EE was greasing the bucket of the unloading crane at the down river dock when EE reached over the bucket and caught a burr (metal shaving) on the forearm causing a 9/16 inch long laceration to EE's left forearm. EE received 2 stitches and a tetanus shot.

Employee has had a confirmed STS in his hearing in both ears. He was tested and re-tested and sent to Family Dr for blockage. He still shows shift that cannot be determined in the 2K and 3K frequencies as to work or medical related. He had substantial loss upon base line in 2/00.

Employee was removing barge wire off of cavel on dock when the wire had flipped under tension and jerked his arm straining his wrist. Wire rope has lay memory and when it stuck in clete the energy in the tensioned wire rope caused it to roll while he had it in his grasp. Doctor indicates strain.
2 ee were replacing cutting edge of Hitachi bucket and dropped the cutting edge on ee 4/5 phalanges smashing them. He had it braced with cribbing and it moved dropping 3 inches onto the deck of the dock. 11 sutures.
EE had a part of a bucket get caught on a bucket elevator, and the part slipped hitting him in the mouth. They were jogging (maintenance)the elevator looking for a pin that had lost its cotter key to repair it. The wear of the pin caused the bucket to move lodging under a lip of the elevator and jogging it caused the bucket to spring hitting him in the mouth as he watched it.

EE was opening cab door of his loader, and slipped while standing on ladder and caught himself aggravating his right elbow. He was originally seen back in November, but it is still bothering so he went to a specialist today that injected his elbow and was prescribed pain management night time drugs now making it recordable. He is back in his loader with no restrictions.
EE was working on Water Truck MQ0373 and he smashed his thumb between the rear leaf spring and pry bar. He was sent for medical treatment and was placed on light duty status, and prescribe a pain medication that was NEVER filled.
Employee was assisting with a maintenance task of re-assembling an auger when he lifted a blade and felt a discomfort in his abdomen that did not go away. He was sent for medical evaluation and has been diagnosed with an inguinal hernia on his left side. He has restrictions that can be met as of now, and is being scheduled for surgical repair.
