Employee was using a 3/4 ton chain ratchet to replace return roller in tunnel. EE wrapped chain ratchet hook around piece of 8" channel, after raising the belt EE bent over to put roller in, the chain came loose causing hook to hit EE in nose and mouth

Employee was taking a two wheeled dolly that carries small rolls of electrical wire up the steps to the loadout when EE slipped, when EE landed EE's ring finger caught between grating floor and the dolly handle causing a fracture and laceration requiring sutures.

Employee was cutting angle with a torch. It fell & came back up and hit EE's left arm, cutting EE's arm.

Employee was pulling a rope across the spillway between pond #1 and pond #2 when ee slipped twisting left knee.

Employee noticed something wrong with eye on way home from work. When ee woke up it was irritated and ee notice something stuck to eye. Employee received medical treatment for this injury at an optometrist office on 10/05/18.

Employee was removing a pipe elbow with a chain fall when it became fouled and shifted suddenly, catching hand in a pinch point between the elbow and an adjacent pipe.

12/1/17 EE & another EE was removing a piece of metal from chute when EE started experiencing pain in lower back. EE completed shift and filed an incident report that lower back was hurting, EE worked the following shift. EE sought medical attention on 12/2/17 and given full release. 12/7/17 went to different hospital and taken off work. Pain management for back since 8/2/12.

While attempting to remove solid concrete blocks from the raw coal crusher chute employee reached in to remove a block and another block fell striking employee on the hand, cutting right ring finger.

Employee was working on a loft step ladder. EE went up the ladder to grind a bevel. EE got to the third step and the extension cord that EE had for the grinder was short. As EE turned to pull on the extension cord to get more slack, feet slipped on ladder rung causing EE to hit side on top of ladder.

On 04/22/2016 this Plant was notified that this employee was diagnosed with pneumoconiosis.

Employee was a passenger in a L7000 Ford Utility truck when the operator of the truck lost control of the vehicle and struck a utility pole and traveled over an embankment.

Employee, along with three other workers, was taking rollers off of a belt line. One of the rollers slipped and fell back into the rollers channel frame. Employee's thumb was in the channel when the roller fell. Employee received 4 sutures due to the laceration on right thumb.

EE was walking from load out back when a gust of wind blew rust into his eye. We feel this should be classified some other way than being a reportable accident this wasn't because of the mine environment employee was wearing his glasses also. This wind had been blowing like this for several days. He didn't cease work on 2-23-12. He didn't seek medical attention until 2-27-12.

Employee was relocating a 1.5 ton chain. When he went to pick the chain up, he felt a pain in left side and groin. Employee continued to work. Went to doctor on 3/13/12 and diagnosed with hernia. Surgery will be scheduled in near future. Employee is continuing to work full duties until the surgery.

EE was closing door on end loader, when door closed his hand was in door frame and door closed on middle finger cutting it. Resulting in laceration requiring stitches.

Member was replacing the cutting edge on a blade for a bulldozer using a 1/2" impact gun. As he was doing this he stumbled striking his back on the tracks of the dozer. Member brought in documentation from his doctor on 5/27/2010

injured was doing a line bore on a dozer and got index finger caught on drill bit causing a laceration his left index finger requiring stitches.

EE was working on the clean coal feed belt building floor. Crew took lunch in load out building to get out of the heat, injured realized he left hard hat and was going to get it. He walked into a piece of angle iron that was sticking out from the steps that was holding the hand rail to the steps (approx 5'6" off the ground) caused a laceration to his head. Not normal walkway.

While installing a 1/2 nut & bolt in a rail over insured's head the nut slipped out of his hand & hit him in the front of tooth. Injured stated he chipped his tooth.

There was a non-injury accident at the #10 coal feeder in the clean coal draw tunnel. The belt went off & smoke was observed at app 9:51 AM. Plant foreman could not determine the source of the smoke so he notified Superintendant & the Safety Dept. Contacted the State at 10:16 AM & the MSHA Hot Line at 10:28 AM. The fire was extinguished at 11:30 AM & verified out at 12:02 PM.

Employee was using a 1" impact wrench with a swivel. The pin came out of the swivel and caught his left thumb causing a laceration. He received 4 stiches and a Tetnus shot.

Employee was doing some housekeeping and while throwing some bags into trash container some of the chemical powder blew into his eyes. The left eye was irritated the worst. Received antibiotic ointment.

Member was helping take a pump apart. Moving the impeller to the side of the pump, it fell over landing on his left foot.

EE was taking engine compartment door off of dozer to gain access to starter. EE squated down to lift door and his right knee started hurting. First day away from work 5/4/06.

EMPLOYEE WAS WALKING OUT ON THE THICKNER AND AS HE WALKED HIS FOOT SLIPPED THROUGH A HOLE WHICH WAS CAUSED BY A DEFECTIVE WELD. HIS LEFT KNEE HIT THE GRATING CAUSING A LACERATION REQUIRING STITCHES.

WALKING BEHIND BELT LINE, TRACING A CUTTING TORCH LINE. WATER WAS APPROXIMATELY ANKLE DEEP SO A SUMP HOLE COULD NOT BE SEEN. THE GRATING HAD BEEN REMOVED AND NOT PUT BACK. EMPLOYEE FELL AND CUT RIGHT HAND.

EE WAS DISMOUNTING A KATMASU ENDLOADER TO CLEAN COAL FROM A COAL PAD WHEN HE SLIPPED AND FELL FROM THE LOADER LOADING ON HIS BACK.

EMPLOYEE STATED HE WAS WALKING ON SHAKER AND EMPOYEE STUMBLED OVER ON HIM AND TWISTED HIS KNEE. CONTESTING COMPENSATION.

EE WAS CHANGING OUT BAD FLOOD LIGHT BULB, BULB BROKE, CUTTING INDEX FINGER.

INJURED WAS OPERATING A TRACTOR-TRAILER AND WAS DUMPING A LOAD OF COAL. ACCORDING TO THE INJURED DRIVER, THE TRAILER WAS 3-4 STAGES UP WHEN SUDDENLY THE LOAD SHIFTED OR SOMETHING BROKE ON THE TRAILER, CAUSING THE TRAILER TO FALL OVER TOWARD THE DRIVER'S SIDE, STRIKING THE GROUND.

EE WAS LIFTING A 42" IMPACT IDLER UPON THE FORKS OF A BOBCAT FORKLIFT, APPROX 1 FOOT FROM THE WORKING PLATFORM AND EXPERIENCED PAIN IN HIS BACK.

REMOVING BELT STRUCTURE W/BOOM TRK EE WAS ON GROUND THE SUSPENDED STRUCTURE CAME APART & A PIECESTRUCK EE ON THE HEAD HE WAS WEARING HARD HAT

EMPLOYEE WAS MOVING A PIECE OF 6' PIPE. HE EXPERIENCED PAIN THAT NIGHT THAT GOT WORSE. THE DOCTOR STATED THAT HE HAD TWO CRACKED RIBS ON HIS RIGHT SIDE ALONG WITH SWELLING.

EE WAS LIFTING ON A HEAVY PUMP IMPELLER. HE FELT PAIN IN HIS LOWER ABDOMIN. EE HAS NOT LOST ANY WORK TIME DUE TO THIS INJURY.

EXPANDED METAL ON FLOOR BROKE THRU RT LEG WENT THRU METAL, LT KNEE STRUCK FLOORING.

WHILE HOISTING CHEMICALS ON AN ELECTRIC HOIST THROUGH A HOLE IN A PIECE OF BAR GRATE. THE BAR GRATE CAUGHT ON THE HOIST CHAIN AND FELL DOWN THE CHAIN CAUSING INJUREY TO EMPLOYEE'S LEFT HAND AN D FINGERS.

THE VICTIM WAS USING PINCH BAR TO ALIGN BOLT HOLES IN THE ROPS STRUCTURE OR A D9G DOZER WHEN THE BAR SLIPPED CAUSING HIM TO FALL APPROXIMATELY 4 1/2 FEET. HE LANDED ON HIS FEET BUT OFF BALANCE CAUSING INJURY TO HIS ABDOMEN.

WHILE REMOVING A 16 FOOT LONG 8 INCH DIAMETER PIPE AN OLD SUPPORT BRACKET WELD BROKE RELEASING THE PIPE TO FALL ON THE VICTOMS LEFT FOOT.

ACCIDENT VICTIM WAS STANDING ON TRACK TO CHECK OIL LEVELS. HE STEPPED ON LOOSE TRACK PAD. PAD TURNED CAUSING VICTIM TO FALL TO GROUND HITTING ARMS ON TRACKS AND FUEL CONTAINER.

WHEN HANGING V BELTS ON OVERHEAD HOOKS IN WAREHOUSE EMP FELT SOMETHING IN HIS EYE. RUBBED IT TO STOP THE ITCHING FEELING. INJURY WAS NOT REPORTED TO OPERATOR UNTIL 9-7-96.

EE WAS HELPING TO REPAIR A DANIELS WASHER. WHILE REMOVE PARTS OF WASHER, I SLIPPED/DROPPED, CUTTING DISTAL RIGHT INDEX FINGER AND FRACTURING DISTAL PHALANX.

EMPLOYEE WAS CUTTING BOLT - SLAG BUILT UP ON END OF TORCH - EMPLOYEE USED LEFT HAND TO REMOVE SLAG FROM END OF TORCH - DID NOT TURN TORCH OFF - WHEN SLAG WAS REMOVED TORCH RE-LIT AND BURNT IND EX FINGER AND BACK OF LEFT HAND.

WHWEN SWINGING THE 2 TON CAPACITY CHAIN HOIST FROM ONE SIDE OF A LOAD TO THE OTHER CHOKER THE HOHIST WAS HUNG ON SLIPPED OFF THE HANGER HOOK OF THE HOIST LETTING IT FALL ABOUT 10 FEET STRIKING EE ON HIS RIGHT SHOULDER AND NECK.

EE SLIPPED ON FLOCK SPILL & FELL DOWN STEPS FROM 5TH FLOOR TO 4TH FLOOR & LANDED ON HIS TAILBONE

INJURED WAS TIGHTENING WATER CLAMP ON GLAND WATER LINE ON HEAVEY MEDIA PUMP END OF SCREWDRIVER SHATTERED CAUSING PC OF METAL TO STRIKE INJURED IN RIGHT EYE.

INJURED WAS REMOVING PIECE OF METAL FROM SIDE OF CLEAN COAL CRUSHER METAL BECAME CAUGHT ON ROLLER PULLING INJUREDS RT HAND INTO ROLLER CAUSING LACERATION TO RT HAND AND WRIST.

INJURED WAS RUNNING UP STEPS-HIT HEAD ON OVERHEAD BEAM-SORE OR BRUISED NECK.

INJURED WAS REMOVING CHEMICAL LINE...HOSE CAME LOOSE SPRAYING 8853 FLOC-CHEMICAL INTO RIGHT EYE.

WELDER WAS WELDING AR LINER IN HOPPER AN EXCAVATOR WAS USED TO HOLD METAL IN PLACE EXCAVATOR BUCKET SLIPPED HITTING WELDER ON TOP OF FOOT BROKE 3 BONES ON TOP OF FOOT-WELDER WAS WEARING HARD T OES AT THE TIME.

WORKING ON HEAVY MEDIA CYCLONES BUMPED RT ELBOW ON PROTRUDING BOLT BRUISED AND SWELLING ON RT ELBOW.

DRIVING PIN OUT OF ARM ON LOADER W LARGE SLEDGE HAMMER MISSED PIN RT HAND HIT ARM LACERATION AND BRUISE TO PALM OF RT HAND.

CAR DOORS BAR SLIPPED HIT IN IN L JAW BROKE PARTIALS.

DESCENDING WALKWAY CAUGHT FOOT ON WALKWAY- TWISTED RIGHT KNEE.

EMPLOYEE WAS CLOSING THE RR CAR DOORS USING LONG BAR WHICH IS PLACED IN CAR OR LATCH OF CAR.BAR SLIPPED OUT OFF STRIKING EMPLOYEE IN TOP OF HEAD.
