Raven Crest Contracting, LLC · Struck by falling object

The victim was retrieving a lifting strap from the tool compartment of the truck. A tarp was partially covering the strap and EE gave it a yank to free it. It was tangled with a sheave (pully assembly). The sheave fell out of the tool compartment striking EE's right foot just behind the steel toe covering. Crushing injury to great right toe. No fractures detected.

Employee was carrying food into the mine office to heat. EE lost balance, stumbled and fell backwards. EE stepped on a water hose lying on floor of porch. EE fell onto a mantrip parked on porch then onto the floor. EE complained of right shoulder pain. EE was treated by MET-M then transported by ambulance to hospital.

Victim had stepped onto the top of a truck tire to gain access to the engine compartment. EE's foot slipped as EE was getting down and EE injured wrist when catching themself. Incurred fractured/dislocated left wrist.

Employee was cutting trees, and fell while walking around the hill to next tree to cut, cutting arm.

The employee was attempting to start the lube truck by jumping across the starter when the key switch ignition did not work. The truck was in gear and it lurched forward onto the employee's left leg just below the knee. Employee sustained a mashing injury to leg. The truck did not go across employee's leg, just caught the outside of the leg.

A rock fell out of the right side of a 992G bucket into the 777 truck bed, driven by the injured. The impact of the rock jarred the truck and the driver. The driver complained on the next shift of pain in the left shoulder and hip.

Employee was dismounting a 777 rock truck to go to the restroom when he stepped off the ladder and twisted his right knee. He continued to work the rest of the shift on Friday and worked on Saturday. He went to the doctor on Sunday Feb 12. He did not notify his supervisor until Feb 23 that he had had medical attention.

EE loaded a rock in his bucket which was too large for his loader. The loader tipped forward and the rock fell out, letting the loader hit the ground. EE was not wearing his seat belt and was bounced out of his seat, striking his arm against the hand bar on the dash.

Employee slipped while climbing on a 992G loader to repair an antifreeze leak. He tried to catch himself with his left hand. He thought it had only sprained his left wrist, and did not go to the hospital. However the wrist did not improve so he went to the doctor on January 6, 2005. His left wrist was broken.

EE SAID THAT A STEP WAS BROKEN WHEN HE GOT ON HIS TRUCK & THAT IT SHOOK HIM A BIT. AFTER TALKING TO HIM ON MONDAY, I FOUND OUT THAT HE THINKS HE MAY HAVE RUPTURED A DISC IN HIS BACK AND TORE A ROTATOR CUFF IN HIS SHOULDER WHEN HE CAUGHT HIMSELF.

HE CALLED THE MINE OFFICE ON 5/19 COMPLAINING OF NECK & SHOULDER PAIN & INDICATED HE DIDN'T KNOW WHAT HE HAD DONE TO IT. THEN HE CALLE
