Employee was using a utility knife to cut a belt to make a skirt. Was holding the straight edge that EE was cutting against with EE's hand. Knife came out of the belting material and punctured EE's right-hand wrist. Direction of cutting was towards EE.

Employee cleaning belt conveyor take-up rollers on house belt #5 with a scraper. Employee was performing cleaning task while conveyor belt was running.

Employee was housekeeping and while doing this hit head on an electrical box in the work area. Employee was not wearing hard hat.

Miner attempted to step on a short ladder leading to a conveyor service platform. Left foot slipped on ladder rung and right shin and knee hit the service platform mezzanine causing trauma to the knee cap. Miner did not have left foot in full contact with the ladder rung, only the tip of EE's boot. Ladder missing hand extension on the West side so did not have 3pt. contact.

While draining a leaking hydraulic hose, oil sprayed onto employees face, entering employees right eye causing irritation.

While cleaning material with a walk behind skid steer, employee's left hand became pinched between the hand controls and a beam. Index finger was cut requiring 2 stitches.

Condition was a hot humid day, no air movement in the area of cleaning. Drank water but not enough and didn't take multiple breaks during activities of housecleaning. At the 9am break didn't eat anything other than drinking water because they left their lunch pail at home.

The miner was repositioning a 50lb bag to be re-inked and twisted back resulting in a muscle spasm.

Line of Fire. Individual was in the shop cutting a conveyor belt with a utility knife. Was in a sitting position and knife slip and punctured left lower leg - calf.

EE had talked to another EE between 10:00 & 10:30 and told other EE that the plant was running ok. At approximately Midnight-12:05 other EE found EE on the shop floor with his eyes open and apparently not breathing. Other EE ran to the scale office and told another EE to call 911 that he had checked for a pulse and could not find one.

Back pain. Operating Quarry haul truck over haul road. Operator indicated being jarred but could not identify actual area of occurrence. Haul road in fair shape other than small rutting due to rain and stripping activities, but not severe.

We had a light snow / ice event that occurred early that morning that made the black top in the area slippery. He was on his way to punch in slipped on the black top and hurt his right leg. At this time doctor is indicating a pulled hamstring in the upper thigh area.

Employee climbed on top of a motor guard to check cleanliness of a Hammer Mill feeder pan. After checking, employee proceeded to climb down off of the guard and in doing so slipped and fell onto his back, striking either the guard or the concrete motor pad. Employee sustained fracture to the 8th rib on right side and sprain right ankle.

Cleaning belt with scraper when scraper was caught and pinched hands into side guard & tightening screw breaking right index finger degloving left index finger and bruising left shoulder.

EE was walking on graveled area, slipped and reached for a oxy-acent torch cart to catch himself and struck his forearm on the hose holder, causing a cut to his right forearm.

Was knocking dust from a fan and stuck his finger through the guard and cut his finger. The cut needed stitches. He will return to work when the stitches are removed.

HE WAS WORKING ON A DUST COLLECTOR WHEN HE SLIPPED OFF A LADDER AND TWISTED HIS LEFT ANKLE. HE WENT TO THE DOCTOR AND DR. SAID IT WAS A SLIGHT SPRAIN. REST IT FOR 24 TO 48 HOURS AND HE CAN COME BACK TO WORK WITH NO RESTRICTIONS.

HE WAS DUMPING A DUMP CART FULL OF MATERIAL AND PINCHED A NERVE IN HIS NECK. HE IS BEING TREATEDMEDICALLY WITH SHOTS AND THE POSSIBILITY OF SURGERY AT A LATER DATE.

WAS CLEANING OUT A SCREW CONVEYOR WITH A COFFEE CAN. THERE WAS LOW HEAD ROOM SO HE WAS BENT OVER AND WENT TO DUMP OUT CAN. WHEN HE DUMPED THE CAN HE TWISTED HIS BACK. WENT TO CHIROPRACTOR FOR AN ADJUSTMENT AND ULTRA SOUND.

GETTING DOWN OFF BAGGER AND LOST BALANCE AND FELL BACKWARDS AND CUT LEFT HAND ON METAL STRAP ON THE CARDBOARD DISPENSER. WENT TO DOCTOR AND THEY PUT 8 STITCHES IN HIS LEFT HAND.

HE WAS LOWERING A SCRAPER ON THE HOUSE BELT WHEN A SPLICE IN THE BELT CAME AROUND AND CAUGHT HIS LITTLE FINGER ON LEFT HAND BETWEEN THE BELT AND SCRAPER.

A CAPACITOR WAS BEING LIFTED FROM THE SCALE PLATFORM TO BE PLACED ONT HE FLOOR. THE LEFT HIP JOINT WAS TWISTED.

HE WAS HOLDING ONTO A PIECE OF PIPE WHEN IT ROLLED OUT OF HIS HAND AND FELL ON HIS RIGHT KNEE CAUSING A 2 INCH CUT WHICH HE HAD 4 SUTURES OUT IN.

WENT TO STEP ON SADDLE ON #2 BAGGER AND SADDLE TRIPPED AND THE MAN HIT HIS CIN ON CYLINDER.

FELL OFF CONVEY BROKE BONE ABOVE LITTLE FINGER. L. HAND.

IT IS UNKNOWN AT THIS TIME WHAT HAPPENED.

ROD FLEW OFF TOP OF HAMMER MILL & HIT HIS HEAD .

TRYING TO PUT BAGGER SADDLE INTO POSITION.

HURT LOWER BACK WHILE CLEANING UP AROUND PLANT

MAN WAS CLEANIN1 A ELEV WITH A ROD WHILE IT WAS STOPPED. ROD BECAME STUCK IN THE MATERIAL. ROD BROKE LOOSE WHILE MAN WAS PULLING ON IT. HE STRUCK HIS ARM ON G EAR BOX BEHIND HIM RECEIVING MUS CLE DAMAGE TO B9LLY PART OF TRICEPT RIGHT ARM

GOT FINGER CAUG2T BETWEEN HANDLE OF PORTABLE DUMPSTER AND METAL GUARD ON 1 & 2 BELT.

EMPLOYEE WAS LIFTING DOOR ON TOP OF TANK TO TAKE MEAUSREMENT CAUSING HERNIA LOWER ABDOMAN.
