The employee was setting out caps and boosters to load the shot. EE stopped at the bulk truck and told EE's co-worker that EE felt funny. EE continued to layout caps and boosters but was unable to walk correctly and EE's right side was numb. EE finally sat down and tried to call EE's spouse but was unable to due to a lack of motor skills on one side.

Employee has operated a front end loader over 7 years. The bouncing from the loader over many years has possibly caused compression of the vertebrae of the neck and bone spurs causing a pinched nerve. Employee experiences pain in the neck radiating to the left arm.

Employee was pulling tools to the top of silos using a bucket and rope. Employee felt a pull in the lower back and could no longer pull.

Employee was holding angle for welder to tack up in hopper. From 11:30PM to 6AM. Employee was closing eyes as welder was welding. Employee went home and woke up with burning eyes.

Employee was climbing a ladder out of a density separator when a metal flake fell into left eye.

Employee was using a 3 LB. hammer to drive a pin into the wear plate on the bucket of the 988G. The hammer hit the side of the top of the pin and broke a metal piece free. The metal chip flew into the left forearm of the employee. 2 stitches were required for the cut.

Employee was lifting a belt hanger onto the backhoe bucket. While holding the hanger close to employee's body, the end nut snagged on the pocket of the vest causing it to drop onto the employee's right pinkie finger, lacerating employee's finger.

Employee was troubleshooting pneumatic scrape. EE was turning the position selector & quickly turned to look at the scrape. EE caught EE's foot on the cleat on the inclined walkway and fell, hitting the left side of EE's head on the lower slide plate along the walkway. Employee became unconscious. EE awoke dazed and walked to the office.

Employee was standing on the back of a truck bed hooking gauges to bottles, when he reached for a wrench to tighten lines, he slipped off the tailgate. It was determined that rain and sand stuck to his boots and made a slip hazard.

Employee was holding cable steady so pin could be driven into Lok-A-Loy. Pin in Klein cable grip broke releasing tension in cable. Klein cable grip struck employee in the left middle finger.

Employee was prying on a rod mill liner with a pry bar and holding it up. The liner shifted, releasing the pressure on the bar. The liner shifted, fell and caught the bar. The bar flew up and hit employee in left cheek.

An X-ray taken during the annual medical surveillance program was read by Dr.as positive for indications of pneumoconiosis. A second reading by different Dr.confirmed the image as positive for indications of pneumoconiosis. A confirmed case of pneumoconiosis with a median ILO profusion category of 1/2.

Injured employee was pushing bottom of a bolt up with hand during removal of a loader bucket cutting edge. Bolt was not fully inserted into the square catch of the cutting edge. A second employee using an impact wrench on the nut started and the bolt turned, cutting the right hand and finger of injured employee. Employee required 15 stitches to close the wound.

Employee was turning to climb stairs on CAT 988G Loader and got "pinkie" finger on right hand caught in engine guard causing a fracture of the bone int the right hand.

Apparent silicosis, but diagnosis is being further evaluated and is expected to be retracted.

Employee was stepping from a stationary ladder onto a grating at the rodmill. When he placed his left foot onto the grating his foot slipped off the grating. His right hand was on the railing. All of his weight suddenly shifted to his right arm and shoulder area. He had gotten PB Blaster fluid on the bottom of his boot, which caused the slip. He has a right shoulder sprain/strain.

Employee was chipping slag at an awkward angle when a small chip went around his safety glasses into his right eye. Employee went for medical treatment on 3/12/08. A metallic chip was removed from his right eye. The eye will heal.

Employee was disloging a rock under the Jaw Crusher. The rock swung loose and hit him in the mouth.-

Employee was lifting a roller track. When he threw it to the side, he hurt his back.

While trying to repair dozer, the blade arm slipped against the dozer tracks and pinned the mechanic's leg between the blade arm and the dozer tracks.

EE WAS WALKING TO RAILSIDE LOADOUT CONTROL ROOM. HE PUT HIS HANDS ON THE RAILS AND WAS STUNG BY A WASP BETWEEN HIS MIDDLE AND INDEX FINGER ON HIS RIGHT HAND. EE IS ALLERGIC TO BEE STINGS.

TRK DRVR WAS CRANKING TARP OFF OF TRAILER & THE CRANK FLEW OUT OF HIS HAND THE CRANK SPUN BACK &HIT HIM ON THE FOREHEAD. HE REC'D A 2" CUT TO THE FOREHEAD & WAS TAKENT O WINCHESTER MED CTR HE RETD IN 2 HRS

AS EE PREPARED TO LOAD THE 4TH HOLE IN FRT ROW WITH AN/FO HE TOSSED HIS TAPE OVER THE CREST TO CHECK THE BURDEN ON THIS HOLE. EE SAID HE REMEMVERED READING 12 FT ON THE TAPE THEN WAS SLIDING DOWN THE FACE. AT ABOUT 8FT DOWN THE FACE HE GRABBED ONTO A ROCK & TRIED TO HOLE ON. NOT BEING ABLE TO HOLD ON HE SLID ANOTHER 30FT TO THE TOE.DURING HIS SLIDE HIS HARD HAT MANAGED TO STAY ON

DRILL STEEL STUCK AND OPERATOR AND HELPER WERE WORKING TO FREE DRILL STEEL.

PILOT LIGHTS ONT HE 3 VAPORIZERS HAD GONE OUT DURING THE NIGHT. EE HAD ALREADY RELIT ONE OF THEM & WAS ATTEMPTING TO LIGHT ANOTHER WHEN IT FLASHED BACK AT HIM AND BURNED HIM

EE WAS HELPING TO CLEAN UP FLOOR AREA AND WAS ASKED TO ASSIST IN REINSTALLING A SUMP PUMP THAT IS USED TO PUMP WATER/SLURRY. MECHANICS HAD FINISHED OVERHAULING THE PUMP AND ASKED FOR ASSISTANC E IN PLACING IT BACK INTO OPERATION. EE WAS WORKING A RACHET TYPE COME-A-ALONG THAT WAS ATTACHED TO AN EYE WELD. THE WELD BROKE CAUSING THE PUMP TO FALL ONTO EE'S RT FOOT.

EMPLOYEE WAS REMOVING SIDEPLATE FROM SHELF WHEN EMPLOYEE SLIPPED & FELL FROM SHELF & BRUISED LEFT RIBS

DRIVING COMPANY PICK-UP FROM MAINTENANCE SHOP TO LOADOUT AREA WHEN STEERING WHEEL WAS JERKED OUT OF HAND CAUSING INJURY TO RT THUMB ROAD CONDITIONS WHERE DRY & CLEAR

BRACKET ON SCREEN MACHINE WAS BEING REPLACED. EMPLOYEE WAS PUTTING IN BOLTS AND HAD HAD TIGHTENED ALL BOLTS. A SOCKET RACHET WAS APPLIED TO NUTS OF BOLTS WHEN SOCKET RACHET SLIPPED OFF CAUSING HAND TO TWIST AND JAMB AGAINST SIDE OF MACHINE.

HIGH WINDS PICKED UP FABRICATED LID (150 LBS) AND BLEW IT AGAINST HIM, HITTING HIM ON THE LEFT SHOULDER AND KNOCKING HIM TO THE GROUND. LOCAL RADIO BROADCAST LATER CONFIRMED FWINDS AT 77 MPH.

LOADING SAND INTO RAILCAR UNTIL CAR WAS FULL.IT MOVED FORWARD DOWN TRACK SLIGHT GRADE ON TRACK WHEN BRAKES FAILED.THE IMPACT OF MOVING CAR AGAINST STATIONARY CARS CAUSED SLAMMING EFFECT WITH E

EMPL UNLOADING 9R TIES FROM PICKUP TRUCK LIFTED END ON TIE STRAINING LOWER BACK.

THE ROD MILL EN8 LINERS WERE REPLACED 02/09/87. THE SPENT LINERS WERE CONVEYED ONTO THE CLEAN,DRY CONCRETE FLOOR. THE INJURED WAS PUTTING THE LINERS ON END AND FLIPPING THEM OVER INTO A WALDON LOADER BUCKET M5NUALLY. THE NEXT DAY 02/10/87, HE COMPLAINED OF A STIFF LOWER BACK AREA.

THE EMPLOYEE WA8 WORKING ON THE REMOVAL OF A PUMP IMPELLER. A PIPE WAS PLACED IN ONE OF THE VANES OF THE IMPELLER TO AID IN THE UNSCREWING OF THE IMPELLER. THE EMPLOYEE WAS STANDING ON THE PIP E AS LEVERAGE W2ILE ANOTHER EMPLOYEE HAMMERED ON THE PIPE. THE PIPE SLIPPED AND THE EMPLOYEE WHO HAD BEEN ON THE PIPE FELL AND SPRAINED HIS ANKLE.

EMPLOYEE WAS IN SCREEN TOWER SPLICING A CONVEYOR BELT WHEN THE UTILITY KNIFE HE WAS USING SLIPPED AND LACERATED HIS RT INDEX FINGER AND THUBMLIGHTING WAS FAIR AND CONDITIONS WERE DRY

EMP HAD POSITIONED POP OUT STAIRWAY DOWN TO RR CARS EMP CLIMBED DOWN AND STAIRWAY FOLDED BACK UP EMP FELL APPROX 15 FT TO THE GROUND THE RELEASE CABLE HAD GOTTEN CAUGHT WHICH PREVENTED THE ST STAIRWAY FROM LOCKING INTO POSITION EMP BROKE HIS RT LEG AND LEFT THUMB
