On 8/14/06 at approx 8:10 am, while servicing the fan, a change in the mine pressure was recorded on Fan Chart and water gauge was noticed. Upon investigation, a roof fall in an intersection 3 X-Cuts from fan was discovered. This fall measures 18' x 18' x 8' and fall partially obstructs air flow and impedes travel of air course. The fan portal is located in the #5 Entry.
Was moving miner cable out of #6 roadway and pulled right side mid-back
Slipped on belt while exiting H-Ride on the south section.
EE was operating a shuttle car and hit a hole in the roadway #2 entry.
While shoveling on belt, employee possibly pulled a muscle in back.
EE WAS STANDING ON BOLTER BOOM IN DOWN POSITION, INSERTING A TUBE OF GLUE INTO A DRILLED HOLE. EE SLIPPED OFF BOLTER BOOM STRIKING HIS LEFT ELBOW AGAINST THE SIDE OF THE BOLTER.
EE was getting onto the mantrip to travel into the mine when he slipped and hit his left foot on the Rib.

WHILE LIFTING WELDER OFF OF 488 SCOOP THE WELDER DROPPED & IT PULLED A MUSCLE IN THE EMPLOYEE'S SHOULDER WHILE DRAGGING UP THE ENTRY.
WHILE TRAVELING SOUTH ON ST. RT. 119, TURNED HIS TURN SIGNAL ON TO TURN TO HOBERT ENTRANCE WHEN HE WAS STRUCK IN THE REAR BY ANOTHER VEHICLE. INJURED EE HAD PAIN TO HIS LOWER BACK & WAS TAKEN TO THE HOSPITAL FOR X-RAYS.
EMPLOYEE WAS MAKING A POWER MOVE ON WEST SECTION AND WAS PULLING SLACK FROM THE FEEDER CABLE. HE ENCOUNTERED A LITTLE PAIN IN HIS RIGHT HIP.
BAD TOP WITH WET WEAK THIN LAYED LAMINATED TOP. THIS AREA WAS HEAVILY CRIBBED. HEIZMANNS JACKS AND JACKS WITH BEAMS. WE USED LONGER BOLTS WHEN DEVELOPED. WE USED 4 AND 6 FOOT BOLTS STAGGERED PATTERN. SOME 8' BOLTS ALL RESIN FULLY GROUTED. THE AREA WAS DEVELOPED IN JAN. OF 2004 AND WAS 55 FT LONG, 20 FT WIDE, 6 FT HIGH. 9000 FEET FROM WORKING FACE.
WHILE LIFTING BELT STRUCTURE HE WAS IN A BENT POSITION AND STRAINED LOWER BACK PUTTING STRUCTURE IN SCOOP
He was on west mains putting belt structure in scoop for a belt move, While loading structured He aledgly had hurt his back but didn't say anything til about the accident til he come out of end of shift 9-2-05. He then said he had hurt a disc in his back his kidney and right testcial.

Employee was building a block stopping ventilation control. He was cutting a block to size by using a hatchet and striking it with a hammer. While striking the 2 metal tools together, a chip of metal broke off hammer and lodged in his left eye. He was not wearing eye protection.
Working on blocking under cuts of feeder while setting feeder onto belt tail. Smashed rt. middle finger between 2 crib blocks.
EMPLOYEE WAS MOVING CONTINUOUS MINER BACKWARD WITH SLACK HOOKED TO BOOM & BOOM SWING TOWARDS HIM. THE MINER WENT OVER A DIP IN THE BOTTOM WHICH CAUSED THE MINER TO PIVIT DOWN IN THE BACK STRIKING HIS LEFT FOOT.
Changing scoop tire that was flat, while lifting tire on scoop he strained his lower stomach.
Forman suspected injured was on drugs, acting strange. Injured left his work site, hit his supervisor, resulting in a fight; alledges wounds to arms. Injured states that a rock hit him on the back of the neck while operating scoop (refused drug test).
Employee is a outside man. Employee stated he injured eye while welding and not using eye protection. Employee was welding without management consent or direction. Welding was not part of his job. Was not informed of injury until 5-14-05 from Worker's Comp.

EE WAS RIDING MANTRIP TO SECTION FROM SURFACE. MANTRIP RAN OVER A PIECE OF ROCK. MANTRIP BOUNCED AND JARRED HIS BACK WHCIH RESULTED IN PAIN LATER THAT DAY. CALLED IN 2/17/05 INFORMING ME ABOUT ACCIDENT, SAID EE WAS GOING TO HAVE BACK CHECKED AT HOSPITAL

Roof fall 3800' from working face with less than 300' of cover. Fall appeared to be layered slate with coal rider seam 8" thick about 10' above coal seam. This area was bolted with 4' - 6' - 8' bolts (resins). This area had about 20 cribs 4' apart and Hientzmans between cribs several jacks with beams. This area developed 12/30/03. Low cover, rotten top, rider seam.

While roof bolting he was drilling a hole when his pin steel stopped up, while dropping his drill pot down he had his hand on machine drill pot. the steel was stuck in the hole and fell out, hitting his index finger on left hand. his finger swelled

EE WAS SUPPLYING #3 ROOF BOLTER. HE WAS LOADING RESIN GLUE ON BACK OF THE BOLTER AND WHEN HE THREW A BOX OF GLUE HE TWISTED A LITTLE & PULLED MUSCLE IN HIS LOWER BACK.
EE WAS PUTTING UP ROOF BOLT AND WAS PUSHING IT UP IN THE HOLE AND PULLED MUSCLE.
ROOF FALL APPROX. 16'2" WIDE X 15' LONG FELL ABOUT 18" ON ONE SIDE AND 4' ON THE OTHER. IT APPEARED TO BE WEATHERED SLATE AND SHELL WITH A LITTLE WATER. (IN #8 ENTRY 2200' OUTBY FACE 2' INBY S PAD 5373). THIS AREA WAS HEAVILY CRIBBED BEFORE FALL OCCURRED.

DURRING THE WEEKLY EXAMINATION OF THE INTAKE ESCAPEWAY, A ROOF FALL WAS FOUND IN THE INTERSECTION OF THE 5TH X-CUT UG IN THE #7 INTAKE AIR ENTRY. APPROX. 20'X 30'X 5'HIGH. 4FT.FULL GROUT RESIN RODS WERE USED IN THIS AREA AND RIBS HAD BEEN NOTICED. THE FALL WAS TIMBERED OFF ON ALL SIDES AND TRAVELWAY WAS RE-ROUTED AROUND FALL.
WHILE HELPING TO MAKE A CONVEYOR BELT MOVE UP ON THE SOUTH MAIN SECTION, EE SAID HE HAD STRAINED HIS LOWER BACK WHILE LIFTING BELT STRUCTURE.

SOMETHING WAS THROWN FORM THE CAR TO WINDSHIELD OF THE COAL TRUCK CAUSING THE WINDSHIELD T O SHATTER. SOME OF THE GLASS FRAGMENTS HIT THE DRIVER IN THE FACE AND PUT A FEW SMALL CUTS IN HIS FACE, NECK, AND EYES.
EE WAS REMOVING THE HAD SCRAPPER ASSEMBLY FROM THE D-BELT DRIVE ON THE 001 SECTION TO REPLACE THE SCRAPPER BLADE. WHILE LIFTING ON SCRAPER ASSEMBLY HE TWISTED & STRAINED LOWER BACK
EE WAS MOVING MINER ACROSS SECTION WHEN A PIECE OF LOOSE TOP DISLOGDED FROM BETWEEN BOLTED ROOF. THIS PIECE OF TOP STRUCK EE ON TOP OF RIGHT FOOT RESULTING IN A FRACTURE OF FOOT.
LEFT KNEE GAVE OUT WHILE ATTEMPTING TO INSTALL ROOF BOLTS. THIS IS AN OLD INJURY IN WHICH HE HAD SURGERY & HAD BEEN OFF WORK FOR 1 YR. HE RED T WORK ON 5/24/99 WORKED 3 HRS HE COULD NOT PUT ANY WEIGHT ON KNEE WITHOUT CAUSING PAIN WHILE ATTEMPTING TO PERFROM HIS ASSIGNED JOB AS ROOF BOLTER OPERATOR
WHILE BOLTING IN 4 RIGHT X-CUT IN LOWER THAN USUAL CONDITIONS, EE WAS BENDING A SIX FOOT BOLT AND FELT A PULL OR TWINDGE IN HIS BACK.
EE BENT OVER TRYING TO STRAIGHTEN A 6' ROOF BOLT SO IT COULD BE PUT UP, WITH THE BOLTER, AGAINST THE TOP, WHEN HE FELT A SHARP, BUYRNING PAIN IN HIS LOWER BACK.
LAMINATED SLATE ROOF SEPARATED ABOVE ANCHORAGE IN THE AN AREA TRAVELED WEEKLY.
