Employee pinched finger between stone causing a laceration.

EE team lifted a stone, lost footing while carrying and fell hurting ankle.

EE moving stone on splitter, stone slipped, laceration to finger.

EE pulled a stone towards EE and felt abdomen pain.

EE was breaking stone in the Chilton shop. While EE was breaking stone, a chip of the stone went up under EE's safety glasses into EE's left eye. EE was given first aid to flush EE's left eye. Afterward, EE still felt discomfort in EE's eye. EE was taken to the clinic and EE's left eye was found to have a scratch. EE was given a prescription for eye drops.

EE was opening a stone with a hammer and chisel when a small piece of stone flew and contacted EE's right 5th digit, resulting in a laceration.

Employee was operating a stone splitter. EE was using EE's right hand to push a piece of stone out of the splitter. At this time, EE's left hand on the controls accidentally dropped the splitter head onto EE's right middle finger resulting in a laceration and two fractures.

Employee was chiseling the top off a piece of stone when it tipped over and fell on EE's right foot.

Employee was picking up a stone next to a pallet when another stone fell off the pallet and struck employee's hand resulting in a laceration to employee's left pinky finger.

Employee was pushing stone into a breaker when the stone stopped and right hand twisted around the stone resulting in a sprain to right wrist.

Employee was flipping a stone and the stone pinched his hand under the fifth finger. Avulsion/laceration to left hand. Restricted left hand work until end of healing.

Employee was striking a chisel with a hammer on a piece of stone when the stone opened and the hammer hit his left index finger, fracturing the metacarpal bone.

Employee was stacking pieces of stone on a wooden pallet. When he turned around the last stone he stacked rolled off the pallet and landed on his foot.

Employee was using hand pitch and hammer to split stone open. When he struck the pitch, it went through the opening and the hammer struck his thumb and injured the thumb.

Employee was lifting and stacking stone from the end of stone splitter when he felt a pain in his lower back.

Employee was using end splitter machine to split stone during his shift, when Supervisor provided instruction on how to safely use machine. 15 minutes after instructions were given, employee struck his thumb with the cutting tooth of end splitter.
