Employee was attempting to hold a wedge in place while striking wedge with a pick hammer employee missed the wedge striking right pinky finger causing a laceration to the end of the finger.

Employee was kneeling while running the miner in 4 left when a rock measuring 48" long 19" wide and 6" thick fell between bolts striking them on the bottom of their right foot.

Employee was trying to remove a fly pad out of the scoops ram which was all the way out, the scoop operator didn't see employee and began to pull the ram in catching EE's fingers in between the ram and scoop bucket.

Roof bolter operator was installing roof bolts. Operator was installing a roof bolt into the chuck of the machine when hand slipped and hit the controls causing the chuck to rotate entangling operator's hand. Resulted in fractures and lacerations of the left hand and ring finger.

While loading a shuttle car conveyor chain onto a rail mounted flat car, a portion of the conveyor chain slid off the car, striking employees ankle and foot. Resulted in a fracture.

Employee stepped in a hole causing injury to EE's right ankle. Employee continued to work until 08/14/2023.

Employee was diagnosed with pneumoconiosis.

Employee had hand on canopy of underground forklift while operating. Canopy contacted mine rib pinching left hand. Resulted in a laceration requiring 13 sutures.

While accessing operator's deck of a scoop, employee came in contact with a weld burr lacerating left arm resulting in five (5) sutures.

Employee was diagnosed with pneumoconiosis.

Employee was handloading steel I-beams by hand onto a rail mounted flat car smashing left middle finger. Resulted in a laceration requiring 4 sutures.

While installing a bolt in the mine roof, miner was attempting to line drill chuck up with bolt head when the rotation was unintentionally engaged. A burr caught the glove and wrapped right ring finger around the bolt. Resulted in stitches and a dislocation.

A non-injury roof fall occurred on the No. 2 Section Pillar Line in the No. 6 Entry. Spad No. 18677. The roof fall measured approximately 20 feet long, 15 feet wide, 5-10 feet high.

Injured was skiving belt at 6A Belt Tail using a utility knife, resulted in laceration requiring 3 stitches.

Miner was handling trailing cable when miner felt pain in right shoulder. Accident was determined to be reportable on 09/19/2022.

While walking around the outby end of the roof bolting machine in the No. 7 entry on No. 1 Section, piece of rock fell from the mine roof striking employee's right arm resulting in a laceration. Injury required 10 stitches. Rock measured approximately 12 inches long X 8 inches wide X 1/2 inch thick.

Draw rock fell from mine roof striking employees leg while operating side controls of roof bolt machine. Employee continued to work until 08/18/2022. Was determined to be reportable injury on 08/31/2022.

Employee was attempting to install a bit insert on the continuous miner by using a ball peen hammer when a piece of metal struck them in the eye. Metal was removed from eye by surgery.

While using jacks to put a derailed locomotive back on the track, one of the jacks fell over pinching hand between jack and crib block. The locomotive was not on the jack when it fell over. Resulted in a fractured right hand.

Miner was kneeling down in the intersection while operating the continuous mining machine, when a piece of roof rock fell from between bolts, striking the right leg. Resulted in a fractured fibula.

Employee was carrying block to a stopping and experienced pain in the lower stomach area. Employee continued to work until 05/25/2022 at which point a hernia was diagnosed. Hernia will be repaired.

Employee was diagnosed with pneumoconiosis.

Employee was guiding scoop operator in to scoop up a bundle of conveyor belt when the scoop bucket struck the employee's right foot.

Miner was walking by roof bolter and struck hand against the machine striking a burred edge. Resulted in a laceration required four stitches.

While drilling the roof in No. 7 Right on the No. 1 Section, a piece of rock came down the drill steel striking employee in the nose.

Employee was exiting the scoop deck and lost footing striking shoulder on the mine rib.

While handling high voltage line during a repair to blade switches in a power box, the cable shielding contacted the miner's left middle finger resulting in a laceration and six (6)stitches.

While hanging the roof bolter trailing cable, arm came in contact with a roof bolt plate causing a laceration on left arm. Resulted in stitches.

While changing out a top cradle in the conveyor belt, employee stumbled and fell causing the cradle structure to fall pinching EE's right middle finger against the water line. Resulted in a avulsion.

While installing a curtain board, a piece of draw rock fell from between bolts. Employee dodged the rock, stumbled and fell injuring right shoulder.

While handling shield off of shuttle car, employee felt pain in groin. Was diagnosed with hernia which will be repaired. Was determined reportable on 05/26/2020.

While installing a cable bolt the glue did not set allowing the bolt to slowly drop out of the hole. Miner attempted to pull bolt out of the hole when EE felt a sharp pain in the left shoulder. Began missing work on 01/14/2020.

An unplanned roof fall occurred in the #14 Cross Cut of the #1 Section's Left Return. The fall was approx. 50' in length, 20' in width, and 7' in height.

While traveling through the escapeway door, the employee's left leg got caught between the door and frame of the Fan House.

While employee was opening the scoop battery lids, they became fouled. When re-positioning EE's hands to get a better grip, the lid fell striking EE's left middle finger.

Employee stated while handling a cable pull rope, employee felt a pain in back/shoulder area. This employee has suffered and continues to suffer from multiple back/shoulder related injuries due to repeated trauma, therefore Marfork Coal Company is submitting this as an Occupational Illness due to repeated trauma.

While traveling in a shuttle car to the feeder, the employee hit a hole in the bottom and injured back. EMPLOYEE DID NOT BEGIN MISSING WORK UNTIL 9/13/2019.

Employee was diagnosed with Pneumoconiosis.

Employee was guiding scoop operating in an attempt to get a roll out of the belt. While EE was instructing EE to come in, the scoop bucket struck the employee on the right foot.

While tramming from #9 entry to the feeder, employee ran over a block causing employee to bounce and strike head on the canopy.

Employee was diagnosed with pneumoconiosis.

An unplanned roof fall occurred in the face of the No. 2 Section approximately two breaks inby spad no. 14625 in the no. 5 entry extending across the cross cuts to the no. 7 entry. The fall was approximately 140 feet in length.

Employee was operating scoop cleaning the No. 9 entry along the rib on the curtain side when the scoop struck a curtain rod causing it to break and enter the operator's deck. Curtain rod struck employee in the mouth resulting in four sutures.

Employee was exiting underground forklift cab when their foot allegedly became caught between the seat and frame causing them to fall backwards landing flat on back. The operator has completed its investigation of the alleged incident and has determined the incident may meet reportability criteria as set forth by the regulation.

Roof fall on No. 1 Section; No. 1 Entry, 1 break outby face near survey spad 14357.

While operating roof bolting machine, the miner was installing a T3 channel strap. During installation, the strap came in contact with the drilling steel, causing the strap to spin around, striking the machine operator's hard hat on the left side of their head. There were no witnesses to the incident.

Miner was dropping trailing cable slack from the mine roof from a roof bolter. While handling the cable the take up reel engaged and began taking up the cable slack. A cable rope hanger hook caught the miner in the webbing of EE's hand between index finger and thumb resulting in laceration/puncture requiring stitches.

Employee was diagnosed with pneumoconiosis.

Employee was pulling curtain from underneath gob and felt a sharp pain in shoulder and forearm.

Employee was diagnosed pneumoconiosis.

Employee was assisting the underground forklift operator with the placement of supplies. Machine ran over employees right foot causing bruising and soreness.

Employee is diagnosed with pneumoconiosis.

Employee was beating two hammers together in an effort to remove a splice pin from a mechanical conveyor belt splice. A piece of metal chipped off one of the hammers striking employee in cheek below the right eye.

Miner was changing tire on four wheeler when the jack suddenly fell over while under load smashing miner's hand between frame of machine and jack. Resulted in sutures.

A mine examiner discovered a roof fall along the No. 6 Belt Conveyor at 15 Break between the No. 1 and No. 3 entries. The fall measured approximately 100 feet long x 20 feet wide x 7.5 feet high. There were no injuries and no equipment was involved. Passage of work crews was not affected.

While traveling in a rail mounted mantrip, miner injured head when the mantrip struck a broken rail causing the machine to come to a sudden stop. Injury was a laceration requiring 6 staples.

While loading bolt machine, miner turned to walk back to scoop and stepped in hole spraining ankle.

Rock fell from between roof bolts striking miner on the back of head and right shoulder causing EE to fall against continuous mining machine striking face on the machine.

While operating scoop, a flyboard bolted to the mine roof, was caught by the scoop and pulled down and fell into the center section of the scoop. A hydraulic fitting was damaged and hydraulic fluid was sprayed into the miner's face and eyes.

Miner was diagnosed with pneumoconiosis.

While bolting the intersection of #6 a rock fell out from between bolts striking miner on right side of neck and right foot.

Employee was diagnosed with pneumoconiosis.

Employee was attempting to cut boot laces out of their work boots when they cut their hand. Resulted in stitches.

Miner was washing continuous miner utilizing miner water line. While washing the pressure pump kicked on causing the miner to lose grip on the hose. While trying to regain grip, miner felt pain in elbow. Continued working at normal duties until 10/17/2017 when miner began missing due to medical treatment being required for the injury.

Employee was using a hawk bill knife in an effort to trim belt rubber when knife slipped resulting in a laceration to right arm. 3 sutures.

Employee strained back while attempting to hang miner cable.

Employee was attempting to remove a pin from the shear jack of the continuous miner. Pin became lodged, employee attempted to use slate bar to pry the pin away from the frame when the bar slipped causing a strain to lower back.

Employee was diagnosed with reportable hearing loss in the left ear.

Employee was at the #7 Belt Head and was changing a fire suppression spray. While working, hard hat brushed the mine roof causing rock dust to fall down in behind safety glasses entering eye. Medical treatment was sought, eyes were flushed and employee was prescribed eye drops.

Injured was clearing a rock at the #4 Tailpiece and slipped on belt structure striking left shoulder.

Employee was changing a hose on the take-up power pack located at the #5 Belt Drive when the wrench EE was using slipped. Employee was supporting self with left arm. When the wrench slipped body moved forward dislocating left shoulder.

While operating shuttle car, employee was bounced around in the operator's deck causing neck to pop. Employee experienced pain in lower back and left arm.

Employee has occupational hearing loss.

Employee has been diagnosed with occupational Pneumoconiosis. Employee was terminated due to workforce reduction on March 24, 2016

Employee has been diagnosed with occupational Pneumoconiosis.

The employee was in the process of removing a rock from ATRS on a Fletcher DDR roof bolting machine when EE felt a pain in stomach. EE was diagnosed with an abdominal hernia on 4-4-16 and began missing work on 4-5-2016.

The employee was in the process of removing a rock from on top of the Joy continuous miner when a piece of rock measuring approximatley 4'L X 28" X 1 to 2 inches thick fell from the mine roof striking the employee on the right arm resulting in a laceration requiring 4 sutures.

Employee was installing track rails. The track rail was in a raised position and while fastening a track tie to the bottom of the rail, the chain supporting the rail broke causing the rail to to fall and strtike the employee on the top of right foot.

Employee was tramming scoop in muddy conditions when scoop slid against rib, causing the rib to roll out onto the deck resulting in multiple injuries. (left wrist-fracture / 3 sutures to chin / 2 chipped teeth).

Employee has been diagnosed with Occupational Pneumoconiosis. The Safety Department was notified of the diagnosis on Jan 29, 2016.

Employee felt pain on lower back and in right leg while bent over to lift a pie pan. Employee was treated at Raleigh General Hospital.

Employee was bolting in #7 entry about 3 rows inby corner. While he was swinging pod out to bolt the corner, rib rolled out striking employee's left ankle. (2 bones fractured) Rib was 78" long x 39" wide x 32" thick. Rib was behind curtain. Employee was treated at Raleigh General Hospital.

Employee was using flexco hammer to put water line together. As he was hitting against plastic coupling, a piece of coupling broke off striking him in his safety glasses resulting in a scratch to his left eye and cheek.

While Employee was setting timbers along the beltline a timber fell against Employee striking back of head and neck.

Employee was throwing 1A track switch when he slipped and fell against J-12 mantrip hitting his lower and middle back. Employee did not have loss days until he went to the doctor on 11/3/15.

While Fire bossing #4 belt Employees ride became low on charge. While attempting to push the ride up a small incline his foot slipped causing him to twist his right ankle. Employee did not seek medical assistance but did not report to work the following day.

Employee stepped into a hole and twisted his knee causing him to fall and striking his shoulder on a rock.

Employee has been diagnosed with Occupational Pneumoconiosis. The Safety Department received notification of his diagnosis on 11/11/2015.

While Employee was loading a shuttle car with the miner; a piece of rock fell out between a row of bolts striking Employee on the back of neck.

Employee has been diagnosed with Occupational Pneumoconiosis. Marfork Safety Department received notification of his diagnosis on March 27, 2015. Also Employee ceased work in April 2014.

Employee has been diagnosed with Occupational pneumoconiosis.

Employee has been diagnosed with Occupational Hearing Loss. Employee ceased working on April 8, 2014.

An unintentional roof fall has occurred on the Arabian Mains 1 break inby spad 3893. The fall was approximately 40ft x 19ft x 5ft. There were no injuries reported and no equipment involved.

As Employee was operating continuous miner a rock fell striking Employee on the neck causing pain in his lower back. Employee was treated at Raleigh General Hospital and was diagnosed with a compression fracture of the L2 and L5. Employee has a follow-up appointment with a specialist.

While Employee was picking up tools from changing inserts on the continuous miner he felt a sharp pain in his lower back and leg.

Employee was hit by a piece of draw rock on his back. Employee was treated at CAMC General Division

While bolting top Employee placed steel against top when a kettle bottom fell striking Employees left shoulder and rolling down his arm mashing his left ring finger between the drill pot and kettle bottom causing a fracture.

Employee has been diagnosed with Occupational Pneumoconiosis. Employee retired on 6-28-13.

Employee has been diagnosed with Occupational Pneumoconiosis. Employee is on disability effective 12-12-13 due to injury.

Employee has been diagnosed with Occupational Hearing Loss. Marfork Safety Department received the diagnosis on 6-3-14.

While pulling miner waterline through stopping, metal coupling came through the stopping, flipping over and striking Employee in the mouth, resulting in a chipped tooth.

Employee has been diagnosed with Occupational Hearing Loss. Employee retired on October 31, 2013.

Employee has been diagnosed with Occupational Pneumoconiosis.

An unintentional roof fall has occurred on Horse Creek Mains #7 Entry Right Return 1 break inby spad 374. The fall was approximately 40ft x 18ft x 6ft to 8ft. No injuries were reported and no equipment was involved.

Employee has been diagnosed with Occupational Pneumoconiosis.

Employee was hanging reflectors when a piece of draw rock fell striking Employee on the head and back. Employee has not missed work but due to a knot on his back his doctor drained fluid out of it on April 10, 2014.

Employee was pulling cable off shuttle car when he felt a pain in his neck.

Employee placed his hand on rail for leverage to stand up when rail slid over and smashed his right pinky finger against another rail resulting in (distal) amputation by physician of right pinky finger below first knuckle.

Employee was struck in the head by a rock. Marfork Coal Company is filing this 7000-1 solely to abate Citation No. 9054035 and does not agree that a reportable accident occurred or that it violated Part 50.20(a) in any respect since no lost time accident occurred at this mine...per Management and Legal.

Employee has been diagnosed with Occupational Pneumoconiosis.

As Employee prepared to operate locomotive he put his finger between frame of motor and shield. As he sat down he placed his bodyweight on shield smashing his right middle finger. The accident was not reported to Marfork Coal by the injured. An internal investigation was conducted and accident did occur on the job. On 11-22-13 finger was amputated due to severe infection.

Employee has been diagnosed with Occupational Pneumoconiosis.

An unintentional roof fall has occurred at the Old #1 Horse Creek Mains 154 spad in the #5 Entry. The roof fall was approximately 50ft x 20ft x 5ft. No injuries were reported and no equipment was involved.

Employee was cutting the belt for the angle of the vulcanizing splice. The knife slipped off the belt striking Employee on the left hand cutting through his gloves. Employee was treated at Raleigh General Hospital where he received 4 sutures left index finger and 2 sutures left hand.

As Employee's were transporting conveyor pot to the continuous miner. The conveyor pot was chained to the forklift. The chain broke causing the conveyor pot to fall striking Employee on the right foot. Employee was transported to Raleigh General Hospital for treatment and was diagnosed a #2 and #3 metatarsal bone fractures.

Employee was unloading belt structure from scoop. The top roller was stuck and as Employee pulled on roller he felt a pain in lower back left leg.

Employee was driving F350 company truck when the truck started to lose power and smoke coming from underneath front end of vehicle. Employee pulled vehicle over and saw flames and immediately started to put out fire; called Whitesville Fire Department to assist.

Employee was diagnosed with Occupational Pneumoconiosis.

Contractor was walking by the #3 Fairchild Scoop and lost his footing when he tripped and hit his chin on the canopy of the scoop. He was treated at Raleigh General Hospital where he received two (2) sutures.

Employee was drilling a test hole in #7 right when a piece of steel fell and struck Employee in the mouth causing a small laceration and chipped two teeth.

An unintentional roof fall has occurred #2 Entry spad #3530. The roof fall was approximately 20' x 20' x 5'. No injuries were report and no equipment was involved.

An unintentional roof fall has occurred at the #2 Entry spad 3556. The roof fall was approximately 60'x 20'x 5'. No injuries were report and no equipment was involved.

Employee was lifting a blower motor off the ground to put in the parts building. As Employee lifted the motor, the motor shifted causing injury to right thumb. Employee was treated at MedExpress and was diagnosed with a sprain/strain to right thumb and given an immobilization splint.

While prying the cable out from around the cable reel on # 10 shuttle car sprocket the pry bar slipped and caused right pinky finger to have a laceration requiring 4 sutures on the tip of finger. the distal phalanges to have a hairline fracture.

EE was hooking pull rope on top of miner around a rock to pull it off and was crossing the conveyer boom to get off miner when a rock measuring 40"x45"x4" thick fell between the bolts strikin g EE in the neck & back while he was still on the miner.

As Employee was gathering block to build a stopping he got out of the scoop and a piece of rock measuring 27" x 32" x 1.5" fell striking Employee on the back of the head. Employee was transported by EMS to CAMC General Division.

A roof fall at Arabian Mains at spad 4108 and 90feet outby spad 4094. Measuring 20 foot in length 20 feet wide and 6 foot high. 48 in glue was used in roof support. This area will need to be cleaned up and overcast installed.

Employee was repairing mandoor in a stopping when a block fell striking Employee on the top of the left hand. Employee was treated at Med Express and xrays were negative.

Employee was filing dozer with fuel when he slipped and began to fall, grabbed handle and injured shoulder. Employee did not miss any work due to this injury but started physical therapy on 6-5-12. Due to the nature of the injury Employee had surgery on 3-12-13.

Employee was holding metal for front of rock box. One side was tac welded, weld broke loose causing metal to fall pinching Employees finger between side piece of rock box. Employee was treated at Raleigh Boone Medical Center where he received two (2) sutures.

Employee was carrying a can of oil when he felt a pain in his back. Employee was treated at CAMC General for treatment.

As employee was changing batteries on the scoop the scoop lid closed smashing his index finger on left hand. Employee was treated at Beckley Appalachian Regional Hospital and was diagnosed with a fractured left index finger.

Employee looked at Arc from welder while holding a piece of metal to be welded. Resulting in flash burn of both eyes.

Employee was dropping miner cable in #2 entry. A rope hanger wrapped around his right hand between the employees ring and pinky finger. Employee's hand was cut between the fingers resulting in 19 sutures.

As Employee was pulling rock off miner with a slate bar the slate bar became hung. The rock started to slide off the miner which caused the rock to roll in the opposite direction landing on the operators right foot. The rock was approximately 3'x2'x8". Employee was treated at Raleigh General Hospital and was diagnosed with a fracture on the tip of his right big toe.

An unplanned roof fall has occurred on #2 Section at crosscut #3 to #4 spad 5876. The roof fall was approximately 65' x 20' x 6'. No injuries were report and no equipment was involved. A revision to the roof control plan has been submitted to the District Manager for approval.

An unplanned roof fall has occurred at the intake #6 entry at spad 262. The roof fall was approximately 60ft x 20ft x 6ft. No injuries were reported and no equipment was involved. Lifeline was rerouted and the miners were made aware of the change.

While Employee was raising the drill pod of the roof bolter, to push bolt up, a piece of draw rock fell striking employee on his hard hat. Employee was treated at Raleigh General Hospital and x-rays were taken.

As Employee was pulling curtain out of gob he stepped on a rock causing his right knee to twist. Employee went to his family care physician the following day 8-3-11 where xrays were ordered for 8-4-11.

While employee was picking up water line to put a fire valve in his back struck a pie pan which had been bent up causing him to fall. The Safety Department was not notified employee missed work or was seen by a physician until 8-8-2011.

An unplanned roof fall has occurred on the #1 Section #6 entry 1 Belt at spad 242. The roof fall was approximately 30'x19'x5'. No injuries were reported or no equipment was involved.

Employee was cleaning 5 entry with a scoop. As he was cleaning the right rib piece of line curtain fell. He went to re-hang the line curtain a piece of rock fell out of the rib pinned right foot. The rock was approx 72x32x16. He was transported to CAMC and was diagnosed with 6 fractures in his right foot. He was given instructions to follow up with an orthopedic surgeon.

An unplanned roof fall has occurred at spad #3916 on the Arabian Mains #6 entry. The roof fall was 20' x 20' x 5'. No injuries involved, no equipment involved or passage blocked.

Employee was bolting a corner bolt in #2lt. Employee was standing in a walk through bolter when a piece of rock fell striking employee on the right hand. Employee was treated at Raleigh General Hospital and was diagnosed with a strain.

Employee was lifting a shield on the #2 Motor to check the wires and plugs on the blue box. As he was lifting the shield he felt a stinging sensation in his stomach. He then noticed a large knot raise up on his stomach. Employee immediately ceased work and drove himself to a medical facility which he was diagnosed with a hernia. Employee continued working until 5-13-11.

Member was pulling a water line with the scoop and got his right hand tangled up in the loop of the water line. The member immediately ceased to work and co-worker performed first aid on him in the nature of wrapping his hand then he transported himself to Raleigh General where he was then diagnosed with a broken hand.

Member was putting bolt into hole when rock fell and caused bolt to hit him in the private area. He immediately ceased work to receive medical attention.

Member was bending and pushing bolt into top and heard his shoulder make a loud pop. He did not cease work but told his supervisor about the injury but didn't fill out an accident report. After the shift, he transported himself to Med Express in Beckley. Marfork Safety Department was not made aware of this until after he had seen a doctor.

An unplanned roof fall has occurred approximately 1400 feet outby #2 Section at spad #3535. The fall was 20ft x 25ft x 8ft. No injuries or equipment was involved.

An unplanned roof fall has occurred in a worked out area of the Appaloosa Mains near spads 4750 to 4620. This area is used to evaluate EP8 and EP7. The roof fall was 800' x 20' x 4'. No injuries or equipment was involved.

Injured stepped off of a flat car, struck his left foot on a rock, causing a sprain to his heel/ ankle area.

Member stepped off of rear of walk thru bolt machine approximately 1 foot off the mine floor and twisted his ankle.

An unplanned roof fall occurred at spad 248. Fall measured 100' Length X 20' Wide x 5' High. No Injuries were reported.

Member was preparing to load top structure into a scoop bucket when he bent down to pick up the structure he heard and felt a pop in his lower back.

Member is in the process of operating a scoop and attached one end of a nylon pull rope to a scoop insert and the other end to the scoop bucket to slew the scoop insert around straight when the nylon rope broke and came back and struck member in the side of his head.

An unplanned roof fall occurred on #3 belt line in the #4 entry near spad 3476. The fall measured 25'L X 30'W X 6'H. The accidents were reported. This area was developed 01/16/2007.

EE was raising the ventilation curtain up to clean the rib and a peice of rock fell from the roof causing a laceration to his right arm that required glue.

While standing on ladder to remove fly pads, ladder fell

Roof fall 20' X 30' X 8' at spad # 3565. 3500' out by an active section. No injuries, equipment involved or passage blocked.

Roof fall in the #1 entry at spad # 4095. Fall was 80' X 20' X 4'. No injuries, passage blocked or equipment involved. Foof fall was 6000' from an active section

Injured attempted to walk around a shuttle car in the 5 right cross cut and a rock approximately 20" X 25" rolled off the shuttle car hitting him on the left knee causing a contusion.

EE was rock dusting with a scoop and bucket duster when he hit a sand jack causing it to fall and hit him in the torso. The injury required two stitches.

Roof Fall on a Joy 14-15 Continuous Miner while performing retreat mining. Fall was 3.5' X 12' X 35'. No Injuries occurred.

There was a roll of fly pad beside rib. Employee picked up roll of fly pad & threw it on to roof bolter & felt sharp pain in left lower back.

Driving a chisel between duster and flinger, hit chisel to loosen it up and the chisel popped out stricking member in face under the rt. eye on the cheek, causing a laceration.

No injuries, primary escape way blocked. Evacuated crews until escapeway was rerouted

Member was taking 4" plastic water line apart when another member hit it with a sledge hammer causing it to burst and a piece of it hit member in the face causing a fracture to his cheek bone.

EE was attempting to reset a 480 volt, 225 amp breaker. When he did it blew up causing a 2nd degree burn to the top of his right hand and a first degree burn to the side of his face and ear.

Member was assisting crew making a belt splice. He reached down, grabbing one of the knockers, at which time the other member struck him on the right hand with a hammer.

A fall of the roof occurred on #2 section #1 entry approx. 70' inby spad #3178, 320' outby section. Fall measured 30' long, 20' wide, 5' thick, no injuries, no passage of men affected.

Injury and other ee had bolted and 1 entry and were moving to #2 entry. Other ee had the cable reel on, while he was backing machine up as he spun around corner the cable came across the mine floor and took his feet out from under him causing him to fall on r. shoulder & r. wrist as he braced his fall causing a fracture to r. wrist

While helping another EE, EE attempted to cut a cable on his leg. The knife slipped causing a laceration to his left leg, right little and rign finger, that required 16 stiches.

While mining through a barrier block between two panels a cut through occurred causing a water inudation.

Member was pulling a loose piece of rib when it fell. He put his hand up to keep it from hitting him in the face. A sharp rock cut his hand causing a laceration that required 3 stitches to repair.

Roof fall in the # 6 entry at the last open cross cut. The fall was 80' in by spad # 2638. Fall was 20' long, 20' wide and 7' high. No injuries occurred, no equiptment was involved and passage was not blocked.

Roof fall in the last open cross cut of # 4 entry, 90' out by spad 2634. Fall was 52' long, 18' wide and 8' to 9' high. No injuries occurred and passage was not blocked

While roof bolting in the # 4 entry a piece of rock fell out striking employee in the face chipping a front tooth that will require dental work.

Member was in #5 entry on #2 section near spad #2580 when a small piece of draw rock fell and struck member in the head causing a laceration that required 7 stitches.

EE was alking down the intake entry and stepped into a hole and tripped, EE caught himslef with his right leg and this caused him to twist his knee that resulted in a spain.

While riding in a Brookville Mantrip, EE's left leg came into contact with the rib, causing fractures and an avulsion.

While pushing coal in the steel area with a D7 dozer, ee ran over a hump causing his head to hit trac box.

Employee attempted to exit a mantrip when his foot slipped causing him to put his hand down on the ground causing a laceration requiring 6 stitches.

Roof fall 1 x-cut inby spad #304. 60' x 20' x 8'. Rerouted intake escapeway. No injuries.

Roof fall in #2 entry at spad #295. 60' x 20' x 6'. No injuries.

Due to the Joy 14-15 Continuous Miner loosing power in the last lift of a pillar block in the #2 entry, a roof fall (40' X 40' X 5'-7') fell on it. No injuries occurred and the Continuous Miner was recovered.

While walking through the cross sect. from #5 to #4 entry, a piece of rock measuring 6' x 2'6" x 3"-4" fell striking EE on the head & back. X-rays were taken with the results being negative.

While attempting to let the roof bolter cable down that was hanging up in the #6 entry, the reel was activated causing employee's right hand to be smacked against the roof, lacerating the index & middle fingers.

Roof fall in #2 entry 80' inby spad #550. 20' x 20' x 5'. No injuries.

Roof fall 120' x 20' x 5' - 7'. Roof fall across belt & track. No injuries.

Roof fall in #4 entry, one crosscut inby spad #942. Roof fell on a 14-15 Joy continuous miner while pillaring. Fall was 7'x25'. No injuries accured

Individual was trimming a piece of belt and knife slipped resulting in a laceration to left forearm requiring 10 stitches. Individual didn't miss any work.
