Examiner found roof fall with msha inspector while doing weekly exam, fall was 25' wide x 30' long x 6 ' thick area will not be cleaned up, escapeway was moved around the affected area, was dangered off an additional support will be added as needed

Employee lost suction on the drill pot, stepped to the back of tray to check suction, then went back to the canopy when a piece of rock fell, striking them on the right side of ear, neck, shoulder area. They went to ER and needed 3 or 4 sutures on their ear and was released back to work. Accident location #2 entry 001 section 144 feet inby spad 12328.

Employee was diagnosed with occupational pneumoconiosis on 10/19/25 we received the notification of the diagnosis on 4/20/2026

EE was diagnosed with occupational pneumoconiosis by a B reader on a chest x ray . EE hasn't worked at our operations since Oct 2023

Employee raised shuttle car up with service jack that's attached to the car, placed part of crib block under jack and a crib under operators deck, was welding a nut onto drag link so EE could pull on it, the crib block failed that was under jack allowing it fall enough to hit EE's hip area causing hairline fractures to pelvis area.

Employee was diagnosed with occupational pneumoconiosis, on 6/20/2025 by a certified b reader for chest x-rays , EE has continued to work.

Employee was working with others to anchor the tailpiece, after drilling the inby hole on the offside EE was moving drill to the outby hole when EE tripped and fell catching themself with left hand causing a laceration on left pinky finger on left hand requiring 2 sutures

Employee was sliding rub rail out of the way. Rub rail slid off the block that was supporting it and EE's finger was caught between the rub rail and the block causing a laceration requiring 6 sutures.

While handloading block into scoop bucket, employee felt pain and a pop in right shoulder. Continued to work until 2-5-25 when employee was taken off work.

Employee was diagnosed with occupational pneumonoconiosis on 10/28/24 by a B reader , EE hasn't worked since 8/23/24 due to EE's vision problems

Roof Fall was discovered 9/17/24 during weekly exam with state inspector on 4 C Belt bk 28. Fall is in intersection, will not be rehabbed, not blocking travel, isn't in an escapeway. Fall is 5' thick x 20 long x 15 wide. Area was dangered off. No injuries. Note: Both agencies are aware of the roof fall.

Two rope bolts had gotten lodged behind drill boom on bolter. EE extended the boom and was pulling on rope bolt when the second one came loose and struck EE in the lip requiring 6 sutures to close the laceration

Employee was diagnosed with Occupational Pneumoconiosis on 7/11/24 we were made aware of EE's condition recently

Employee was unloading bolt supplies from the roof bolter for repairs, while moving a stack of roof straps EE's left ring finger was caught between the straps resulting in a laceration requiring 5 sutures to close the wound Equipment Type

Tramming scoop in track entry had rt arm arm outside operator's compartment, ran scoop into rib catching arm between scoop and rib causing laceration, resulting in sutures needed to repair the wounds

Bending 5 foot roof bolts to make it easier to install in lower mining conditions employee felt pain in left shoulder, mining height was 55 inches

EE put the pusher into drill pot, put finisher in hole, was suing up pressure to match up the two pieces of steel when EE said slipped and applied too much up pressure and pinched right index finger between the finisher steel and pusher resulting in 16 sutures and taken off work on 8/11/23.

Employee was installing a 10' cable bolt in the 6 entry when the extension and finisher got stuck in the hole as employee was reaching to remove the pusher from the drill pot the extension and finisher fell out of the hole striking employee on the right pinky finger causing a laceration requiring sutures

Employee was replacing a damaged bolt in the #9 entry before bolting the 20' cut, when a piece of rock hit EE in the back. As EE tried to get away another piece of the same rock fell , hit EE in the lower right leg causing a compound fracture. Total size of rock when measured was 4' W X 94" long X 7" thick. Only a portion of the rock struck the employee.

Employee was tramming the left bolter from the inch tram control to install the last row of bolts when a piece of rock measuring 13 1/2' fell and struck the employee on the left arm and hip causing a bruise and scrape. EE went to Dr on 5-22 and Dr gave EE 2 days off work.

Employee was bolting in the #5 entry, when removing the steel from the roof the finisher separated from the pusher allowing the pusher to come out of the chuck striking EE in the safety glasses on left side of the face, EE was taken off work at this time

Employee was bolting in the #1 entry on last row of bolts as EE placed EE's starter steel in the pot and pushed it up a piece of bone 15x10x1 inch fell striking EE's right hand causing a laceration requiring 3 stitches

Employee was helping remove a set of batteries out of battery tray on surface using a lifting beam hooked to the forklift, two other jacks was being used on each side to aid in raising battery out of tray, as they bent over to move the jack the lifting beam came unhooked hitting employee in the face, employee was taken to hospital and diagnosed with sinus cavity fracture

We were notified by MSHA by letter this employee is now a part 90 miner on 03/03/2023 and needs to work in low dust area.

EE was walking down #4 entry to get a roll of fly pad. EE bent over to pick up the fly pad, draw rock fell hitting EE in head and back & knocking EE into the muddy floor. Rock measured 12" wide x 24" long X 11/2" thick, EE was taken to hospital and found to have no broken bones but was taken off work until seen by specialist

Employee had finished setting bits on #1 cm 6322 and was picking up unused buckets of bits off c/m and felt sharp pain in left shoulder. EE didn't cease work or miss any time until 6-6-23 when EE had surgery to repair the left shoulder and was taken off work then.

EE was walking to drop cable slack in preparing for power move, an electrician was trouble shooting reel on the #2 car, when the electrician pecked on the valve the cable reel engaged as EE was stepping across cable, it came up striking EE on the mouth area causing laceration to lip requiring couple stitches.

Employee was bolting in 3 left, as EE was drilling EE put the finisher steel in the hole EE tried to guide the pusher steel to connect the steel with left hand as EE was raising drill pot EE pinched hand against mine roof causing laceration requiring stitches

Employee and the foreman were putting up belt hangers in belt entry, after installing EE's hanger, employee swung out to install a hanger for the waterline once this was done EE swung the boom back in, EE's hand was caught between canopy and mine roof causing a injury to EE's hand

Employee was roof bolting the mine roof in # 7 entry when a piece of rock measuring 8 inches long x 5.5 inches wide struck EE on the forearm causing a laceration that required stitches to close back up

Employee was kneeling down as they were cutting 8 right when a portion of inseam bone in the coal rib fell out striking them in back area causing a simple fracture to a unspecified vertebrae in spine. EE was taken to hospital and released back to work with a follow up appointment with a orthopedic doctor on 8/26/22.

Employee was found unresponsive sitting on EE's four wheeler, outby the #4 section EE was given CPR and taken to the surface then transported to Hospital by ambulance. The ER Doctor stated EE died of Cardiac Arrest. This should be listed as Degree 8 according to Part 50 reporting because dying of natural causes.

Employee was diagnosed with Occupational pneumoconiosis on 5/2/22 by chest x ray. We were notified on 6/20/22 about the employee's diagnosis.

Employee had just bolted #7 heading and some T5 channel 16 feet long straps fell over that was stored on the bolter as EE let the ATRS down. They struck EE on the right lower leg causing fractures to EE's leg, surgery was needed to repair the bone fractures.

EE was installing a draw head on a flat car, as EE was lifting the draw head over a ledge on the car EE felt pain in lower back. EE continued to work until 7-20-22 when EE was taken off work by the Dr and prescribed physical therapy.

Employee was removing a 5 foot bolt from the tray of the roof bolter when the employee struck left ring finger against the bolt head and the ATRS causing a simple fracture to the tip end of finger

Employee was Fast Feeding the drill pot back down to put in EE's two piece drill steel and EE inadvertently got EE's left leg/knee too close to the drill pot and it struck EE in the knee while EE was bringing the drill pot back towards the ground causing a fracture to the tibial bone

Injury date 2-24-22 employee was checking the #3 face as they started to change direction, rt knee popped, they continued to work until they had surgery on 4-21-22, EE wasn't scheduled to work on 4-22. EE had scheduled vacation prior to having surgery from 4-25 thru 5-2 they returned back to work on 5-3-22.

Employee was helping change 1000' of belt when the blue rope got hung on bottom hanger. EE flagged scoop to stop and went over to free the rope when the chain holding sheave wheel broke allowing sheave wheel to hit EE in the left side. EE was taken to hospital and released back to work but now is off work. All scans and test was negative when done at hospital

Employee was standing at the dump end of the shuttle car checking why the feeder stalled out, scoop was cleaning the center dump as operator got out of scoop EE hit the PTO lever and it shot oil into EE's left eye behind EE's safety glasses

Employee was installing a 5 foot bolt when they straighten the bent bolt to push it into drill chuck they caught a pinky finger between the bolt head and drill pot resulting in a fracture to their finger

Down on EE's knees pulling fly pad off the curtain board when EE felt a pop in EE's chest, EE finished EE's shift then went to hospital next day, was diagnosed with strained chest muscles told to remain off work at least two weeks

EE was loading in 2 left crosscut when the roof dribbled, EE jumped up too quick and twisted EE's right knee. EE was taken to hospital and released, EE followed up with a specialist who took EE off work for further treatment

Employee was getting into deck of roof bolter and twisted rt knee. EE has worked since the incident, but now has been taken off work on 10-11-21 for further treatment.

While installing a roof bolt, employee was straightening the 5 foot bolt to be pushed up into the hole when the bolt twisted causing a pop in left shoulder

Employee had chest x ray done on 7-1-2020 ,it was read by a B reader at that time, we were notified by email on 5/18/21 that employee was diagnosed with occupational pneumoconiosis.

Was picking up miner cable to hang, felt pain in lower back

As EE was exiting the back of the HDDR roof bolter in the #3 heading, EE's foot slipped off the landing and EE landed on EE's left foot and ankle causing a fracture.

Employee tripped on a rebar mat and bumped EE's shin on an adjacent steel beam.
