Emp. was standing on the track hoe tracks filling with DEF. When EE finished, EE turned to the left and EE's left foot slipped off the tracks and EE fell landing on the fuel trailer that was parked beside the track hoe. EE suffered 4 fractured ribs and nicked EE's lung on left side. The area received a heavy rain the night before, the area, the tracks, and EE's boots were muddy.

Foreman suffered a fatal wound during a non-mine related firearm discharge event.

A repair job completed, a chain was attached to the excavator bucket. The bucket & the chain were on the ground. When the excavator was moved, the employee was standing on the chain as the bucket was swung. Employee was tripped, but did not fall.

Employee was replacing skirt board on stacker belt. Wrench slipped and impacted finger. First aid completed. Employee reported Jan 20 finger was infected and went to clinic. Clinic provided antibiotic/meds; employee released to return on Jan 25.
