE.

Emplyee stated that while fixing a tire he hit his right index finger with a hammer.

Employee was operating D-11 dozer and track slipped off large rock causing dozer to slide into a hole, which resulted in a jar to the ee. The result was pain in lower back.

EMPLOYEE WAS HELPING ELECTRICIAN WORK ON LIGHT ON FRONT END LOADER IN HIGHWALL MINER PIT(021). ROCK FELL STRIKING LEDGE BREAKING ROCK APART. A PIECE OF ROCK STRUCK EMPLOYEE ON THE RIGHT HIP.

Ee while cleaning in shop, pulled two buckets apart causing liquid and dirt to go into left eye.

Ee was installing a tire on a wheel rim & caught his finger between tire & wheel rim. This incident resulted in a minor injury to the ee's left middle finger. Ee may have temporary loss of fingernail.

THE CONVEYOR CHAIN ON THE HIGHWALL MINER HAD BROKEN. THE INJURED & A CO-WORKER WERE ATTEMPTING TO REPAIR THE CHAIN & WHEN THE CO-WORKER HIT A CHAIN LINK WITH A SLEDGE HAMMER, A PIECE OF THE CHAIN FLEW OFF BECOMING EMBEDDED IN THE INJURED'S LEFT CHEST JUST BELOW THE COLLAR BONE. INJURED WENT TO A MEDICAL FACILITY TO HAVE METAL REMOVED & RECEIVED 2 STITCHES.

Lumbar sprain occurred to lumbar region of employee. Employee hired as off-road truck driver. Injury was reported as a result of over-exertion from off-road hauling.

EE WAS CUTTING OLD BOLTS OUT OF A HIGHWALL MINER PUSH BEAM WITH A CUTTING TORCH. HE PASSED TORCH OVER THE INSIDE OF HIS LEFT WRIST, BURNING IT. THE EE RECEIVED MEDICAL TREATMENT FOR A BURN ONLY. NO LOST WORKDAYS WERE INVOLVED.

EMPLOYEE WAS OPERATING THE 773 CAT WATER TRUCK WITH THE WINDOW DOWN AND MATERIAL WENT INTO HIS LEFT EYE. THE MATERIAL WAS REMOVED ON 4/16/04 BECAUSE IT WAS IMBEDDED. THIS IS A MEDICAL TREATMENT CASE ONLY. NO LOST WORK DAYS WERE INVOLVED.

WHILE SHOVING WITH A D11R DOZER, A ROCK ROLLED UNDER THE BLADE & THE LEFT TRACK ROLLED ONTO THE ROCK. THE DOZER SLID OFF OF THE ROCK, CAUSING A SUDDEN FORWARD MOTION TO THE EE, RESULTING IN AN INJURY TO HIS LEFT SHOULDER & NECK. THE EE CONTINUED TO WORK WITH NO MEDICAL TREATMENT UNTIL 4/5/04 - AT THIS TIME INCIDENT BECAME REPORTABLE.

EE re-injured his back (original occurrence 3-7-98) while operating rock truck and stated he was jarred by rought road.

LIFTING A FLOATER HOSE (SUCTION LINE) OFF OF THE BACK OF THE WATER TRUCK #633, HE TURNED TO THROW THE FLOATER INTO A WATER HOLE & INJURED HIS BACK.

EE WAS OPERATING 777 CAT ROCK TRUCK TRAVELING FROM LOADER TO DUMP AREA. HE SLID INTO A HOLE JARRING HIS LOWER BACK CAUSING SHARP PAIN.

EE WAS IN THE PROCESS OF CLEANING OUT THE BED OF 1 TON PARIS TRUCK, AND WHILE CLIMBING UP INTO THE BED, HIS FOOT SLIPPED AND HE STRUCK HIS MOUTH ON THE SIDE OF THE BED. THE RESULT WAS LACERATI ON REQUIRING 2 STITCHES AND A TOOTH WAS KNOCKED OUT. EE DID NOT MISS ANY DAYS WORKED.

EE WAS CUTTING A U JOINT FROM A SERVICE TRUCK WITH A TORCH, HOT GREASE SPALSHED OUT UNDER HOOD AND CAUSED MINOR BURNS TO HIS FACE.

EE WAS STEPPING FROM HIS MECHANIC'S TRUCK. HE STEPPED ONTO FROZEN MATERIAL FALLING TO THE GROUND. HE CAUGHT HIMSELF WITH HIS LEFT ARM CAUSING A FRACTURE TO HIS LEFT WRIST.

EE INJURED HIS SHOULDER MOVING A SPOOL OF CABLE ON 1/2/04. HE DID NOT SEEK MEDICAL ATTENTION UNTIL 2/17/04. HE CON'T TO WORK TO THIS DATE. DUE TO CONT'D PAIN HE WAS SEEN BY A ER DR ON 2/17/04 THIS DR WAS NOT SURE OF INJ & REF'D HIM TO ANOTHER DR ON 2/23/04. THIS WAS THE EARLIES APPT AVAILABLE, THE EE WAS EXAMINED & IT WAS DETERMINED NO DAMAGE TO SHOULDER.

EE WAS WELDING ON PIECE OF EQUIPMENT WHEN A GRINDER BEING USED BY ANOTHER WELDER STRUCK THE INJURED EE ON LEFT KNEE CAUSING A LACERATION WHICH REQUIRED STITCHES. EE DID NOT MISS ANY DAYS WORKED.

EMPLOYEE WAS HOLDING A PIECE OF METAL GETTING READY TO WELD IT INTO PLACE WHILE IN THE BUCKET OF A MANLIFT. THE METAL SLIPPED OUT OF HIS HAND & SLIP DOWN HANDRAIL OF BUCKET & STRUCK IN LOWER P ELVIC & GROIN. NO MEDICAL TREATMENT WAS REQUIRED & DID NOT MISS ANY DAYS WORKED. EMPLOYEE HAD ONLY ONE DAY OF RESTRICTED DUTY ONLY.

EMPLOYEE WAS CLEANING BACK WINDOW OF D11 CAT DOZER. HE SLIPPED AND STRAINED HIS ARM DUE TO HAVING HOLD WITH HIS HAND.

EE HAD HAND IN DOOR OF ROCK TRUCK, WHEN TRUCK MOVED FORWARD DOOR CLOSED ON LEFT THUMB. THE RESULT WAS A FRACTURE TO END OF THUMB. EE DID NOT MISS ANY DAYS WORKED AND RETURNED TO REGULAR JOB AT FULL CAPACITY. THIS IS MEDICAL REPORT ONLY.

Mechanic was talking old cutting edges, held on with bolts and nuts, off of d5h cat dozer blade. use an air wrench 3/4 drive to take the nuts off he used his thumb to hold the hand bolt down into a countersunk hole, while air wrench spin. the bolt at high speed the bolt came out of hole pinning.

WE WERE INFORMED THAT A MINOR TEAR TO HIS KNEE EXISTED ON 2/13/03 AND THAT THE EE COULD NOT WORK AND MAY NEED SURGERY. AT THIS TIME THE INCIDENT BECAME REPORTABLE.

EMPLOYEE LAID LADDER AGAINST PUSH ARM OF A D11R STARTED TO CLIMB OFF LADDER SLIPPED OUT AND EMPLOYEE FELL TO GROUND ON RIGHT FOOT.

EE WAS WELDING WHEN HIS WELDING ROD STUCK TO THE METAL. HE BROKE THE ROO FREE AND WAS CHANGING THE WELDING STINGER TO HIS OTHER HAND WHEN THE ROD WENT UNDER HIS SHIELD AND GLASSES INTO HIS LEF T EYE.

EE STARTED MISSING WORK 8-7-02 WITH BACK PAIN, WHICH HE TURNED IN UNDER COMPANY INSURANCE. HE DID NOT REPORT AN INJURY, LATER HE INFORMED US THAT HE AND HIS DOCTOR WERE FILING FOR COMPENSATION ALLEGING A BACK INJURY AT WORK. THE OCCURRENCE IS STILL UNDER INVESTIGATION.

EE WAS OPERATING FRONT END LOADER, THE COVER FOR THE AIR CONDITONING VENT FELL OFF. HE REACHED INTO VENT TO SEE HOW THE COVER WAS SECURED, HIS FINGER CAME INTO CONTACT WITH THE FAN, THE RESULT WAS A LACERATION WHICH REQUIRED THREE STITICHES. THE INJURY REQUIRED MEDICAL ATTENTION ONLY, NO WORK DAYS WERE LOST.

EE WAS CHARGING COOLANT IN #309 785 CAT ROCK TRUCK. AS HE WAS CLIMBING THE ACCESS LADDER HIS FOOT SLIPPED DUE TO OIL ON HIS BOOT & WET STEPS. HE FELL TO THE GROUND ONTO HIS BACK. THE EE CONTIN UED TO WORK AT HIS REGULAR JOB UNTIL 7/2/02. AT THIS TIME THE INCIDENT BECAME REPORTABLE DUE TO DAYS LOST.

THE EE RECEIVED LACERATIONS THAT REQUIRED STITCHES, EE DID NOT MISS ANY DAYS WORKED (MEDICAL ATTENTION ONLY.

MEDICAL EVIDENCE RECEIVED BY WV WORKER'S COMPENSATION INDICATES EE HAS IMPAIRMENT RESULTING FROM CARPAL TUNNEL SYNDROME.

EMPLOYEE WAS INSTALLING A MOUNT FOR A CITIZEN BAND TWO WAY RADIO. HE WAS USING A POCKET KNIFE FOR A SCREW DRIVER. THE BLADE ON THE KNIFE CLOSED AND CAUSED A LACERATION TO THE RIGHT INDEX FINGE R. THREE SUTURES WERE REQUIRED, NO WORK DAYS WERE LOST.

EMPLOYEE HAD COMPLETED FUELING #211 FRONT END LOADER, WHEN HE PULLED NOZZLE FROM WIGGINS SYSTEM ON THE LOADER, THE SYSTEM DID NOT SHUT FLOW OFF AND FUEL WAS SPRAYED INTO EMPLOYEE'S FACE AND EY ES.

EMPLOYEE WAS GRINDING ON PUSHARM OF DII DOZER TO REPLACE STEP. SOMETHING WENT INTO HIS RIGHT EYE. HE WAS WEARING EYE PROTECTION AT THE TIME OF THE OCCURRENCE.

EE HAD COMPLETED SERVICING #116 DOZER AND WAS PUTTING HIS TOOLS INTO THE SERVICE TRUCK. HE TURNED TO ENTER THE TRUCK AND HIS LEFT FOOT CAUGHT IN THE MUD. THE EE TWISTED HIS LEFT KNEE TEARING L IGAMENTS IN HIS KNEE.

EE WAS CLEANING BACK WINDOW OF 988 FRONT END LOADER WHEN HE TRIPPED AND FELL FORWARD OFF THE EQUIPMENT BETWEEN THE HANDRAILS.

EE ALLEDGED THAT HE BACK OVER A ROCK WITH D10 DOZER & WAS JOLTED IN THE MACHINE. HE DOES NOT REMEMBER ANY DETAILS OF THE INCIDENT & NO ONE WITNESSED THE OCCURRANCE. INCIDENT IS STILL UNDE R INVESTIGATION TO DETERMINE CAUSE & COMPLETE NATURE OF INJ

EE STATED THAT HE FELL WHILE DISMOUNTING DOZER. NO WITNESS TO ALLEGED INCIDENT & EE DID NOT REPORT INCIDENT TO MGT PRIOR TO PROPERTY ON HIS SHIFT. EE DID NOT REPORT INCIDENT FOR 4 DAYS(DAYS OF F IN HIS REGULAR WORK SCHEDULE). THE EE STATED THAT HE HAD SEVERAL ITEMS IN HIS HANDS & SLIPPED & FELL IN A SITTING POSITION. THE INCIDENT IS UNDER INVESTIGATION, SUBMITTING DUE TO TIME FRAME

REPORTED CARPAL TUNNEL BOTH ARMS, RESULTING IN SURGERY REPETITIVE MOTION OPERATING EQUIPMENT.

EMPLOYEE WAS ON TOP OF LOADER BOOM HOOKING UP HOIST LINES, OIL WAS ON BOOM, EMPLOYEE SLIPPED AND HAD RIGHT FOOT HUNG, TWISTING RIGHT KNEE.

EMPLOYEE WAS CUTTING BRUSH WITH CHAIN SAW. THE SAW CAME IN CONTACT WITH HIS LEFT LOWER LEG CAUSING A SMALL LACERATION, RESULT WAS 2 SUTURES AND NO MISSED WORK DAYS.

EE WAS HELPING TO INSTALL BLADE ON CO. #120 D11 DOZER. HE WAS GUIDING THE RIGHT HOIST JACK TO THE CONNECTING BLOCK, THE JACK WAS NOT ALIGNED CAUSING CONNECTING BLOCK TO FLIP BACK CATCHING HIS HAND.

EE WAS TRYING TO GET A LADDER OUT OF THE BACK OF THE SERVICE TRUCK, HE COULD NOT GET IT OFF THE RACK BECAUSE THE WELDING MACHINE WAS SLID OVER TOO CLOSE, HE WAS USING A PIECE OF PIPE TO PRY IT OVER. LOW BACK STRESS.
