Operating air lance/hose; Chicago fitting came apart and fitting on hose struck employee on the face. Cut required 4 stitches to close. It appears employee was not wearing required face shield.

Employee was moving grease drum after greasing machine.
Employee was grinding a weld on a stainless steel liner when an object struck the employee in the right eye. Employee was wearing long sleeved (FR) shirt; gloves; safety glasses; grinding shield; (FR) long pants; steel toed leather boots.
Employee was removing fuel injector from D6R Cat Dozer. Injector was removed and employee had his hand over injector hole. Helper was instructed to turn engine over and air from injector part blew air between his skin on the hand and was filled with air.
EE was attaching 6" lay flat suction/discharge line to fill water truck approx. 6' in length with cam lock fitting.
Employee slipped on ice and fell forward.
On 2/5/12 EE was climbing onto D6r-lgp Cat dozer when he slipped on the track and fell with his right shoulder hitting the track. He reported it and said he was ok, X-ray taken next day, was ok. On 3/12/12 was still in pain, doctor said get MRI, torn subscapllaris, did not want anything done. On 5/7/12 he decided to have operation at this time on his shoulder, reportable accident.

Driver had loaded with ash and was slightly overweight so he had opened the tailgate to lighten the load. A chunk of ash impeded the gate closing fully so he tried to pull the lump out of the way. As the lump dislodged the gate slammed shut and got the 5th finger (pinkie) of his right hand caught between the gate and the bed.

After placing the portable heater filling hose in the bed of the pickup truck, employee stepped off the rear bumper and twisted his ankle.
The EE was tightening a fitting on an air lance. After the fitting was tightened the hose attached to the chicago fitting on the pressurized side of the valve came unattached to the chicago fitting. The hose violently whipped out of control & struck the EE in the leg & face. The pipe clamp (worm gear clamp)still attached to the hose, cut his lip and chipped a tooth.
The employee strained his lower back while clearing material using a pneumatic power tool from underneath BC-10 belt rollers.

Employee was using an air lance to unplug the fuel chute above the "b" hammermill located within the ro-pro fuel handling system. He strained his right hand, between the thumb and the wrist area. He was transported to local hospital, x-ray was negative.

EE was cleaning the Ro-Pro area. The wind was gusting very hard. In the process of coming back into the building-the door was coming closed, he took a quick step to avoid getting hit by full force of door. In the quick step he unintentionally stepped on snow shovel that the wind had knocked down & slipped & fell. In the fall he bruised his elbow & strained his back.

DISLODGE CHUTE BUILD UP OF COAL BY STRIKING EXTERIOR OF DUCT WORK WITH HAMMER. HAMMER SLIPPED FROM HAND, EE TRIED TO CATCH IT AND THUS PINCHED HIS FINGER. THE LEFT HAND RING FINGER TIP WAS BROKEN AND REQUIRED STITCHES TO CLOSE THE WOUND.

A METAL SHARD CAME OFF A DRAG CHAIN LINK AS THE CONNECTING PIN WAS BEING REMOVED & STRUCK THE EE IN THE UPPER LEFT LEG. THE MECHANIC WAS ASSISTING ANOTHER MECHANIC WHO WAS USING A SLEDGE HAMMER, DRIFT PINS & HOLDERS. AS THE PIN WAS BEING STRUCK WITH THE HAMMER, THE HAMMER STRUCK THE CONNECTING LINK CAUSING THE FRAGMENT TO STRIKE EE.
WHILE TRYING TO REMOVE A ROCK 2' BY 2' FROM THE MOVING CONVEYOR BELT, HIS HAND BECAME PINNED BETWEEN THE ROCK AND THE CONVEYOR BELT HOOD COVER BRACKET.
