Dust flew into eyes while shaking off tool box cover

While exiting 1394 Haul Truck, Left hand holding rail, right foot stepped down on next step but foot slipped. Landed on buttocks on the bottom of last step.

While dismounting from Excavator, foot got caught between steps. There was a dip in the ground which caused employee to lose balance while releasing hand rail. Employee fell backwards and landed on right elbow.

While lifting rocks off the Drill Pattern, strained lower back.

While operating the Skidsteer, the employee traveled over a slight incline and the skidsteer jerked, causing EE's body to jerk as well. Shortly after, EE felt tingling and slight pain in neck and back.

Employee used right foot to move hose. Slight pain in right ankle. 8/9/22 appointment, return to full duty. Follow up appointment on 11/4/22, employee was placed on restrictive duty on 11/17/22

While removing an a/c line from vehicle, lost balance while pulling it from the evaporator core and fell on hand to brace self.

While cutting with the Plasma, air blew debris under safety glasses and hit left eye

While holding bearing race. A piece of the material broke and flew in left hand while another employee punched the race out.

Employee twisted right ankle while exiting Conveyor #5 walkway, stepped to ground and fell.

While loading blast holes. EE was moving side to side, left to right, and front and back. Allegedly strained left muscle calf. EE sought medical attention on 9/13/2021

While removing bolts on the top of the tail pulley guard, the guard slid down and cut into employee's left ring finger

Working on an oil tank, Crusher #3, when a hose not connected, swung and hit employees head. Area struck, swelled up to approximate size of a golf ball.

On 3/12/21, employee reports for the first time that they have right ear pain/ringing from noise exposure for approx. 10-15 minutes when training conveyor belt in Primary Plant on **2/19/21 without hearing protection which company provides. **Employee was not at work on 2/19/21.

Employee was walking towards the tool room when they stepped off the concrete slab to the ground and twisted an ankle. Employee forgot about the slight drop off from the concrete slab to the ground.

While moving trougher on C-2, trougher slid off C-2 frame and hit 3 fingers on employee's left hand.

Employee stepped off from skytrak lift onto uneven ground and felt pain to right knee.

While walking on the catwalk between crusher 5 & 6, EE forgot that the hatch door to gain access to the crusher landing was open and fell through the opening striking left side abdomen against the catwalk opening. Access from the catwalk to the landing area is about 3' down. A miner, was down below cleaning the landing area but did not witness the fall.

Operator of 773 haul truck reported a leak in truck. As they traveled to a safe spot, the hydraulic ram on the right side of the truck wedged between right rear inner tire & frame causing tire to come off the rim. Operator in the cab alleged to have ringing in left ear from the tire blow out.

On 10/03/18 Miner woke up in the morning with pain in left knee. Miner believes ee strained left knee from climbing in and out of manlift basket and leaning on basket rails when working on conveyor 23 bearings. Miner reported injury on 10/03/18

While welding on the excavator bucket, Miner pulled welding gloves off, proceeded down the ladder & positioned right hand onto the bucket to balance, when the stinger fell. The stinger fell from the top of the bucket, hit side of the bucket and the air arc cutting rod burnt top of right hand. MINER SOUGHT MEDICAL TREATMENT AND REPORTED INJURY ON 9/12/18

While exiting through the hopper hatch for crusher 14, miner felt a sharp pain below the right knee cap. Miner continued to work and felt knee getting tighter. Miner reported the injury the next day at the start of the shift.

EE saw co-worker at Screen 15 with the hood opened 3/4 of the way. The hood was blocked by screens stacked in front of it. In an attempt to lay the hood flat, EE crouched under the hood to lift/push screens out of the way and ee felt pain in lower back thereafter. Pain did not go away, EE out on injury from 3/15/17 and sought medical attention on 3/17/17.

On 11/17, while operating eq. 2754 Loader in phase 2, EE said EE could feel the twinge in the shoulder. When EE woke up on 11/18 EE said it was more sensitive. As the day progressed on 11/18 at work, the repetitive motion of manually leveling the bucket sent sharp pains up the arm into the shoulder. EE contacted supervisor at 12 pm.

While trying to jack up tank 3 leg, the 4" pipe popped out and fell onto hardhat then upper back of miner.

Employee operating 773 Haul Truck (1389) proceeded to drive down main haul road above primary plant, when they noticed another Haul Truck driving up. They immediately braked to a complete stop. Following behind employee was a 600 Komatsu loader (2799) operator. The Loader bucket hit the haul truck. Immediately after, employee felt pain to their back.

On 7/6 EE and Mansand crew was changing out conveyor 60 belt. EE claims the manual tugging & pulling of the belt might have caused lower back pain that was felt later that day. Pain worsened the next morning, but EE ignored it. Pain continued on for the next 2 wks. EE REPORTED INJURY on 7/20/15.

While laying out a pipe into a trench, employee went to pull the pipe when employee stepped on a 2 to 3" rock and twisted employee's right ankle

On 11/4/14 EE reported that on 10/30/14 he had a hard time lifting the cover for crusher 4 hydraulic controls that was covered with material. Several days later he noticed a lump by his belly button and went to see a doctor on 11/4/14. His doctor referred him to a specialist, on 11/17/14 diagnosis of hernia confirmed. No lost time on this injury as of yet.

While working in the area of Tank 5 & 6, EE was positioning a new water chute in place, when he removed the come-a-long on his side and the chute dropped and crushed his right hand.

While driving haul truck #1384, descending to a lower level of quarry haul road in phase 1 pit area, EE travelled over 2 depressions in the roadway causing him to bounce hard on his seat. Over the remainder of his work shift he began to feel pain to his lower back.

While putting away 1/4" screens for 3&4 bottom decks, employee was on one end of the screen bending down and co-worker on the other. Employee, in the process of standing up, tripped on the wood that is used to keep screens off the ground. As he started to fall backwards, he caught himself before falling to the ground. In the process, he twisted his back

Miner was cutting off electrical conduit down from the batch plant, unistrut clamp was not secure and the conduit fell about 4 to 5 feet hitting employee on the hard hat. Upon impact, the hard hat suspension liner pressed into miners forehead, causing laceration. Miner received sutures on open wound.

Employee was walking around the shop truck in the shop, when his right knee buckled. Employee managed to catch himself and did not fall to the ground. The cause of knee buckling could not be established, employees knee was not struck by or against anything, nor did he step on anything or walk on uneven ground. Incident occurred 15 minutes into the start of the shift.

Employee was operating bench loader 2754 removing material close to a 30' face in phase 2. A rock, about 4' to 5' in diameter, slid down a 15' section and struck the edge of the loader bucket causing the equipment and operator to jerk. About an hour later the base plate broke off the seat and employee reported that his back was sore and left for the night.

Working on a driveshaft center support bearing race, EE tried to crack the race with a hammer & chisel. A small metal shard pierced through his rubber glove, causing a small laceration on left palm. On-site first aid administered. Weeks later EE felt a bump on his palm. On 5/10/13, EE sought medical attention, foreign body material removed. EE received sutures.

While descending on a fixed ladder from conveyor 54 down to the rooftop of Air Separator 1 (AS1), EE was unaware of the last 2 rungs that were missing (26" gap), as he continued down, he had to catch himself from falling onto the rooftop of AS1 and injured his right shoulder. Employee continued to work and thought the pain would go away. On 4/3/13 EE sought medical treatment.

While walking down C31 catwalk with a empty 1-gallon paint can in his left hand, about 10-15 feet from the head box, employee slipped and broke his fall by holding onto the handrail. EE did not feel any immediate pain. Later on that night employee felt pain to his lower back.

While operating 773 haul truck (equip# 1391) with a load, EE was driving down the Phase-2 haul road between the L-1 bench and H-3 bridge, the truck hit a bump in the haul road and EE tried to brace himself with his legs to cushion the bump. In that process he strained his inner right thigh. EE reported incident at the end of his shift on 1/28/13

EE was working behind the cab of a 773, reaching into the compartment in a bent postion to trouble shoot air brake system. When he came down from the truck he felt pain in his lower back. (Eq.# 1384)

On 8/18/12 EE stated he came into work with his right leg muscle tight, drove 773 and alleged that the pain got worse. In 8/31/12 EE reported as a "near miss". On 9/7/12 EE sought medical attention and placed off work as a work related injury.

While working on IR Drill pounding out a pin with a hammer, metal fragment broke off and cut into the palm of employees right hand.

While pulling on a wrench with left hand and pushing the ratchet with right hand to tighten a bolt for a v-belt guard on screen 31, employee felt his shoulder pop. Possible shoulder strain/sprain.

At approximately 12:15 am while inside the bowl of crusher 2, employee was hand unloading the jammed crusher of loose rocks. Several loose pieces of rock fell from an overhead chute onto employee. The initial rock knocked employees hard hat off and other loose rocks fell and struck employee in the head and upper back.

While working on Dozer, equipment number 2550, employee was standing on the Dozer tracks replacing hydraulic hose and o-ring, when his foot slipped off the tracks and he fell about 4 to 5 feet to the ground, landing on his back.

While in a skid steer loader traveling down an inclined roadway with bucket up and filled with material, employee felt that the loader was going to tip forward. Employee reacted by trying to brace himself in the equipment and accidently hit the control stick for the bucket. The bucket came down and smashed his left leg, above the ankle, into the equipment.

While cutting through a retention bracket that held down a compressed spring, with a welding torch, the force of the spring popped up, causing a 1" piece of the retention bracket to ricochet against employee. As a result, employee sustained a laceration and contusion to the left chest area.

While servicing a 773 haul truck (equip. 1389), employee was preparing to put power steering fluid in the tank, however the cap was tight. He placed his left forearm against the muffler to gain leverage to open the cap. He did not realize the muffler was hot, as a result, he sustained a burn to his left forearm.

While digging out material that backed up in crusher II, employee had to reach down with his left hand to pull rocks, transfer rocks into the right hand & reach up to pass the rocks out of the crusher. Due to the high wall of the crusher, it made it difficult for employee to position himself without stretching and twisting repetitively, causing strain to his back, neck & shoulder.

While opening the hood of the water truck (Equip# 669), the stop cables broke causing the hood to over extend and strike employee on the top of his head. Employee was wearing a hard hat.

While working in Phase II, employee was operating the bench loader, moving material from the face of the wall to bring a boulder down. The boulder slid, dropped and hit the front bucket of the loader, causing the rear of the loader to lift 3' to 4' off the ground. Upon landing employee felt pain to his back and neck. Employee reported incident at the end of the shift.

Employee was walking on the wet concrete walkway in the feeder tunnel when he slipped and fell on to the concrete walkway. Employee stated that he fell on his left side of his body, using his left hand to cushion his fall.

Employee was seen wiping the windows on the CAT-773 haul truck and about 5 to 10 minutes later he was seen by the Stockpile Operator, laying on the ground in front of the haul truck. Employee sustained a laceration to his head and broke his right wrist.

While operating overhead Gantry Crane controls with right hand, employee guided and latched hook into the completed catwalk with his left hand. Upon pressing the up button to raise the catwalk, the hook twisted, crushing his left thumb between the hook and the catwalk causing laceration.

While walking on ground level, employee slipped in muddy area and bent his right knee. Employee regained his balance and did not fall to the ground, however he heard a pop noise from his right knee.

Employee and co-worker, was carrying a acetylene tank to load onto the welding truck, when co worker let go of his side and the acetylene tank crushed injury's left pinky finger against the mounting bracket on the truck

After extracting a hammer bit from crusher 2 with co worker, both EEs carried it down the stairs into a truck. While walking to the truck, injured stepped into a muddy area with surge rocks and twisted his left ankle.

While working in the tertiary plant, employee stepped onto screen #5 to retrieve a piece of wood when his left leg slipped and twisted causing pain to his left knee.

On 7/1/09, employee called supervisor to report that he was not coming in due to lower back pains. Employee thinks that while operating the dump loader on 6/30/09, the jerking action while driving and hauling materials from Phase II to the Primary Crushing unit, is what caused his back to be sore.

While using the impact gun to unscrew 3/4" flat head bolt on a backhoe bucket, employee pushed on the flat head bolt with his left hand to prevent it from spinning. The flat head bolt began to spin and the edges of the bolt sliced into his left index finger causing a 1/4" deep laceration that required 5 sutures.

On 2/17/09, employee was operating a Dozer (equip# 2550) to remove a toe (rock protrusion) that required more time and force than normal to remove. At the end of the day employees back was sore from all the jolting of the equipment. Employee worked a full shift the next day and his back was still sore. He reported the injury to the supervisor after his shift on 2/18/09.

While working with other employee relocating 6" water hoses about 25' long, into a trench in Phase I, employee allegedly twisted and strained his back. Employee worked 8.0 hours the next day, and did not show up for work on 2/9. EMPLOYEE REPORTED THE INJURY TO THE NIGHT SUPERVISOR ON 2/9/09.

While working with two other employees ratcheting a come-a-long to lift a ripper claw, the chain on the come-a-long broke and the ripper claw fell and hit employee above the right knee.

Employee was working with a crew of 6 people unloading bags of anfo and saran explosives from the container to the magazine container when he started to feel discomfort in his back. He continued working and thought that the discomfort would go away. By the end of the work day employees lower back ached. Employee sought medical attention on 11/28/08.

While changing screens on screen #5, employee began pounding the wedge down on the screen when pieces of metal flew into his left eye. Employee was wearing safety glasses, however they were not the appropriate type for the work he was doing.

Employee was unloading 55 lb bags of anfo explosives from th econtainer, passing it on to employees outside the container for most of the morning. Later in the afternoon, after loading holes for a blast, he back started to stiffen and he would receive shocks of pain in his lower back when he twisted a certain way.

While carrying a screw for the diaphram up the stairs in the primary plant, employee felt his knee pop. Employee sought medical assistance on August 7, 2008.

While checking conveyor 3 bearings in the primary plant, EE stepped onto a pile of rock material, sunk into the material and strained his lower back. EE thought that the material was hard before he stepped onto the pile.

While working with another employee changing bits on drill (2641), employee's finger tips of his right hand got caught (smashed) inbetween the seem and wrench of the drill. Employee was wearing a glove. Doctors initial reports received on 5/19/08 notes pain medication was given, therefore this is a recordable injury.

While operating the 2754 Loader in phase 1 moving secondary rocks on the floor, employee drove the loader into the face of the wall and felt pain in his back. He thought that the pain would go away but it didn't. Reported incident on 4/10/08.

While working near crusher 4 cleaning the area, employee pulled on the water hose and strained his lower back.

While servicing the drill, the intake hose from the bag house had filled up with mud. Employee then disconnected the hose clamp and pulled the hose off of the intake housing. In the process, employee felt pain in his lower back. At the end of the day, employees back started tightening up.

After climbing down from truck #1392, employee began walking towards the lunch room when his back froze up on him. EE believes this is an aggravation to prior injury. EE off of work on 03/03 and 03/04 due to illness. EE submitted medical certificates for being out sick and reported he was out for being sick and not because of injury.

While doing pre-trip or pre-check of the water truck (equipment #0668) employee alleges that the hood of the truck was stuck at the time. While pulling to get the hood open employee felt a tightness to his lower back.

While working with the powder crew on C-Rdge, they needed to move the container of explosives with a tractor. Employee was turning the handcrank to raise the container when his hand slipped causing his body to go forward and hitting his head on the crank. As a result employee sustained about a 2" laceration on his right eyebrow.

While cleaning out the bypass chute behind crusher 2, employee stepped on section of the catwalk that appeared to be rusted and fell partly through hitting his right thigh on the electrical conduits and the tail pulley guard. As a result employee sustained abrasions on forearm, contustion of the thigh and laceration to left palm.

While getting out of the truck to buy lunch, employee slipped and fell to the ground landing on his back.

While working in the parking lot behind the lunch room cutting down trees, employee was standing behind the tree that another employee was cutting when the tree slid back and hit employee in his inner right thigh.

Around two weeks prior to June 1, 2007, EE claims to have suffered a lower back strain from the jerking of the body in a sitting position while operating a defective loader over a period of time. Allegedly the controls to operate the bucket is defective thus causing more constant use of throttles and pedals. Employee reported injury on June 12, 2007.

While climbing the stairs carrying a return idler weighing approximatly 40 lbs, employee felt a pop in his left knee followed by pain shortly thereafter.

Employee was working on conveyor 7 pulling the belt over the head pully when the belt ripped causing employee to hurt his lower back. Employee filled out a injury report but did not seek medical assistance until April 3, 2007 and has been off work since.

EE hit right knee on stairway after near fall from rock on ground.

EE was welding a plate for S-9 when the slag popped and entered his left eye through the safety glasses.

While holding safety latch open on the compressor #3068, a truck backing up made contact with the compressor, smashing employee's left pointer finger between Track Bar and Tow Bar.

While climbing into crusher 2 chute, Employee strained his left elbow.

While returning to work from lunch break, employee struck his right knee on the trailor hitch of the working foreman's truck.

While climging the ladder to enter into the cab of truck 1391, employee slipped off ladder straining his left ring finger, wrist and shoulder.

While walking down the stairs from the mansand room EE felt pain in his left heel. EE did not report incident and thought the pain would go away but it got worse. Reported incident on 12/22/06.

While cutting out CR-2 Bowl liner from bowl asssembly, torch flame made contact with rusted area and hot material blew back towards employees face

EE was assembling a hammer assembly for an air track drill. EE was attempting to screw the nut on a long bolt to hold the assembly together when the hammer dropped and smashed EE's finger on his right hand.

While working with the powder crew getting ready for an afternoon shot, EE carried two powder bags (30 # each) placed it on the gound container floor. When he turned, his back started to hurt, his legs gave way and he fell to the floor. EE had a hard time standing up, so an ambulance was dispatched.

While operating the CAT 773 off-highway truck #1391 at the bottom of the quarry haul road, EE drove over a bump in the road that allegedly tossed him around in the cab. He struck his right hand on the gear shifter handle. He continued to drive until his palm started to swell.

While working on the grating walkway on S-31, EE was positioned at the bottom of the walkway coiling up the welding lead when another worker began lowering the welding cable. The rope broke causing cable and stinger to strike EE, who was positioned directly below, on the right shoulder.

EE was loading metal plates to repair a chute onto his welding truck when one of the metal plates fell onto his left pinky finger causing a deep laceration.

EE was welding threaded studs from morning till lunch. After lunch he noticed numbness in his right leg from hip to foot and thought that his leg "fell asleep". He came into work the next day and his right leg still had numbness.

EE had the bed of his truck tilted up and hit the overpass with the corner of the dump. The jolt pushed his right arm back into his shoulder while steering the truck. EE felt a little discomfort but waited to see if he could shake it off. As a result of the impact EE strained R arm and shoulder.

While checking screens in the screen room in the tertiary, employee picked up an old rusted screen, whcih fell apart and landed on his right foot. The sharp rusted edge punctured his boot and into this big toe.

While helping the welders remove guard on the driver fan, EE removed the bolt and kicked the guard with his left foot to loosen it. As a result his big toenail hooked onto the safety shield guard in his rubber boot and ripped the toenail back.

While approaching the H-3 underpass from Phase II in truck #1389, EE locked on his breaks when he noticed our service truck standing by to let him pass. When employee locked on his brakes it caused the truck to bounce up and down, thus aggravating his back.

While stepping up on service truck to turn on valves, foot slipped and EE strained left hip and thigh area.

The feeder plate from conveyor 1 fell and blocked up the bypass chute behind conveyor 2 and material started to back up. Employee went into the chute to push the feeder back onto the conveyor and in the process, employee strained his lower back.

While working with two other welders who were welding metal side skirts onto the primary feeder, EE alleged to have sustained possible flash burns.

While climbing up the steps on the hydraulic drill (equipment 2041) to check the radiator water, EE alleged to have slipped on some oil film that was on the steps and fell back. EE landed on his feet and claimed to have strained his lower back.

WHILE WORKING ON THE LOWER LEVEL OF THE PRIMARY PLANT TO INSTALL A NEW TAIL PULLEY GUARD FOR C-1, EE NEEDED TO CLEAN THE TAIL SECTION OF THE MATERAIL THAT SPILLED OFF THE CONVEYOR BELT. WHEN HE TURNED ON THE HIGH PRESSURE WATER HOSE, THE RUBBER GROMMET ON THE HOSE WAS WORN AWAY THUS CAUSING THE WATER NOZZEL TO FLY OFF AND LACERATED HIS LEFT SHIN

AFTER PARKING THE JOHN DEERE EXCAVATOR IN AN AREA OF LOOSE GROUND TO SERVICE A HAMMER EQUIPMENT, HE ALLEGES TO HAVE STRAINED HIS LEFT FOOT WHILE STEPPING OFF THE MACHINE ONTO LOOSE GROUND

While working with another employee to separate and park the tractor from the trailer using a forklift and chains. Employee began to pull on the chain to position the lift, when the chain got stuck and pulled employee's left shoulder out of socket.

While walking down from conveyor #3 catwalk, employee slipped on a water hose that was buried with material. In the process of breaking his fall, employee strained his lower back.

While using a chisel and hammer to remove broken grease fitting from the rock buster hammer (equip# 2514) a foreign metal piece of material flew off and lodged into EE's left index finger.

While tightening the come-a-long, the pad eye broke off striking EE in the right leg, right below the knee. EE did not seek medical attention until 5/7/05.

While working with another employee under scale #2 installing new load cell and brackets, EE strained his lower back while moving heavy jacks and curbing to jack up scale platform. Manuvering space was limited to where EE's could not stand up straight.

While removing the pump head from the air operated grease pump, employee scraped his right hand, as a result, received a lacerated wound.

While working up on tank 22, EE was lowering tool bags down with a rope when it got stuck on the lower concrete ledge. Leaning over the hand rail, EE began to push and pull on the rope to free tool bags. After work, EE started to feel his back tightening up.

While assembling the crusher components, EE was hitting hte head lock nut with a heavy metal ram and twisted his right wrist.

While replacing middle hanger bearings and shaft, the shaft had to be removed by hammering it loose. While hammering the shaft, a piece of metal came off and lodged into employee's left hand.

EE was putting back a concrete form. He used a prybar to move the form and it slipped from his hand. He subsequently hit his hand on the concrete form.

EE was putting the bit for a rockbuster on Equipment #2514 back on. EE was physically reinstalling without the aid of operator. While lifting the bit into the rockbuster housing, EE pulled muscle in lower back.

EE was working on a rockbuster #2514 when a low back sprain occurred.

While Truck 1390 was parked at the Primary Plant to dump its load, hydraulic oil caught fire and EE jumped from the truck to the ground. When EE hit ground, he reports that he hurt his back.

EE was grinding metal plate on "B" screen project. He was wearing all safety equipment at the time. However, a foreign item (metal flakes) got into his right eye.

While EE was changing screens on S-11, he pulled his shoulder muscle.

EE was pulling wrench to tighten conveyor belt when he felt a strain in his groin area.

Another employee was replacing mixer chutes on Mixer #518. Job scope required him to pound a chute bolt out. While pounding the bolt from the chute, the bolt flew out and struck injured employee in lower abdominal area. First aid was performed on the job site.

Employee was loading powder. While carrying an ANFO bag (55 lbs) from loader bucket, ee twisted his back while lowering bag to the groung.

DRIVER OF POWDER TRUCK FORGOT ABOUT SPEED BUMP WHICH LAUNCHED EE (WHO WAS SITTING IN THE BACK) OFF THE SEAT LANDING DOWN HARD; CAUSING PAIN TO LOWER BACK.

AMERON PROVIDES A YEARLY HEARING TEST TO ALL EMPLOYEES AND A HEARING BOOTH IS USED. EE OFF LOADED HEARING BOOTH FROM TRUCK AND PLACED BOOTH IN MAIN OFFICE; STRAINED RIGHT SIDE OF LOW BACK/HIP MUSCLE.

EMPLOYEE WAS REPLACING BELT ON SNATCH BLOCK PULLEY. CHAIN SEPARATED FROM BLOCK, CAUSING PULLEY TO FALL ON RIGHT ARM.

EMPLOYEE WAS POUNDING CHUTE TO REMOVE OLD CHUTE LINER. PIECES OF DUST AND RUSTY METAL STARTED TO FALL, GETTING INTO HIS RIGHT EYE.

WHEN EE WAS CUTTING FROZEN BEARING ON PULLEY, HAMMERED BEARING, CAUSING A PIECE OF METAL TO LODGE INTO HIS LEFT HAND BETWEEN THUMB AND INDEX FINGER.

EE OPERATING EQUIP #1384 RETURNING EMPTY FROM PRIMARY PLANT TO PIT LOADER ON HAUL ROAD NEXT TOLIGHT PLANT EE MIDSJUDGED ROAD WIDTH. FRONT TIRES HIT ROCK FOOTING SHIFTING TRUCK AS OPERATOR HIT FACE ON DOOR

WHILE EMPLOYEE WAS HOOKING UP AIR COUPLING TO IMPACT WRENCH, AIR GOT INTO THE PUNCTURE WOUND ON HIS LEFT HAND NEAR HIS THUMB. THE PUNCTURE WOUNDWAS FROM A PREVIOUS INJURY.

WHILE DISASSEMBLING A HOLE HAMMER, THE EMPLOYEE SPLIT THE CYLINDER AND USED A SLEDGE HAMMER TO REMOVE THE OUTSIDE CYLINDER. THE PISTON BROKE WHEN THE EMPLOYEE HIT IT WITH THE SLEDGE HAMMER AN D A PIECE PUNCTURED THE EMPLOYEE'S RIGHT LEG.

WHILE INSTALLING A 2" WATER PIPE TO THE C-12 HEAD PULLEY AND TRANSFER TOWER, THE PIPE WAS TEMPORARILY HELD IN PLACE WITH TIE WIRE UNTIL THE UNISTRUT SUPPORT COULD BE WELDED ONTO THE CONVEYOR. THE WIRE DID NOT HOLD AND THE PIPE FELL ON THE EMPLOYEE'S RIGHT HAND. THE PIPE SHOULD HAVE BEEN CLAMPED DOWN INSTEAD OF USING WIRE.

THE EE WAS ASSISTING TO FREE A LARGE PIECE OF CONCRETE THAT WAS WEDGED IN THE FEEDBOX. A CRANE, FITTED WITH A SLING, WAS ATTEMPTING TO PULL THE PIECE FREE WHILE THE EE WAS POSITIONING THE PIEC E WITH A BAR. THE BAR SLIPPED AND THE EE FELL ABOUT 6 FT UNTO A CATWALK. FALL PROTECTION WAS AVAILABLE BUT WAS NOT UTILIZED BY THE EE.

THE EE WAS ATTEMPTING [TO] FREE A CONVEYOR BELT WHICH GOT STUCK ON THE CHANNEL IRON. AS HE ATTEMPTED TO CUT THE EDGE OF THE CONVEYOR BELT, THE KNIFE SLIPPED & THE MOMENTUM OF THE KNIFE CAUSED A LACERATION TO THE EE'S LEFT ARM [FOREARM].

SCREENS WERE BEING HOISTED UP TO THE PRIMARY PLANT THE CRANE LOWERED THE SCREENS TO THE CATWALK. EE LOOSENED THE WIRE HOLDING THE SCREENS CAUSING THEM TO FALL APPROX. 3" ONTO HIS LEFT FOOT.

THE EE WAS PUTTING OIL INTO THE GEAR BOX ON CONVEYOR 10 AND SLIPPED ON THE FEED BOX. THE EE SPRAINED HER RIGHT ANKLE.

WHILE REMOVING A BROKEN RECOIL SPRING FROM A DIESEL HOSE REEL OF A SERVICE TRUCK, THE SPRING UNWOUND AND HIT THE EMPLOYEE ON THE FOREHEAD. THE EMPLOYEE SUFFERED A LACERATION BETWEEN HIS EYES.

AFTER OPERATING THE LOADER ALL DAY ON THE MOISTURIZER PILE, THE EMPLOYEE COMPLAINED OF PACK PAIN BECAUSE THE LOADER SEAT COULD NOT BE ADJUSTED. HIS SUPERVISOR WAS UNAWARE OF THIS SEAT PROBLEM.

WHILE ATTEMPTING TO UNHOOK A TOW BAR FROMA TRUCK PINTAL THE EMPLOYEE INADVERTENTLY PLACED HIS LEFT HAND IN A POTENTIALLY DANGEROUS AREA BETWEEN THE TOW BAR EYE & THE PINTAL HOOK UPPER JAW

WHILE SERVICING A ROAD GRADER, THE EMPLOYEE ATTEMPTED TO LIFT UP A 5 GALLON BUCKET OF OIL INSTEAD OF USING A SMALLER CONTAINER. THE EMPLOYEE THEN INJURED HIS BACK WHEN HE TWISTED AS HE WAS LIF TING THE BUCKET.

AFTER DUMPING HIS LOAD AT THE TOP OF "C" RIDGE. INSTEAD OF AVOIDING THE AREA, OR PROCEEDING WITH CAUTION, EE ELECTED TO DRIVE OVER THIS AREA. HOWEVER, HE DID NOT PREPARE HIMSELF FOR THE SUBSEQ UENT "JARRING" ACTION IN THE CAB OF THE TRUCK. EE STRAINED HIS BACK (STRAINED/SPRAINED) AS A RESULT OF THE JARRING ACTION.

WHILE EMPLOYEE WAS TRAVELINGON FORKLIFT FROM THE PRIMARY PLANT, THE TRANSMISSION JUMPED OUT OF GEAR. AS THE FORKLIFT GAINED SPPED, THEMPELOYE EPUMPED THE BREAK TO SLOW MACHINE DOWN. HOWEVER, THE FORKLIFT CONTINUED TO PICK-UP SPEED AND BEGAN TO SWERVE. THE MACHINE THEN HIT HT ERIGHT SIDE BERM AND ROLLE DOVER ON THE LEFT SIDE.

AFTER FILLING FUEL TANK O #2545 CAT D9N BULLDOZER, EMPLOYEE ATTEMPTED TO TWIST HER BODY AROUND WHILE STANDING ON TRACKS OF DOZER. EMPLOYEE WANTED TO PUT THE FUELING NOZZLE AND HOSE ONTO A NEA RBY SERVICE TRUCK. EMPLOYEE TURNED, BUT GRIPPING PREVENTED HER LEFT FOOT FROM TURNING WITH HER BODY.

INSTEAD OF WAITING FOR FOREMAN TO PICK HIM UP, EMPLOYEE RODE THE WATER TRUCK TO THE LUNCHROOM. SINCE TRUCK WAS NOT MEANT FOR PASSENGERS, EMPLOYEE SAT ON PLYWOOD BOARD. WHILE GETTING OFF OF TRUCK, EMPLOYEE SLIPPED, LANDED ON BOTH FEET, BUT FELL ON LEFT SHOULDER.

EE STEPPED ON FLAT BAR TO REPOSITION WEAR PLATE TO ENSURE PROPER ALIGMENT. WHILE LIFTING WEAR PLATE EE STRAINED BACK.

WHILE ATTEMPTING TO REMOVE A ROCK THAR WAS WEDGED AT THE TOP OF THE SCREEN, THE BAR THAT EMPLOYEE WAS USING SLIPPED. EMPLOYEE WAS HIT ON THE LEFT SIDE OF HIS FACE ON THE UPPER CHEEK AREA.
