On Monday 8-14-06 at approx 8:10am, while servicing a fan, a change in the mine pressure as recorded on fan chart and water gauge was noticed. Upon investigation, a roof fall in an intersection located 2 x-cuts underground from the #4 Portal at the Camp Creek #1 portal. This fall measures 16' x 50' x 10' high.

On 8/14/06 at approx 8:10 am, while servicing the fan, a change in the mine pressure was recorded on Fan Chart and water gauge was noticed. Upon investigation, a roof fall in an intersection 3 X-Cuts from fan was discovered. This fall measures 18' x 18' x 8' and fall partially obstructs air flow and impedes travel of air course. The fan portal is located in the #5 Entry.

Roof fall 3800' from working face with less than 300' of cover. Fall appeared to be layered slate with coal rider seam 8" thick about 10' above coal seam. This area was bolted with 4' - 6' - 8' bolts (resins). This area had about 20 cribs 4' apart and Hientzmans between cribs several jacks with beams. This area developed 12/30/03. Low cover, rotten top, rider seam.
