Employee was going to show co-worker how to hook up a chain. Walked by conveyor/MBC unit and cable arced/blowed out. Stuck left hand out to block flash and ran away from arcing. Received burns to hand and blistering to left side of face.

EE was attempting to bump conveyor chain, in this process, his arm or shoulder engaged the right tram causing the bridge to move forward, catching his left leg between the mobile bridge and low-low structure. Fractured his fibula and cut his calf (sutures).

EE WAS RECOVERING A 4" WATER LINE AT TURTLE PORTAL USING A BATTERY POWERED SCOOP, HAD WATER LINE TIED TO SCOOP BUCKET WITH ONE JOINT OF PIPE EXTENSION BACK OVER SCOOP ON OFFSIDE WHEN PIPE GOT HUNG ON CANOPY POST AT PORTAL CAUSING PIPE TO SWING AROUND STRIKING EE IN THE FACE. EE SUSTAINED A FRACTURED NOSE & LACERATION ABOVE HIS RIGHT EYE.

WHILE TRAVELING FROM TOOL CAR TO THE CONTINUOUS MINER HIS RIGHT FOOT GOT CAUGHT ON A PIECE OF 4' X 3' BELT LAYING ON THE MINE FLOOR CAUSING HIM TO TRIP AND FALL. HE TWISTED HIS RIGHT KNEE.
