Evansville Mill: 76 MSHA citations Metal/Non-Metal

Employee has had a confirmed STS in his hearing in both ears. He was tested and re-tested and sent to Family Dr for blockage. He still shows shift that cannot be determined in the 2K and 3K frequencies as to work or medical related. He had substantial loss upon base line in 2/00.

Employee was removing barge wire off of cavel on dock when the wire had flipped under tension and jerked his arm straining his wrist. Wire rope has lay memory and when it stuck in clete the energy in the tensioned wire rope caused it to roll while he had it in his grasp. Doctor indicates strain.

2 ee were replacing cutting edge of Hitachi bucket and dropped the cutting edge on ee 4/5 phalanges smashing them. He had it braced with cribbing and it moved dropping 3 inches onto the deck of the dock. 11 sutures.

EE had a part of a bucket get caught on a bucket elevator, and the part slipped hitting him in the mouth. They were jogging (maintenance)the elevator looking for a pin that had lost its cotter key to repair it. The wear of the pin caused the bucket to move lodging under a lip of the elevator and jogging it caused the bucket to spring hitting him in the mouth as he watched it.

EE was opening cab door of his loader, and slipped while standing on ladder and caught himself aggravating his right elbow. He was originally seen back in November, but it is still bothering so he went to a specialist today that injected his elbow and was prescribed pain management night time drugs now making it recordable. He is back in his loader with no restrictions.

EE was working on Water Truck MQ0373 and he smashed his thumb between the rear leaf spring and pry bar. He was sent for medical treatment and was placed on light duty status, and prescribe a pain medication that was NEVER filled.

Employee was assisting with a maintenance task of re-assembling an auger when he lifted a blade and felt a discomfort in his abdomen that did not go away. He was sent for medical evaluation and has been diagnosed with an inguinal hernia on his left side. He has restrictions that can be met as of now, and is being scheduled for surgical repair.

EE was operating a river winch on the lower crane dock, when a wire cable broke on the sheave causing a whiplash action which recoiled back around a protective guard, striking employee in the arm causing a laceration.

night time security guard (an employee of The Ira E. Clark Detective Agency, Inc.) was walking up steps to guardhouse when he slipped and fell causing a laceration to his right arm requiring sutures.

Using an impact wrench to remove a bolt from a loader bucket. The bolt head turned and cut left middle finger requiring sutures.

EMPLOYEE WAS ADJUSTING A ROLLER STRIKING IT WITH A HAMMER. ON THE BACK SWING WITH THE HAMMER HIS HAND CONTACTED ANOTHER ROLLER CAUSING A LACERATION TO HIS LEFT HAND REQUIRING SUTURES.

INSTALLING A METAL SCREEN IN A SHAKER WHICH RESULTED IN A STRAINED CHEST MUSCLE.

Ee was leaving restroom and slipped on the floor causing her to fall landing on her hip resulting in a bruised hip.

Removing a grease gun end from a fitting. The grease gun fell striking the victim on the mouth causing a chipped tooth.

VICTIM WAS HELPING SWITCH A BARGE USING A WIRE CABLE WRAPPED AROUND A CAVEL. THE WIRE CABLE POPPED OFF THE CAVEL AND STRUCK HIS RT. LEG ABOVE THE ANKLE CAUSING A BRUISED LEG.

LIFTING A 5 GALLON OF OIL UP ONTO A TRUCK BED & STRAINED HIS BACK.

EE WAS CARRYING A CONVEYOR ROLLER AND STRAINED HIS ABDOMEN.

VICTIM JUMPED FROM THE LAST STEP OF A CRANE TO THE DECK AND LANDED ON A HOOK AND CABLE CAUSING A SPRAIN TO HIS RIGHT ANKLE.

VICTIM WAS LIFTING SOME WOODEN 2" X 4" CONCRETE FORMS AND STRAINED HIS LOWER BACK.

VICTIM WAS TAKING APART 2-5 GALLON BUCKETS THAT WERE STUCK TOGETHER. THEY CAME APART UNEXPECTEDLY AND ONE HIT HIM IN THE FACE CASUING A CUT TO HIS CHIN AND A CHIPPED TOOTH.

STEPPED OFF A TRACTOR & SPRAINED HIS KNEE

WAS DESCENDING STEPS ON NOSE OF TRAILER WHEN HE SLIPPED AND FELL CAUSING A STRAINED BACK.

INSTALLING A UNIVERSAL JOINT ON HIGHWAY TRUCK. THE U JOINT SLIPPED CAUSING A CUT ON HIS LITTLE FINGER REQUIRING SUTURES.

STEPPED DOWN FROM FORKLIFT & SPRAINED ANKLE.

VICTIM WAS CLIMBING OUT OF GRAVEL WASHER AND PULLED A MUSCLES IN HIS RIGHT LEG RESULTING IN A STRAINED MUSCLE IN HIS LEG.

REMOVING A BEARING WITH A BEARING PULLER. THE PULLER SLIPPED OFF AND STRUCK VICTIM IN FACE CASUING A LACERATION TO HIS FACE.

REMOVING PINS FROM CRANE BOOM SECTION WHEN PENNANT LINE CAME LOOSE & STRUCK VICTIM ON HIS CHIN CAUSING A BRUISE

EMPLOYEE WAS USING A CHAIN SAW, CUTTING TREES ON THE RIVER BANK, WHEN HE SLIPPED ON A ROCK AND FELL ONTO THE CHAINSAW CAUSING A LACERATION TO THE BACK OF HIS NECK.

EMPL. WAS WORKING ON DREDGE TIED TO BARGE. HE JUMPED OVER WICKET HOLE ON BARGE & CUT HIS HAND ON BURR ON CARGO BOX.

EMPL. WAS TAKING A SWING SHAFT APART ON A CRANE WHEN TWO PARTS CAME TOGETHER & MASHING HIS HAND CAUSING CONTUSION.

EMPLOYEE WAS BE3DING TO TIE A STEEL CABLE TO A POST AND STRAINED HIS KNEE.

EMPLOYEE WAS LI0TING A WATER PUMP AND STRAINED HIS BACK

EMP WAS ENTERING TRUCK HE HAD GREASE ON HIS SHOES AND SLIPPED AND FELL HITTING THE RUNNING BOARD OF THE WATER TRUCK, RESULTING INPOSSIBLE FRACTURE OF COCCYX

A HYDRAULIC PUMP FELL OFF A TRUCK BED AND STRUCK THE EMPLOYEE ON THE LEG.

EMPLOYEE WAS HELPING SWITCH GARGES WHEN A 5/8" DSTEEL CABLE BROKE STRIKING THE EMPLOYEE ON THE LEG.

EMPLOYEE WAS PULLING ON A STEEL PLATE TO REALIGN A CHUTE. THE PLATE GOT STUCK AND HE PULLED ON IT AND IT CAME LOOSE AND MASHED HIS FINGER.
