Was loading rock out of #7 face. The shuttle car was making the turn out of the entry. EE was caught between the shuttle car and the rib.
EE was taking belt structure apart. As EE was attempting to pull a belt rail loose from the spreader bar by moving it up and down, the chain came out of the eye of the spreader bar allowing the belt rail to fall down striking the top of the right foot.
Putting scoop on charge raising battery lids and lid fell back down hitting EE's right hand and thumb, causing a closed fracture to very tip of the thumb.
Employee was preparing for a belt move, when the belt slipped off the pile employee went to straighten up the belt with channelocks and they slipped off the belt causing employee to lose balance, falling to the ground twisting right ankle and resulting in a fractured right ankle.
This mine experienced a roof fall in the intersection of #5 entry at crosscut #10 on 8B10 belt line.
This mine experienced a roof fall in the secondary escapeway at Break 24 on 8B1 track entry.
The employee was operating a 4-wheeler (personnel carrier) when it stopped tramming. The employee went to the back of the machine and was pecking on the outside of the control box with a sledge hammer. The 4-wheeler jumped into tram backwards towards the employee, pushing EE approximately 10' before the machine stopped.
Employee was making a cable splice on the left continuous miner when a piece of rock measuring 65" Long x 8" thick x 8" thick and a piece 28" wide x 43" long x 2" thick became dislodged from the roof and rib striking employee in the upper back and neck.
This office was notified on 06/21/19 that EE was diagnosed with Occupational Pneumoconiosis on 4/18/19

PLACED HAND ON THE EDGE OF TOOL BOX TO PUT UP TOOLS AND THE LID CAME DOWN SMASHING LEFT THUMB.
The mine was notified on 1-25-19 that EE has workers occupational pneumoconiosis, EE had a chest x ray on 11-15-18 that showed positive for the disease.
On 1/10/19 at 6:00 pm a shuttle car operator started experiencing pain in left side back/kidney area. The pain started as EE was descending a grade in the mine floor. EE finished shift & completed a report. On the way home the pain increased to the point EE called spouse to come meet EE. Spouse took EE to hospital where 1st diag. was pneumonia, 2nd was contusion.
Employee was diagnosed with pneumoconiosis by a board certified doctor in internal medicine 10/15/18. I received the e-mail on the 10/23/18. The employee quit work November 2015.
An unplanned roof fall occurred on the #7 belt track entry in the #37 intersection 40 back toward the #36 intersection in the #2 entry. The fall was danger and timber off. The track and life line was run to the #1 entry. The fall will be cleaned up to the rehabilitation cleanup plan.
In a dip on old #6 Belt Track, the employee was a passenger in the first of two man buses. As the first man bus went down the dip and was topping over, it flashed its lights for the second bus to proceed, when the first bus stalled out and rolled back down causing the second bus to hit it. This employee began missing work on 06/04/2019.
Installing J-Seal when worker caught glove in the auger which cut off tip of index finger up to the first joint on the left hand.
An unplanned roof fall occurred on the 8B Belt at X-cut #2 inby spad #7924. The fall was about 70 foot long, 19 foot wide and 8 foot high. The fall will be clean up and rehabilitation to the cleanup plan.
Employee was diagnosed with pneumoconiosis by a board certified doctor in internal medicine on 3/10/2018. Received the e-mail on 5/2/2018. The employee is still working.
While making a belt splice on 6B2 beltline, EE was pulling a piece belt from under the beltline with a pair of channel locks, when EE felt a sharp pain in lower back on the left side.
An unplanned roof fall occurred in the belt and track entry on the #2 section 3 X-cuts out by section F8. The fall will not be cleaned up. It will be crib and dangered off and move the section.
An unplanned roof fall occurred on the #7 belt at x-cut 58 at spad #13701. The fall was approximately 60' long, 18' wide, and 10' high. The fall will be cleaned up and rehabilitation to the clean-up plan.

Employee was tightening down a load of water line with a chain ratchet, and the pin that connects the hook of the chain ratchet to the chain broke allowing the chain to snap back, striking the employee in mouth and chin, causing a laceration that required sutures to close.
Employee was changing out supply cars at the rock dust switch, and put one in the straight and two in the switch and brought one back out. As the employee was attaching the safety chain, the car in the straight rolled back catching the employee's foot between the supply cars. The incident resulted in a laceration to left ankle requiring sutures to repair.
A non-injury roof fall approximately 19 feet wide, 70 feet long, and between 7 - 8 feet in height occurred between breaks 13 & 14 on the 8B4 belt line.
Employee observed a large rock going through the feeder that was going to contact the "dead-man" cable going across the feeder. EE attempted to hold the lever that is pulled into the e-stop to avoid the feeder from stopping, but the rock hit the cable & pulled the lever mashing EE's finger between the lever & the e-stop causing it to fracture the end of EE's little "pinky" finger.
Employee was carrying rock dust to dust belt line after a belt move, and as they were walking beside the 6" waterline that was being pulled into place to be installed, a fire valve caught the lower-back part of their left leg causing them to fall.
This employee was diagnosed with Occupational pneumoconiosis on 2/1/17 we received the notification on 4/28/2017.
Employee was loading a Roof Bolter and slipped and fell while carrying a handful of roof bolts, landed on knee causing a fracture to left knee cap.
Employee was removing rock from the miner when a piece approximately 3' x 4' x 1-8" thick slid off the miner hitting left foot resulting in a simple fracture.
On Jan. 12 2017 this office was notified that this employee has been diagnosed with Occupational Pneumoconiosis.

We were notified on 9/9/16 this employee was diagnosed with pneumoconiosis, employee is still working.
On 08/23/2016 this mine was notified that this employee has been diagnosed with occupational pneumoconiosis.
This employee was laid off during a RIF at the mine on 11/01/2015. This mine received on 8/12/2016 that this employee was diagnosed with occupational Pneumoconiosis.
Employee was pulling on a chain ratchet and felt burning in neck and right shoulder.
The employee has been diagnosed with Occupational Pneumoconiosis on 01-14-16 the company was informed on 05-27-2016 the employee has left work for an on the job injury on 07-09-2015 and hasn't returned as of this date. The employee left work on 7/9/15 on an injury not associated with EE's employment.
The injured employee was installing a rib bolt when a piece of draw rock fell from the top hitting EE on upper back and pushed EE to the ground. The employee was taken to the Logan Med Center ER and given a full release to return to work, EE went to Dr on 05-31-2016 and was taken off work for a possible pinched nerve, the employee started missing work on 06-01-2016.
Employee was helping load a piece of belt and fell landing on SCSR. EE went to the doctor on 05-03-16 and was given a full release to return to work but was still having pain and went back to the doctor on 06-13-2016 is off work due to lumbar strain.
Employee was diagnosed with pneumoconiosis by a board certified doctor in internal medicine on 2/2/2016. I received the e-mail on 4/19/2016. The employee is still working.
Employee was diagnosed with pneumoconiosis by a board certified doctor in internal medicine on 04-04-2016, I received the email on 06-29-2016 the employee is still working.
Employee was carrying a bag of dust over a belt crossover as EE was going down the steps EE felt strain on EE's left shoulder. The employee worked up to 02/27/2016 before starting to miss work.
Employee was diagnosed with Pneumoconiosis by a board certified doctor in internal medicine on 11/25/15. I received the e-mail 2/9/16. Employee is still working.
On 01-25-16 we were notified this employee has occupational pneumoconiosis but still continues to work.
Employee was lifting a scoop door when EE felt pain in EE's lower back, right shoulder and elbow.
Employee pick up a draw head to put on the gravel car and feel pain in the left shoulder. This was on 12/17/2015. He continue to work and went to the doctor on 12/28/2015 and he still continue to work until 04/01/2016 when he started to miss work to have surgery on his shoulder.
Employee was loading 8 ft. timbers on a 4 wheeler and felt pain in his back.
employee was making a right turn on a shuttle car and turned too soon and the tire hit the corner of the rib causing him to bounce up and hit his head on the canopy of the shuttle car
While exiting an underground forklift he slipped and fell on his right leg fracturing a bone

On July 9, 2015 this employee was terminated. On 08-18-2015 this mine was notified that this employee was diagnosed with an Occupational Illness of Pneumoconiosis.
Employee was diagnosed with occupational pneumoconiosis on 3/30/2015 we was notified on 7-29-15 of this diagnosis.
As employee was loading belt structure into scoop he felt a sharp pain in his upper right arm. Employee began missing work 4/28/2015. (Bicep tear)
Employee was diagnosed with simple pneumoconiosis on 3/2/15. We were notified on 5/15/15.
We was notified on 12/2/2014 that he has hearing loss, he was tested on 10-13-14 and determined this was a reportable illness.

We was notified on 12-2-14 that he had been diagnosed with occupational pneumoconiosis.
We were notified on 9/9/14 that he has been diagnosed with coal workers pneumoconiosis.
Employee was standing beside the electrician at the feeder, when the number 1 shuttle car pulled onto the dump end of the feeder catching his right leg, causing 5 fractures to his foot and ankle, he was treated onsite and taken to the hospital then transferred to CAMC were surgery was performed on his foot and ankle. Msha has conducted an investigation into this incident.
Two employees was picking up a drawhead to install on a flatcar they were moving, as the other employee picked up his side of the drawhead it allowed it to roll over onto the injured employees right hand, causing a fracture. He was wearing a mechanic's glove.
Employee was tearing down flypads in the #7 entry 50'outby spad number 10757 to move to a new location as he was pulling on the flypads, the flyboard broke allowing a loose piece of drawrock that was above the flyboard to fall onto the ground hitting him on the left ankle resulting in a chipped bone.
Employee was working on the 001 MMU in the # 8 Heading, he finished installing his inside bolt and the right rib rolled out, a portion of the loose rib struck his right calf muscle. He refused an ambulance and was taken to the Logan ER and diagnosed with a hairline fracture to his fibula.
We was notified on 1/12/15 that he was diagnosed with simple pneumoconiosis on 6/24/14. He hasn't worked at this mine since 11/21/2014 because of a reduction in the workforce due to weak market conditions.
While leaving the bathhouse ee told me he hurt his knee, I asked him if it was ok, and if he wanted to fill out a accident report and he said no that he was ok , they wasn't never a clear event or incident to how this employee was injured if he was injured at this mine. Note this was his second day at the mine, his first day was finishing his training.
Placing 5 roof bolts on top of #1 roof bolter when he felt sharp pain in lower back.
No Event occurred. Employee was roof bolting on his knees when he straightened his leg out it began hurting. He was seen at WMH where ER. who referred him to orthopedic for evaluation and release. This 7000-1 is being submitted for the 10 day notification only. It is undetermined if this is reportable incident. An appointment with an orthopedic has not been made.
Two employees were packing metal and when they sat it down one got his finger caught between the metal and a crib block causing a laceration that required 14 stitches.
On February 04, 2014 this mine was notified that this employee has been diagnosed on 01/17/2014 with pneumoconiosis. This employee was terminated on 10/29/2013.
Employee was operating continuous miner in #5 entry when right rib rolled out striking him in the back and pushing him over into shuttle car rolling his right ankle and injuring his lower back.
Employee stated on 11-11-13 he injured his back lifting cinder block. He finished his shift and went to Logan ER and received a full release. He RTW on 11-12-13. On 11-16-13 he went to CAMC and was dx. with Sciatica and He requested time off work. On 12-11-13, we were advised by OIEI that this incident was reportable.
A roof fall was found between the #3 to #4 crosscut at the #6 Belt head near survey station 2744. The fall measured approximately 30' in length, 17' wide and 7' high. No equipment was involved and no injuries reported.
Employee alledges he injuired his back while loading timbers in a scoop bucket on 7-13-13. The employee was taken to the hospital and released to return back to work on 7-15-13. He sought medical attention from his family doctor who took him off from work, based on his initial release from the ER Room allowing him to return to work we feel he's able to perform his normal job.
Employee worked at Ruby Energy for approx 1 month he went out on short term Disability and hasn't worked at this location since that time.
Employee took a hearing test on 5-15-13 and he has declined a retake of his test so therefore we are reporting his decline in his hearing.
We were notified by msha about the part 90 miner and were unaware that we needed to complete a 7001- form on him until now. I apologize for it being late.
Employee was going to change batteries on the scoop. As he was getting out of the scoop, he twisted his left knee.
EE was spraying gob from rock box when he slipped and fell causing him to strike the left side of his head on the back of the tailpiece causing a laceration.
Employee was tramming his roof bolter out of the #3 entry when he accidentally slewed himself into the boom of the C/M that was located in 3L.
Employee was picking up trash and other materials around newly set tailpiece when he twisted his right knee and fell to the ground.
Employee was loosening a bolt on a feeder when his wrench slipped causing him to stumble and striking his left ear against the track of a feeder.
Employee was getting out of scoop and had the door open. When he put his hand down to exit the scoop, the door shut catching his left hand. Scoop was parked on a incline.
Employee was installing an outside bolt in #6 right. After he finished, he went to swing his pot in to install the inside bolt when his right knee gave out. The height in 6 right is 53". He had a previous injury to this knee which required surgery. There was no event or condition that caused this incident. We are requesting this as an occupational illness.
Employee was taking a water box to the #2 section when a piece of draw rock fell striking him on the back of the head causing a laceration.
Employee was installing an outside bolt. As he went to get a pizza pan to put on the bolt, he slipped on a rock and twisted his right knee.

The employee was helping pull rail out of a cross cut into the track entry, the crew was using a manbus to pull the rail, the pull rope that was being used to pull the rail broke causing the rail to swing and hit the employee in both legs.
An unplanned roof fall occurred on the 6C Belt at break #20. Fall was approximately 80' long by 20' wide by 8' high.
Employee was hanging C/M cable when a piece of draw rock measuring 12"X10"X1.5" thick fell, striking the top of foot just behind the metatarsal guard of the boot. Height in area was approx. 6.5'
Employee was lifting 2+20lbs buckets of bits. When he turned to stand upright he heard a popping sound in his lower back. Going to his knees he could not stand up or straighten his back after that.
Employee was welding on a shuttle car when a piece of hot slag burned through ear plug and fell into ear canal causing damage to ear drum. Employee continued to work full duties up until 1/31/13 when he was scheduled off for surgery.
Employee was removing a rub rail from the left continuous miner at the demister cover so he could access the demister. When he pulled the pin from the rub rail the rail fell to the ground and smashed his right middle finger.

An unplanned fire occurred at the #2 belt head inside the contained transformer. The flame of the fire did not escape the inside of the transformer.

EE was spot bolting along 6 belt break 31 when a piece of rock 7" X 9" X 1.5" thick fell striking him on the left shoulder and cheek. This injury resulted in a laceration requiring stitches.
EE was picking up empty oil cans and putting them on a flat car. One of the empty oil cans has a jagged sharp edge on it. He cut his left middle finger requiring stitches.
An unintentional roof fall was discovered by the mine examiner on 8/9/2011. Fall in located one break in by spad #1425. Fall measured 30' long by 18' wide by 6' high. Fall will not be rehabilitated.
An unintentional roof fall was discovered by the mine examiner. Fall measured 18' Wide by 6' High by 50' Long. Fall is located between #6 & #7 approx. 400' from the working section across from survey spad #6414. This area will not be rehabilitated. No injuries. No equipment involved.

EE was installing shuttle car cable anchor when a piece of cap flew off and struck him in the face, leaving a laceration to his upper lip.
An unplanned roof fall was discovered by the mine examiner on his weekly air way examination. Fall is a contuniation of a fall at this site at spad #1461 in #6 entry return. This fall will not be rehabilitated. Fall measured 18' L X 18' W X 6' H.
Continuation of a previously reported roof fall at spad #3067 was discovered. Fall measured 25' L X 18' W X 6' H. This area will not be rehabilitated.
An unplanned roof fall was discovered by the mine examiner. Fall is 1 x-cut outby spad No. 5646 approx. 4000 ft outby the faces. Fall measured 30'L x 20'W x 6'H. This area will not be rehabilitated.
An unplanned roof fall was discovered by the mine examiner at spad #4512 1st left panel AZI 119 degrees 34' 29". Fall does not affect passage of persons. 25" L. x 18' W. X 6' H. Fall will not be rehab'd.
EE was getting a box of glue off the back of the roof bolter to supply his pin tray. He struck his right elbow on a sharp edge of a pizza pan, resulting in a laceration requiring stitches. Note: The Safety Director was not notified that EE had received stitches until 2/16/2011.
An unplanned roof fall was discovered by the mine examiner and MSHA inspector. Fall located on 1st left panel off 1st north west submanins in #3 entry at spad #4504. Fall measured approximately 25'L by 18'W by 3'. Area will not be cleaned up.
An unplanned roof fall was discovered by the weekly examiner 2100 ft outby the #2 section right return #7 return near spad 5753. Fall measured 60' long by 19' wide by 12' high. This area will not be cleaned up.
An unintentional roof fall was discovered by the mine examiner in panel off 2nd NW sub mains. 2 x-cuts inby spad #5184. The fall measured 40' long X 19' wide X 7' high. Area will not be rehab.
An unintentional roof fall was discovered by the mine examiner in panel off 2nd NW sub mains. Fall meas. 100' L. x 19' W. X 7' H. No equipment or miners were involved. Area will not be rehab'd. 1 X-C inby spad #5190.
Employee was hauling supplies to the #2 section at #5 head went into a dip. The AL Lee locomotive was parked in the dip. EE dropped into this dip and struck a parked track Locomotive. At this time we have no diagnosis of injury. First day away from work 10/6/2010.
We were notified by MSHA about the part 90 miner and were unaware that we needed to complete a 7000-1 form on him until now. I apologize for it being late.
On 8/19/2010 was notified that Dr. has taken EE off work due to a strained left knee. EE was supplying roof bolter when a rock fell between the roof bolts raking him down his left side, pushing against rib, injuring his left knee. Rock was approximately 5' long by 6" thick.
An unplaned roof fall was discovered by the mine examiner on 07/27/2010. The fallwas in the out by intake at spad 1615 in #5 entry. The fall measured approximately 30' long by 19' wide by 8' high.
An unplanned roof fall was discovered during the weekly exam of air ways. No miners or mining equipment involved. Fall located inby spad 3317. Fall measured 90' long x 19' wide x 9' high.
A roof fall above the anchorage of the bolts was discovered by the mine examiner. The fall measured 70' long X 19' wide X 6' high. at spad # 2865 in the #1 entry.
EE was taking down a miner cable that had been hung. The cable was under tension, the metal hook on the cable hanger caught his left ring finger causing a laceration and a fracture to his finger tip.
An unintentional roof fall was discovered by MSHA inspector and mine examiner. This fall is one break outyby survey spad # 1584. The fall measured approximately 20' wide X 35' long X 10 to 12' high. No injuries or escapeways blocked.
Setting up the continuous miner to rehab this area when the rock fall occurred. It was 20' long x 16'wide x 6' high. This area was being rehabed for this mine. This area will be cleaned up and rebolted. This fell in the secondary escapeway. The lifeline was rerouted and the men trained on the change of the escapeway. The fall was at Break 38 on the #6 belt in the #3 entry.
Injured was using a come along when his hand slipped causing his hand to hit his safety glasses causing a laceration above his left eye.
EE was removing bolts out of belt structure. He stepped off the takeup frame & his feet slipped causing him to injure his left knee. Was treated & given a full release to return w/no restriction or directions to see specialist. He hasn't returned to work. We did an investigation & concluded based on his release he would be able to perform his normal duties per Part50 Item 30
Roof fall was found, fall is 100' long x 18' wide x 7' high. Fall is dangered off and stopping lines re-routed around fall area. MSHA investigated the fall area. The roof is laminated it occurred at break 10 on #6 belt on the #1 entry.
Injured dropped a roller and mashed right little finger.
UNPLANNED ROOF FALL OCCURED ON THE #5A BELT MEASURING 80 FEET IN LENGTH x 20 FEET WIDE x 6-8 FEET HIGH AT X-CUT #31. THE FALL WAS REPORTED TO THE HOT LINE AND ALL ENTRANCES WERE DANGERED OFF. A ROOF FALL CLEAN UP PLAN HAS BEEN SUBMITTED AND APPROVED.
An unplanned roof fall occurred below anchorage but had roadway blocked in primary escapeway. Men were withdrawn & escapeway rerouted. This was phoned in to hotline. The area was located in 1 crosscut inby SS 1536 adjacent #4 belt. The area has been rehabilitated. A cleanup rehabilitation plan was submitted.
An unplanned roof fall occurred in the #4 entry at break #40 on the no.4 belt flight. The fall was 1 x-cut inby s.s.# 1536 the size was 20' wide X 40' long and app. 8' high. The area was dangered off and a plan submitted to rehabilitate the area. The fall was called in at 12:16 p.m.
Injured was struck in right hand with a piece of drill steel causing a laceration requiring 3 stitches.
Injured was in the process of swinging his boom out whan a piece of rock fell striking him on the arm causing a laceration requiring stitches.
Injured was walking and a piece of rock fell from the roof, striking him on the left side of the head, causing a laceration requiring stitches.
EE was squatting down & felt pain in his knees as he was going down & stated as he was getting back up felt a pop in his rt knee. He ceased work & reported to ER. No diagnoses was determined, ee has history of knee surgery & knee injuries. Other documents will follow as it becomes available. EE has been referred to the Doc that done previous surgery.
An unplanned roof fall was discovered on a weekly exam in the #7 entry on the 5 a belt return entry at 5.5 #3963. The size of the fall was 210'long x 8' high x 20' wide. The area was dangered off & does not affect travel of ventilation. EE of MSHA & fire boss ee discovered the fall. This area will not be rehabilitated.
Injured was stacking a stopping & dropped a block on his left hand causing a fracture to the ring finger.
Injured slipped on top of roof bolter while cleaning it. He went to ER and was released to return to work on 8/17/09. On 8/18/09 he returned to orthopedic dr. and was taken off work until 8/24/09. No diag. was made therefore the co. had not made a determination on reporting until 08/26/09. The mine was idle until 08/24/09.
Unplanned roof fall occurred in #6 entry a S.S. #4099 app. 1050' from the #2 working section. This was breakered off & will not be rehabilitated the size of the fall was 160' long x 20' wide x 6' thick. Phoned in at 7:22 am
Employee was organizing storage trailers, moving spool of steel cable, lifted spool up to roll, strained muscle in groin area.
Injured was prying on a track switch with a bar. The bar slipped causing his to slip. As he tried to catch himself with his right leg he stated he felt something pop 3 times in his right foot.
Fall on 5A belt intersection and 2 ft Inby - Spad # 3988. Fall is 1 x cut inby this spad. Fall was approx. 18 ft wide x 50 ft long x 8 ft high. No rider was present. Slate top and laminated.
Fall in the # 5 belt entry at spad no. 4101 fell in the intersection on the # 2 section. It was approx. 8' to 10' high. It was cleaned up and rebolted in the area. No persons or equipment was affected.
While setting an I-beam on another I-beam, injured mashed his fingers causing a lcaeration requiring stitches.
Injured was replacing a filter housing. As he tightened the case he stated he felt a tear in his right shoulder and was hurting all the way to his finger tips. Note: He worked until 5 am & then went to Dr. He was given a release to return to work by an orthopedic specialist & scheduled for an MRI that was negative. EE worked until 4/28/09. He was taken off work after another scope was done.

Member was attempting to put a load of rail in a switch when the coupler pin came out and the car broke the safety chain. The chain caught a piece of water line causing it to spring and hit him in the head. He received 2 stiches.
