Glens Falls Quarry Metal/Non-Metal

Tingling and numbness in both hands and fingers, shooting pain in fingers and hands at times. NOTE: First date employer was made aware of this issue was 8/5/13. First time employee sought medical attention was 8/8/13. This report is being filed due to employee having surgery on 12/20/13 with no loss of time and no restricted duty.

The employee slipped on ice while dismounting from the track hoe, twisting his right knee.

Employee slipped on snow covered ice. Severe bruise to hip trunk area.

The employee was performing his regular duties when he felt a twinge in his left shoulder.

The employee got out of his truck to walk down the road to the Quarry. The road was icy, and the employee fell; resulting in pain to neck, lower back right shoulder and elbow.

The employee was cutting tubing for air conditioner at the quarry crusher, when a foreign object went into employee's right eye.

This is not an Injury Report. The employee has a 10 db shift in his hearing in comparison with last year's testing. This loss is not related to any specific injury date or current job exposure.

GROSS ELECTRIC WAS PULLING OUT A DAMAGED ELECTRICAL CABLE FOR THE 904 CONVEYOR BELT. DURING THE PULL, THE DAMAGED CABLE BROKE & SNAPPED BACK OUT OF THE CONDUIT AT THE TAIL END OF THE 904 BELT, HITTING GROSS ELECTRIC EE THAT WAS MONITORING THE CABLE PULL.

EE was checking the 903 belt as it was off track & having problems with the north tail pulley bearing. When he put his hand to the bottom of the guard to feel for heat, the next thing he knew something grabbed his finger. The EE thinks the zero speed paddle caught his fingerr & pinched it against the guard, causing a laceration to the middle finger of his right hand.

The employee was walking to his car at the end of his shift. The employee slipped on ice that was under water, fell and twisted his right knee.

THE EMPLOYEE WAS CLIMBING UP THE LADDER ON THE LOADER WHEN HE ALLEGEDLY FELT PAIN IN HIS RIGHT ARM.

THE EE WAS CLOSING THE QUARRY GATE UPON LEAVING FOR THE DAY. WHEN CLOSING THE GATE, THE TOP HINGE CAME APART ALLOWING THE GATE TO FALL. THE GATE STRUCK THE EE'S LEFT HAND, AND BARB WIRE ON THE GATE CUT HIS LEFT FINGER. THE GATE ALSO STRUCK THE EE'S LEFT ARM AND LEFT SIDE OF BACK, CAUSING SOME SCRAPES.

EE WAS CLEANING BLOCKS OUT OF THE CRUSHER. A BLOCK FELL ON HIS FINGER, CAUSING A LACERATION & FRACTURE TO HIS LEFT INDEX FINGER

THE EE ALLEGES CARPAL TUNNEL SYNDROME IN THE RIGHT WRIST, ALLEGEDLY DUE TO DRIVING A LOADER FOR 10-15 YEARS IN HIS EE. SURGERY ON 3-20-02 PUT EE OUT OF WORK FOR INCIDENT DATE OF 10-31-02.

THE EE WAS BLDG A WALL IN THE QUARRY SUPERVISORSOFFICE. WHEN THE EE SWUNG THE HAMMER TO HIT A NAIL INTO THE WALL HE HIT HIS MIDDLE FINGER ON HISLEFT HAND INSTEAD

EMPLOYEE WAS ALLEGEDLY IN THE PROCESS OF REMOVING A STONE THAT WAS WEDGED BETWEEN THE FEEDER ANDTHE SIDE OF THE HOPPER. WHILE HE WAS POKING AT THE STONE WITH A BAR, THE BAR GLANCED OFF THE SID E OF THE STONE, AND PINCHED HIS SMALL FINGER ON HIS LEFT HAND BETWEEN THE BAR AND THE SIDE OF THE HOPPEER.

THE EE WAS REMOVING THE COVER TO THE SANDER. HE LOST CONTROL OF THE COVER AND IT FELL ON THE RING FINGER OF HIS LEFT HAND.

THE EE HAD PLACED A HEATER [HOG] INSIDE A FROZENAIR COMPRESSOR TO UNTHAW IT. WHEN THE EE WENT TOREMOVE THE HEATER HE FELT A PULL IN HIS LOWER BACK.

WHILE OPERATING FRONT END LOADER EMPLOYEE HAD TO TURN HIS HEAD AND NECK CONSTANTLY TO VIEW BACKING DOWN RAMP WITH EA BUCKET OF STONE HE WAS REMOVING FROM QUARRY.

EMPLOYEE WAS LIFTING ONE END OF LIGHT PLANT TO HOOK UP TO BALL ON PICK-UP TRUCK AND FELT PAIN IN LOWER BACK TWO HOURS LATER.

WHILE GETTING O0F OF GRADER SLIPPED ON GROUND AND TWISTED RIGHT KNEE.

DRIVER WAS BACKING TO HOPPER TO UNLOAD. BACKED ON TOP OF BERM STOPPED. SHIFTED INTO FORWARD GEAR & RELEASED BRAKE-HIND WHEELS WENT INTO HOPPER. CAUSING DRIVER TO BUMP HIS HEAD & BRUISE IT.

MOVING A 55 GAL OIL BARREL INTO PLACE FELT PAIN IN BACK

TRUCK DRIVER PULLED OVER TO LET OTHER TRUCK PASS & HIT HOLE AN EDGE OF RAODWAY.

EMP WAS CHANGING OIL FILTER SHARP EDGE CUT FINGER ON LEFT HAND HE WAS NOT WEARING GLOVES AT THIS TIME
