Dredge Miss Kate Metal/Non-Metal

While putting bolts in pipe flange in ladder on dredge. Holding a bolt when another employee hits it with a hammer. The hammer slipped and struck right pointer finger.

The top row of paddles was installed on dual log-washer shafts. While using a pipe wrench to turn shaft for additional paddle installation the unbalanced weight of the shafts caused the shafts to rotate quickly, the quick rotation caused the pipe wrench to turn with the shaft, pinning employee's wrists between the pipe wrench the adjacent metal enclosure.

Repairing the log washer, walking to the job and fell into. Received laceration to head, and scrapes on both legs.

storing the 2" X 100 ft water hose used to wash the dredge deck and barges. As ee was pulling the charged hose, ee twisted and felt a sudden lower back pain.

Replacing fitting elbow near sweco shaker from the catwalk. the person holding it dropped it the other tried to catch and smashed left pinky finger

Port deck at gravel conveyor hand wench. Moved conveyor flap out, dogged it off handle at hand wench struck the foreman above his left eye.

Dredge mining winter repair - replacing and realign #3 sand screw liner plate - weight 300-400 lbs. was 4' X 8" X 1/4" steel. Dropping liner plate into screw box landing on hand of employee.

Was putting wire rope on gravel barge and the wind took off, pushed the barge and the wire rope got tight, knocked him to the deck of the dredge and landed on right shoulder. Compensated till 4-5-14 then MRI (4-14-14) to rule out fracture or tear

Was tightening lugs in the starter box, box blew out.

Lifting chute on the front of the sweeco shaker with a come along. Slipped and caught finger on the Sweeco trough.

EE was welding a brace on logwasher. EE placed 6' (2x8) board on 3rd rung of ladder and guardrail of logwasher, (unsecured)for platform. Scaffold board slid, causing him to lose his balance and fall into the logwasher, approx 44" from bottom of the board.

Employee was putting the flipper door out on conveyor. As he was turning the handle, the pulley broke, handle spun out of employee's hand and handle struck employee in the forehead.

OTHER EMPLOYEE WAS MOVING A BARGE WITH WINCH. AS BARGE WAS MOVING THE WINCH CABLE UNEXPECTEDLY CAME OFF STRIKING EMPLOYEE IN THE LEGS

EE WAS PLACING A CABLE ONTO THE QUARTER CAULE OF A TOWING BARGE, WHEN HE BENT OVER TO PERFORM HIS TASK HE FELT A SHARP PAIN IN THE MIDDLE OF HIS BACK.

FOUR EMPLOYEES WERE ATTEMPTING TO LOWER A METAL PLATE ONTO DREDGE PIPE. PLATE SLIPPED & SMASHED ONE EMPLOYEE'S FINGERS.

EMPLOYEE WAS CARRYING HUBS FOR GRAVEL SCREEN DOWN STEPS TO BE PUT IN JON BOAT. EMPLOYEE WAS UNAWARE OF HIS INJURY UNTIL 10-25-01.

LOWERING CHUTE AND CAUGHT HAND IN PINCH POINT. CUT LEFT FINGER ADN REQUIRED 9 STITCHES. EE RETURNED TO BOAT PILOT DUTY AFTER TREATMENT.

EE ASSISTING BY HOLDING PIPE FOR WELDER WHEN A PIECE OF SLAG WENT IN LEFT EAR.

EMPLOYEE TIED JON BOAT TO BARGE (SWINTEK) AND CROSSED OVER JET PIPE STEPPING ON TIMBER HEAD, HIS FOOT SLIPPED AND RUPTURED ACHILLES TENDON/ LEFT LEG.

WAS PULLING CABLE ON BARGE AND GOT FINGERS BETWEEN BARGE AND CABLE AND BURR ON CABLE CUT FINGER.

WAS PULLING BARGE CABLE OVER TIMBERHEAD TO DROP BARGE DOWN & SPRAIN LEFT ELBOW.

WAS TURNING CRANE ON WINCH TO MAKE FLAP CHUTE ON END OF CONVEYOR THOW SAND TO SIDE OF BANRGE BEING LOADED AND HANDLED ON CRANE SLIPPED OUT OF HIS HAND STRIKING HIM ABOVE EYE CAUSING A SMALL CU T AND BRUISE.

EMPLOYEE WAS TAKING 5/8 CABLE FROM LOADED BARGE TO HAND TO OTHER ON EMPTY BARGE AND DID NOT HAVE ENOUGH SLACK. WINCH OPERATOR WAS TO SLACK CABLE. HE HIT WRONG BUTTON, THIS TAKING UP SLACK & HI TTING EMPLOYE IN LEG WITH CABLE KNOCKING HIM DOWN ON BARGE.

WAS STARTING A PUMP BY PULLIN RECOIL ROPE, WHEN HE PULLED ROPE IT PULLE DTO STOP, INJURING RIGHT HSOULDER.

WAS REPLACING 6" PIPE ABOUT 3' LONG FROM SAND TANK TO SAND SCREEN & STRAINED LOW BACK.

WAS PUMPING BARGES AND PUMP VIBERATED TOWARDS EDGE OF BARGE & EMPLOYEE GRABBED UP PUMP.

WAS WORKING AROUND A SAND SCREW WHEN HE SLIPPED AND CAUGHT HIMSELF AND PULLED HIS BACK.

WAS WALKING ON LINE DECK ON BARGE & TRIPPED HITTING LEFT KNEE

WORKING ON GRAVEL CHUTE WITH A PERSON CUTTING AND HOT PIECE OF STEEL SLAG WENT INTO BOOT AND BURNED ANKLE.

WAS MOVING OR DRUM FROM MT BARGE TO ANCHOR FLAT WHEN HE FELL ONTO LOVE DECK OF ANCHOR FLAT.
