An employee operating a Caterpillar 770 haul truck hauling material to repair the road to the stripping area. As the employee dumped the load the bed contacted overhead powerlines. The employee exited the truck to extinguish a fire. Upon returning to the truck EE reached for the handrail. When EE touched the truck, EE was electrocuted.

While an employee was splicing a 42" conveyor belt using a belt cutter, the cutter became unbound and slipped due to the excessive force. The employee's forearm contacted a double headed nail that caused a laceration.

Employee was dismounting a volvo track hoe. Employee slipped and struck their face on the track and the back of their head on concrete floor. Resulted in a laceration to the nose requiring 5 stitches, a broken nose, and a contusion to the back of the head.

The rock breaker was breaking oversize down small enough to run to primary crusher. A rock came back and hit the cylinder and then ricocheted back toward the cab entering the cab hitting employee in the right side of the head.

Changing a return roller. The employees thought that the brackets that hold the roller in place were on top of the frame of the convoyer but were not, they were on the bottom. One employee let his side loose first, letting the roller free to swing into the other employees finger, smashing it into the frame.

EE was attempting to remove the race from the shaft of a bearing when a piece of metal broke off and penetrated into his left forearm. Two stitches was the result of having to extract the piece of metal.

EE was punching holes in pieces of belt with hammer and hole punch and had already punched out 6 holes. On the 7th hole he missed the hole punch with the 2 lb. hammer and struck his thumb on his left hand. X ray showed a hairline fracture on the knuckle. He was given a splint to wear. He was wearing gloves. Area was muddy and punch was too small to hold.
