While removing a machine guard on the overland belt tail pulley, the miners left hand was pinched between the machine guard and frame. The tail pulley was filled with rock. A rock moved while removing the guard causing the guard to shift and pinch miners hand. The miner suffered a crushing injury/contusion to left hand. No open wound was caused and an x-ray revealed no fracture.

The miner was feeding a new belt onto a belt conveyor when the spreader board on a chain hoist skipped free. The board flew out and struck the miner in the chest.
The miner slipped and fell on EE's back and rolled 3 to 4 feet. The miner had right shoulder pain. Original diagnosis of right shoulder sprain on 4/24/17. The miner had no restrictions/full duty and has had no lost time from the accident. On 7/7/17, diagnosis changed to a torn rotator cuff and is scheduled for Right Shoulder Surgery on 7/12/17.

When swinging a sledgehammer at a rock, missed the rock causing hand to contact the discharge chute that the rock was lodged in, causing a laceration to the finger and cutting the tip of the finger off.
TRUCK HAD MECHANICAL FAILURE, MINER WAS STRUCK BY THE OPEN DOOR KNOCKING HIM TO THE GROUND CAUSING LEFT KNEE, RIBS AND ANKLE PAIN. MINER WAS GIVEN Rx FOR PAIN AND SWELLING, ICE PACK AND KNEE IMMOBILIZER.
WHEN SHOVELING MATERIAL FROM UNDER A CONVEYOR, FELT PAIN IN LEFT LEG AND LOWER LEFT BACK. UPON EXAM AND X-RAYS FOUND TO HAVE HERNIATED DISC, LUMBAR SPINE.
WHILE CLEANING UP MATERIAL AROUND CRUSHING AND SCREENING PLANT WITH A BOBCAT, A ROCK BOUNCED OFF OF THE SCREEN AND ENTERED THE BOBCAT, HITTING THE OPERATOR ON HIS LEFT KNEE
