EE went to step out of EE's work truck and stepped in a hole and twisted EE's ankle.

Employee was replacing the barrel on 251 pump, and had trouble getting the value removed so EE got a slate bar and began to pry on it, the slate bar slipped mashing the right ring finger, resulting in a fractur to the tip of EE's finger

Employee stated was using a utility knife to trim a screen panel for the Unit #224 Screen when the knife slipped and EE cut their upper left thigh. The employee received 3 sutures.

Employee stated they were assisting in the installation of a drainage ditch, positioned up hill to spot for the dozer. The dozer moved some material that had an old bent up section of gas line in it and when this happened, it caused the other end of the gas line to shift and it struck the employee across the back of their shoulders.

Employee stated that they were stepping out onto the crawlers of the 325 CAT Longarm when the cat pad they stepped on shifted causing them to lose balance and they fell off the crawlers to the ground injuring their head, back, and neck.

Employee stated was mounting the 980G Front End Loader (C/N#21105), when EE go to the 2nd step and fell. EE fell backwards landing on left arm causing it to break.

Took a big step out of skytrack and knee popped.

Witness stated that themselves and another were in the process of moving a speed reducer across the 4th floor of the prep plant. Witness was using a slate bar to walk the speed reducer and injured employee was pulling the speed reducer with a strap. Injured stated that on the 5th pull they felt a sharp pain in lower back.

Employee was working on a D10 dozer idler when employee was trying to get out employee had lifted self up and sat on the push arm and twisted body to throw feet up on the push arm when employee felt pain in lower back.

Employee was struck by a hood from a 777C truck. Employee lifted hood up to work inside of motor compartment. Hood was missing a safety latch. Gust of wind blew and the hood fell over striking the employee on crown of head. Employee was transported to area hospital where the employee received 8 staples.

Employee was setting a dewatering pump along a beltline. While trying to get the pump into the correct position the latch on hook broke. The pump moved causing the employee to slip and employee's left knee struck the pump.

Employee was lowering down a counter weight when they slipped on spillage. Employee worked up until 10/31/2019...went off on comp on 11/01/2019.

Employee stated EE was working to remove suction line on the bottom floor. Employee stated EE was standing in coal fines when EE turned to reach for a wrench but EE's foot remained stuck causing EE to twist EE's knee and ankle. Employee reported incident on 5/8/2019 and sought medical treatment. Doctor placed injured employee on restrictions due to sprain ankle until 5/13/2019

Employee stated EE was grinding a piece of metal, when EE felt debris/metal go into EE's eye. Employee went to the Dr. and received prescription eye drops, making it a medical reportable.

Employee stated employee was in the process of changing a sump pump. As employee walked around the sump employee felt pain in employee's foot and fell to the ground in pain. This resulted in a tendon strain.

Employee was dismounting the 980 loader when a foot slipped and employee fell to the ground. Employee landed flat footed on the ground and felt pain in the right knee. Employee was diagnosed with a right knee strain and taken off work. The employee began missing work on 3/21/17 due to this incident.

Employee stated EE and another employee were loading a spool of cable into the bed of a truck. The spool was lifted with a forklift, and EE guided the spool into the truck. EE was holding the end of the cable with left hand. When the forks lowered, EE's left arm was pinched between the forks and truck bed. This resulted in a contusion and restricted work days.

Employee was moving screen when he smashed finger against screen and angle iron

On September 19, 2014 mine management was notifiedofan occupational illness. The emplyoee was awarded benefits due to occupational pneumoconiosis.

Pulling a cylinder pin from a 777D Rock Truck when a bolt shot out hitting him in the face

Employee was diagnosed with Occupational Pneumoconiosis.

Operator received notification on 3/18/2014 the employee has been diagnosed with black lung. This employee retired on 1/31/2014.

Employee was using a cutting torch to cut metal and his glove became saturated with oxygen and apparently a spark from the hot metal ignited the oxygen causing his hand to be burned.

Employee was diagnosed with hearing loss

Employee was working on the #1 slurry pump and while walking across the floor, he tripped over the forklift fork and fell to the ground. His right hand struck the spare water pump. The employee continued working but was diagnosed with a fracture and placed off of work on 3/8/2013.

The dozer operator entered onto the clean coal stockpile in preparation to push coal to the number 325 feeder to load a train. The stockpile had a void over the feeder and the d-9 dozer that he was operating slid sideways into the void. The operator struck his head inside of the cab, this injury resulted in 4 staples on his head.

Employee was pushing raw coal on the #1 stacker as he backed up and turned the dozer to the right, the dozer became stuck in the #1 variable feeder. There were no injuries with this occurance

Medical evidence received indicates 1.3% hearing loss.

On April 17, 2014, Employee was notified of a partial permanent disability involving carpal tunnel in his wrists.

Medical evidence received indicated a 0.183% hearing impairment.

Employee was diagnosed with a hearing loss.

Employee stated while moving safety chain from counter weight structure of U-50 refuse belt he was moving loose end of chain over top of sheeting. Employee stated at this time he believes his small finger tip went thru a hole in the chain link and was struck against sheeting girt or beam at the same location.

Employee Dx - Coal miners Occupational Pneumoconiosis.

EE stated as he was exiting the 463 bob cat, he felt a pain in his thoracic area of his back. He stated that he took some advil to relieve the pain, but after 2 hrs he states that it is feeling worse. He aslo stated that when he was moving scrap vessel chain around yesterday (10/30/08), he may have aggravated his back.

Employee states that while removing splash guard from the 265 R.C screen, the guard slipped causing employee to lose his grip. When the splash guard assembly dropped, it caught his right hand between the screen side plate and guard. Employee was wearing leather gloves.

Employee states he was installing a u-bolt style pipe hanger and as he raised it in place he felt a pain in his shoulder. Employee did not miss work until 5-10-08 due to his regular work schedule.

The employee has been diagnosed with occupational hearing loss.

EE came out early & was cleaning belt sprays on the #6 truck dump belt.EE turned & the belt off and climbed up on the structure to access the spray bar,he was climbing he slipped & fell to the ground,twisting his left ankle & brusing his left heel.He reported the accident to his normal boss 3hrs later,he continued to work his shift.

EE was closing feeder gates inside the clean coal reclaim tunnel. he then let up on the ring tinking it was still latched on the hook and he claimed when the gate closed his hand was in the ring causing his left arm to be pulled up overextend and resulted in an injury to his left shoulder.

The employee was climbing from the feed end toward the discharge end of the unit #224 course refuse screen to check the oil levels. His foot slipped and he fell nearly seven feet onto the plant floor below. He was taken by ambulance to the ER where he was treated and released. He started missing work on Saturday, April 28, 2007.

Upon an investigation the dozer operator was pushing/spreading coal to permit an additional storage radius around #2 stacker. During pushing/spreading the coal an undetected bridged area broke free creating a void at the #4 fixed feeder, the dozer proceeded to slid down into the void backwards and rested in a vertical position.

OP Claim: The Occupational Pneumoconiosis Findings (02/06/07) to justify a diagnosis of OP. The company received the medical on March 27, 2007.

EE was leaving the pipe room and tripped in the walkway and caught himself on the wall, causing pain in his right shoulder, left and right knees. Diagnosis of the shoulder was bones and soft tissue unremarkable with no evidence of fracture or dislocation. The right knee was diagnosed with degenerative changes with periostic ostephytes. He started missing on 11/11/06.

Walking down truck parking long on graveled slope and twisted ankle.

Electrician was helping move scaffolding and pinched his hand between the 2 sets.

Welding angle to a beam and brought welding rod down some how it went through his arm and hit an artery.

EE stated he was cutting a piece of metal at the metal rack. While working, he stepped on a piece of angle, which turned or rolled and this caused him to twist his knee. EE finished his shift.

EE stated he was desending on walkway from unit 92 refuse belt, stepping to ground level. EE missed last step. EE stated that the lights were to his back he misjuged the last step and sprained or twisted his left knee.

Two employees were in the process of unloading two boxes of metal clamps from the back seat of a SUV to transfer the boxes to a pick up truck so they could be delivered to another location. He slid one of the boxes out, then turned to allow other room to assist him in lifting the box. He thinks the box tilted and the weight of the box shifted, then felt pain in his lower back.

EE stated that he and another ee were replacing the splash sheild to move it into position to be bolted. The pry bar slipped and/or the injured ee lost his grip on the bar causing him to fall backward striking his wrist on the floor and the apex that had been removed from the b-3.

Diagnosised with carpal tunnel syndrome.

Employee was lifting 3 1/2" pipe (rigid conduit) when he strained his stomach. The injury was reported but follow up was not done till 9/19/2005. At that point employee had been laid off effective date 8/26/2005.

Employee was measuring the froth dosage rate for the B-side cells. He felt something splash on his face. He wiped his face with a rag and went back to his job. After finishing his shift he showered; afterwards he felt a slight stinging in his right eye. When he got up the next day his eye was very red and appeared to have two small blisters on the right corner.

Employee stated he was using a utility knife to strip the insulation from a wire, the knife slipped, he cut the top of his left index finger.

Cleaning components from transmission using air cleaning gun and dirt went under safety glasses and into right eye.

DIAGNOSIS OF CARPAL TUNNEL SYNDROME.

Employee stated he was walking across floor by the new side sump pump when he slipped on some magnetite that was in the floor, twisting his left knee. The employee went to the doctor on 08-03-04, he stated he still had pain in his left knee. The employee started missing work on 08-03-04 under doctors care.

MACHINERY REMOVED FROM COUNTERWEIGHT CAUSING CLAMPS TO LOOSEN. LOST SLACK.

EMPLOYEE STATED HE WAS REMOVING AN 8"PULLEY SHEAVE BLOCK FROM TROLLEY BEAM LOCATED OVER PLANT FEED BELT. EE STATED THIS CAUSED PAIN IN HIS LOWER BACK.

EE HAD DEGENERATIVE DISC DISEASE OF THE BACK. HE HAS BEEN TREATED FOR THE CONDITON SEVERAL TIMES IN THE LAST 3 YRS. ON 5/11/04 EE STARTED MISSING WORK DUE TO THIS CONDITION.

EE WAS HELPING INSTALL A PUMP MOTOR PUSHING MOTOR IN PLACE. HE FELT PAIN IN LOWER BACK.

EMPLOYEE STATED HE WAS DISMOUNTING THE 966 LOADER HE INTENDED TO STEP OVER TO THE BANK, HE MISSED THE STEP ON THE BANK. HE GRABBED THE HAND RAIL WITH HIS RIGHT HAND, HE FELT SOMETHING PULL IN HIS NECK. HE SAID HE FELT A BURNING SENSATION IN HIS NECK AFTER A FEW MINUTES.

WORKING ON A BROKEN STICK ON AN EXCAVATOR. WHILE TRYING TO SEE IF A PATTERN FIT THE MACHINE, THE MACHINE SETTLED AND PINCHED FINGER BETWEEN THE BUCKET AND STICK.

EMPLOYEE STATED HE WAS WELDING OVER-HEAD, WHEN A HOT PIECE OF METAL FROM THE WELD FELL BURNING A HOLE IN HIS SHIRT. HE WAS HIS SHIRT BURNING AND SLAPPED IT, CAUSING THE HOT METAL TO MOVE FURTH ER DOWN INSIDE HIS SHIRT, IT REACHED HIS WAIST. THIS CAUSED HIM TO HAVE SEVERAL SMALL BURNS FROM HIS CHEST TO HIS WAIST.

OCCUPATIONAL HEARING LOSS.

EMPLOYEE STATED HE WAS CLOSING A RAILROAD CAR DOOR WITH A RAILROAD DOOR BAR. AS HE WAS DOING THIS HE FELT DISCOMFORT IN HIS RIGHT GROIN AREA. HE HAS SLIGHT SWELLING IN THIS AREA.

OCCUPATIONAL PNEUMOCONIOSIS. DATE OF DIAGNOSIS 05-15-03. DATE DIAGNOSIS RECEIVED 09-18-03.

EMPLOYEE STATED HE WAS DRIVING THE NUMBER 906, 777 TRUCK. WHEN HE WENT THRU A HOLE ON THE REFUSE DUMP AREA CAUSING THE STEERING WHEEL TO JERK. WHEN THAT HAPPENED HE FELT A PULL IN HIS RIGHT EL BOW, CAUSING PAIN. PLEASE NOTE: EMPLOYEE DID NOT START LOST TIME UNTIL 5/12/03.

EMPLOYEE WAS CHANGING STARTERS ON A 777D CAT TRUCK. FEET WERE MUDDY. FEET SLIPPED FORWARD WHILE REMOVING STARTER, STARTER DROPPED TRAPPING RIGHT HAND BETWEEN STARTER AND THE FRAME OF THE MACHI NE.

EE STATED THAT HE HAD BENT OVER AND THEN COULD NOT RAISE BACK UP. AFTER A FEW MINUTES HE WAS ABLE TO RAISE UP. COMPLETED ASSIGNED WORK FOR THE DAY BUT WAS FEELING A LITTLE MINOR DISCOMFORT. EE HAS A HISTORY OF BACK TROUBLE. X-RAYS GAVE THE IMPRESSION OF "DEGENERATIVE DISC DISEASE" L5-S1 EE STARTED MISSING WORK ON 10/05/02.

EE IS MISSING WORK DUE TO DEGENERATIVE DISEASE OF THE LEFT KNEE. HE HAS HAD KNEE SURGERY TWICE ON LEFT KNEE.

THE EE STATED HE WAS ATTEMPTING TO OPEN THE DESLIME PROTECTIVE SCREEN OVER B WING TANK WITH A 10" SECTION OF 1/2" RIGID CONDUIT. THE TOP OF THE CONDUIT CAUGHT HIS HARD HAT AND SCRAPED THE TOP OF HIS HEAD.

THE EMPLOYEE REPORTED TO HIS SUPERVISOR THAT DURING HIS ACTIVITIES OF OPERATING THE SLATE TRUCK THAT HE HAD AN INCIDENT WHICH CAUSED BACK PAIN. X-RAYS TAKEN WERE NEGATIVE. THE EMPLOYEE HAS A H ISTORY OF DEGENERATIVE DISC DISEASE.

THE EMPLOYEE STATED HE WAS CLEANING THE LUNCH ROOM ON THE FOURTH FLOOR OF THE PLANT WHEN THE MOP BUCKET TIPPED OVER AND SPLASHED SOME DETERGENT WATER ON HIS LEG. THE EE DID NOT REPORT THIS ON THIS SHIFT. THE EMPLOYEE HAS A PRE-EXISTING CONDITION ON HIS LEG WHICH WAS IRRITATED. THE EE STARTED MISSING WORK ON 9-27-01.

EE STATED HE WAS HELPING REPLACE A PIPE FITTING BY USING TWO PIPE WRENCHES AND ONE OF THE WRENCHES SLIPPED ON THE FITTING AND CAUGHT HIS FINGER BETWEEN THE WRENCH HANDLE AND A SHEETING GRID AG AINST THE WALL.

ON THE LOCATION WHERE SOME OLD DRYERS HAD BEEN TAKEN OUT FOR NEW DRYERS; NEW CHUTES WERE BEING INSTALLED. ONE OF THE CHUTES TURNED OVER AND THE EMPLOYEE TRIED TO HOLD IT FROM HITTING THE FLOO R HARD ENOUGH TO DAMAGE THE CERAMIC LINING. HE DID NOT EXPERIENE PAIN AT THIS TIME, BUT HAD A KNOT ON HIS GROIN AND PAIN THE NEXT DAY.

THE EE STATED HE WAS WORKING ON INSTALLING THE COVER ON THE CLEAN COAL CRUSHER WHEN HIS RIGHT INDEX FINGER BECAME CAUGHT BETWEEN THE COVER AND THE BASE. EE WORKS ONLY ON WEEKENDS. DID NOT LOOS E ANY TIME.'

EE STATED THAT HE WAS COMING DOWN THE STEPS FROM THE PLANT SAMPLER ON THE CLEAN COAL STORAGE BELT, WHEN HE SLIPPED ABOUT 6 STEPS FROM THE BOTTOM OF THE STEPS. HE FELL DOWN TWO STEPS TO THE CON CRETE PAD, HIS LEFT LEG TWISTED AND CAUGHT UNDERNEATH HIM.

WALKING, HIT HEAD AGAINST ROOF BOLT, FELL BACK LANDING ON RESPIRATOR.

DESCENDING STEPD FELL APPROX 15 STEPS.

EE SUFFERED A HEART ATTACK WHICH WAS RULED AS A DEATH FROM NATURAL CAUSES BY THE ATTENDING PHYSICIAN.

(ONLY WORKS WEEKENDS) THS INCIDENT WAS DET ERMINED TO BE A REPORTABLE INJURY ON 12-28-1999. A SERIES OF TESTS HAD TO BE CONDUCTED TO RULE OUT A PRE-EXISTING MEDICAL PROBLEM.

EMPLOYEE STATED HE WAS STRIKING HEAVY VESSEL CONVEYOR FLIGHT WITH A SLEDGE HAMMER TO STRAIGHTEN THE BENT FLIGHT WHEN IT FLIPPED UP AND STRUCK HIS LIP.

EE STATED HE WAS THROWING CABLE HOIST INTO END LOADER BUCKET TO TIER OFF MAST WHEN A LADDER FELL AND STRUCK HIS ARM.

EE SAID HE WAS WORKING WITH SC2000 FOR ABOUT 9 1/2 HOURS AND GOT SICK.

THE EE STATED HE WAS OBSERVING ANOTHER EE WORK WHEN HE SHIFTED HIS WEIGHT HE FELT A PAIN IN HIS LEFT KNEE.

WORKING WAS IN SWING SEAT AND COWORKER USED CABLE DOWN CONTROL. VICE GRIPS CAUGHT THE BOOM UP LEVER CAUSING BOOM TO RAISE PULLING WORKER BACKWARDS INTO CONTROL LEVERS INJURY LOWER BACK.

EE STATES THAT HE WAS DISMOUNTING FROM HIS D-8 DOZER AND HIS FOOT SLIPPED CAUSING HIM TO PULL HIS BACK. EE STARTED MISSING TIME AT A LATER DATE. RECEIVED MEDICAL NOTES 2/1/99 AND DETERMINED RE PORTABILITY AT THAT TIME. ACCIDENT IS STILL UNDER INVESTIGATION.

EE WAS ATTEMPTING TO ENTER DOORWAY OF LOADOUT BUT WAS BLOCKED BY CJX PERSONNEL. WIND BLEW DOOR SHUT CATCHING HIS LT INDEX FINGER. THIS REPORT WAS FILED TO ABATE CITATION. EACC RESERVES THE RIG HT TO CONTEST THIS REPORT WITH REPORTABILITY ISSUES DETERMINED BY CONFERENCE.

EE STOOD LADDER UP IN WORK LOCATION, CLIMBED TO TIE LADDER OFF W/OUT HAVING BOTTOM OF THE LADDER HELD. LADDER SLIPPED OUT, EE FELL, LANDING ON HIS FEET.

SLIPPED AND FELL ON TOP STEP OF STAIRWAY LANDING ON RIGHT ELBOW.

A DRILL FELL FROM AN I-BEAM ADN STRUCK ARM BIT FIRST LEAVING A CUT.

HIT TOP OF LEFT HAND WITH HAMMER WHILE WORKING.

EE STATED HE WAS UNLOADING 6" STEEL PIPE FROM A 4-WHEEL CART. WHEN HE PUSHED THE PIPE OFF, IT LANDED ON HIS RIGHT FOOT.

EE STATED THEY WERE REMOVING A HYDRAULIC JACK ATTHE AUGER BUILDING. INJURED STATED WHEN HE UNHOOKED THE BUCK HEAD FITTING THE JACK FELL OFF THE HOPPER STRIKING HIS LEFT FOREARM CAUSING A LACER ATION.

EE STATED HE SUSTAINED A LACERATION TO HIS RIGHT INDEX FINGER WHILE TIGHTENING BOLTS ON THE SKIRT RUBBER OF THE CLEAN COAL COLLECTING BELT. HE STATED THAT AS HE WAS TIGHTENING A BOLT THE SOCKE T HE WAS USING SLIPPED OFF THE NUT CAUSING HIS RIGHT INDEX FINGER TO STRIKE THE EDGE OF THE BELT STRUCTURE.

EMPLOYEE STATED HE WAS GOING TO CLEAN OUT TH EFLOOR DRAIN AT THE DUST COLLECTOR, WHEN HE LIFTED THE GRATING IT FELL BACK DOWN CAUSING A CUT ON HIS LEFT HAND MIDDLLE FINGER.

EMPLOYEE STATED THAT HE AND 3 OTHER EMPLOYEES HAD PLACED A JIG APRON ON A SHEET METAL CAR AND WERE PUSHIGN IT ACROSS THE FLOOR WHEN THE CAR TURNE DOVER, PINNING HIM BETWEEN THE HANDRAIL AND A PRON.

EMP STATED HE WAS CLEANING OUT A CMI AND HE WAS KNEELING DOWN ON LEFT KNEE. LATEER ON HE SAID HIS KNEE WAS BOTHERING HIM. WENT TO DR ON 11-21-96AND HAD FLUID DRAINED OFF KNEE. THIS WAS NOT REP ORTED UNTIL 11-21-96.

EE STATED HE HAD HEATED THE INTER RACE OF A MOTOR BEARING ON TEH DRIVE AND OF AN ELECTRIC MOTOR IN PREPARATION TO CHANGE THE BEARING. AS HE WAS TAPPING THE FACE THE SLIVER OF METAL BROKE LOOS E AND WENT THROUGH HIS SHIFT LODGING IN HIS STOMACH. (METAL WAS APPRXO 1/4"LONG AND AS BING AROUND AS A PIN HEAD). AN INCISION HAD TO BE MADE TO REMOVE METAL WHICH RQUIRED SUTURES.

EE STATED HE WAS ASSIGNED TO DRIVE THE R-50 TRUCK. HE SAT IN THE CAT 773B TRUCK TO RIDE 1/4 MILE TO THE R-50. HE PLACED HIS ARM ACROSS THE BACK OF THE DRIVER'S SEAT. WHEN THE DRIVER SHUT HIS D OOR CLOSE, HE CAUGHT EE'S THUMB IN THE DOOR HINGE POINT.

EMPLOYEE STATED A TRUCK WAS DUMPING BESIDE OF HIM. WHEN THE TRUCK BESIDE OF HIS TRUCK THE TAILGATE SLAMMED SHUT THE AIR COMPRESSION CAUSED DAMAGED TO HIS LEFT EAR.

EE STATED HE WAS TURNING FORK LIFT ATTACHMETN OVER TO UPRIGHT POSITION WHEN HE CAUGHT HIS RIGHT RING FINGER BETWEEN FORK ARM AND BACK PLATE.

INJURED STATED HE WAS CUTTING 2 OLD DRIVE BELTS OFF THE MOTOR. WHEN HE CUT INTO THE BELTS, THE KNIFE STRUCK HIS LEFT LOWER ARM.

EMPLOYEE STATED HE WAS CUTTING 2 1/2 GALLON PLASTIC WATER BOTTLE APART WHEN KNIFE SLIPPED AWAY FROM THE BOTTLE AND STUCK IN HIS STOMACH.

WORKER WAS PREPARING TO CLEAN LAB COL CRUSHER. HE RAISED CRUSHER LID. WHICH RAISES BELT GUARD ON CRUSHER, THE BELT GUARD WAS FORCED UP UNDER LAB SINK. SPRINGING LID BACK DOWN. THE WORKERS RIGH T HAND WAS CAUGHT BETWEN CRUSHER LID AND BASE OFCRUSHER. BONE RIGHT HAND BROKEN.

AS HE LANDED ON FEET HE STATED HE FELL BACKWARDS CATCHING HIMSELF WITH RIGHT HAND. AS A RESULT EMPLOYEE INJURED HIS NECK SHOULDER & MIDDLE BACK.

EE STATED HE WAS CLEANING A PAINT TRAY WITH EQUIPMENT CLEANED WHEN SOME LIQUID SPLASHED UNDER HIS SAFETY GLASSES INTO HIS RIGHT EYE.

EMPLOYEE STATED HE FELT PAIN IN HIS LOWER BACK WHILE HOLDING A METAL SHAFT.SUPERVISOR WAS NOT NOTIFIED UNTIL 01-17-91 OF THIS INCIDENT.

EMPLOYEE STATED HE HAD REPLACED A FAN BELT ON THE ROM FAN.HE STARTED FAN MOTOR TO CHECK BELT ALIGNMENT PRIOR TO REPLACING GUARD.AS THE BELT WAS COASTING TO STOP HE REACHED FOR HIS CAP LIGHT AN D CAUGHT HIS FINGER BETWEEN BELT AND PULLEY.
