Employee was attempting to remove a v-belt from a sheave on the main drive of a wet screen. Another emloyee was assisting to remove tension on the belt by raising the drive end of the belt pulley while the injured employee attempted to lift the belt off the sheave. While grasping the belt near the motor end the belt moved back over the sheave & pinched injured employee's finger.

Miner was climbing a six foot step ladder. Upon reaching the third rung of the ladder his foot slipped off the rung and he fell to the ground. The ladder was on a concrete pad at a wash plant and soles of boots were wet.

Miner was climbing on and off a crane during shift. He does not recall a specific injury event but experienced pain in his left knee at shift end. He was initially evaluated and diagnosed with a possible cartilage tear in the knee. He continued to work at normal duties until October 24, 2007 when he had arthroscopic surgery to repair the torn cartilage.

Miner was positioning a tie-down chain on a floating conveyor. While using his foot to hold the chain in position on the deck as he wrapped the chain around a cleat, the chain slipped under his foot causing him to twist and strain his back.
Miner walking along beltline when foot got stuck in mud. While pulling foot loose he strained knee. Continued to work at full capacity until 9/18/06 when he was placed on restricted work activity by his doctor. Area had experienced several days of heavy rain prior to date of injury.
Miner was attempting to free a stuck conveyor using a 36 inch aluminum pipe wrench and a 5 foot cheater bar. While pulling down on the wrench/bar with his hands above his head, the wrench handle broke. Miner fell and landed on his tailbone. Wrench had previously been bent.

1) The employee was standing with one foot on the framework of a conveyor and the other on a ledge of a tunnel. 2) The employee was elevated off the ground. 3) The conveyor was running while performing maintenance. 4) He suffered an almost severed pinky finger, lasceration on the palm, belt abrasions and swelling on shoulder and elbow.

EE WAS REMOVING TEMPORARY SAFETY HAND RAILS & PLYWOOD COVERS ON GRIZZLY BARS ON DREDGE. WHILE REMOVING THE LAST OF THE PLYWOOD, HIS FOOT SLIPPED THRU THE BARS. HE USED HIS RIGHT HAND TO CATCH HIMSELF.

EE WAS TRYING TO GET HEAD ROLL BACK INTO POSITION BY PUSHING AND PULLING WHICH RESULTED IN A BACK STRAIN.

EE WAS PRYING CHUNK OF ASPHALT FROM A FEEDER OPENING. THE PRY BAR SLIPPED CAUSING IT TO COME AGAINST OTHER METAL WITH HIS SMALL FINGER IN BETWEEN. TIP OF THE FINGER WAS INJURED INCLUDING SOME TISSUE & BONE DAMAGE.
