Employee was moving a plate when it slipped from EE's hands and landed on EE's foot.

Employee lifted gas can and felt pinch in shoulder. Later, employee was fixing a belt when it hit EE in the forearm.

Was going into crusher to do maintenance when EE felt EE's back tense up. When EE tried to crawl back out EE couldn't move.

EE was hit by rock that came out of bin.

EE was using high pressure hose and the pressure resulted in arm being thrown in the air. Possible rotator cuff tear.

EE cut hand on angle iron while shoveling.

EE bent over and back tightened and started to have spasm.

cutting belt; knife slipped; cut left forearm requiring stitches.

Emp passed out due to extreme heat, dehydration.

EE was performing start-up procedure and went to grab hand rail and slipped, dislocating right finger.

Emp was welding and a piece of steel fell off the lip onto left foot.

A piece of angle iron fell onto employee's leg just bruising it while working on the chute.

Employee was trying to hose counter weight and got splashed in eye with dirt.

While emp was moving long arm to weld cutting edge on bucket, a piece of metal off of the track arm flew off and hit emp in the chin.

While employee was repairing the grizzly car at the feeder the backhoe operator went to set the grizzly bar and when it was lifted the grizzly bar swung hitting employee causing him to fall approx. 13 feet to the ground onto # 1 belt. (9/22/05 call to company - possible paralysis)

Emp was attempting to push rocks off of the belt and strained shoulder muscle.

EE WAS ATTEMPTING TO REMOVE SAFETY BLOCKS FROM HYD. CYLINDER OF CRUSHER. INSTEAD OF DECREASING PRESSURE, HE INCRESED PRESSURE AND CRUSHED BLOCK. BLOCK FLEW OUT AND HIT EE IN ABDOMEN.
