come-a-long slipped hitting employee in the mouth

Employee was tramming a ManLift and hit a bump forcing employee to strike against the basket.

Employee was walking, felt a pop in the back and needed medical attention.

The Plant Manager was contacted by the Cross VFD stating that a fire had started in the ditch on County Line Rd in front of the quarry. The fire spread onto mine property before it was extinguished.

The operator was sitting in an Off Road Truck being loaded when Excavator swung around and struck the catwalk with the counterweight, shaking the operator on the inside of the cab.

Employee was loosening bolts on top of the Davie FS2-E Drill mud pump to troubleshoot a noise. Upon using an adjustable wrench, it slipped causing EE to strike EE's left hand near the pinkie knuckle. Employee was wearing Hexarmor impact/cut resistant gloves at the time of the incident. The injury occurred between the cut protection and knuckle protection area on the glove.

The EE was installing a pin on a crane. Using a hammer the EE made contact with the pin. During impact a piece of the hammer came and struck the EE in the left forearm. The piece went into the forearm.

EE was removing a section of wear plate steel with a torch. As the final cut was made the plate fell and landed on the EE's left hand dislocating three fingers and lacerating the palm.

EE was using a side grinder when it kicked back and up striking him in the forehead causing a laceration above his Rt. eye that required stitches to close.

Technician was attempting to place a portable hydraulic press, weight approx. 100 lbs., onto wood crib blocks. Hydraulic press fell off of wood crib blocks and fell. Techician tried to hold onto press and sprained left shoulder.

Moving toilet tank & hurt lower back.

Moving electric motor with fork lift; while adjusting forks on fork truck, he smashed his finger.

Employee was moving the forks on a fork truck when he caught his finger between the slide bar and the fork.

Employee was walking around the rear of the crusher station. He attempted walking over a small pile of clean-up material. As he did this, he twisted his ankle and felt pain in his foot.

Injured employee was climbing up the ladder on side of customer's truck to examine material when he felt a pain in his left chest.

EE WAS USING HANDTOOLS, PERFORMING MAINTENANCE IN PLANT ALL DAY. HE NOTICED SOME PAIN THAT NIGHT, AND TREATED IT AT HOME. A FEW DAYS LATER, HE REPORTED THE INCIDENT, AND WAS SENT TO THE DR. XR AYS REVEALED A FRACTURE OF ONE OF THE BONES IN THE WRIST, BUT IT COULD NOT BE DETERMINED WHEN THE FRACTURE HAD OCCURRED. THE EE SAID HE HAD FRACTURED HIS WRIST IN A MOTORCYCLE ACCIDENT YEARS

EMPLOYEE WAS HELPING STEM BLAST HOLES, MOVING ROCK IN 5 GALLON BUCKETS. AS HE LIFTED A BUCKET, HE FELT A PAIN IN HIS LOWER BACK. HE WAS SENT TO A DOCTOR AND DIAGNOSED WITH BACK STRAIN. HE CONT INUED HIS NORMAL DUTIES. IN MARCH 2002 HE RETURNED TO THE DOCTOR AND WAS DIAGNOSED WITH A HERNIATED DISC.

INJURED AND OTHER EMPLOYEE WERE LOOKING THROUGH THE SCRAP YARD FOR SPARE CONVEYER PARTS. INJURED STEPPED ON A NAIL PROTRUDING FORM A PIECE OF WOOD, PUNCTURING HIS FOOT. FIRST AID TREATMENT WA S PROVIDED AT THAT TIME. ON OCT 8 HE WAS ADMITTED TO THE HOSPITAL WITH AN INFECTION IN HIS FOOT.

STEPPED OFF BACKHOE FOR AN ONSITE MEETING. ABOUT A HOUR LATER, RETURNED TO EQUIPMENT AND KNEE STARTED HURTING.

EXHAUST LEAK ON DUMP TRUCK (OFF ROAD).

EMPLOYEE WAS LIFTING BUCKET WITH APPROX. 30 POUNDS OF ROCK SAMPLES. THE HANDLE TWISTED, CAUSING BUCKET TO ROTATE, CAUSING STRAIN TO TENDONS IN THE LEFT WRIST.

EE WAS CLIMBING ON SIDE OF CONVEYOR FRAME WHEN HIS FOOT SLIPPED AND HE FELL 4-5' TO THE GROUND. EE WERE PREPARING TO CHANGE CONVEYOR BELTING.

EE AND ANOTHER EE WERE WORKING FROM A BOAT, CONNECTING A DISCHARGE HOSE TO A PIT PUMP. THE BOAT MOVE UNEXPECTEDLY (IT WAS SECURED TO THE PUMP), CAUSING THE EES TO DROP THE HOSE. A FLANGE ATTAV CHED TO THE HOSE STRUCK EE ON THE LEFT KNEE. THE ACCIDENT WAS REPORTED THEN, BUT EE DID NOT SEEK MEDICAL TREATMENT UNTIL AUGUST OF THIS YEAR.

EE ENTERED THE DRAGLINE BUCKET TO GREASE THE DUMP BLOCK. THE BUCKET WAS WET FROM A LIGHT RAIN, AND HE SLIPPED FALLING INTO THE BUCKET & AGAINSTTHE HOIST CHAIN. THE CONTACT WITH THE CHAIN CAUS ED A SEVERE BRUISE ON HIS RIGHT THIGH JUST ABOVETHE KNEE

STRUCK BY ROCK FROM CONVEYOR BELT OR VIBRATING SCREEN CAUSING LACERATION TO THE HEAD.

WHILE TIGHTNING JAW CRUSHER BOLTS EMPLOYEES WRENCH SLIPPED CATCHING HIS THUMB BETWEEN WRENCH & CRUSHER FRAME EDGE OF WRENCH CUT HIS RT INSIDE THUMB AT THE KNUCKLE JOINT REQUIRING (5) FIVE STIT CHES

EE WAS LIFTING A BUCKET OF CRUSHED MATERIAL TO DUMP ON A SHAKER USED FOR TESTING STONE. THE LIFTING OF THE BUCKET CAUSED THE LOWER BACK INJURY.

EMPLOYEE HAD METAL TO FALL AND STRIKE LEFT LEG ABOVE KNEE AND LAND PARALLEL TO LEFT LOWER LEG ABOUT 12 IN ABOVE ANKLE

ATTEMPTING TO LIFT TOO HEAVY AN OBJECT

EMPLOYEE CUT FINGER WHILE CLOSING DOOR ON DRAGLINE EMPLOYEE DAMAGED LEFT INDEX FINGER REQUIRING TWO STICHES

EMPLOYEE WAS STRIKING A WEDGE WITH A HAMMER; WEDGING OF FLYWHEEL OF SCREEN; AND WEDGE CAME OUT STRIKING EMPLOYEE ON RIGHT EYE BROW.
