Victim had stepped onto the top of a truck tire to gain access to the engine compartment. EE's foot slipped as EE was getting down and EE injured wrist when catching themself. Incurred fractured/dislocated left wrist.

Employee was cutting trees, and fell while walking around the hill to next tree to cut, cutting arm.

The employee was attempting to start the lube truck by jumping across the starter when the key switch ignition did not work. The truck was in gear and it lurched forward onto the employee's left leg just below the knee. Employee sustained a mashing injury to leg. The truck did not go across employee's leg, just caught the outside of the leg.

Employee was cutting a tree, a fellow employee cut a tree into the first employees cutting area, striking the employee on the back.

EE WAS CLIMBING DOWN LADDER FROM LOADER - LEFT FOOT ON STEP AND WHEN STEPPING DOWN TO UNEVEN GROUND, FOOT WAS CAUGHT AT STEP AND TWISTED RIGHT KNEE WHEN LOST BALANCE.

After working on fender lights of equipment, employee stepped from fender back onto deck, hitting and slightly cutting his face above the right eye. He then fell to the ground.

EE complained of back discomfort while operating 400 Komatsu Excavator. He stated seat was uncomfortable. EE finished shift and then sought medical treatment on 3/27/07 for back pain.

Ladder slipped and employee fell while fueling equipment.

A rock fell out of the right side of a 992G bucket into the 777 truck bed, driven by the injured. The impact of the rock jarred the truck and the driver. The driver complained on the next shift of pain in the left shoulder and hip.

EE was loading shot rock with a 992G and jarred the loader in some hard rock and hurt his lower back.

Employee was dismounting a 777 rock truck to go to the restroom when he stepped off the ladder and twisted his right knee. He continued to work the rest of the shift on Friday and worked on Saturday. He went to the doctor on Sunday Feb 12. He did not notify his supervisor until Feb 23 that he had had medical attention.

Employee suffered back pain as he climbed onto his 992G loader.

Employee stepped down of a Komatsu 400 Excavator and rolled his ankle.

Employee was performing a pre-shift inspection on his 777 Truck. He claims his lower back was injured after straightening up from bending over to look at the under side of the truck.

EE was driving a rock truck down a hill toward the Cedar Grove deep mine when his a rough spot in the road. Foggy conditions was present.

EE was pulling a fuel hose out to fuel the rock crusher, when he sprained the middle of his back.

EE loaded a rock in his bucket which was too large for his loader. The loader tipped forward and the rock fell out, letting the loader hit the ground. EE was not wearing his seat belt and was bounced out of his seat, striking his arm against the hand bar on the dash.

EE STEPPED OUT OF THE CAB ONTO THE DOZER TRACK AND WAS BENDING OVER TO HAND A PRESHIFT TO A MAN ON THE GROUND WHEN HE EXPERIENCED BACK PAIN.

EE was loading a 992C bucket pin into a mechanic's truck by hand. The pin slipped and mashed his little finger on his left hand between the pin and the truck's tail gate.

EE WAS STEPPING UP ON A SERVICE TRUCK WHEN HE TURNED TO CLIMB HIGHER, HIS RIGHT KNEE POPPED.

Employee was transferring product from an anfo bin into a anfo truck, when the clam shell bucket on the anfo bin closed on his finger, cutting and breaking the ring finger on his right hand.

Employee slipped while climbing on a 992G loader to repair an antifreeze leak. He tried to catch himself with his left hand. He thought it had only sprained his left wrist, and did not go to the hospital. However the wrist did not improve so he went to the doctor on January 6, 2005. His left wrist was broken.

EMPLOYEE BACKED HIS D10N DOZER OVER A WINDROW ANOTHER DOZER HAD MADE AND STRAINED HIS BACK.

Employee slipped on slick coal surface & fell, landing on his left shoulder and elbow. He bruised his shoulder.

EE WAS USING A TORCH TO CUT A DRIVE LINE FREE AND A SPARK WENT BEHIND HIS SAFETY GLASSES AND BURNED HIS EYELID.

Employee was removing a hose clamp from an oil level sight glass on a 120 excavator when the screwdriver slipped and punctured his left hand.

MECHANICS WERE REMOVING COUNTERWEIGHT BOLTS ON A 345 EXCAVATOR, WHEN ONE OF THE MECHANICS DROPPED A PIPE WRENCH, WHICH STRUCK HIM IN THE FOREHEAD ABOVE HIS LEFT EYE. HIS PUPILS WERE UNEV EN, SO HE WAS TAKEN TO THE HOSPITAL.

EE SAID THAT A STEP WAS BROKEN WHEN HE GOT ON HIS TRUCK & THAT IT SHOOK HIM A BIT. AFTER TALKING TO HIM ON MONDAY, I FOUND OUT THAT HE THINKS HE MAY HAVE RUPTURED A DISC IN HIS BACK AND TORE A ROTATOR CUFF IN HIS SHOULDER WHEN HE CAUGHT HIMSELF.

MUD THAT HAD ACCUMULATED ON THE STEP CAUSED THEM TO BE SLIPPERY. EE WAS LEAVING THE MAIN OFFICE WHEN HE SLIPPED AND FELL.

WHILE LOOKING FOR OIL LEAKS A DROP OF OILY MUD FELL INTO HIS LEFT EYE.

THE EE WAS LOADING A C/02 BOTTLE IN STORAGE CABINET OF WELD TK. WHEN HE RAISED BOTTLE IN PLACE TO CHAIN IN PLACE HIS L.H. THUMB HIT SIDE OF CABIENT AND MASHED HIS THUMB.

EMPLOYEE WAS CUTTING A BUSHING OUT OF A LIFT ARM PIT, WIND BLEW FLAME THROUGH BORE AND BURNT RIGHT SIDE OF HIS FACE AND RIGHT ARM.

EE WAS CUTTING BUSHINGS OUT OF A D10R HOIST JACK AND INHALED FUMES.

EE GOT FUEL SPRAYED ALL OVER HIM. WHEN HE CLIMBED INTO THE GREASE TRUCK TO GET WATER TO WASH HIS EYES OUT HIS FOOT SLIPPED ON THE STEP. HE HAD A HOLD OF THE STEERING WHEEL, ALL HIS WEIGHT WAS ON ONE ARM. EE WORKED 5/15, 5/16, 5/17 WITHOUT COMPLAINT. HE CALLED THE MINE OFFICE ON 5/19 COMPLAINING OF NECK & SHOULDER PAIN & INDICATED HE DIDN'T KNOW WHAT HE HAD DONE TO IT. THEN HE CALLE

HE WAS GETTING OFF HIS TRUCK AND TWISTED HIS KNEE.

EMPLOYEE SAID WHEN HE WAS GETTING OFF HIS TRUCK HE MISSED THE BOTTOM STEP HE LANDED ON HIS RIGHT HIP. EMPLOYEE WENT TO HOSPITAL IN AN AMBULANCE X RAYS INDICATED A BROKEN HIP AND PELVIS.
