Employee was tarping a truck. When climbing down employee slipped and fell. Conditions were cold & snowy.

Employee was putting down a 100 lb bag when EE felt a sharp pain in their back.

Employee was running palletizer, when EE brought the table back EE's right index finger got caught in the pinch between the first and second table.

Railing broke after employee leaned over it to unjam a bag causing EE to fall and sprain their back.

Employee was working on palletizer and pinched a finger on the bed of palletizer, scraping some skin off.

Employee went to grab a bag of sand as it was falling off palletizer and hurt/sprained elbow.

Employee went to remove a remove a broken bag from the incline conveyor belt when they tripped over the wire support and fell on left knee.

Employee was working on trying to get the packer unjammed when EE cut EE's hand on a piece of metal.

A piece of platform grating near the surge elevator was missing (maintenance was working on it) EE didn't see it and one leg fell through.

Normal work station position sliding 80 lb cement bags on air table while palletizing bagged product. Worker stated he heard pop in shoulder and felt pain after incident in right shoulder.

EE was loading a flatbed trailer. EE placed his hand on the forklift mast and crushed three fingers when the forklift operator moved the mast.

Employee was injured attempting to turn on baghouse control switches for dryer fan motor. Control panel exploded hitting employee in face and chest.

EE STEPPED ON NAIL WHILE WALKING TO BREAK ROOM. HAD CHANGED OUT OF HIS REQUIRED STEEL TOE SHOES. EE REMOVED NAIL AND WAS SENT TO BE TREATED. TETNUS SHOT ADVISED.

IMPROPER LIFTING OF BAGGED MATERIAL. NOT QUALIFIED TO ASSESS DEGREE OF DAMAGE. ASSUME MUSSEL PULL OR LOWER LUMBAR MUSSEL SPRAIN OR STRAIN.
