Employee was adjusting the jaw crusher when the spring retaining washer failed causing a laceration and broken right pinky finger.

Employee stated ee felt a pain in back while climbing ladder getting into haul truck

employee tried to pull himself into a grader using the grader ladder and strained his knee

EE was cleaning underneath crusher and poked his right index finger on a piece of barbwire.

EE was using a pry bar to remove ware plate from a screen, when he felt pain in his lower back.

While attempting to get into the cab of one of the trucks, he grabbed one of the handholds and stepped onto the running board and twisted his knee.

Two EEs were making a connection. One pulled the Kelly up while the other set the slip in the table. The joint was set lower than usual. The two tongs were installed. The driller engaged the break-out tong 2-3 times unsucessfully. They decided to use the make-up tong to break the joint. When he engaged the clutch the tong came around the derrick leg and broke victim's hand.

Employee slipped on unknown object near screen rack area. To brace the fall, employee reached for the top screen and lacerated his hand.

EE was bit by a spider while shoveling in the conveyor tunnel.

Employee was tigtening tooth of loader by pounding pin back into tooth when a sliver of metal came off pin and struck employee in abdomen.
