EE operating dozer on base stone pile, complained of soreness in his lower back area. He was given X-rays and MRI which showed new injury to old condition on 11/9/07. He was medically treated on 11/29/07 when doctor administered cortisone shots. The area that the EE had surgery on years ago was reaggravated by operating dozer.

Electrician was working on deteriorated overhead electrical line from manlift. A section of line ran over the shop. The electrician exited the manlift onto the roof. While walking on roof he stepped on sky light & broke through the ceiling. He fell to the shop floor below. Safety harness was worn but there was nowhere to tie off.

Employee was driving in a beam with a sledge hammer. The handle hit against an obstruction causing the strike to be redirected. The result was a smashed right thumb. Laceration to right thumb which needed stitches.
Employee was cutting a belt to splice another section of belt in. Employees hand slipped downward on the utility knife that he was using to cut belt. His pinky finger came in contact with the blade causing a laceration to the finger.

Employee was changing screen cloth. Screen cloth was being tightened down with tension pin and wedge when the wedge shot upward and hit employee on the lip cutting the upper lip and loosening three teeth.
