EE was operating a front end loader in the clay shed. EE opened the door and thought it latched open. EE reached out to get some food and the wind blew the door shut striking EE's left hand between thumb and pointer figure. EE required 8 stitches.

The employee was removing excavator teeth from a metal bin. As EE was removing a tooth, EE's left hand struck a piece of sheet metal. The sheet metal cut EE's left index finger. As a result, stitches were required.
Contractor employee was climbing the ladder steps on a CAT 50 ton haul truck to check the oil. A foot slipped and they hit a knee. Employee informed a supervisor the next day and was taken to the Dr. The Dr. said the knee was bruised and employee was placed on light duty.

Employee was shoveling surface soil into drum when he felt a sharp pain in in shoulder. Employee was transported to local ER for treatment. ER physician determined employee had a muscle spasm. Employee claimed he was in severe pain. Employee received injections for pain control and muscle relaxer. Employee was released to return to work his next shift without restrictions.

Excavator was shutting down at end of shift. Excavator was top loading trucks, the operator cut a ramp to walk excavator off the level it had been operating from. While walking down ramp, excavator started bogging in dirt and suddendly stopped, caused cab to tilt over. EE released seat belt, hit forehead on windshield, deep cut. 2 witnesses called foreman(called 911).

Left the pit loaded, came to a point in the haul road where setting sun hits the mirror. Other truck passed on his left in opposite direction, sun hit mirror & blinded him. He was close to the edge of the road next to the berm & drifted even closer right tire caught the berm & almost came to a stop. Berm gave way/truck rolled down slope 60 ft. climbed out window and climb up hill.

Employee was leaving work area to go to break room. As he took 3 steps after leaving work platform, his feet slipped out from under him. This caused him to fall to floor & after landing on his right elbow, he injured right shoulder. Floor was somewhat slick due to rain & high humidity conditions. He suffered torn ligaments, tendons to right shoulder (torn rotator cuff).

EE WAS REMOVING SHOVEL FROM BACK OF SERVICE TRUCK. HE SLID HIS HAND UP THE HANDLE & GOT FIBERGLASS SPLINTER IN HIS RIGHT HAND BETWEEN THE LITTLE FINGER & RING FINGER. HE WAS WEARING GLOVES AT THE TIME.
EE WAS WASHING THE OFF-ROAD TRUCK WITH 1 1/2" FIRE HOSE. WHEN PULLING SLACK OUT OF HOSE HE STRUCK THE METAL PIPE RACK ON THE TRUCK WITH HIS LEFT THIGH. EE RETURNED TO WORK FULL DUTY ON 2/2/04 BUT STARTED HAVING PROBLEMS WITH THIGH & RETURNED TO THE DR. ON 2/9/04. AT THAT TIME HE WAS GIVEN PERSCRIPTIONS & PUT ON RESTRICTED DUTY UNTIL 2/23/04.

SUBJECT THOUGHT HIS TRUCK WAS FINISHED LOADING. HE WHEN AROUND TRUCK TO CHECK LUG BOLT THAT WORKED LOOSE. WHILE CHECKING LUG BOLT. A PIECE OF CLAY FELL FROM TRUCK, STRICKING HIM IN TOP OF H EAD. THIS REQUIRED 7 STICHES. BACKHOE OPERATOR DID NOT KNOW SUBJECT WAS ON THAT SIDE OF TRUCK.
