FALLING SPRINGS QUARRY: 1 muerte registrada Metal/Non-Metal

EE is believed to have received insect bites on 10/11/25. On 10/14/25, the EE sought treatment due to swelling, redness and itchiness at the bit site. EE received a steroid injection, prescription antibiotics and was advised to go home and rest. EE went to urgent care for follow-up on 10/16/25. Operator notified on 11/17/25, when urgent care invoice was received.

Drive motor belts broke on horizontal impact crusher. They wrapped around the shive. The first few belts were tough to cut. The second to last belt cut very easy because the last belt broke while cutting the second to last belt. Cut the inside of left forearm with razor knife. 8 stitches.

Employee was climbing out from changing a screen in Tower 1. Employee was wearing gloves and PPE and caught a sharp piece of screen with their hand, cutting it through the glove.

Employee was in Tower 1 moving screen 1. Cut their hand while in the act of moving screens from one side of floor to the other. Small cut.

The employee was shoveling under a conveyor inside a tunnel. Lifting material onto belt with the same shove. On light duty. Cleared to drive EE's truck. Physical therapy. Employee hoped the injury would get better. Waited until 10/2/2020 to have it checked out even though it happened 9/28/2020.

No one on site witnessed the accident. It is presumed that EE fell off the ladder of the 777D EE was driving at the end of the shift. EE remembers nothing from the fall or the events that took place after. Said nothing about the fall when talking w/management, but complained of back pains and drove home. No one was aware of the fall until checking in with EE after hours.

Was walking on concrete and missed the step off and rolled right ankle. EE worked the rest of shift, no problems, was off 9/30/19 for personal day, came back 10/1/19. Sent to medical and put on light duty on 10/1/19. Came back to work and given return to full duty on 10/7/19. Will report on 4th Quarter injuries report. Did not miss work for ankle.

While EE was carrying a section of handrail with another ironworker EE stepped over the end of a cart and stepped down on a piece of metal, which rolled causing EE's ankle to roll.

Employee was changing wire screens. Lifting the screen into place strained arm and finger. The quarry management was not notified until 7/25/2019. The employee was hoping it would get better. Sent to doctor and was placed on light duty and physical therapy follow up appointment.

Two EEs were taking a fuel tank off a pavement roller. There was pressure in the pump that sprayed when the line was taken off. Both EEs were wearing safety glasses. The EE did not notice irritation until EE woke up the following day (Saturday). Sent EE to be checked Monday morning. Urgent care gave EE prescription eye drops. No lost work time.

The employee was filling a 777F with diesel fuel at the start of the shift. Strained right arm while doing so. The employee went back to full work hoping it would get better. We were informed on 10/5/2018 at the end of the shift. The employee wanted to see if it got better with rest. The employee asked to go to the doctor the following week. Physical therapy assigned 10/25/2018.

The EE was inside the man lift basket changing a grease line. The basket caught and slid off the steel frame causing a bouncing effect. Safety lanyard pulled EE back in basket. Bruised lip laceration, closed fracture of right side of maxilla and fracture of transverse process of spine w/o spinal cord lesion. ER said EE could return to work next day. See specialist. Sore body.

The employee bent over to pick up a metal chute door in tower 1 of the screening plant. When the employee came to me ee looked to be in pain and was having trouble moving. Took employee to an urgent care where the doctor prescribed a muscle relaxer at the employees request. Put on light duty and went back to work on the same day.

Mine EE annual hearing test came back over the acceptable level of 25db on 8/9/17. EE was re-tested on 8/25/17 per the regulations and again was over the 25db level. EE continues EE's enrollment in the Hearing Conservation Program (HCP) and all engineering & administrative controls are being reassessed.

Shoveling out the rock and grease accumulation around the bucket boom of a 992K and felt discomfort in forearm. Still had movement. Sent for examination. First doctor returned employee to work with no PT. At a check up a second doctor recommended PT on 4/20/2017 with sessions starting 4/21/2017. At work on light duty.

Employee (EE) was discarding a conveyor idler when finger got caught in idler, pulling right arm/shoulder. EE reported injury but did not seek treatment. EE sought treatment on 10/2/2015 as shoulder soreness did not improve. Diagnosed with unspecified shoulder strain, and ordered physical therapy. Returned to duty w/no restrictions on 10/2/2015.

Employee (EE) visited clinic on this 8/13 with complaint of stiff/sore neck resulting from work performed on 8/10. On 8/10 EE was working on Primary Crusher removing, cleaning, and replacing wear plates. EE was working in an awkward, stooped position. On 8/13 Dr. diagnosed neck sprain/back strain and ordered physical therapy. EE returned to work same day.

Employee (EE) was working inside primary crusher changing impact hammers. When witness struck the impact hammer with a sledge, a shard broke loose from the impact hammer and struck EE in the arm, lodging just below the skin. EE was transported by supervisor to local ER where Dr. cut out shard and applied 3 sutures. EE returned to work with no restrictions same day.

The truck landed on its' right rear corner in the dumped material the truck then fell forward causing the left front corner of the cab to striking the rock wall and sloughed or rolled back to the position. He seem lucid but bloody from cuts on his left arm and right hand and some cut on his head, was no visible bleeding at that time.

WHILE PUSHING AN ELECTRIC WELDER UP A SLIGHT INCLINE, EMPLOYEE FELT A "POP" IN HIS RIGHT KNEE. THE TERRAIN WAS NOT UNEVEN OR OF A DANGEROUS SURFACE. HE HAD ASSISTANCE IN PUSHING WELDER.

EMPLOYEE WAS REMOVING A PIN FROM THE TOOTH ADAPTOR BY DRIVING IT WITH A SLEDGE HAMMER. WHEN HE STRUCK THE PIN A SMALL SLIVER CAME OFF AND IMBEDDED IN HIS KNEE.

THE RIGHT FRONT OUTRIGGER ON CRANE WAS BEING RETRACTED. EE'S RIGHT HAND WAS CAUGHT BETWEEN OUTRIGGER PAD AND TOP OF THE OUTRIGGER. THE OUTRIGGER CRUSHED THE EE'S LITTLE FINGER AND DAMAGED HIS RING FINGER. EE'S LIITLE FINGER WAS AMPUTATED AND HIS RING FINGER WAS PINNED.

EE WAS LOADING CAPSTON AND STRIPPING INTO 100 TON TRUCKS WITH A CAT 992G WHEN HIS BUCKET TEETH HOOKED INTO A SEAM IN THE CAPSTONE. WHILE TRYING TO LIFT THE BUCKET, THE BACK OF THE MACHINE CAM E OFF THE GROUND. WHEN HE BACKED AWAY FROM THE WALL, THE BACK OF THE MACHINE CAME DOWN, JARRING HIM, CAUSING HIM TO BOUNCE IN THE SEAT.

EE WAS HELPING THE MECHANIC INSTALL A LIFT CYLINDER ON A 5O TON TRUCK. AS IT WAS BEING LIFTED INTO PLACE. THE EE GOT HIS HAND BETWEEN THE EYELET ON THE CYLINDER AND PIN BOSS ON THE BED. THE CY LINDER PINCHED HIS SMALL FINGER PUT ABOUT A 3/4"CUT IN HIS FINGER.

EE STEPPED ON PIPE WRENCH, WRENCH SKIDDED ON STEEL GRATING.

EMPLOYEE WAS CHECKING THE BUCKET LEVELER ON THE HIGHLIFT. AS HE WAS CLIMBING DOWN HE STRUCK HIS HEAD ON THE RIGHT REARVIEW MIRROR BRACKET.

USING PRY BAR IT SLIPED & HE FELL STRIKING HIS CHIN ON A STEEL OBJECT

EE HAD HAD A HISTORY OF BACK PROBLEMS. HE SAYS HIS PRESENT COMPLAINT IS NOT THE RESULT OF AN ACCIDENT BUT AS THE RESULT OF HIS OCCUPATION AS A TRUCK DRIVER. HE CANNOT POINT OUT ANY SPECIFIC RE ASON FOR HIS BACK PROBLEM.

REPAIR WORK BEING DONE TO HUD. JACKHAMMER AT PRIMARY CRUSHER. HAMMER HAD BEEN PUT ON A STEP OF A STAIRWAY WHILE BEING FIXED. IT SLIPPED OFF THE STEP & CAUGHT THE EDGE OF THE FOOT JUST BEHIND T HE STEEL TOE. PINCHED THE SKIN & CAUSED SLIGHT FRACTURE TO LITTLE TOE.

EE HAD SPENT THE PREVIOUS NIGHT MOVING HIS BELONGINGS FROM HOME THAT WAS EXPECTED TO BE COVERED BY THE FLOOD HE WAS OBSERVED BEFORE WORK WALKING WITH CONSIDERABLE DISCOMFORT SHORTLY AFTER STAR TING WORK LOADING SHOT ROCK HE CLAIMS TO HAVE RUN INTO A LEDGE IN FLOOR CAUSING BACK PAIN

STUCK HIS FINGER THRU GUARD OF RUNNING HEATER FAN IN TRUCK. ALL HE SAYS WHEN ASKED WHY IT HAPPENED IS THAT IT WAS A STUPID THING TO DO.

EMPLOYEE WAS CHANGING POSITIONS WITH A LIT TORCH IN HIS HAND AS HE TURNED HIS BODY HIS FOOT SLIPPED CAUSING HIM TO PASS THE LIT TORCH CROSS HIS LEFT CHEEK (FACE) AS A RESULT HE RECEIVED SMALL 1ST DEGREE BURN ON HIS LEFT CHEEK

HE WAS ON HIS BACK UNDER A TRUCK CUTTING THE DRIVESHAFT OFF WHEN A SMALL PIECE OF SLAG FELL INTO HIS EAR.

TRUCK WAS GOING DOWN HILL DRIVER MOVED TO EDGE OF ROAD TO AVOID A ROCK AND WHEEL DROPPED IN AN AREA THAT HAD WASHED OUT BECAUSE OF RAIN.HE LOST CONTROL AND DROVE UP ON A BANK AND TRUCK TURNED ON ITS SIDE.STRAINED HIS RIGHT WRIST.DRIVER WAS WEARING SEATBELT.

EMPLOYEE WAS USING A ROCK AS A STEP. ROCK WAS WET AND FOOT SLIPPED TWISTING ANKLE.

HYDROLIC HOSE ON A LIFT CYLINGER BROKE WITH BED OF TRUCK RAISED BED FELL & JARRED THE TRUCK

DRIVER WAS REPREMANDED FOR UNSAFE DRIVING ON 1-10-90ON 1-11-90 REPORTED HE HAD HURT HIS BACK WHILE DRIVING A TRUCK HE SAID HIS SEAT HAD BROKED DURING THE DAY & THIS HAD CAUSED INJURY INSURANCE CO IS INVESTIGATING

TIGHTENING BOLTS WITH WRENCH FELT MUSCLE IN NECK PULL.

EMPLYEE WAS WALKING & HIT HIS HAND ON A PIECE OF IRON CUT HIS HAND REFUSED MEDICAL CARE BUT WAS SENT TO HOSP.BY SUPERVISOR

OPERATOR CLAIMS MACHINE GIVES HIM WIPLASH NO PARTICULAR OCCURANCE OR ACCIDENT CAUSED HIS BACK CLAIM.
