A roof fall was discovered in the #7 crosscut between the #5 & #6 headings. The fall was a continuation of a previous 2006 fall. The fall measured 40 x 16 x 5 to 6 feet in thickness. The area was initially bolted with 4 foot conventional bolts. The area was timbered off to prevent access and six 80 ton jacks were installed around the edges of the fall.

A scoop operator received lacerations to his middle and ring fingers on his left hand when he was moving a shuttle car and the shuttle car dropped off of a small ledge in the mine floor. He had positioned his left hand outside the shuttle car compartment and his fingers were smashed between the operators deck and the mine floor.

The ee was hauling loose material from a settling pond and stepped from the cab onto the dump bed steps to check the placement of material. The ee was standing on the 3rd step when the step collapsed. The ee was holding on to the top of the bed and did not fall to the ground, but did slip down and made contact with the broken step-resulting in a laceration to the leg.

A loaded shuttle car damaged the ventilation curtain leaving the #3 heading. The ee repaired the curtain and placed a rock on the bottom of the curtain to hold the curtain in place. The ee received a laceration to his left hand from the sharp edges of the draw rock - requiring four sutures.

The employee was bolting in the #6 heading. The height was approximately 8 feet which is abnormal for this seam. The operator had completed drilling the hole for the bolt and was lowering the drill boom when his middle finger on his left hand was caught-between a hydraulic fitting on the drill pot motor and the drill boom.

A ROOF FALL was discovered during the weekly exam one break inby station 2636. The fall area was bolted with 42" fully grouted rods with 8' cable bolts installed in the intersection on a five bolt pattern as supplemental support. The fall measured 20' wide x 40' in length & 8' in thickness. All entry points were timbered & dangered off to prevent access to fall area.

The contract employee entered the mine at 9:00 a.m. He stated to two co-workers that he had injured his back over the weekend. At 1:05 p.m. he called to the mine surface and alleged to the mine superintendent that he had injured his back at the #5 belt drive. Abby Contractors is controverting the workers compensation claim.

An additional ROOF FALL was discovered at a previous rock fall location on the #1 belt at crosscut #7. The original fall was reported in 2002. The roof falls combined measure 18 feet wide X 30 feet long X 3 feet in thickness.

Emp. was positioned in the tram deck of the machine looking over his shoulder with his left arm positioned outside of the confines of the operator deck. His left arm was caught-between the machine & the coal rib resulting in scratches & bruising to the arm.

The employee was riding a rubber tired personnel carrier in a laid-back position, due to the mining height. When he exited the personnel carrier his right foot was lodged under the personnel carrier holding his foot stationary while his upper body turned. This created a twisting motion at the knee, resulting in a knee injury. Lost time began on 8-6-10.

EE was drilling a bolt hole for the 3rd bolt in the 1st row of a cut just taken out in the no. 2 heading. After drilling approx. 8 - 10" into the roof the drill steel hit a crack in the roof. The rock infront if him started to work and he started to run back to the back of the machine. A slick ensided slip fell from the roof. A piece of this rock hit the side of his foot.

The emp. alleges to have contracted CWP while working at this mine. Emp. was diagnosed with the same classification of CWP when he was initially hired with this company. In 2006 he had a NIOSH sponsored chest x-ray which he did not bring to us until November 2009. Although this is being reported now, the emp. had this condition when he came to work for us initially.

A claim was received on 2/11/09 for a diagnosis of alleged coal worker pneumoconiosis that was made on 8/21/07 amd again on 9/28/08. Employee allages to have contracted CWP whole working for Dominion Coal Corp. Management received notification of this diagnosis on 2/11/09.

The EE was injured when a rock measuring 13" W X 13.5" T X 17" L fell from the rib near his postion and caught his left ankel. The section is pillaring and the EE had just started taking a miner cut with the cont. miner out of the left hand pillar. Shuttle car circuit breaker kicked & EE stated he would eat while breaker was put back in SC. Eating lunch when rock rolled of rib.

The roof fell from the pillared area into the intersection of the No. 2 Bleeder Entry. The fall occurred approximately 1400' inby the active section.

A roof fall occurred just outby survey station No. 28 approximately 500' inby the drift mouth of the No. 5 intake entry. The fall was composed of laminated sandy shale and measured approximately 20' long x 17' wide by 40" thick. The fall was cleaned up and the area re-bolted.

The belt transformer at the No. 3 Belt Drive caught on fire. The CO system sounded an alarm and the section crew was notified. The crew was brought down the intake outby the fire & some of the crew members put the fire out with fire extinguishers and water after it was determined there was no power on the transformer.

The injured and co-worker loaded one end of the track vehicle with rock dust and placed a mandoor on top of the dust. The injured unloaded rock dust at the No. 2 - 5 belt drives and was proceeding to the #6 belt drive when the mandoor caught the roof and was dragged over the top of the track vehicle and struck him on the left arm.

A roof fall occurred in the #6 Intake entry at survey station #2317, approximately 300' out of the face area. The fall measured approximately 30' long X 30' wide X 5' thick. The area had been bolted with 42" fully grouted roof bolts. The roof in the area was a laminated sandy shale.

The section was being moved around and a new belt was being installed. The belt tail piece had been disconnected and was to be moved to the belt being installed. A scoop was hooked to the tail piece to move it. The EE did not move back far enough. One of the legs of the tail piece fouled on an offset in the mine floor. It swung around and the back of the tail piece hit the EE.

The fall caught the high voltage cable running up the entry. The fall is located approx 4400' outby the active working section.

The employee was operating the continuous miner by remote when some draw rock fell on the miner. The employee backed the miner out of the place in order to remove the drawrock. As he was pulling on a large piece of drawrock, a piece 37" long x 14" wide x 0" - 3.5" thick broke out and slid down onto his little finger and outside of his right hand.

The fall was in a slip form and was composed of sandy shale. 42' fully grouted resin bolts were installed in the area.

AS HE WAS TRAVELING THE TRACK ENTRY AND STARTED DOWN AN INCLINE, APPROX 3-4 BREAKS INBY THE DRIFT MOUTH A SLATE BAR THAT HAD BEEN LEFT ON TOP OF THE PERSONNEL CARRIER SLID FORWARD, STRUCK A PROP SET ALONG THE BELT. THE BAR HIT EE'S LEFT ARM BREAKING IT & CUT UPPER LIP REQUIRING 7 STITCHES.
