The employee was operating a CAT D11R Dozer. EE stated EE ran over a rock, jarring EE's body. There were no witnesses to the incident. EE's first day away from work was 9/17/24. NOTE: The operator concluded the incident investigation on 10/9/2024.

Stated that EE was cutting down a tree while doing clear cutting work. At the time EE did not notice that a grapevine attached to the tree was underneath the leaves & was under EE's left foot. When the tree fell, the grapevine went with it, tangling EE's foot in it & drug EE down the hill 10'-15'. It chipped EE's left ankle. 2 Screws and a plate were put in EE's foot, & 6 staples.

The operator has been notified the employee has occupational pneumonoconiosis.

EE slipped off the bottom step on the customers lube truck and fell to the ground hitting EE's Back and Wrist. The jobsite had about 12 inch's of snow and EE said EE was wearing all EE's PPE. (Steel toe Boots, Glasses, Reflective Cloths and Gloves)

EMPLOYEE WAS PERFORMING AN ELECTRICAL INSPECTION ON A SERVICE TRUCK. EE HAD TO REMOVE THE MIG BOX FROM THE SERVICE TRUCK IN ORDER TO CHECK THE GROUND. EE WAS ON EE'S KNEES PUTTING THE MIG BOX BACK INTO PLACE WHEN EE FELT PAIN IN EE'S RIGHT ELBOW & BICEP. EMPLOYEE'S PHYSICIAN HAS NOW PERSCRIBED PHYSICAL THERAPY. THIS IS THE REASON WE ARE NOW REPORTING THE ACCIDENT.

EMPLOYEE WAS HELPING UNLOAD SUPPLIES FROM THE BACK OF A PICKUP TRUCK. THERE WAS A PALLET WITH ONLY ONE ROW OF WATER ON IT THAT HAD TURNED SO THE FORKS OF THE FORKLIFT WOULD NOT LINE UP FOR IT. EMPLOYEE ATTEMPTED TO SLIDE THE PALLET AROUND WITH RIGHT ARM AND FELT A PAIN IN RIGHT BICEP.

Employee was cutting a tree on a bench / road when a standing dead tree became dislodged from above where EE was working. Tree fell striking employee in the head.

Was cutting clutch brake out of service truck bell housing. The torch wore out while cutting. Removed torch out of bell housing inspection hole and a flash back of fire shot out of the inspection hole. Right side arm & chest had 2nd degree burns.

While removing side panel on 988B loader the top door pulled out of hinge fast then the employee jumped to ground instead of falling. Employee landed on EE's feet but was jarred from the impact

Driver did walk around on truck after leaving scales & step was slick getting in truck & employee slipped & fell on tailbone. ER dr. says bruised tailbone & go back to work in 2 days.

Employee was changing a wheel bearing on a service truck. With a pry bar EE was removing the wheel bearing. When the bearing came loose EE fell to the ground. EE caught self with left hand and suffered a laceration and a puncture wound to the left thumb requiring two sutures.

Employee was helping crib the rear of ST768 lube truck so the rear springs could be replaced. The floor jack that was located on the right rear of the truck slipped out allowing the truck to set back down on the floor. The tires were still on the truck but the crib block EE was inserting under the axle kicked out and mashed EE's finger between the crib block and the truck frame.

Dozer operator was asked to come to a different location. When ee arrived at new location ee trammed over a rock and the dozer slammed to the ground causing a whiplash type injury.

Employee was dismounting a front end loader & while climbing down the last set of steps to the ground, the second rung down broke causing the employee to fall to the ground. Employee's left leg went inside of the next rung down causing a small abrasion to the left calf. Employee landed on right arm causing sharp pain in the right shoulder & a bruise to right arm.

Employee was using a hawk-bill type knife to remove the outer layer covering electrical wire. The knife slipped and lacerated the left thumb.

Security guard was not feeling well, called to be relieved. Another party found them unresponsive and slumped in a chair at the guard shack. Taken to hospital by EMT's. ***Several full statements attached to original file***

Employee finished changing the brake calipers on the hose reel of the highwall miner. EE was retrieving a fitting that was laying on the hose reel frame when EE slipped and fell. EE tried to catch self with left hand and EE's fingers went through the metal grating injuring the hand and fingers. EE also struck EE's head as EE fell resulting in a small cut above the right eye.

While dumping a load of coal on the stock pile, the employee felt the pile set down below EE's trucks rear wheels. EE dropped EE's bed with the full load of coal still in the bed. The truck then proceeded to set down and raise up on it's tail shoot. This caused the EE to be thrown sideways in the cab with a whiplash effect. The employee did not strike EE's head and was wearing

Employee was cleaning a small ditch out with a long handle shovel. The ditch had already been dug with a small back hoe. EE placed hand between the end of the shovel handle and EE's leg to gain leverage. When EE applied leverage with EE's leg, EE felt pain in the right hand in the area of the pinky finger.

It was raining and the employee had an umbrella in hand. Employee was walking around the corner of the security post in order to enter the building. Employee caught the toe of employee's boot and fell forward landing on left arm on the pavement. Employee fractured the radius bone in left arm. Employee's first day away from work was 2/20/2018.

While stepping down from the mounting ladder on a drill to the ground, the employee started to turn and walk away from the drill. EE's right foot stayed stationary and the rest of body turned in the direction EE was turning causing pain to right knee and right hip. Employee did not fall. Employee's first day away from work was 1/24/18.

Employee has been diagnosed with occupational hearing loss. This is a code 26 occupational illness. *Notified on 1/10/2018.

Employee fell on a log as employee was walking around a hill.

Installing roller on a D-10R dozer, using sledge hammer to knock roller loose, a small sliver of metal clipped off roller, penetrating employee's upper right thigh causing laceration. Note: employee returned back the next scheduled day of work 6-6-2017.

Dozer operator was pushing shot material. Fog conditions moved in and restricted the operators visibility but EE continued to push material. The operator became lost and drove over the high wall.

Employee has been diagnosed with complicated pneumoconiosis. Employee is not currently employed at this mine

The operator was notified on 8/29/2016 that the employee (Last Exposure Date Worked Feb 2014) has been diagnosed with Occupational Pneumoconiosis

The employee was prying on a guard to retrieve a dropped tool. As EE did, the pry bar slipped allowing the employee to be struck in the mouth by EE's own hand breaking a tooth. On Friday June 3, 2016 it was decide to repair the tooth and repair began. The employee informed the company of the repair work being started on Monday June 6, 2016.

Employee was using a pry bar to turn a final drive on a D-11. Employee had one knee resting on a roller. As EE re-positioned the bar, the roller moved getting the employee off balance resulting in the employee falling to the ground. At the time of the incident, the employee felt fine. EE completed the shift and worked the next scheduled shift.

Employee plus one was thawing a water line. One was using a weed burner to thaw ice while the other was twisting and bending the line to help break the ice. Air was also being used to push the ice. A large amount of ice broke loose at once causing the line to move abruptly. The coupling of the line struck the employee on the knee cap.

A brush fire of unknown origin made its way to the edge of our permit area. The fire was then reported.

The employee had his left hand resting on an eyelet on the launch. The trolly operator lowered the trolly to pick up the next beam. The trolly creates down pressure the pick up process. This down pressure forces the eyelet to raise and as this took place the finger was smashed between the eyelet and the mono-rail.

Smoke was seen in the distance. It was not clear if the smoke was from an incident on property. The smoke was from a brush fire. The fire was moving close to the outer boundary of the property. The fire was found to be 2000' from disturbed ground. No miner or equipment was in the area. The fire is believed to have started off property but moved to outer limits of the permit.

Employee was tasked to fuel the PC 3000 before the start of shift. The PC 3000 had trammed from the working pad at the end of the shift on the previous day. The tramming had created windrows of material. The employee tripped and fell over the material while dragging a hose.

On this date we were notified of an employee receiving an award of 10% for Occupational Pneumoconiosis.

Employee was greasing a loader when a grease fitting had come loose. With grease hose in hand, ee was coming from under the loader to get tools to replace the fitting. The employee accidentally discharged the grease. The grease struck the employee and embedded under the shin.

Approximately 1:05 pm smoke was noticed in an area not yet mined. Upon investigation it was found to be a brush fire of unknown origin. 911 was called so fire department persons could respond to extinguish the fire. The WV Department of Forestry also responded. The incident remains under investigation. The MSHA Hot-line was used.

Employee was using a tag line to steady a load when the load started to shift . He then released the tag line and grabbed the load with his hands and caught his elbow between the crane and load breaking his right radius bone next to his elbow.

Employee was walking between dozer and bus. Employee slipped on what is believed to be a small rock. The slip resulted in a fall. The employee tried to minimize the fall by placing the left hand out. The left arm received a fracture just above the wrist.

He was jacking front of truck up when jack kicked sideways and came to the ground with the spring hanger hitting his left shoulder.

Dozer track came in contact with a large rock near the surface causing dozer to slide sideways resulting in a roll over.

He was exiting the cab of truck to do his walk aground when he lost he's footing and could hold on and fell to the ground.

Lifting & lowering 100 & 150 ton hydraulic jacks inside the car body of a 495 shovel. The space is very tight, working in awkward positions. The jacks weigh somewhere close to 100lbs or more.

EE was dismounting equipment. 1st step to ground, stepped on a rock causing him to loose balance. Trying to regain balance he slipped in gravel and fell to ground. Placed his right arm out to break the fall and injured the right wrist. Saw a doc on 7-19 and x-ray was positive. Brace was applied. Cast was applied on 7-23. No time has yet been lost.

EE was setting 3 inch pump on bottom step of steam truck and had to reposition himself to move pump up to the bumper of truck. He then picked up the pump and felt pain in his lower back.

Employee was dismounting equipment. When he stepped from the last step on the equipment to the ground, he stepped on a rock causing him to loose balance and falling to the ground. He tried to brake the fall by extending the right arm. The fall created damage in the shoulder area and it determined this month that outpatient surgery was needed. No lost time til surgery on 11/8/13.

Eq. operator was traveling on a dozer at start of shift. While traveling the operator adjusted the seat. After adjusting the seat he placed his right hand behind the seat to ensure clearance between seat and power inverter that was behind the seat. While his hand was behind the seat the dozer jarred enough to pinch his hand between the seat and the mounted inverter.

The date and time of incident represents data and time I notified that the EE has been diagnosed with Coal Workers Pneumoconiosis. The EE has missed no work to date.

Right in front of the building on the drivers side of a truck, fell and caught himself with his left hand. Ice in the parking lot. When he fell and caught himself with his hand, it bent his hand backward.

EE was refueling grader, when he was climbing down his foot slipped off step and he was tangled in hose causing him to fall backwards onto ground landing on his right hip, then to right shoulder.

He was overloaded and had to drop off while pulling on tailgate his right hand got caught between the latch and pin.

During the installation of bucket link on a Cat 992D loader a bottle jack was being used to assist with alignment of the bucket link and the connection point. The jack slipped and struck the employee just above the knee. The impact created a laceration requiring sutures to close the wound.

EE was standing at the controls of a Chieftan 1800 power-screen with plans of adjusting the speed of the belt. The EE opened the control panel door & was using the left hand to turn the needed switch. The right hand was resting on top of the control panel door. The control panel is located below the belt. A rock left the belt and struck the right hand. suture. X-rays were negative.

He was dumping off from being overloaded and had a piece of coal stuck in between bed and tailgate. While cleaning it out, the tailgate slipped out of his left hand smashing his right hand the first three fingers.

2 welders were side by side Arc Welding cracks on Ex 2500 Bucket. Eyes became irritated overnight.

Installing transmission on a 773B, chain popped and allowed transmission to shift pinning left foot between frame and transmission.

The driver smelled something burning pulled over and stop to check it out saw flames went to grab fire ex. but didn't have time jumped out away from truck.

EE was working freshly poured concrete with a tamping bar. Concrete was poured to hold a post for a sign. As EE was tamping the concrete he managed to strike himself in the mouth with the pole breaking a tooth. Thurs 9/8/11 the EE saw a dentist. The dentist repaired the tooth thus making this incident reportable. The EE had no lost time. 9/9 & 9/10 are normal days off.

2 mechanics were going to change transmission & transfer case on Chev 2500 pickup. The truck tires were placed on crib blocks for more clearance & blocked. Park brake was set according to mechanics. Rear portion of drive shaft was disconnected with no issure. When front portion was disconnected, truck began to roll. Mechanic under the truck tried to roll out & was struck on hip.

The employee placed a pipe wrench on the stem of a ball valve. The intent was to allow water to flow to a hose. As he turn the stem using the pipe wrench, the stem of the ball valve broke allowing the top of his hand to strike the next ball valve to the left. This striking of the top of the hand created a laceration on the top of the hand that needed 4 sutures to close.

EE was cleaning tailgate and misjudged where his left hand was and it was due to being overloaded. It was his left index finger.

A mechanic was searching for the cause of a leak on a loader, # RI 7400. The EE went to the top of the engine compartment to pull off a cover inorder to see the area for the engine. The EE removed the bolts holding the cover in place and grabbed the handles provided to remove the cover. While pulling the cover a knot appeared in his groin. The surgeon will see him on 11/19.

EE was placing caps and boosters on the bench stepped into a jack pad hole, left by a drill, landing awkwardly twisting his right knee

He was doing his walk around at break check when he noticed a broken spring and went to double check it. When he was crawling back out his hard hat fell off and he caught a piece of metal which cut his head open.

The member was raising the hood of a 6370 Hatachi Excavator to check the fluids as part of the pre-shift inspection. While attempting to raise the hood, the member lost control of the hood and it immediately started back down. The unexpected change in direction placed strain on his lower back.

Installing battery cable on D10R, standing on crossbar of machine, slipped and caught self with (R) elbow and (L) knee, strained back.

While loading shot material, the operator drove into material and hit solid material stopping the loader suddenly.

Three mechanics were working on a EX-1900 excavator. Injured EE was tightening bolts on the right bucket cylinder guard. The mechanic was using a 1/2" impact wrench,when the socket attached to the impact wrench slipped off of the bolt and came off of the impact wrench. The socket struck the EE on the right side of the upper-lip.

EE was stemming a hole while doing this he hit a rock hidden in the dirt. He instantly felt pain in his right shoulder.

On 5/5/10, the employee was staarting the shift and was climbing the mounting ladder of a 835 Volvo articulated truck. The member's foot slipped and he grabbed the handrail with a free hand & in doing so straind his lower back. He missed no days. In September the member again saw a Dr and after diagnostics were completed was placed off work on Sept 27, 2010.

Using a service truck boom to lift an idler, a chain hook slipped allowing the idler to fall striking the employee. This incident was found on a part 50 audit

While installing air intake tubes, strained low back. Surface mine: Warehouse lot.

Stepped off fuel truck onto a rock. Ankle made a "popping" sound.

The member was checking his service truck to see if a rock had been lodged between the tires. While walking around the truck, the member slipped on ice and fell on his right shoulder. The member completed his shift. **The member did not miss any shifts. The out patient repair to shoulder was completed on Aug 12, 2010.**

He was dumping off because of being over weight and then while cleaning off tailgate so it would latch his hand slip and got caught between rod and dog ears smashing his left index finger.

Treating bed at stock pile back, Started back down steps and slip causing sprain, strain to right wrist, lower back and right hip.

Shovel bucket struck the tail chute of a rock truck

EE was working on a tandem axel dump truck.One EE was in the cab and was operating the PTO the other EE was knelt next to the PTO pump.The EE placed his right hand on the pump for support and balance. His left hand using a tool trying to push the dump plunger.The tool slipped off of the plunger and struck his right hand forcing the hand into the pump shaft.

While walking across the EE parking lot to his car he slipped in the mud (not falling to the ground) and felt his left knee "pop". Didn't want medical attention untill 1/29/10. EE seen a doctor and was put on light duty.

Crew was lifting a guard onto the crowd drum with a winch. The lifting eye broke and the winch cable struck an employee on the forehead above the left eye resulting in a lacerations requiring stitches.

Truck bed was removed from trailer used to haul coal. The ee was trying to un-hook the cable used to lift the bed. The tension released from the cable, backlash from the cable pinched the ee hand between the cable and a small near-by rock. The pinching resulted in a fractured of the tip of the ring ginger. No splint was needed and the ee returned to work on the same day.

Employee was stemming holes when he felt "pain shoot from his lower back down through [his] legs". Employee was taken to Summersville Memorial Hospital and given light duty till the 1st of September.

2 ee were pulling a final drive. Final drive was connected to boom of a service truck. Before the drive became free ee grabbed back of drive to guide the drive when it was free. As drive became free it immediately moved backwards a pinched/lacerated the ee hand. Lacerations were to the ring and small finger and required 7 stiches total.

Truck coming down hill lost control and collided with highwall. Cracked rib - bruised knee - mild concussion

Employee was driving a rock truck, while traveling loaded he drove into a dip in the road jarring the operator. He visited a chiropractor for treatment

Employee was stemming holes when something in his shoulder gave. He reported the incident and sought medical attention the next morning.

Employee was working under a rock drill. As he was exiting he raised up too soon striking his back on a fuel tank mounting bolt.

Employee struck bumper of truck with right knee on Saturday. Went to family dr on Monday. Still suffered from numbness and swelling, went back to doctor and was told to take off from work.

The operator was using a 992G model to clean up a pit. He was loading the remains of a pad previously used by a PC 3000. While loading the bucket, the machine was stopped by a rock. The sudden stop of the machie caused the operator's wrist to be pushed against the joy stick and resuled in a sprain of the wrist.

A coal truck driver had dumped off coal due to being over weight. The driver then went to the back of the truck to ensure the tail gate would completely shut and to sweep the tail gate area of any loose coal. He pulled the tail gate by hand to allow additional coal to fall. The additional coal was keeping the gate from completely closing. As he pulled the gate, he lost

Employee stepped off ladder and knee gave out.

Fell to the ground while changing a air filter on a G model loader. This incident was found during a Part 50 audit

While pulling an oil hose from a grease truck the employee slipped in gravel twisting a knee

On 1/8/2009 the employee was diagnosed with Carpal Tunnel. The employee missed work from 6/18/2009 to 9/7/2009 for medical procedure.

Employee was removing cones from hole prior to loading when he felt back give out (reaggravated injury from 10/2).

Getting into cab when the door swung open too hard. employee grabbed the door but lost footing in the process and catching himself by grabbing a handrail. Allegedly straining the shoulder.

While checking oil and antifreeze levels in rock truck, EE was pulling up ladder, when he felt a slight pain in lower back. He continued to work through one more truck, then decided to have it checked out. No emergency care was required and EE refused ambulance.

Employee was stemming a hole when he felt a "pop" in his back. He stopped stemming and stretched...operated a truck for a bit but his back continued to hurt so he requested to seek medical attention.

EE exited his car & was on his way to the coal truck to begin the preshift. While walking to the truck, he stopped to speak with other members. At the end of the conversation, he turned to continue on to the truck. At the point of turning he twisted his knee & fell to the ground. He was treated at a local hospital & a knee brace was applied.

The mechanic was assigned to replace the tracks on a D-11. He was using a pry bar to get the clip, or gator, to the down position. The bar slipped caused the mechanic to fall to the ground. At the time the bar slipped, his foot was under the master track pad. The pad was being held up by a crib block.

Employee was climbing ladder on a drill when felt pain in the shoulder. Incident was discovered during a Part 50 Audit

Work was being done on the chain wrench for DR 820. At the time of incident, an air line was being aligned.

Employee exited cab of service truck and turned to turn on work lights. Incident was found on a part 50 audit

EE was mounting a 785 C Cat truck, while maintaining 3/pts of contact. EE was mounting from ground to the 1st step when he felt some pain in the lower back area. EE reported the incident at 1244pm on the same shift and completed the shift. He was seen when the shift was over.

Truck needed coal dumped off and driver got finger in tailgate.

Employee was stemming holes when he felt back pain. Went to local hospital, was examined and released with no restrictions.

A belly pan was being installed after work was completed on a hydraualic line. The pan was being lifted in place by a cable. Once the pan had been lifted, the mechanic moved into place to install the bolts that would permanently hold the pan in place. As this was done, the cable failed and the mechanic was struck by the pan.

Employee was removing clamp on bin elbow. Hand came into contact with sharp piece of metal causing a cut to the index finger.

Cleaning tailgate area. Right index finger smashed.

He was stepping up into the bucket approx 1' off the ground with his left leg when his right foot slipped. All his weight shifted over to his left leg as he twisted.

Ee was machining faces on nose cone of a d10r dozer. I stopped the drill and steped around to look at face & I stepped on a small rock and twisted my knee as I stepped around to look at the face.

EE DRIVING ROCK TRUCK UP OUT OF STEEP PIT WHEN THE LOAD SHIFTED TO THE REAR CAUSING THE TRUCKS FRONT END TO RAISE UP 4-5 FEET AND THEN SLAM DOWN JARRING THE TRUCK CAUSING PAIN IN LOWER BACK.
