Tingling and numbness in both hands and fingers, shooting pain in fingers and hands at times. NOTE: First date employer was made aware of this issue was 8/5/13. First time employee sought medical attention was 8/8/13. This report is being filed due to employee having surgery on 12/20/13 with no loss of time and no restricted duty.

GROSS ELECTRIC WAS PULLING OUT A DAMAGED ELECTRICAL CABLE FOR THE 904 CONVEYOR BELT. DURING THE PULL, THE DAMAGED CABLE BROKE & SNAPPED BACK OUT OF THE CONDUIT AT THE TAIL END OF THE 904 BELT, HITTING GROSS ELECTRIC EE THAT WAS MONITORING THE CABLE PULL.
EE was checking the 903 belt as it was off track & having problems with the north tail pulley bearing. When he put his hand to the bottom of the guard to feel for heat, the next thing he knew something grabbed his finger. The EE thinks the zero speed paddle caught his fingerr & pinched it against the guard, causing a laceration to the middle finger of his right hand.
The employee was walking to his car at the end of his shift. The employee slipped on ice that was under water, fell and twisted his right knee.

THE EMPLOYEE WAS CLIMBING UP THE LADDER ON THE LOADER WHEN HE ALLEGEDLY FELT PAIN IN HIS RIGHT ARM.
THE EE WAS CLOSING THE QUARRY GATE UPON LEAVING FOR THE DAY. WHEN CLOSING THE GATE, THE TOP HINGE CAME APART ALLOWING THE GATE TO FALL. THE GATE STRUCK THE EE'S LEFT HAND, AND BARB WIRE ON THE GATE CUT HIS LEFT FINGER. THE GATE ALSO STRUCK THE EE'S LEFT ARM AND LEFT SIDE OF BACK, CAUSING SOME SCRAPES.
EE WAS CLEANING BLOCKS OUT OF THE CRUSHER. A BLOCK FELL ON HIS FINGER, CAUSING A LACERATION & FRACTURE TO HIS LEFT INDEX FINGER
