An non-injury occurred between Brks. 7+8 and extending to Brk 11 on Panel 1 off 1 South Submains. The fall measured approx 279' long, 20' wide and 8' high (Roof Fall).

An unintentional roof fall was discovered on the 004 section in the #4 entry between #3 and #5 break. Size of fall was estimated to be 120' L X 7-8' H X 18-20' W. The area had been supported with 6' fully grouted resin bolts. No injuries, no equipment caught or ventilation was disrupted. Fall will not be cleaned up.

An unintentional roof fall was discovered in the #2 return 8 breaks outby the #3 set of seals. Size estimated to be 20'W x 6'H x 20'L. Area had been supported with 6' fully grouted resin bolts. No injuries, ventilation not interupted, no equipment caught. Fall will not be cleaned up. MINE has been in non-producing status since Aug 1, 2009.

An unintentional roof fall was discovered on the #2 West mains in the #5 entry at #23 brk at spad #9780. Size estimated to be (5'H x 18-20'W x 80'L). Area had been supported with 4' fully grouted resin bolts with supplemental support consisting of 10' cable bolts. Fall will not be cleaned up. No injuries, no equipment caught, ventilation was not interrupted.

An unintentional roof fall occurred in the #4 entry at spad #9491. The fall was estimated to be 4 ft high by 18 ft wide by 60 ft long. This area was bolted with 4 ft resin bolts with 10 ft cable bolts in the intersection. No injuries, track & belt was caught, ventilation was not interupted. The fall will be cleaned up.

An unintentional roof fall was discovered at old #15 belt in the #5 entry @ 33 brk, one brk inby spad 9079. Size: 18'W x 20'L x 4-5'H. The area had been bolted with 4 foot fully grouted resin bolts. No injuries, no equipment caught, ventilation was not interrupted. Fall will not be cleaned up.

An unplanned roof fall was discovered at old #15 belt in the track entry, size was est. to be 30'L x 18-20'W x 4-5'H. The area had been bolted with 5' fully grouted resin bolts with supplemental support consisting of 9' cables bolts, no injuries, no equipment caught or ventilation was not altered. fall will not be cleaned up. location of fall was #33 break track entry spad #9075.

Struck head against top of inby air lock door on 1 North belt, while riding in mantrip.

A pillar fall occurred on the MMU 004-0 in the #20 crosscut in the #5 pillar, catching the continuous miner, size of the fall undetermined. No injuries, ventilation was not interrupted.

Unitentional roof fall occured in #4 entry outby the tailpiece of 004 section.The fall occured 20' inby spad station #7701.The fall measured appx. 20' W x6' to 8' H x 120 L.area was supported by 4' fully grouted bolts.#3 Stamler MBC was caught by this fall, ventilation was not interrupted & roof fall will be cleaned up.

Attempting to raise belt head with lifting jack to reset crib, his hand slipped and jack bar came out and hit him, resulting in a cut to his chin requiring sutures.

Injured was kneeling on his right knee at intersection, 4 rows back from face operating continuous miner, taking 1st cut of of 1 left gob eye on 004 Section. A rock fell out from between bolts striking him on his back & right hip, resulting in fracture to rightt hip. Rock measured approx. 71" long x 36" wide x 1/4" to 5" thick & mining height was approx. 82".

EE was positioned between #3 mobile bridge and the low-low structure, right foot on the low-low belt, left foot on mine floor. EE was attempting to bump conveyor chain, in this process, his arm or shoulder engaged the right tram causing the bridge to move forward, catching his left leg between the mobile bridge and low-low structure. Fractured his fibula and cut his calf (sutures).

Employee was tramming scoop through block curtain battery end first when curtain came in contact with canopy. Employee said his vision was blocked. He raised curtain up with his left hand inching scoop thru curtain when his left hand got caught between rib and canopy. Employee sustained fractures to his index finger and thumb.

Employee was riding in an open mantrip in the 3 west #4 belt entry when a rib rolled as the mantrip passed this area striking the employee on his left shoulder and left rib cage. Rib roll was approximately 126" L x 1"-4" thick x 46" H, height in the area was 86". Employee sustained enclosed concussion, collapsed left lung, fx left ribs.

A pillar fall occurred on the MMU 003-0 in the #11 crosscut in the #3 pillar, catching the continuous miner, size of fall undetermined. No injuries, ventilation was not interrupted.

An unintentional roof fall occurred at the #14 brk, 3 West #2 belt in the #3 entry. Size of fall was estimated to be (5'H x 18'-20'W x 70'L). Fall will not be cleaned up. No injuries occurred, no equipment caught, ventilation was not interrupted. Area had been bolted with 6' torque tension bolts.

An unintentional roof fall occurred on the 001 section in the last open break in the #2 entry. Size of fall was estimated to be 60' L x 18'-20' W x 8'-12' H. Fall will not be cleaned up. Section has been abandoned, equipment being removed from area. No injuries occurred, no equipment caught, ventilation was not interrupted.

An event occurred (ignition) in behind the old #13 belt seals, one of the 7 seals was dislodged, eliminating the separation between the active and sealed area of the mine. Cause of ignition unknown.

EE was observing a 6" water line that he had helped relocate from the off side of the 001-0 section belt to the track side of the belt. When pipe was pressurized, a roustabout coupling came loose allowing pipe to swing around and strike him on his forehead. He sustained lacerations above left eye and right temple, requiring sutures.

Employee was removing belt structure when his index finger on his left hand got caught between two pieces of belt structure. Employee sustained a fracture/laceration to the distal joint of the index finger.

Employee stated while loading supplies in scoop bucket a piece of rock fell from top striking him & knocking him to mine floor, trapping him underneath the rock. He was retrieved from under rock, transported to surface, transported to Holston Valley Medical Ctr, Kingsport, TN. He was kept overnight for observation, released (02/06/06).

EE was struck on the back by a rock brow that broke out from around roof bolts (10-12" thick, 18-48"wide and 90-96"long). EE sustained fracures to left ribs, lower back and neck. Accident occurred on the MMU 003-0 section.

Ee sustained injury to his left big te when an airlock door came in the deck of personnel carrier he was riding in EE sustained crushing injury to his left big toe. EE big toe was amputated to the first joint.

An unintentional roof fall occurred #3 North Belt and Track entry at spad #1902. Size of fall was estimated to be 150'L x 20-22'W x 4-7'H. The area had been supported with 5' fully grouted resin bolts with supplemental support consisting of 9' cable bolts. No injuries, ventilation was not interrupted, belt and track were caught under fall. Fall has been cleaned up.

An unintentional rock fall was discovered in the #1 entry beginning at spad #2129. The area had been supported with 4' fully grouted resin bolts. No injuries, no equipment caught, ventilation was not interrupted. Fall will be cleaned up.

Employee was helping make belt move, belt was being taken out, in the process of removing top belt rail ee's index finger on right hand got caught between belt rail and mine roof. Employee sustained laceration/ contusion to index finger (rt) requiring sutures.

Employee was operating miner on the 002-0 south section, was taking a lift cut in the #1 gob eye when a rib roll struck employee on the back of his left leg knocking him down. Employee sustained fracture to the tibia bone just above the ankle.

Employee stated while removing a bag of rock dust (40 lb) from scoop, he felt pain in his lower back. Employee started missing work on 3-14-05. EE was diagnosed as having low back strain.

AN UNINTENTIONAL ROOF FALL WAS DISCOVERED IN THE #1 ENTRY ON 2 LEFT OFF 2 NORTH MAINS 1 BRK INBY SPAD #2646. SIZE 60' LONG X 4' HIGH X 18-20' WIDE. WAS SUPPORTED WITH 4' FULLY-GROUTED RESIN BOLTS. NO INJURIES, NO EQUIPMENT CAUGHT, VENTILATION WAS NOT INTERRUPTED. FALL WILL NOT BE CLEANED UP.

An unintentional roof fall was discovered on the 001-0 (West) Section in the #2 entry, spad #2576. Size: 50'L x 4-9"H x 18-20"W. Had been supported with 4' fully grouted resin bolts with supplemental support consisting of 9' rope bolts. No injuries, no equipment caught, ventilation was not disturbed. Fall will not be cleaned up.

An unintentional roof fall occurred on the 001-0 (west) section in the #4 entry (belt entry) spad #2613. Size: 4'-5'H x 60'L x 18'-20'W; had been supported with 4' fully grouted resin bolts. No injuries, no equipment caught, ventilation was not interrupted. Fall has been cleaned up.

Employee was getting out of mantrip and sprang right ankle.

An unintentional roof fall was discovered int he #7 entry 3 west mains 2 breaks outby spad 651. estimated size 6'h x 20'w x 90'l. area had been supported with 6' torque tension bolts. no injuries, no equipment caught, ventilation was not interrupted.

Long term exposure to using hands in repeated motions.

Power supply failure to slope brake car made car in operative for approximately 2 hours.

EE WAS CARRYING A MANDOOR AND SAID HE TWISTED HIS KNEE.

EE WAS RIDING PERSONELL CARRIER THAT WAS TAVELING FROM BOTTOM OF SLOPE TO SECTION. EE WAS IN THE OUTBY END OF CARRIER, AND WAS ON HIS KNEES FACING DIRECTION OF TRAVEL. CARRIER PASSED UNDER NEATH OVERCAST, EE RAISED UP STRIKING HIS HEAD AGAINST OVERCAST.

EE STATED HE WAS DRILLING HOLES FOR ANCHOR BOLTS IN MINE FLOOR FOR BELT HEAD DRIVE USING HYDRAULIC SHORDER DRILL WHEN DRILL BIT GOT HUM=NG IN CRACK CAUSING DRILL TO SURGE/TWISTING STRIKING HIM ON HIS LEFT FOREARM. EE SUSTAINED FRACTURE TO THE UINA BONE LEFT ARM.

EMPLOYEE WAS SITTING ON MANTRIP WHEN ANOTHER MANTRIP COULD NOT STOP AND STRUCK THE MANTRIP SITTING CAUSING EMPLOYEE TO HURT BACK.

EE WAS RECOVERING A 4" WATER LINE AT TURTLE PORTAL USING A BATTERY POWERED SCOOP, HAD WATER LINE TIED TO SCOOP BUCKET WITH ONE JOINT OF PIPE EXTENSION BACK OVER SCOOP ON OFFSIDE WHEN PIPE GOT HUNG ON CANOPY POST AT PORTAL CAUSING PIPE TO SWING AROUND STRIKING EE IN THE FACE. EE SUSTAINED A FRACTURED NOSE & LACERATION ABOVE HIS RIGHT EYE.

WHILE TRAVELING FROM TOOL CAR TO THE CONTINUOUS MINER HIS RIGHT FOOT GOT CAUGHT ON A PIECE OF 4' X 3' BELT LAYING ON THE MINE FLOOR CAUSING HIM TO TRIP AND FALL. HE TWISTED HIS RIGHT KNEE.

WHILE EE WAS BENT OVER TO CHECK A PUMP, A ROCK FELL FROM THE BACK & HIT EE ON THE RT. PORTION OF HIS LOWER BACK.

WHILE SETTING UP TO BOLT, A ROCK FELL AROUND A BOLT AND HIT EMPLOYEE ON HARD HAT.

EMPLOYEE ATTEMPTED TO STEP OVER A BAGLINE WITH AIR FLOWING THROUGH IT. HE STEPPED ON PART OF BAGLINE, SLIPPED AND FELL.

WHILE BENDING OVER TO TAKE APART A JACKLEG FOR BOLTING, ANOTHER JACKLEG FELL OVER & HIT EMPLOYEE ON BACK OF HEAD.

WHILE TAKING JACKLEG OUT OF HOLE, EMPLOYEE SLIPPED ON A ROCK AND JACKLEG HIT EMPLOYEE ON RIGHT LEG ABOVE ANKLE.

EMPLOYEE WAS HELPING TO WORK ON A DRILL, WHEN A ROCK FELL FROM THE TOP STRIKING HIM.
