Miner was working in a conveyor head box and had set a 1-ton Lever hoist on the head pulley above EE. When EE bent over, EE's hardhat fell off and the lever hoist slid off the head pulley and struck EE in the head.

Miner rolled R-ankle while walking from thickener to get parts for a maintenance project.

Drug impaired trespasser entered mine and was throwing rocks at haul trucks from pit slope. Authorities were called, & when PD tried to coax them up out of pit, they chose to head down & tumbled approx 100' down slope wall to pit floor. They then got up and ran before collapsing. FD transported them offsite. Awaiting PD report.
Slipped on shaker deck stairs trying to get more slack for welding lead, causing them to roll an ankle.

Miner was operating haul truck and while being loaded a large boulder struck the inside of the bed jerking the haul truck and operator cab side to side. As a result, the miner in the haul truck cab reports headache, neck and back pain.
Miner attempted to access the side of the jaw crusher when EE slipped and fell injuring EE's left ankle. Fall protection was in use at the time and the injury resulted in a broken ankle and torn ligament.

Miner was cutting a conveyor belt with a utility knife when the knife slipped cutting hand.
Miner was removing belting from a conveyors return side. When miner went to exit conveyor frame they slipped stepping down and hit right ear, forearm and thigh on conveyor frame. Contusion to head with Avulsion of right ear, contusion with abrasion to right forearm and right thigh.
Two miners were moving a bearing when one miner's fingers caught between the bearing and the stairs, causing crushing injuries to right middle and ring fingers and fracturing right middle finger.

Miner was inspecting the train before loading. Miner was dismounting off of a piece of equipment when an article of clothing got caught causing the miner to fall. Miner received a cut to elbow and a bruised hip.

Injured was helping on a project to fix an electrical problem and needed the man lift to reach. The area he drove equipment down was loose dirt and rather steep. He turned it too tight and lost traction thus sliding and making an abrupt stop on a dirt bank up against the gravity take up guard, throwing him around inside the basket.

IT IS UNKN OWN HOW THE INJURY OCCURRED. W/COMP IS DELAYING AND INVESTIGATING CLAIM. DOCTORS WILL MAKE DETERMINATION AS TO WHETHER OR NOT INJURY IS INDUSTRIAL/NON-INDUSTRIAL BEFORE DECISION IS RENDERED.
