Employee was clearing blockage from a closed conveyor belt gate; employee attempted to clear blockage with hands, moving conveyor belt moved the blockage while employee had hand on it and hand became wedged between belt and blockage causing injury to fingers on left hand.

manlift basket was used improperly as a lifting device and the employee was pinched during the lifting process.

Employee was closing a shale gate. The gate did not fully close and EE used the handle lever (detachable square tubing) to strike against the gate. In doing so, EE hit EE's hand on the gate fracturing the tip of EE's left ring finger. Also lacerated the finger the required stitches.

Overheated material fell onto the clinker conveyor belts and caused the skirt rubbers to catch on fire. The resulting fire heat caused electrical damage to the e-stop causing the belts to stop. Employees tried extinguishing the fire with fire extinguishers. The employees called the fire department. No employees were injured, property damage will exceed $10,000.

Employee was cleaning inside of a Reed Gunite Pot and got left hand glove caught between feed wheel and rock shear. Severed tip of middle finger and index finger.

Injured employee was swinging a sledge hammer and missed the point of contact and hit right hand pointer finger on shaft. Broke right hand pointer finger.

Windy conditions stirred up dust and entered employee left eye. Left eye irritated that required medicated eye drops.

I've been gone since Feb returning on April 5th so this didn't get reported. EE was helping to align a shim under a pillow block bearing when the pry bar slipped while tightening the belts causing the pillow block bearing to drop on EE's left thumb. EE has a non displaced fracture of distal phalanx and laceration without foreign body damage to nail.

While the EE was shoveling material the shovel hit the floor then EE felt a pain in upper rt Arm/Shoulder causing a muscle strain.

EE says EE was doing clean up work with a shovel in the tunnel and pulled a muscle around EE's Rt rib/chest area, but then says it could have happened when EE was mowing the lawn?

EE states was cleaning a conveyor table with a T-rake, the T-rake got into the roller pulling EE's hand into the roller. EE went to the ER, X-rays were taken and all was negative and no restrictions. EE went back to our work comp clinic on May 17th and was given restrictions to also start physical therapy on May 22nd.

EE worked on Saturday in the tunnel welding and moving metal pieces and felt something in right eye but didn't want to go to our work comp Doctor, EE went on own to EE's own Dr and didn't turn it in until EE received the bill. EE had a metallic foreign body in the right eye.

EE was moving steel plates to center grips for the forks, the plate slipped off falling onto right foot. X-ray showed a small fracture.

EE was hammering off a bearing when a piece broke off and flew up into his face cutting his lip. He received four stitches and a chipped tooth.

EE had removed a cover to clean out a hopper, when extending the shovel into the hopper and removing material he was cut by a sharp piece of metal cutting through his gloves.

EE WAS STEPPING OUT OF A DUMP TRUCK, HE STEPPED OUT ONTO SOME LOOSE MATERIAL TWISTING HIS RT ANKLE AND HEARD IT POP. THE CLINIC DID AN XRAY, IT DIDN'T SHOW A FRACTURE. THE DR STATED IT WAS A SPRAIN.

EE was prying on a crusher housing to set it back in place. He was standing on a wooded board covering a work hole used for maintenance on the crusher. The board broke and he fell through the hole catching himself with his arms chest high with his left leg/foot landing on an I-beam. The employee sustained a sprained left shoulder, leg/knee and ankle.

A CONTRACTOR WAS INSTALLING KEY BRICK IN A ROTARY KILN. A KEY BRICK SPRUNG OUT HITTING HIS LEFT INDEX FINGER. THE INJURY CONSISTED OF STITCHES AND FRACTURED THE TIP OF HIS LEFT INDEX FINGER.

EE was getting ready to install a bearing. He squatted down to pick it up and his right knee popped. He notified a supervisor the same day, but didn't want treatment at that time. On March 31st his knee was no better. He then was sent to the clinic. Diagnosis is a Knee Impingement with restrictions.

EE states accident was reported to management on 3/18/14. EE and two others were lifting a 1x6x20' metal bar when he felt a sharp pain in his abdomen. He went to OP Regional ER on 3/17/14 and left with restrictions, but I was not notified.

For about three days the EE had been working in a confined area on his knees bent over at times using a Mag Drill to place steel plates to the wall and to secure the adjusters in the holes.

EE WAS INSTALLING AN EXHAUST ON TOP OF A ROAD GRADER, HE SLIPPED AND FELL BACKWARDS ON THE TIRE. THE DOCTORS FINDINGS IS A BACK CONTUSION AND SPASM OF MUSCLE.

EE was doing his cleanup work, shoveling material and picking up and stacking brick, while doing so he felt a sharp pain in his lower abdomen. The Doctor says he has a Hernia NOS. He is to see a General Surgeon.

EE was getting ready to go down stairs, he tripped on the top step falling down approximately 7 steps to the landing. He fell with his right leg bending backwards underneath him. The Dr put him on crutches and sit down work only until he has an MRI on Nov 14th.

EE was laying underneath a dump truck installing a starter. A piece of metal fell into his left eye. A foreign body was removed leaving a residual rust ring which has to be cleaned out by an Ophthalmologist MD. A Tetanus shot was also given.

EE was getting ready to replace a fan belt. He didn't close the fan louver's and put a board in the fan to keep it from turning as a safety, the fan started turning backwards he tried to stop it with his right foot. The belt pulled his foot between the belt and sheave. It broke both bones in his ankle.

Employee was stepping down the ladder of a Highloader , his Rt foot got caught on the last step causing him to step down on his LT foot sideways. He sprain his Lt Ankle. They put him in a boot when he is to be active. He was told to take over the counter meds for pain.

Employee Arc welded from 10am - 5:30pm.

Employee reported he fell 3/4/2011. At that point he was okay but sore. Monday A.M. his Rt shoulder was hurting and wanted to be seen. He stated that he was carrying a oxygen bottle when he slipped and fell in the mud falling forward and catching himself with both arms, mostly with the right arm, but also stated his left shoulder was sore also.

EE was jackhammering kiln brick and concrete out of a kiln. He stumbled over the pieces laying on the ground. He sprained his Lt ankle.

Employee states he was moving lifters at the Kiln while doing maintenance work and twisted his back. On Tuesday, November 10th he said his back wasn't any better, he wanted to go get checked out. The Doctor put him on modified duty with therapy.

Employee states he was pulling down on a ratchet with a cheater bar, as he was pulling down on the ratchet it slipped off the nut and it came back on his right finger tip. He sustained a Distal Phalanx (fractured finger tip)

Employee reported to Plant Manager that he kinked his back again by moving a crusher roll. At this time I have not spoken with employee or received a work status report. Employee stated he did not use the electric hoist that is provided.

Employee states he was shoveling when his back started hurting. The Doctor is sending him to therapy for one week, then will re-evaluate.

Employee states he was laying on a metal rail replacing rail rubber. He felt a pain in his right hip area.

Employee states he was on a forklift moving a couple of crusher rolls. One of them looked like it was going to roll off, so he got off to put a piece of wood under the roll. As he pushed against the roll he felt a pop and a tear feeling in his lower back.

Employee states he was putting bolts in a lifter, he turned, tripped and fell on his shoulder. The Doctor states a strain of his right shoulder.

Employee said he picked up a piece of shaft and felt a pull in his left groin area.

Employee states he was changing a grinder wheel on a hand grinder, when he flipped the switch it slipped cutting through his glove and cutting the fourth knuckle of his left hand. He received about eight stitches.

Employee states he was helping to replace a belt on the scalper. When they were replacing the guard he got his Rt. hand caught between the motor and guard. He had soreness to his hand. The Doctor put him in a splint and was given Prednisone.

Employee was going up the ladder to get into a L150 High Loader when he felt his thumb pop. On Monday (11/17/08) he spoke with his Supervisor who then sent him to the Clinic. He was diagnosed having a "Mallet Thumb", he will have a pin put in on 11/19/08 for 8 weeks. The nurse said he will have no restrictions from them, only what he feels he can and can't do.

Employee said he was shoveling with his left hand. He felt a pop in his left shoulder and back, causing a left cervical strain and left shoulder strain. This was not reported to Management until 6/30/2008.

He was trying to jerk open the door (instead of using the pry-bar provided) to change from crushing to scalping.

Employee was using a ratchet pulling down to tighten a bolt. He was using his right leg as leverage to balance himself, but his leg slipped injuring his lower left back.

Employee was picking up a bearing housing and strained his left groin.

Employee was rodding material above a head pulley. The head pulley caught the rod, twisting & bending his hand.

Employee was shoveling under a conveyor belt and felt a pull in his back.

EE said he climbed down a ladder and jumped off the bottom step, which is 14" to the ground, but another EE (who does not want to be identified) seen him jump from the top of the platform to the ground, which is 6'.

EE had been doing shoveling work. He said his wrist felt sore, but he continued to shovel the material. On 10/31/07, the secretary noticed his wrist was wrapped. He said it was fine, "just a little sore". He refused treatment. On 11/2/07, he came to the office, his wrist was swollen so we sent him to the clinic.

Employee was lifting and changing screens on a shaker. He felt pain in his lower and upper back. the work comp. said he had minimal thoratic strain. On 9/18/07 he reported he still had pain. He returned to the doctor. He's now on light duty and physical therapy.

Injured was drilling a bolt out of a shaft on top shaker. He remembers rubbing his eye. Later that evening his right eye started paining him. He went to ER where they removed a piece of metal and advised him to follow up @ eye clinic. They cleaned out a rust ring that had developed and excused him from work the following day. He returned to full duty July 18th.

Employee was laying under a crane replacing a starter. He went to sit up hitting his head on the frame.

Employee was jackhammering, he slightly turned on his right leg and felt a pull in his right hip. An MRI was scheduled for 3/16/07.

Employee was jack hammering midway inside a kiln. He looked up above him to check the bruicks, doing so the jack hammer bounced down on top of his left foot.

Employee was cleaning outside the kiln area, a gust of wind blew dust into his eyes.

Employee was walking by the lower shake when the counterweight slipped off hitting the guide and bounced forward hitting ee's legs in both the knee and shin area.

On 4/29/05 EE was mounting feed belt to belt scale, frame of feed belt slipped & fell on his hand. He went to ER, they xrayed his hand & said it wasn't broke. Follow up on 5/2 @ OHS Compcare, they didn't re-xray his hand, he was released to full duty. 5/9 he came in complaining of pain & hand was still swollen. Sent him back to OHS & they did xrays showing hand was broken.

EMPLOYEES WERE JACKING UP AN OVERSIZED BALL OF MATERIAL AND PLACING BRICKS UNDERNEATH IT TO GET IT UPON THE COOLER GRATES SO THEY COULD GET THE BALL TO MOVE. THE MAINTENANCE MAN REACHED UNDER THE BALL TO MOVE A BRICK; THE JACK SLIPPED OFF THE BALL OF MATERIAL AND THE BALL FELL ON HIS RIGHT HAND.

EMPLOYEE WAS UNCLOGGING A HOLE WITH A SLEDGE HAMMER. HE HIT THE HANDLE OF IT AND FELT PAIN DOWN HIS ARM (ELBOW).

EMPLOYEE WAS CLIMBING OUT OF AN APRON FEEDER HOPPER, HE SLIPPED HITTING HIS RIGHT ELBOW ON THE CORNER OF THE HOPPER. EMPLOYEE WAS GIVEN A SHOT OF CORTIZONE IN HIS ELBOW.

EE WAS DRILLING. HE WAS HOLDING 5THE DRILL WITH HIS LEFT HAND & A BOARD WITH HIS RIGHT. THE BIT BROKE & JUMPED OVER CUTTING HIS RIGHT FOREARM. NO STITCHES--RECEIVED TETANUS SHOT.

EE WAS GRINDING METAL AND A PIECE FLEW UP INTO HIS LEFT EYE EMPLOYEE STATED HE WAS WEARING SAFETY GLASSES.

EMPLOYEE WAS OPERATING A DOZER PUSHING UP A STOCKPILE. IT LUNGED FORWARD JERKING HIM.

EE WAS HOLDING A SHEAVE BETWEEN HIS KNEE'S TAKING BOLTS OFF OF IT. THE SHEAVE SLIPPED TURNING SIDEWAY CUTTING HIS KNEE.

EE WAS WALKING INSIDE OF A KILN. HE TRIPPED, FALLING FORWARD. HIS HARD HAT FELL OFF. WHEN HE RAISED UP, HE HIT HIS HEAD ON TEH END OF A BURNER PIPE, CUTTING HIS HEAD.

EE WAS USING A CUTTING TORCH. THE HOSE BLEW APART CATCHING HIS RT. HAND GLOVE ON FIRE. BURNING THE 5TH FINGER. ER DR SAID IT WAS 3RD DEGREE. NEXT MORNING HE WAS SEEN AT WORKCOMP DR. HE SAID IT WAS 1ST DEGREE BURN.

EE WAS STEPPING OVER A DITCH. HIS FOOT SLID DOWN INTO THE DITCH.

EE WAS WALKING IN THE DARK AND STEPPED INTO A DITCH.

EE WAS REMOVING A MUFFLER OFF A MOTOR GRADER. HE CUT HIS RT MIDDLE FINGER. EE CLEANED & PUT A BANDAGE ON IT, AND THOUGHT IT WOULD BE OK. AFTER A FEW DAYS, IT BECAME SORE & SWOLLEN. HE REPORTED THE ACCIDENT ON 7-5-01 TO HIS SUPERVISOR & WAS SENT TO THE CLINIC.

EE WAS TRYING TO JUMP START A TRUCK & HAD IT CONNECTED WRONG. THE BATTERY EXPLODED INTO BOTH EYES.

EE PICKED UP A SOCKET. IT HAD A SHARP EDGE ON IT, CUTTING HIS 4TH LEFT INDEX FINGER.

EMPLOYEE'S WERE PUTTING A HUB ON A LOWER SHAKER. A HELPER PUSHED THE HUB ON BEFORE EMPLOYEE MOVED HIS RIGHT HAND.

EE WAS BENT OVER IN A SMALL AREA & TRIED TURNINGTO TIGHTEN SOME BOLTS WHEN HIS BACK POPPED.

EMPPLOYEE WAS WORKING WITH PORTA POWER, IT FELL OVER ONTO HIS RIGHT TOE AREA.

EE WAS USING A PORTA POWER TO CHANGE A HAMMER ON A CRUSHER MACHINE. IT SLIPPED OFF HITTING HIM IN THE FACE. HE HAD SIX STITCHES ON LEFT SIDE OF FOREHEAD AND A CONTUSED NOSE BONE WAS NOT BROKE.

EE WAS INSIDE A SYNTRON HOPPER PICKING SHALE. ITBROKE LOOSE COVERING HIM TO HIS KNEES.

EE WAS OUTSIDE WALKING TO HIS JOB, IT WAS VERY WINDY. A FOREIGN BODY BLEW INTO HIS LEFT EYE. THE EE STATES HE WAS WEARING SAFETY GLASSES.

EMPLOYEE HAD A STEEL PLATE CLAMPED TO A HOIST; THE HOOK SLIPPED OFF CAUSING THE PLATE TO FALL &SLIDE DOWN EMPLOYEE'S LEG.

EE WAS GRINDING WHEN A FOEIGN BODY FLEW INTO HIS EYE.

EMPLOYEE WAS HOLDING A GUARD THAT WAS HANGING FROM A CRANE. THE GUARD SWANG AROUND HITTING A HANDRAIL, WHICH BOUNCED BACK HITTING HIS LEFT FOREARM.

EMPLOYEE WAS LIFTING A 20-GALLON BARREL OF WASTE OIL. HE PULLED A MUSCLE IN HIS RIGHT SHOULDER & UPPER BACK.

EMPLOYEE OPERATES A SHALE CRUSHER; A LARGE CHUNKOF SHALE GOT LODGED--WHEN HE OPENED THE SIDE DOOR TO DISLODGE IT, HE REACHED IN AND A LARGE CHUNK OF SHALE FELL ON HIS LEFT HAND.

EMPLOYEE WAS ROLLING A LARGE BLOCK OF WOOD END OVER END. EMPLOYEE SAID HIS SHOULDER POPPED OUT AND BACK INTO PLACE. HE WENT TO THE CLINIC 4/18;THE DOCTOR DIAGNOSED A STRAINED SHOULDER. NO X- RAYS WERE TAKEN. EMPLOYEE SAID IT HAD GOTTEN WORSE. HE WENT BACK ON 8/16. THE DOCTOR NOW THINKS HE HAS A ROTATOR CUFF TEAR. A MRI IS SCHEDULED. NO MODIFIED DUTY WAS REQUESTED.

LAST MAY OF 1999 EMPLOYEE COMPLAINED OF RIGHT SHOULDER PAIN CAUSED BY RODDING SHALE ABOVE HIS HEAD. DURING THIS TIME HE FAVORED HIS RIGHT ARM USING MORE OF HIS LEFT. HE NOW HAS THE SAME SYMPTO MS IN IS LEFT SHOULDER. AN MRI SCAN HAS BEEN SETUP.

EMPLOYEE WAS REPAIRING A HYDRAULIC HOSE. IT BLEWTHROWING HYDRAULIC OIL ONTO HIS FACE AND IN HIS EYES.

EMPLOYEE WS PRYING A CHUNK OF SHALE OUT OF A CRUSHER. WHEN THE CHUNK BROKE LOOSE IT POPPED OUT HITTING HIS RIGHT INDEX FINGER.

EE SLIPPED & FELL GOING UP STEPS.

EMPLOYEE WAS FEEDING WIRE THROUGH A BEARING HOUSING. HIS HAND SLIPPED, HITTING HIS LEFT PINKY FINGER ON THE BEARIONG RACE.

EE HAS TO ROD SHALE DAILY ABOVE HIS HEAD. DOING SO FOR ABOUT THREE YEARS THIS HAS CREATED PAIN IN HIS RIGHT SHOULDER. HE WAS FIRST TREATED FOR SHOULDER SPRAIN. ON 7-15-99 HE WAS SENT FOR AN MR I, 7-19-99 IT WAS CONGFIRMED HE HAD A ROTATOR CUFF TEAR. SURGERYNIS SCHEDULED FOR 8-6-99.

EE WAS DOING SOME SHOVELING WORK AND STRAINED HIS LOWER BACK ON THE LEFT SIDE.

EMPLOYEE WAS SWEEPING ON TOP OF SILO'S AND DUST BLE INTO HIS EYES.

EE WAS SHOVELING SHALE FROM AROUND A CONVEYOR HE PULLED THE LOWER MUSCLE IN HIS BACK

EE WAS JUMP STARTING THE MAINTENANCE TRUCK AND THE BATTERY EXPLODED INTO HIS FACE AND BOTH EYE.

EMPLOYEE WAS MOVING A CONVEYOR WHEN SOMETHING BLEW INTO HIS EYE.

EMPLOYEE WAS CHANGING A HYDRAULIC LINE ON A PIECE OF EQUIPMENT, THE HOSE BROKE BLOWING HYDRAULIC FLUID INTO HIS EYES.

EE WAS WAQLKING DOWN STAIRS. WHEN HE REACHED THE BOTTOM HE STEPPED INTO A HOLE CAUSING HIM TO FAL. HE TORE LIGAMENTS AND CHIPPED A BONE IN HIS LEFT FOOT.

EE WAS PULLING METAL SHAVINGS FROM UNDER A LATHEAND CUT HIS PINKY FINGER ON METAL SHAVINGS

EE WAS WELDING AND A PIECE OF SLAG FLEW INTO HISRIGHT EYE

EE WAS WELDING AND FELT SOMETHING IN HIS EYE

EMPLOYEE WAS WALKING DOWN STEPS, HE HIT A PUDDLE OF WATER AND SLIPPED, FALLING DOWN THE STEPS.

EMP WAS MEASURING A TRAILER. WIND WAS BLOWING AND DUST PARTICLES BLEW INTO HIS EYE.

EE WAS UNCLOGGING A HOLE WITH A ROD WHEN IT POPPED BACK HITTING HIM IN THE MOUTH.

EMPLOYEE WAS CUTTING PAPER WITH A KNIFE,IT SLIPPED OUT OF HIS HAND,STABBING HIM IN THE LEG.

EE WAS CLIMBING DOWN A LADDER OUT OF HEAVY EQUIPMENT AND HE TWISTED HIS KNEE.

EE WAS RODDING SHALE AND THE ROD BROKE IN HALF IN HIS LEFT HAND CAUSING A PUNCTURE WOUND.

EMPLOYEE WAS PICKING UP A BEARING PULLER AND THE JAWS CAME TOGETHER SMASHING HIS RIGHT PINKY AND RING FINGERS.

EMPLOYEE WAS USING A SLEDGE HAMMER AND THE HANDLE OF THE HAMMER HIT HIS LEG, CAUSIN THE HAMMER TO DEFLECT HITTING HIS HAND.

EMPLOYEE WAS CLOSING A DOOR AT THE TOP OF A BUCKET ELEVATOR. IT SLIPPED, FALLING ON TOP OF HIS LEFT FOOT.

EMPLOYEE WAS TIGHTENING A BOLT. THE BOLT BROKE CAUSING HIS HAND TO COME BACK AND HIT A LATHE GUARD.

EE HAD CLIMBED TO THE TOP OF A STOCKPILE TO ADJUST A WATER SPRINKLER. HE SLIPPED ON ICE AT THE TOP & SLID DOWN TO THE GROUND.

EE WAS CLEANING DRYED DIRT OUT OF DOZER TRACKS WITH A METAL BAR & PROPANE TORCH. A PIECE OF METAL IMBEDDED IN HIS LEFT EYE ABOVE THE PUPIL.

EE WAS REPLACING BELS ON A CRUSHER & GOT HIS RIGHT HAND STUCK BETWEEN TWO SHEAVES...HE BADLY BRUSIED HIS RIGHT HAND, CUTTING THE TOPSIDE OF THE THIRD FINGER & BACKSIDE OF THE UPPER PALM.

TWO MAINTENANCE MEN WERE PUTTING NEW BELTS AND SHEAVES ON A SHAKER AND GOT HIS FINGER CAUGHT BETWEEN THE BELT AND SHEAVE - SMASHING IT.

HE WAS USING A GRINDER ON A BELT GUARD WHEN THE CLAMP HOLDING THE GUARD CAME LOOSE. THIS CAUSED THE GRINDER TO KICK BACK CUTTING HIS ARM ABOVE HIS LEFT WRIST.

CUMULATIVE INJURY DEVELOPED OVER A PERIOD OF TIME DUE TO KNEE BENDING AND WALKING UP STEPS.

EMPLOYEE WAS GOING TO REPLACE GEARBOX & MOTOR WHEN #2 MAINTENECE MAN LEFT TO LOCK-OUT STARTER BOX WHEN #1 MAINTENCE MAN STARTED DISMANTLEING EQUIPMENT BEFORE #2 MAN GAVE ALL IS CLEAR.

EMPLOYEE WAS CARRYING WIRE MESH SCREENS UPSTAIRS WHEN HE PULLED GROIN.

WAS RUNNING MATERIASL ON THE CONVEYOR BELT WHEN IT STOPPED, HE WENT TO CHECK THE FUSE BOX, 2 WERE BAD. HE REPLACED THE FIRST ONE WITH NO PROBLEM. WHILE PUTTING THE SECOND ONE IN, IT BLEW, BU RNING THE TOP OF HIS HAND AND FLASH BURN TO HIS RIGHT EYE.

THE INJURED PERSON WAS HELPING THE LOADER MAN LIFT A STEEL 2 TON ROLLER WITH A CHAIN, THE EMPLOYEE'S LEFT INDEX FINGER GOT CAUGHT BETWEEN THE CHAIN AND ROLLER, SMASHING IT.

EMPLOYEE WAS LOOKING THRU OBSERVATION GLASSS TO KILN WHEN THE COAL MILL BACKFIRED CAUSING THE GLASS TO BREAK

EE WAS RAISING DOZER UP WITH BLADE AND RIPPER, A WOODEN BLOCK COLLAPSED CAUSING THE DOZER TO JAR THE PLATE OFF THE PRESS. THE PLATE FELL ON HIS FOOT.

EMPL WAS PUTTING BEARINGS ON THE PARTS SHELF & A BEARING FROM THE TOP ROLLED OFF FALLING ON HIS FINGERS.

ELMPLOYEE WAS CARRYING SOME SHAKER SCREEN SUP SOME STEPS, WHEN THEY SLIPPED OUT OF HIS HAND SLIDING DOWN THE LEFT SIDE OF HIS LEG CAUSING A PUNCTURE WOUND.

EMPLOYEE WAS HOLDING A BOLT BETWEEN THE BEARING HOUSING AND FRAME,THE BEARING HOUSING SLIPPED FALLING ON HIS FINGER.

EMPLOYEE WAS WORKING WITH METAL FELT SOMETHING FLY INTO HIS EYE.LEFT EYE FELT SCRATCHY.TWO PIECES OF SHINY METAL WENT INTO HIS LEFT EYE.

THERE WAS AN ELECTRICAL FIRE. THE EMPLOYEE WAS GOING INTO BUILDING TO EXTINGUISH THE FIRE, WHEN HEAT AND SOOT GOT INTO HIS LEFT EYE.

INJURED WAS HOLDING IN THE STARTING COIL ON THE KILN DRIVE ELECTRIC MOTOR WITH A SCREWDRIVER.THE ELECTRIC MOTOR LEAD SHORTED OUT IN THE MOTOR JUNCTION BOX.WHEN IT DID THE BREAKER DID NOT TRIP AND EXPLODED THEN CAUGHT THE WIRING ON FIRE.HIS JEANS WERE ON FIRE,BURNED FIRE ON FACE LIPS UNDER HIS NOSE AND A SMALL PLACE ON HIS LEG.

GRINDING, HAD SAFETY GLASSES ON , WINDY DAY GOT STEEL IN ONE OF EYES, RIGHT EYE

HE WAS JACKHAMMERING CEMENT OUT FROM UNDER THE OLD SCRUBBER. HE HIT AN AIR POCKET IN THE CEMENT AND THE JACKHAMMER DROPPED ABOUT 2 FEET, SMASHING HIS INDEX FINGER BETWEEN THE HAMMER AND A PIEC E OF ANGLE IRON.

THE INJURED WAS BREAKING FREE A ROLLER THAT WAS STUCK,THE ROLLER BROKE LOOSE AND PULLED HIS HAND INTO THE CONVEYOR,IT WAS CAUGHT BETWEEN THE BELTING AND A PICE OF ANGLE IRON AND THE BELT KEPT MOVING,RUBBING HIS HAND.

EMPLOYEE WAS JACKHAMMERING IN THE KILN. HE THOUGHT HE MUST HAVE TWISTED HIS BACK. EMPLOYEE ISN'T REALLY SURE WHEN INJURY OCCURRED, BECAUSED HE DIDN'T EXPERIENCE ANY PAIN UNTIL SEVERAL DAYS AFT ER HE WAS JACKHAMMERING. THIS JACKHAMMERING WAS THE ONLY THING HE COULD THINK OF THAY MAY HAVE CAUSED HIS BACK PAIN.

HE WAS ADJUSTING ROLLERS ON COAL MILL & IT WAS WINDY COARSE HAYDITE DUST HAD BUILT UP AROUND THE COAL MILL & THE WIND BLEW SOME OF IT IN HIS EYES

HE WAS LIFTING A WATER PUMP AT THE CREEK, THE WEIGHT OF IT WAS TOO MUCH FOR HIS BACK AND HE PULLED A MUSCLE.

HE WAS WORKING ON A SECTION OF THE SCRUBBER THE HIGH LOADER WAS LOWERING HIM WITH A CHAIN AND THE CHAIN CAME UNHOOKED AND HE FELL ON SOME FIREBRICK THAT WAS ON THE GROUND LANDING ON HIS BAC K

HE WAS WORKING ON A ROTOR WHICH WAS BEING HELD UP BY AN ELECTGRIC HOIST HE WAS PULLING BEARING OUT WHEN THE BVRAKE ON THE HOIST GAVE WAY SMASHING HIS RT.HAND BVENEATH IT ALSO HIT SHIN BELOW TH E KNEE ON THE SHAFT

HE WAS BENT OVER PULLING A BLANKET UP TO COVER THE SCALPER. HE PUT HIS BACK OUT OF PLACE DUE TO THE WEIGHT OF THE BLANKET.

HE WAS HAULING OUT RAW SHALE DUST IN TRUCK AND THE WIND WAS BLOWING VERY HARD SOME DUST BLEW IN HIS EYE

HE WAS USING A PRY BAR TO MOVE A CHUNK OF RAW SHALE STUCK IN THE CRUSHER,THE CHUNK BROKE LOOSE AND THE PRY BAR STRUCK HIM JUST BELOW THE RIBS.

HE WAS INSIDE T2E BED OF THE TRUCK MEASURING IT CLIMBING OUT HE SLIPPED AND FELL BACK INTO TRUCK

HE WAS WALKING 6ACK FROM THE LOAD TO THE LOADING SHACK,HE DUCKED TO WALK UNDER THE CONVEYOR TO THE SATURATOR AND HE CAUGHT HIS SHOULDER ON THE CONVEYOR,KNOCKING HIM TO THE GROUND.HE WAS LOADIN G SHACK WHEN HE3WENT UNDER THE CONVEYOR.

THE EMPLOYEE WA8 LOOKING INTO THE INSPECTION DOOR BY THE STARGATE FEEDER ON THE COAL MILL WHEN THE MILL BACKFIRED AND PUFFED COAL DUST INTO HIS EYES AND THE FORCE OF THE AIR PUFF PUSHED HIM O FF OF THE CATWA5K.
