While operating bagging machine, contract employee inadvertently pinched right thumb while adjusting the saddle of the bagging station.

Miner was replacing drill steel on a bench drill when pinched thumb between two pieces of drill steel.
Miner was using "walk behind" skid steer to clean spillage off the ground under the roll crusher return conveyor. The employee trammed the machine into the belt structure causing their left hand to be caught between the top of the machine and the metal belt frame.

The office manager was helping a delivery driver carry boxes to the vehicle. One of the boxes began to break. The office manager reached to help. EE fell and struck a vehicle with the left side of face. The employee did not wish to seek medical attention at that time but after persistent pain in the neck decided to pursue it. EE saw a chiropractor 4/26/16.
Employee was removing ice from their car after work in the parking lot. As they walked around the back of car they slipped. They felt pain in left arm but didn't think much of it. At home their arm started to swell & they told their boss but still didn't think there was anything wrong. When the swelling didn't go down they decided to go to the ER & an X-ray showed a broken arm.

While placing the door back on the screw auger of bag house 17 the employees hand got caught smashing his right thumb.
Employee was changing a screen on the 90 hammer screen when his wrench slipped and struck the tip of his left index finger. The impact forced his index finger down on the edge of a screen rail. Employee reported to his supervisor and was taken for evaluation. The laceration on his finger was closed using glue and the employee was given restrictions.
While setting the bearing housing into position at the primary crusher the employee felt a sudden pop in his right shoulder. He immediately notified the Maintenance Superintendent. The result was a strain to his right shoulder
While descending the stairs leaving the breakroom a mine foreman's right foot slipped from under him. He believes it may have been a wet stair or his foot simply landed wrong. He managed to catch himself on the handrail but his right foot slid towards his left foot. He was treated at a local ER where it was determined he had broken his tibia at the ankle.
Employee was loading a tanker with 10 mesh. He was pulling a sample with sample cup, He filled cup and let it rest on tanker lid to get better grip on handle when it fell off lid and he tried to catch it but the weight of cup and sample in cup together was too much and he felt a pop in right shoulder and felt pain go down his arm into finger tips.
EE was driving a brass bushing in the flushing head core, when a chip came off the shaft and went into his left hand. He was working on the fletcher drill. On 05/15/2015 the Operator was notified that the employee decided to have chip surgically removed.
Employee was crossing a roadway enroute to his car parked in an employee parking area. He was going to start his car prior to clocking out. As the employee transitioned from the pavement to the gravel lot he slipped and fell on newly fallen snow. As he fell he landed on his ankle causing a sprain.
The injured employee was walking from the head of the TVA Silo Siding when he slipped and fell on ice that had accumulated in the area. When the incident occurred the employee was leaving the area and heading to the scale house to get coffee and warm up. The employee requested medical attention where he was diagnosed with a strain.

Employee was attempting to tighten a 4" pipe coupling in a vise. As he put pressure on the handles to tighten the vise the coupling slipped. This allowed the vise handle to free wheel causing employee's hands to slip and loose his grip. The sudden jerking motion injured his shoulder.
Employee was using the welding machine to air arc/source a large dump truck bed and while leaning to one side a hot spark/slag piece hit his ear.

Employee has been in the position of a office worker for 19 years. Reported symptoms of numbness tingling which was diagnosed as Carpal Tunnel Syndrome on October 9, 2012. Had surgery to correct this on January 30,2013

On 6/2/11 employee slipped on the steps of his roof bolter as he was attempting to dismount the machine. He reached with his hand to attempt to catch himself and was successful in preventing a fall. However, he noticed some pain in his middle to lower neck region.

Employee was rolling up welding leads when his right foot slipped and caused him to transfer his full bodyweight in a twisting motion to his left leg. This force created a simple fracture of the left fibula.
Employee was attempting to refill a drill with diesel and as he was walking he sprained his ankle when he stepped on a rock.
EE was walking in the back the back door to the lab when he stepped on a rock turning his ankle.
EE was dumping over a stockpile with a Cat haul truck when the stockpile failed which caused the truck to overturn. MSHA was immediately notified due to serious potential of accident.

Employee was lowering a chute with a hand crank device. He lost control of the crank sending it into a freespinning motion contacting his face. This caused a broken nose, fractures to bones in the face, and lacerations.
Most likely the cause of the fire was leaking roof that caused an electrical malfunction leading to an electrical failure.The resulting fire resulted in a loss of the building. The building will be replaced and that would elimate any roof structural concerns and any possible electrical malfunction that would have led to the cause of the fire.
Employee was attempting to move his impact wrench when he contacted a hand rail causing numbness in his right arm.
Lightning strike on surface initiated underground face rounds. No injuries or damage. Mine was utilizing electric intiatition system for nonel face rounds. Potential pathway for lightning to induce current in blasting system was determined.

IE noticed that one of the straps was not going to contact the forks as co-worker was attempting to lift the super sack with the forklift. IE tried to place the strap on the fork on the fork, catching his left middle finger between the strap and the fork. This mashed his finger and he went to the emergency room. A splint was placed on the finger.
EE was realing in a anfo hose when he dislocated his left shoulder.

1906 Loader caught on fire. EE was operating the machine. He thought the flames were actually coming into the cab (they were not) and turned the machine off and bailed out of the cab. He injured his foot/ankle dismounting the machine, but was able to get the fire extinguisher. He did not have to use the extinguisher because the flames went out on their own.

Employee was attempting to close the bottom slide on rail car 103697. He caught his finger in the slide and mashed his left index finger. This caused a laceration that took 3 sutures to close.

Employee was hooking up a trailer to the ball hitch on the bumper of a pickup truck. He was moving the tonque of the trailer back and forth to get the tonque to seat when it mashed his finger.
Employee was removing material with a hand shovel when he grazed a sharp edge of metal with his arm.
Miner was climbing railcar, rung broke causing miner to fall.
EE dumped at the crusher and did not lower the bed on the haul truck. The bed impacted the brow at the second level entrance. Jamming the cab of the truck into the back.
Repetitive action
Employee notice that his eyes were irrated after being around diesel smoke. He was take to the doctor and given prescription eyedrops. The doctor was unsure of the cause of the irritation.

Employee was shoveling under the crusher when he felt a sharp pain in his lower back. The doctor said he had a back strain.
EMPLOYEE SLIPPED AND FELL ABOUT 2-3 FEET AND LANDED ON HIS HAND. THIS CAUSED HIM TO SPRAIN HIS RIGHT HAND. HE WAS WALKING ON A CONCRETE PAD OF A SILO AND SLIPPED OFF THE PAD ABOUT 2-3 FEET.
Unkown (8/4/04 per call to Brian Bell, exact time and circumstances could not be determined. Employee currently on Workman's Compensation).
EE WAS CUTTING SOME SKURTBOARD FOR THE SCREENS AT THE AGG PLANT. HIS KNIFE SLIPPED FROM HIS HAND AND CUT A WOUND ON THE INSIDE OF HIS LEFT FOREARM. THIS REQUIRED FOUR SUTURES TO CLOSE THE WOUND. THE DOCTOR RELEASED HIM TO WORK THAT SAME NIGHT.

A LOAD BEING LIFTED WITH NYLON KEVLAR FIBER FLAT SLING OVER THE EDGE OF THE FORK TIRES. AS THE TINES WERE SLIGHTLY POINTED UPWARD WHILE LIFTING THE LOAD, THE WEIGHT OF THE SAID LOAD CAUSED SLI NG TO SLIDE DOWN TINES & CUT AND BURNED THE FIBER, CUTTING THE SLING INTO. THE LOAD THEN FELL ONTO THE RT LEG OF THE INJURED EMPLOYEE.

EE WAS RESTRICTED DUTY FOR 10 DAYS DURING THIS TREATMENT.

EMPLOYEE WAS DRIVING A PIN INTO CYLINDER WITH A HAMMER WHEN A BUR CAME OFF OF THE PIN AND GOT IN HIS FINGER. HE WENT TO A DOCTOR ON -12-7-89 AND GOT THE SPLINTER REMOVED AND THE DOCTOR WROTE HIM A PRES. FOR ANTI-BIOTICS.
DOWN LEVER ON BOOM HIT WHILE BUCKET OF LIFT WAS ON LEDGE BOOM PULLING DOWN UNTIL PRESSURE GREAT ENOUGH TO FORCE BUCKET OVER LEDGE PUSHING TRUCK AWAY FORM FACE BUCKET HIT FLOOR BUCKET TURNED UP WITH EMP INN BUCKET.
EMPLOYEE WAS MOVING DRILL FROM ONE HEADING TO ANOTHER 100 FT AWAY WHEN THE DRIVE SHAFT CAME OUT AND THE BRAKES FAILED. THE DRILL ROLLED BACKWARD AND STRUCK A PILLOW. THE EMPLOYEE S NECK WAS SH IP LASHED WHEN THE DRILL STRUCK THE PILLOW. EMPLOYEE WENT TO DR ON 2-6-89 AND STARTED LOSING TIME SAME DAY.
