Was hauling out excess powder from storage tank. Was using rod tool to A-line loading spout to opening in top of the dust truck. Was pulling rod toward them when rod slipped off the spout, momentum carried them off the platform they were standing on. Platform is a portable platform with steps and handrailing on both sides. They landed on the froze ground and broke Right Ankle.

Employee was using a utility knife to cut a belt to make a skirt. Was holding the straight edge that EE was cutting against with EE's hand. Knife came out of the belting material and punctured EE's right-hand wrist. Direction of cutting was towards EE.
Employee cleaning belt conveyor take-up rollers on house belt #5 with a scraper. Employee was performing cleaning task while conveyor belt was running.

Employee was housekeeping and while doing this hit head on an electrical box in the work area. Employee was not wearing hard hat.

Miner attempted to step on a short ladder leading to a conveyor service platform. Left foot slipped on ladder rung and right shin and knee hit the service platform mezzanine causing trauma to the knee cap. Miner did not have left foot in full contact with the ladder rung, only the tip of EE's boot. Ladder missing hand extension on the West side so did not have 3pt. contact.

While draining a leaking hydraulic hose, oil sprayed onto employees face, entering employees right eye causing irritation.
While cleaning material with a walk behind skid steer, employee's left hand became pinched between the hand controls and a beam. Index finger was cut requiring 2 stitches.

Condition was a hot humid day, no air movement in the area of cleaning. Drank water but not enough and didn't take multiple breaks during activities of housecleaning. At the 9am break didn't eat anything other than drinking water because they left their lunch pail at home.

EE had talked to another EE between 10:00 & 10:30 and told other EE that the plant was running ok. At approximately Midnight-12:05 other EE found EE on the shop floor with his eyes open and apparently not breathing. Other EE ran to the scale office and told another EE to call 911 that he had checked for a pulse and could not find one.

Back pain. Operating Quarry haul truck over haul road. Operator indicated being jarred but could not identify actual area of occurrence. Haul road in fair shape other than small rutting due to rain and stripping activities, but not severe.
We had a light snow / ice event that occurred early that morning that made the black top in the area slippery. He was on his way to punch in slipped on the black top and hurt his right leg. At this time doctor is indicating a pulled hamstring in the upper thigh area.

Employee climbed on top of a motor guard to check cleanliness of a Hammer Mill feeder pan. After checking, employee proceeded to climb down off of the guard and in doing so slipped and fell onto his back, striking either the guard or the concrete motor pad. Employee sustained fracture to the 8th rib on right side and sprain right ankle.

HE WAS WORKING ON A DUST COLLECTOR WHEN HE SLIPPED OFF A LADDER AND TWISTED HIS LEFT ANKLE. HE WENT TO THE DOCTOR AND DR. SAID IT WAS A SLIGHT SPRAIN. REST IT FOR 24 TO 48 HOURS AND HE CAN COME BACK TO WORK WITH NO RESTRICTIONS.
