The miner was feeding a new belt onto a belt conveyor when the spreader board on a chain hoist skipped free. The board flew out and struck the miner in the chest.

The miner slipped and fell on EE's back and rolled 3 to 4 feet. The miner had right shoulder pain. Original diagnosis of right shoulder sprain on 4/24/17. The miner had no restrictions/full duty and has had no lost time from the accident. On 7/7/17, diagnosis changed to a torn rotator cuff and is scheduled for Right Shoulder Surgery on 7/12/17.

When changing screens inside of screen deck pulled or strained neck muscle

When swinging a sledgehammer at a rock, missed the rock causing hand to contact the discharge chute that the rock was lodged in, causing a laceration to the finger and cutting the tip of the finger off.

TRUCK HAD MECHANICAL FAILURE, MINER WAS STRUCK BY THE OPEN DOOR KNOCKING HIM TO THE GROUND CAUSING LEFT KNEE, RIBS AND ANKLE PAIN. MINER WAS GIVEN Rx FOR PAIN AND SWELLING, ICE PACK AND KNEE IMMOBILIZER.

WHEN SHOVELING MATERIAL FROM UNDER A CONVEYOR, FELT PAIN IN LEFT LEG AND LOWER LEFT BACK. UPON EXAM AND X-RAYS FOUND TO HAVE HERNIATED DISC, LUMBAR SPINE.

WHILE CLEANING UP MATERIAL AROUND CRUSHING AND SCREENING PLANT WITH A BOBCAT, A ROCK BOUNCED OFF OF THE SCREEN AND ENTERED THE BOBCAT, HITTING THE OPERATOR ON HIS LEFT KNEE

WAS DRILLING A BROKEN BOLT FOR REMOVAL WHEN DRILL BIT BROKE A PIECE OF THE DRILL BIT GOT INTO THE EYE. WENT TO THE DOCTOR AND HAD THE FOREIGN BODY REMOVED

While standing on the bank of fresh water pond, employee lost balance and slid down bank, twisting ankle.

Heavy rains caused flooding of Brush Creek causing it to overflow its banks and our berms and filling the pit where we are currently mining with water.

The employee was tightening a tail pulley when he felt a pop in his shoulder/neck

The employee placed floor grating but did not secure. Came back to work Monday am and forgot the flooring was not secured climbed on it causing flooring to give way. Employee cut lip and broke tooth.

Employee was in Loader and Loader bottomed out causing employee to jar his back

EMPLOYEE WAS WORKING AT HOLNAM CEMENT PLANT AND WHILE WORKING ON RAW MILL LINERS WAS STRUCK BY AMILL LINER WHICH FELL FROM UPPER SIDE OF RAW MILL, GLANCING HIM ON THE RIGHT SIDE OF HIS BODY

THREE PEOPLE INCLUDING INJURED WERE MOVING A BOX OF PARTS OFF OF THE NEW SCREEN THAT HAD JUST ARRAIVED WHILE MOVING THE BOX NEAR EDGE WE REALIZED IT WAS TOO HEAVY TO MOVE BY HAND & PUSHED IT B ACK UP ON TO THE SCREEN CAUSING IT TO LAND ON EEHAND

EMPLOYEE WAS ATTEMPTING TO REPAIR THE CONE CRUSHER THEY WERE TRYING TO PULL OUT THE OLD PART & IT BECAME JAMMED HE STRUCK THE OLD PART WITH A HAMMER A PIECE OF STEEL FRACTURED OFF & PUNCTUR ED HIS ABDOMEN

EMPLOYEE WAS WORKING ON THE CONE CRUSHER AND HIS WRENCH SLIPPED CAUSING HIM TO LOSE HIS BALANCE. HE FELL OF OF THE CONE BACKWARDS, HE WAS ABLE TO TURN AROUND AND TRY TO CATCH HIMSELF. HE FELL 10 FEET, LANDING ON HIS FACE AND UPPER BODY.
