Employee stated was attempting to hang line curtain when EE felt a pain in the left shoulder. Injured employee did not file a report until the following Monday (11/13/2023). After filing the report, EE drove self to Raleigh General where EE was taken off work for 5 days. Employee remains off work, but due to medical diagnosis we now feel that EE meets the Part 50 criteria.

Employee stated EE was attempting to place EE's drill steel in the drill pot of the Roof Ranger RR2-13 machine, EE lost balance and fell into the controls which caused the drill pot to rotate. This caused hand to be twisted as the steel rotated. EE was diagnosed with fractured bone in right wrist.

Employee stated they had pulled belt through the tailpiece and was going to cut the belt to make a belt splice as they cut the belt they cut their right thumb with a utility knife. Received 5 sutures in the cut on their thumb.

Employee stated was in the #8 entry. As EE was setting the miner over in the cut, EE was walking backward, slipped on a rock and fell. Employee stated felt pain in left ankle and left knee.

Employee stated EE was welding a slide in a Rock box and was reaching to finish welding and something popped in EE's right ankle

Employee Stated after hanging a back-up curtain in #8 entry, as they turned to walk away they slipped on a fly pad that was laying on the mine floor hurting their right knee. We are submitting this under protest per MSHA recommendation, we feel we don't have enough medical proof until we receive the results of the MRI.

Employee stated: was using a small bar to break a lock off a tool box, the bar slipped out from under the lock and struck employee in the lower lip.

Employee stated that they were lifting a 5 gallon can of hydraulic oil out of a scoop bucket and felt a pain in stomach. The employee finished the shift but did not return to work. They called the mines the next day and informed management that they were referred to a surgeon. The employee returned to work on 7/18/2022.

Employee stated EE was bringing EE's stack steel out of the top the steel came apart from each other and struck EE in the left side hitting EE in the ribs, Employee left the mine site in an ambulance for treatment EE notified us there was no broken bones doctor told EE to take 2 days off.

Employee stated was changing a tire on the 488 Diesel scoop. EE was using a slate bar to work the tire loose from the studs. As EE was prying, EE felt a pain in the lower back and groin area. The injury was not reported due to unknown if this is pre existing on medical condition based on employee history.

Employee stated they were walking from #6 entry to #7 entry to hang fiber cable, when they stepped in a hole that was approx. 10 to 12 inches deep, and felt a pain in left knee, Employee started missing work on 10/6/21 when they went to the doctor, we do know they have had problems in the past, not knowing if this needed to go on a old claim is why we are late filing it.

Employee was bent down cutting a piece of belt in a cross cut behind a diesel mantrip. A diesel scoop turned into the crosscut striking the employee on left torso. Employee sought medical treatment on 09/23 and was released to come back to work. Employee went to doctor again and was placed off until 10/4 where results show contusions to rib area.

Employee sought medical treatment after experiencing back pain while unloading trash. Operator notified on 8/16/21 that employee was diagnosed with pre-existing degenerative spondyloarthropathy, a form of arthritis, after results of an MRI indicate no significate changes from a previous MRI conducted on 6/2/2020.

Employee stated was walking down the #1 entry at 33 crosscut when employee stepped in a hole and felt pain in left knee. The employee worked until 8/10/2020 when employee had surgery to repair meniscus.

Employee was getting out of the Diesel scoop when they put pressure on right knee it buckled causing immediate pain around the knee cap.

Employee stated that EE was in the process of checking the section feeder and was walking around it when EE stepped into some muddy conditions and twisted EE's right knee.

Employee was taking slack out of cats on #2 MRS . EE went to remove grease hose from the grease fitting, the grease fitting broke causing grease that was under pressure to penetrate right hand.

Employee stated EE was attempting to pull the oil feel hose out to put oil in the MRS when the hose came loose employee fell against the plow of the MRS hurting EE's ribs

Employee stated that EE was in the process of pulling cable off of the #362 Shuttle Car when EE slipped on slick mine floor causing EE to twist EE's right knee. Employee stated that EE's knee started to tighten up immediately.

Employee stated they were carrying a reel and fender over to shuttle car in front of buggy and felt a sharp pain in their groin. Employee sought medical treatment and was taken off work.

Employee stated they were walking on section when they stepped into a hole and pulled something in their buttocks. Employee sought medical treatment and was placed off work.

Employee stated was riding the Diesel mantrip to the section when the ride hit a bump causing employee to jar back. Employee was taken off work under doctors care due to back strain.

Employee stated EE was attempting to cross over a 6" water line when EE's rescuer got caught on a piece of guarding causing EE to fall backwards off the waterline injuring EE's back. Employee sought medical attention and was diagnosed with a fractured L5, employee was placed off work.

Employee stated was rock dusting #8 rt entry when foot struck a rock causing employee to stumble while trying to catch self. Employee states during this motion employee strained lower back and left knee. Employee sought medical treatment on 5/21/2019 and was placed under doctor's care.

Employee stated was walking backwards as EE was backing the miner up when EE twisted and injured the knee. Employee sought medical attention and was placed off work under doctors care.

Employee stated was cleaning out the deck of the shuttle car when EE re-injured an old injury in EE's middle back area. Employee called in the following day.

Employee stated employee bent over to pick up a bundle of plates when employee felt a pain in employee's back. Employee sought medical treatment and was taken off work.

Employee stated EE was operating a rubber tire diesel mantrip when EE hit a bump in the road causing EE to strain EE's back.

Employee stated was hanging miner cable with the assistance of 4 others when EE re-aggravated a lower back injury. Employee did not work the following shift.

Employee was trying to set a bushing in the frame of a miner. Employee stated they held a long handled sledge hammer up against the bushing as another employee would strike injured employees sledge hammer. As the employee swung, they hit the frame of the miner and it deflected and hit injured employees wrist causing a fracture.

Employee stated EE was in the process of plastering a stopping when a glob of plaster splattered off the stopping onto EE's glasses into EE's eyes. Employee sought medical treatment and was given medicated eye drops due to irritation.

Employee stated EE was helping move CM cable out of the way when MRS cable pulled down and struck employee in the back of the head straining EE's neck. Employee sought medical treatment on 10/30/2018 and was given three days away from work.

Employee stated they stood on drill pot to insert stick of glue. When the employee dismounted, they stepped on a rock and twisted their ankle. Employee was given modified duty, however company could not accommodate restrictions.

Employee was loosening the top suction hose, on roof bolter when a rock fell from the top striking them on top of the head. Employee complained of lower back pain. Employee was placed under doctors care for 2 days.

Employee stated ee was carrying a timber from the scoop bucket to the rib when legs felt weak and ee fell to the ground. Employee complained of back spasms.

Employee stated they had loaded roof straps on top of the scoop (battery end). Employee stated they backed up battery end of scoop and hit the rib and pushed the straps into the operators compartment striking their left hand causing a laceration. Employee was placed off work under doctors care.

Employee was walking when they tripped and stepped on miner cable causing them to sprain their ankle. Employee was taken off work under doctors orders.

Employee stated was lifting a tool bag from ground level to the top of a scoop when employee felt a sharp pain in back. Employee was taken off work under doctors orders.

Employee stated was operating a shuttle car when ran over a rock causing the employee to be jarred into the canopy. Employee complained of back and neck pain. Employee was taken off work on 6/5/2018.

Employee stated ee was walking when ee tripped over gob in a water hole and began to fall at which time ee caught self on the rib with right arm which resulted in pain in right shoulder and lost work days.

Employee was kneeling working on a mantrip when employee tried to stand up left knee popped and employee felt a sharp pain which resulted in lost work days.

Employee was installing a belt head with a lifting jack when the jack bar came in contact with employees head resulting in loss of consciousness. No lost or restricted work days.

Employee was operating a shuttle car when the shuttle car struck a rut causing back pain. Employee was initially given a full release but on 4/30/2018 ee returned to doctor and was taken off work at which time the incident became reportable.

Employee's right ring finger was caught between the lid of an SCSR Cache resulting in a laceration that required 11 sutures.

Employee stated EE was operating a shuttle car when the shuttle car struck a small rut causing the employee to jar EE's back resulting in a sprain/strain injury and lost work days.

Employee stated EE was lifting a belt scraper when EE felt pain in EE's lower back and both legs resulting in sprain / strain injury and lost work days.

Employee was lifting timbers out of the bucket of scoop when they felt a pain in the lower back resulting in a sprain/strain injury and restricted work days.

Employee slipped and fell into a pile of small sharp rocks resulting in a laceration the required 4 sutures.

Employee stated EE was shoveling material onto the belt when EE felt a pop in the right shoulder. Employee sought medical attention on 1-30-2018 and was placed off work.

Employee was pushing open the air lock door when the employee heard something in shoulder pop resulting in sprain/strain injury and lost work days.

Employee was lifting a tool box off of a charging cable when ee felt a sharp pain in lower back resulting in a sprain/strain injury and lost work days.

Employee was moving continuous miner trailing cable out of the roadway when ee felt pain in lower back resulting in a sprain/strain injury and lost work days.

Employee was climbing the bank under the stairs to retrieve something that ee dropped thru the stairs. Employee stated they fell and stubbed the left shoulder resulting in a strain/sprain and lost work days.

Employee was helping load an injured employee into ambulance when they felt pain in their back resulting in a sprain/strain injury and lost work days.

Employee was found laying on the ground in front of the feeder by another employee. Employee stated employee had fallen and twisted employee's back. Incident resulted in a sprain/strain and lost work days.

Employee stated that the man-trip hit a hole and jarred their back. Pain started going down the right leg immediately. This resulted in sprain/strain and lost work days.

Employee stated employee picked up a flyboard and employee turned and felt a sharp pain on the right side and middle of employee's back, which resulted in a sprain/strain injury and restricted workdays.

Employee stated was exiting a man-trip when ee stood up and felt a sharp pain in lower back and groin. *On 11/3/17 the incident became reportable when Rockwell Mining, LLC received information confirming a work related injury.

Employee stated that as ee was getting out of a shuttle car ee foot slipped and twisted left knee. *On 11/8/2017 the incident became reportable when Rockwell Mining, LLC received information confirming a work related injury.

Employee stated employee was removing the bottom spray block guard on a continuous miner when the guard fell against employee's right ring finger smashing it against the continuous miner resulting in a laceration which required sutures.

Employee was diagnosed with Coal Workers Pneumoconiosis.

Employee stated was loading timbers into bucket of scoop and felt pain in lower back. This resulted in a sprain and lost work days.

Employee was using spray paint to paint a center line in the #3 entry on the #1 section. As they painted the center line the air movement in the entry caused paint particles to blow into their eyes. Employee states they were wearing safety glasses. This resulted in an irritation and prescription eye drops. No lost work days.

Employee stated ee was operating a shuttle car in the #8 entry and the shuttle car got close to the line curtain. In order to prevent tearing the curtain down the employee jerked leg over to hit the brake. When ee did this ee felt pain in groin area. This resulted in a strain and lost work days.

Employee was operating a shuttle car on the #2 section when the car bottomed out and became hung in the intersection. When the shuttle car was freed from the hole in the mine bottom, the employee was jarred inside the operators compartment. This resulted in a neck/back strain and lost work days.

Employee stated ee was operating the diesel scoop when the machine overheated. EE heard a leaking noise from the rear of the scoop, and as ee walked toward the rear of the scoop to see where the noise was coming from, the cap covering a reservoir blew off. This caused hot fluid to contact the side of face, resulting in burns and restricted work days.

Employee stated was crossing the #2 belt and when stepping down on the other side of the belt felt pain in left foot/ankle. The employee continued to work until 5/9/17, when employee was diagnosed with a strain, and taken off work. The incident became reportable on 5/9/17 when the employee began missing work.

Employee stated was working on the Co. No. 262 CM and while lifting the rub rail back into place to reinstall the rub rail felt pain in right shoulder. This resulted in a strain and lost work days.

On 4/12/2017 Employee diagnosed with an occupational illness (carpel tunnel) started missing work due to surgery.

Employee stated EE was operating the shuttle car on the #1 Section when the shuttle car went over an irregularity in the roadway, jarring EE. This resulted in a back strain and lost work days.

Employee stated EE was operating the shuttle car on the #1 Section when the shuttle car went over an irregularity in the roadway, jarring EE. This resulted in a neck strain and restricted work days.

Employee stated employee was riding the #865 mantrip to the section with the rest of the crew when the mantrip hit a hole in the roadway jarring employee's back. This resulted in a strain and lost work days.

Employee stated EE was supplying the right side of the #451 Roof bolter when EE injured lower back. Employee stated EE was handed a handful of pie pans as EE turned and twisted and felt a sharp pain in lower back. The incident resulted in a lost time accident.

Employee stated ee was riding in the passenger side of a 14 man diesel mantrip. The mantrip was backing into a break to turn around when the right back corner caught the rib jarring the employee and straining lower back.

Employee stated employee was riding in the back of a diesel mantrip when the mantrip hit an irregularity in the mine road causing employee to strain back. The result of the strain was lost work days.

Employee stated they were helping pour oil into the continuous miner. They were pouring a 5 gallon can of oil into another 5 gallon oil can when they felt pain in their back. This resulted in a strain and lost work days.

Employee stated EE was bent over the bucket of the scoop measuring timbers to be cut for the pillar line when a piece of draw rock fell from the top and struck EE on the right shoulder. The rock measured approximately 11.5" x 7.5" x 1.5".

Employee stated EE was installing roof bolts on the No. 2 Section in the No. 5 Entry. EE drilled the hole, inserted the glue and was placing the bolt head into the wrench. EE's left hand was pinched between the bolt/wrench, and the rotation was engaged. This resulted in the amputation of the tip of the left pinky finger, and dislocation and laceration of the left ring finger.

Retreat mining on the #1 Section in the #1 Entry. Lifts 1 and 2 had been mined and while in the process of moving to the 3rd lift, a roof fall occurred. The fall blocked the #1 MRS.

Employee stated EE was lifting the continuous miner power cable above head to hang it against the mine roof when EE felt a sharp pain in the abdomen. This resulted in a hernia. The employee began missing work due to this incident on 12/2/2016.

Employee stated was operating a scoop along the #2 Roadway and hit a hole, jarring employee inside the operators compartment. This resulted in a back strain and lost work days.

Retreat mining on the #1 Section. The CM broke down in the #2 Entry for approx. 2.5 Hrs. The pillar line began to work. Once the CM was repaired, the #2 jack was moved up approximately 5' and reset. As the pressure was relieved from the #1 jack, the gob area began to fall in the #1 Entry and rode over into the #2 Entry onto the jacks. The fall was approx. 19'W x 28'L x 6'H.

Employee stated they were using a come-a-long. The come-a-long was hooked to a roof plate when it fell striking employee on left hand. Employee received medical attention and had to relieve pressure underneath nail bed by puncturing nail.

Employee stated was loading supplies onto the roof bolt machine. As EE was walking back to the scoop to get more supplies, EE slipped on uneven bottom and fell on right knee. This resulted in a strain and lost work days.

Employee was walking across the section when stepped on a rock and fell, landing on left knee and left thigh. The incident was deemed to be reportable on 9/19/2016, due to diagnosis of a contusion and lost work days.

Employee stated ee was operating the shuttle car on the No. 2 Section. EE states ee hit a hole in the roadway, which caused ee to bounce in the operators compartment. This resulted in a strain and lost work days.

Employee stated employee was cleaning out the bucket of the maintenance scoop. A conveyor motor was in the bucket and needed to be removed. The employee pulled on the motor to slide it out of the bucket. When employee did this, employee felt pain in employee's lower back. This resulted in a back strain and lost work days.

Employee stated that employee was loading a drill mast into the back of a diesel mantrip when employee felt pain in lower back. Employee leaned the drill mast against the ride and lifted the other end when the overexertion occurred. The employee sought medical evaluation on 3/21/16, was diagnosed with a strain, and placed on restricted duty.

Employee stated employee was assisting in hanging the roof bolt machine cable. Employee lifted the cable up, and as employee brought employee's hand back, employee's left thumb came in contact with a pie pan. This resulted in a laceration, which required six (6) sutures to close the wound.

Employee stated employee was walking to the #1 Starter Box to put the breaker in. As employee was walking, employee slipped on muddy, wet, uneven bottom, causing employee to twist employee's right knee. This resulted in a strain, and lost work days.

Employee stated they were working on a SC on the #1 Section when they saw a piece of draw rock fall from the top. They state they moved to get out of the way of the falling rock when they struck their left knee against the shuttle car and twisted their right knee. The employee continued to work until 1/18/2017 when they began missing work due to this incident.

Employee stated employee was shoveling along the #2 belt line, and when employee pushed the shovel into the material, employee felt pain in employee's right shoulder. This resulted in a shoulder strain, and restricted duty.

Employee stated was walking past the feeder in a bent over position. As EE went by the corner of the block, struck head against a rock brow. This resulted in a neck strain and lost work days. The employee began missing work due to this incident on 5/10/16.

Employee stated that he was operating the shuttle car in the area of SS3: 2362, there was a hole in the road that caused the shuttle car to suddenly bottom out. This jarring motion caused the employee to deal pain in his back.

Employee stated that he was attempting to hang miner cable when he felt a pain in his lower back.

Employee stated that as he was attempting to close the battery lid on a scoop, he lost his grip on the lid and the lid struck his left hand resulting in a fracture.

Employee stated that as he was operating the C/M while pillaring, a piece of rib sloughed off and struck him on the lower right leg causing pain in his calf and ankle. Employee returned to work his normal job and shift until 8/10 his job class was changed. The employee did not miss any scheduled days.

Employee stated that he was attempting to hang a bulk bag of rockdust while standing beside the scoop bucket. He was stretched out attempting to hook chains through the bag when he felt a pain in his uppper back between his shoulder blades.

Employee stated he picked up a hand full of roof bolts while loading the bolt machine and felt a pain in his left shoulder. Employee was taken off work on 7/31/2015.

Employee stated that he was assisting in building a stopping on #2 section when he tripped over the tongue of the flat car the block was on and struck his right knee on the mine floor.

Employee stated that as he was attempting to untangle 2 chain hoist chains, one of the hooks struck him in the mouth causing a chipped tooth.

Employee stated that he was attempting to hang the high voltage cable during a power move as the power center was being pulled up into place. The CM waterline that was tied to the power center tripped him and he fell, this is when he felt a pain in the upper middle part of his back.

Employee stated that as he was walking to the face area he stepped in a hole causing his left knee to twist. This caused pain in his left knee. The employee continued to work his normal shift until Monday 4/20/2015 when he was taken off work by a doctor for 2 days.

Employee stated that he was walking in the #3 Entry 60' inby SS#:2249 when he traveled through a set of fly pads he slipped and fell, striking his head on the mine floor which caused pain in his neck and left shoulder.

On 12/8/2014 mine management was notified of a retired employee who was diagnosed with an occupational illness of Pneumoconiosis. EE retired on 2/1/2012.

Employee stated that as he was attempting to remove the drill steel from the hole he had just drilled in the mine roof, a section of draw rock fell out and struck him on the head and face.

Employee stated that he was operating the right side of a Fletcher DDR-13 and had just finished installing a rib bolt. He stated that he was preparing to swing the boom inline to drill the next hole in the pattern, when he mistakenly engaged the boom swing out function of the bolt machine.

Employee stated he picked up a bundle of plates and they came apart. While pushing the plates against the frame of the machine with his arms, the corner of a plate cut the inner side of his left wrist resulting in a laceration that required sutures. The employee reported to work for his next scheduled shift and did not miss any work days due to this incident.

Employee stated that he was attempting to remove a bent piece of drill steel from the top when he accidentally engaged the rotation and the bent steel struck him on the right wrist.

Employee stated that as he was hanging a curtain in the #8 entry he felt a sharp pain in his middle back.

Employee stated that he and 2 other coworkers were attempting to hang a section of 6" aluminum water line. When the employee lifted his end of the water line, he felt a sharp pain in his lower back.

Employee stated that he was in the process of installing a rib bolt when his drill steel became stuck in the rib. He stated that he grabbed onto the drill steel and with a jerking motion attempted to pull the steel out of the rib. When he did this he felt a pain between his shoulder blades and down into his left arm.

Employee stated that as he was tramming the diesel scoop in the haul road at break 46 he ran over a rock that caused him to strike his head against the canopy inside the operators compartment causing pain in his neck and back.

Employee stated he was throwing the CM cable out of the roadway when he felt pain in his lower back.

Employee stated that he and another coworker were carrying a timber when he felt a pain in his lower back causing him to fall to his knees. Employee Retired effective 6/23/2014.

Employee was diagnosed with carpal tunnel syndrome in both wrists

Employee was putting a six gallon box of water in the scoop bucket when he felt pain in lower back. The box weighs aprox. 48 lbs.

Employee was diagnosed with Black Lung.

Medical evidence received indicates 10% permanent partial disability as a result of the occupational disease carpal tunnel.

Employee stated after he backed the continuous miner out of a finished cut he turned to walk away and stepped on a loose rock lying on the bottom and twisted his right ankle

Bolter opp. was tramming the bolter from the deck and was trying to back around the corner of 5 L.T. He backed the bolter into the rib and brock the canopy jack that held the canopy up. When the jack broke the canopy fell down striking the employee in the shoulders and back.

The employee stated that he felt something hit the corner of his eye as he was installing a roof bolt. He removed his glasses to wipe his eye and felt a burning sensation in his eye and then flushed his eye but it got worse and he decided to go to the Dr. to have it examined the Dr. prescribed him antibotics to prevent infection and placed the employee off work for two days.

EE stated when he arrived on the section he picked up a 50 lb bulk box of nails and carried them to the section tool shed. He stated he felt pain in his back at that time. He continued to work the rest of his normal shift. The EE called in the next morning and stated he was seeking medical attention. The EE has not returned to work and is currently under doctors care.

Employee stated he was sitting on his heels with his toes pointed down making a belt splice when his left foot slid to the side causing him to fall back. When this happened he felt his ankle or lower left leg pop.

Employee was diagnosed with carpal tunnel syndrome in his Right wrist.

He was backing the miner out of #6 entry. He stopped the miner to take a cable rope off the head. He then stepped in an off set in the bottom causing the accident.

Employee stated that he was loading half-headers on the side of the pinner. He attempted to toss a half-header onto the pinner and felt a sharp pain in his left shoulder. The mine site received notification from workers compensation on 4/25/2013 stating that the employee suffered a rotator cuff tear on 3/14/2013 that will require surgery to correct.

Operator was notified via e-mail on 2/22/2013 that employee was diagnosed with Carpal Tunnel Syndrome.

On 5/25/2012 the operator was notified employee was diagnosed with degenerative disc disease.

Normal - heading out mines, had done went 3 to 4 breaks hit a low spot in roof with his head.

On 12/14/11, the employee stated he was tramming the 550 scoop in the inby direction heading towards the #4 face. He stated he went through a set of fly pads and ran the scoop into the rib causing the scoop to jar and striking his right shoulder and elbow inside of the deck. The incident was deemed a lost time reportable on 1/11/12 for a strain to the shoulder and neck.

Employee has been diagnosed with carpal tunnel syndrome and began missing work due to the illness on 10/7/11.

The employee stated that he was down on one knee tightening a fitting to a new section of water line he had just installed to the miner. As he put pressure on two pipe wrenches he stated he heard a pop in his lower back and immediately felt pain and tingling sensation travel down his left leg. The ee began missing work on 8/30/11 and was diagnosed with a lower back strain.

Employee stated he was removing a 1: shield from the junction box cover on the 251 miner, the shield slid onto his left middle finger and caused a laceration that required 8 sutures to close.

EE stated that while installing an eye bolt, he was holding the bolt and swung the boom of the bolter to the bolt and pushed the wrench onto the bolt but did not remove his finger. The finger tip became caught between the bolt and the wrench causing a avulsion to the finger and a fracture. The avulsion required stitches.

He had put bolt in hole and was putting bolt onto wrench. Then stated that coat sleeve had got caught between the bolt and wrench, he began to spin the head to align the bolt into wrench, sleeve became caught between the 2. it caused his right arm to twist around the bolt wrench. The ee continued to work until 4/27/11, he then began missing work due to injury.

EMPLOYEE WAS BOLTING TOP WHEN A FOREIGN OBJECT FELL BETWEEN HIS SAFETY GLASSES AND FACE AND INTO HIS LEFT EYE. HE WASHED EYE BUT THIS DID NOT HELP. HE WAS TRANSPORTED TO THE SURFACE AND TAKEN TO AN EYE DR.

EE WAS DEPARTING FROM THE CAB OF HIS TRUCK AT THE TIME AND HE STEP DOWN ONTO GRAVEL AND TWISTED HIS ANKLE AT KOPPERSTON TIPPLE DUMP.

THE OPERATOR OF THE STANDARD 10 S/C -32 SHUTTLE CAR WAS TRAVELING IN THE NO. 3 ENTRY HAULING COAL TO THE FEEDER, CRUSHER WHEN HE RAN OVER A PIECE OF ROCK LYING IN THE ROADWAY. THE BUMP CAUSED THE OPERATOR TO BOUNCE FROM THE SEAT AND INTO THE ROOF OF THE S/C CANOPY.

TWO EMPLOYEES ON THIRD WHIFT WERE BOLTING ROOF IN THE NO. 6 ENTRY WHEN ANOTHER EMPLOYEE (WHO WKS MAKING UP ROOF BOLT). HE COULD OPERATE THE OFFSIDE OF THE ROOF DRILL. THE OPERATOR ALLOWED EE TO DRILL THE THIRD ROW OF BOLT SWAS BEINGINSTALLED. AS THE DRILL STEEL BEGAN ROTATINE. THE UPWARD XXX AAPPLIED APPARENTLY WS EX. CAUSING THE DRILL STEEL TO BOW OR BEND. WHEN THE DRILL STEE

EMPLOYEE WAS LIFTING A SHUTTLE CAR CONVEYOR CHAIN WITH A SLATE SAP. WHEN HE FELT PAIN IN LOW BACK.

EE STATED THAT WHILE OPERATING THE SCOOP HE RAN OVER A ROCK OR LARGE PIECE OF COAL. THE BUMP CAUSED HIM TO BE THROWN INTO THE ROOF OF THE SCOOP CANOPY RESULTING IN NECK INJURY.

AT APPROX 8:00 PM THE CRUSHER/PUMP MOTOR ON THE OWENS FEEDER FAILED. THE SECTION FOREMAN AND THE SHUTTLE CAR OPERATOR WERE PREPARING TO ASSIST THE ELECTIRCIAN IN REMOVING THE MOTOR. AS THE TWO APPROACHED THE WORK AREA, THE FOREMAN OBSERVED THE RIB WAS SLOUGHING. HE UTILIZED A SLATE BAR &PULLED DOWN THE PROBLEM RIB THEN THE 2 BEGAN WORKING ON FEEDER. THE RIB SLOUGHED FURTHER HITTING

AT APPROX. THE CRUSHER/PUMP MOTOR ON THE OWENS FEEDER FAILED. THE SECTION FOREMAN AND THE OFF SIDE S/C OPERATOR WERE PREPARING TO ASSIST THE ELECTRICIAN IN REMOVING THE MOTOR. AS THE TWO APPRO ACHED THE WORK AREA, THE FOREMAN OBSERVED THAT THE RIB WAS SLOUGHING. HE UTILIZED A SLATE BAR AND PULLED DOWN THE PROBLEM RIB THEN THE TWO BEGAN WORKING ON THE FEEDER. SOON THE RIB SLOUGHED F

WHILE THE DAY SHIFT OFF STD. SHUTTLE CAR OPERATORS SHUTTLE CAR WAS BEING LEAPED A RIB ROLL. APPARENTLY OCCURRED IN THE NO. 3 ENTRY. THE OPERATORTRAVELED FROM THE MINER TO THE FEEDER, HE RAN OVER THE RIB ROLL CAUSING HIM TO BE THROWN INTO THE ROOF OF THE SHUTTLE CAR CANOPY A CERVICAL STRAIN RESULTED .

EMPLOYEE HAD COMPLETED MAKING A BELT SPLICE IN THE STOCKPILE CONVEYOR STACKER BELT. WHILE RELEASING THE WIRE ROPE HOIST [COME ALONG] ON BELT CLAMPS, THE HOIST HANDLE STRUCK EMPLOYEE'S LEFT SMA LL FINGER DISLOCATING THE FINGER AT THE PROXIMAL PHALANGE.

EMPLOYEE WAS LIFTING A SHUTTLLE CAR CONVEYOR CHAIN WITH A SLATE BAAR WHEN HE FELT PAIN IN LOW BACK.

EE STATED THAT WHILE DIGGING WITH A SHOVEL, AT NO 2 BELT DRIVE, HE FELT PAIN IN HIS BACK. EE INDICATED THAT FOLLOWING THE ONSET OF PAIN HE FELL AND COULD NOT GET UP.

WHILE SHOVELING NO. 2 BELT DRIVE, EE WAY STANDING IN MUD AT HE LIFTED LOADED SHOVEL TO BELT, HIS BODY TWISTED AND HIS FEET REMAINED STATIONARY IN MUD. HIS LEFT KNEE LIGAMENT APPARENTLY TORE.

THE MINER OPERATOR HELPER WAY ASSISTING THE MINER OPERATOR IN MOVING THE CONTINUOUS MINER FROM THE NO.4 TO NO.3 ENTRY. WHILE PULLING ON THE 2/0 SHIELDED CABLE, THE EE FELT PAIN IN THE GROIN AR EA.
