Anderson Creek Quarry Metal/Non-Metal

While installing a metal collection box via use of a forklift to position in place and a ratchet strap being used to align bolt holes, employee placed hand on flange of metal box to brace self while releasing the ratchet strap. As the collection box made contact with the existing structure, the employees index finger became caught between the bolt flanges causing injury.

Installing fan belts on 773F Haul Truck. Caught left middle finger between tensioner pulley and cross member. Laceration requiring 11 stitches.

Injured right tendon in calf closing gate.

Employee was using a 1 inch impact wrench to tighten the tension bolts on the primary jaw crusher. The tension bolt broke and forced the employee's hand backwards. He mashed his ring finger between the impact wrench and toeboard.

While attempting to install a new tail pulley in a Thor conveyor the employee's finger was caught between the bearing housing and the frame of the conveyor.

FOREIGN BODY IN EE'S RT. EYE. NOTICED LATER AND REPORTED NEXT SHIFT. DR. REMOVED OBJECT FROM EYE AND WROTE A PRESCRIPTION MEDICATION.

EE WAS CLIMBING INTO HEAD PULLEY BOX OF #6 CONVEYOR WHEN HE STEPPED ON LOOSE ROCK AND TWISTED LEFT KNEE.

EMPLOYEE WAS USING AN ADJUSTABLE WRENCH ON A NUT ON A CRUSHER WHEN THE WRENCH SLIPPED AND PINNED THE RING FINGER OF THE LEFT HAND BETWEEN THE BOWL OF THE CRUSHER AND THE WRENCH HANDLE. END OF JOINT OF THE FINGER WAS FRACTURED AND LACERATED CAUSING LOSS OF FINGER NAIL.

EE WAS WALKINGFROM EE PARKING AREA TO WHERE HE HAD PARKED YARD LOADER. HE ATTEMPTED TO PUT HIS SAFETY GLASSES ON AND STUCK THE EAR PIECE INTO THE RT. EYE SCRATCHING THE CORNER.

BENCH FAILURE OF THE ACTIVE DRILL SITE WHILE EE WAS DRILLING NEXT PRODUCTION AREA.

EMPLOYEE WAS UNABLE TO STOP HAUL TRUCK ON PIT RAMP. HE STEERED INTO WALL TO STOP TRUCK. DAMAGED EQUIPMENT WAS REPORTED AND OPERATOR WAS TAKEN TO DR. FOR POST ACCIDENT DRUG SCREEN. HE REPORTED HE WAS NOT INJURED AT THAT TIME. EMPLOYEE WORKEDNEXT TWO SHIFTS BEFORE GOING TO HIS DR. WITH BACK PAIN. HIS DR. SENT SLIP THAT HE NOT RETURN TO WORK UNTIL 9-12-95.

EMPLOYEE HAD HAND STRUCK BY PIPE WRENCH WHILE ATTEMPTING TO MANUALLY HOLD DRILL STEEL FOR HAMMER/BIT ASSEMBLY.
