A PORTABLE TOILET WAS MOUNTED ON A RAILCAR. ALONG THE SIDES OF THE TUNNEL, AT 200 FEET INTERVALS ARE LIGHTS, ALONG WITH JUNCTION BOXES. THIS LIGHTING SYSTEM IS 480 VOLTS. AS THE RAILCAR WAS BE ING MOVE, THE VICTIM WAS ATTEMPTING TO RAISE THE 480 VOLT CABLE AND JUNCTION BOX TO PREVENT IT FROM RUBBING ON TH TOILET. AT THAT TIME THE ELECTROCUTION OF THE VICTIM OCCURRED.

WHILE LOADING RAIL BY HAND ONTO A FLAT CAR, INJURED EE PINCHED HIS LEFT MIDDLE FINGER BETWEEN A PIECE OF RAIL & THE FLAT CAR.

EE WAS INSTALLING BOLTS IN THE TBM GRIPPER SHILED WHEN HE LOST HIS GRIP ON THE DRILL AND DROPPED IT ON HIS LEFT HAND. TAKEN TO ER FOR TREATMENT AND RELEASED TO WORK. ON 6-5-01 HIS HAND BECAME SWOLLEN AND HE WAS TAKEN TO KEARNY CLINIC AND X-RAYED. A FRACTURE IN THE THIRD METACARPAL WAS DISCOVERED AND HIS HAND WAS CASTED.
SCISSOR LIFT DEVELOPED A HYDRAULIC LEAK AND THE SAFETY SWITCH KICKED IN AND LOCKED IT IN PLACE.
