Nelson Brothers detonated an explosives blast in the R3 Coal of the 6-West Pit of Coal Creek Mine. Burning coal dust was carried by north winds and ignited a grass fire south of the pit perimeter, at the fence line between Coal Creek Mine and Dennis L. Edwards. The fire burned 12.1 acres of prairie in the northeast corner of the Edwards property.

Employee had stepped over the tongue of a skid steer trailer carrying 3 rolls of cord. As the employee's second foot contacted the ground after step over, his foot slipped and he lost balance causing him to fall backwards onto the tongue of the skid steer trailer fracturing three ribs on his right side. Walking surface was melting snow and ice.

THE INJURED WAS PUTTING THE DRIVE SHAFT BACK ONTO A HAUL TRUCK. WHILE LINING UP THE SHAFT ON THEOUTPUT FLANGE THE SHAFT SLIPPED, FELL AND PINCHED THE LAS DIGIT OF HIS THUMB BETWEEN THE U-JOINT AND THE FRAME CROSS MEMBER BELOW. THIS BROKE THE LAST DIGIT INTO 8 PIECES.
