EE was coming down the plant from cleaning the walkways. When EE got behind the old #1 screen EE slipped on ice. EE went to catch self and tore muscles in shoulder.

Employee was using haul truck to load barges. As employee was being loaded the truck in staging area began to back up and the rear right corner of the bed hit the cab of the employee being loaded truck door. The employee was able to maneuver through the cab & out the passenger side door. Lacerations & abrasions to the groin area were a result. Accident under investigation.

EE was in the adjustment area on the jaw crusher trying to slide the block behind the toggle plate cover to center it up. The cylinder on the porta-power was at an angle and when pressure was applied, it kicked out and hit EE in the face and left arm. This resulted in 3 stitches between the nose and mouth.

EE was running the water truck. When EE exited the water truck EE stepped on uneven ground and rolled right ankle. This resulted in EE seeking medical attention and was given restricted duty sitting work activity only.

EE was running the WA/700 in the pit when a piece that someone had added to the loader fell and hit EE's arm. The piece held the pre-shift tickets for the equipment. EE did not seek medical attention until 12/04/2017. EE received medical treatment at this time. Fluid was drained from the left elbow and returned to work at full duty.
EE was loosening the compressor fill cap of a Sandvick T35 with an adjustable wrench. When the cap let loose, the wrench pinned EE's middle finger against the site glass busting the finger. EE went to the doctor and received six stitches.

EE was checking the oil in a Komatsu WA600 when right hand struck the engine compartment door. 24-48 hours later EE was diagnosed with cellulitis of the hand. EE was prescribed Rx to treat the cellulitis.

EE came to the office and complained of eye irritation at 3:15 PM, and stated at some point in time he got something in his eye. He drives a truck stripping dirt with cab closed. Flushed, and went to Dr who prescribed antibiotic drops.

EE was climbing out of the skid steer when he missed the step and he twisted his leg when he fell.
Employee has experienced a MSHA reportable 25 decibel shift in hearing.

EE was installing a troughing roller and he smashed his fifth finger on right hand between frame of conveyor and roller. He had assistance available, but tried to perform job without assistance, resulting in a smashed finger. He is on light duty.
EE released the locking mechanism on the tag line for the cable crane bucket, and it uncoiled and it violently struck him in the left shoulder / shoulder blade a couple of revolutions. He was wearing all his equipment and a life jacket took a majority of the energy. He was sent to doctor for examination and treatment for some bruises/abrasions.

Climbing out of a hopper barge using a rope to pull himself out. The rope broke and he fell back down into the barge causing a sprained ankle.
Using a cleanup loader to scrape the bottom of a coal barge. The loader bucket hit a burr on the barge floor causing the loader to stop. He hit the dash with his knee and leg resulting in a cracked bone in his leg. Did not go to the doctor until 8-13/07.
