Employee was trying to move a larger rock with the loader. The rock slid out of the bucket causing the back end of the loader to slam down, jarring the operator. The employee was taken to OMP for evaluation and placed on restricted duty.

EE was driving a pit 60 Ton Euclid from the jaw back to the pit empty. EE blacked out, due to personal medical reasons, and drove through a berm & off a 20 foot descent and wreck injuring face and leg with sutures. EE was buckled in.

USING A PRYBAR TO PRY ON A MOTOR. THE PRYBAR SLIPPED OFF THE MOTOR CAUSING HIM TO LOSE HIS BALANCE. HE FELL BACKWARDS AND HIT HIS HEAD ON CONCRETE CAUSING A LACERATION TO HIS HEAD BEHIND HIS EAR REQUIRING SUTURES.
EE WAS BENT OVER UNDER A 6" IBEAM TO TURN ON A WATER VALVE. WHEN HE STOOD UP HE STRUCK HIS HEAD ON THE STEEL IBEAM CAUSING A LACERATION TO HIS SCALP REQUIRING SUTURES.

VICTIM WAS HELPING ANOTHER EE REMOVE BOLTS & NUTS FROM A SCREEN DECK, ABOUT 7' HIGH. ONE OF THE NUTS DROPPED AND HIT THE VICTIM IN THE MOUTH, CAUSING A BUSTED LIP AND LOOSENED BRIDGEWORK INHIS MOUTH (REFERRED TO DENTIST).
VICTIM WAS USING A SHOVEL TO REMOVE LOOSE MATERIAL IN THE SCREENING AREA WHEN HE STRAINED HIS RIGHT SHOULDER.
VICTIM WAS CLEANING A SECTION OF A DOZER CUTTING EDGE. HE TURNED IT OVER AND PINCHED HIS FINGER BETWEEN THE EDGEAND ANOTHER PIECE OF STEEL CAUSING A LACERATION TO HIS LEFT RING FINGER REQUIRIN G SUTURES.
VICTIM WAS REMOVING A COUPLING FROM A PIECE OF 3INCH CONDUIT WHEN ANOTHER PIECE OF CONDUIT ROLLED ONTO HIS FINGER CAUSING A LACERATION REQUIRINGSUTURES.
