While lifting crusher liner, the hooked chain slipped off of the fork. This caused the liner to roll, resulting in the injury (bruising) of the employee's right leg.

While EE was pulling water hose, EE didn't realize there was a kink in it. The hose came out of EE's hand and the nozzle made contact with the EE's tooth and put a small chip in it. Was not repaired by dentist until August 2nd.
Employee was exiting the service truck and a foot got hung on the step causing them to fall on the right leg and elbow. Employee waited several days before seeking medical attention.

THE PIECE OF STEEL SMASHE D HIS HAND(LEFT) BETWEEN IT AND THE GROUND. HIS THUMB WAS SPLIT AND REQUIRED THREE STITCHES, THERE WERE NO BROKEN BONES.

EMPLOYEE WAS DRIVING THE WATER TRUCK THROUGHOUT THE QUARRY. ON THE THIRD LOAD, HE WAS GOING BACK DOWN INTO THE PIT TO WATER DOWN THE MUCK PILE WHEN STATED THE BRAKES FAILED. HE TRIED TO APPLY THE BRAKES SEVERAL TIMES, EACH TIME THE TRUCK CONTINUED TO GAIN SPEED. IT GAINED SPEED AT A FATER RATE AND HE DECIDED TO JUMP OUT OF THE TRUCK.
CAUGHT R. M. FINGER BETWEEN BOLT AND STEEL BODY PINCHING IT.
COMPLAINING OF SHOULDER, CHEST PAINS WHILE ON PREMISES, POLICY REQUIRED AN AMBULANCE BE CALLED, DID NOT TRANSPORT.
