While walking on the catwalk between crusher 5 & 6, EE forgot that the hatch door to gain access to the crusher landing was open and fell through the opening striking left side abdomen against the catwalk opening. Access from the catwalk to the landing area is about 3' down. A miner, was down below cleaning the landing area but did not witness the fall.

Operator of 773 haul truck reported a leak in truck. As they traveled to a safe spot, the hydraulic ram on the right side of the truck wedged between right rear inner tire & frame causing tire to come off the rim. Operator in the cab alleged to have ringing in left ear from the tire blow out.

On 10/03/18 Miner woke up in the morning with pain in left knee. Miner believes ee strained left knee from climbing in and out of manlift basket and leaning on basket rails when working on conveyor 23 bearings. Miner reported injury on 10/03/18

While welding on the excavator bucket, Miner pulled welding gloves off, proceeded down the ladder & positioned right hand onto the bucket to balance, when the stinger fell. The stinger fell from the top of the bucket, hit side of the bucket and the air arc cutting rod burnt top of right hand. MINER SOUGHT MEDICAL TREATMENT AND REPORTED INJURY ON 9/12/18

While exiting through the hopper hatch for crusher 14, miner felt a sharp pain below the right knee cap. Miner continued to work and felt knee getting tighter. Miner reported the injury the next day at the start of the shift.

EE saw co-worker at Screen 15 with the hood opened 3/4 of the way. The hood was blocked by screens stacked in front of it. In an attempt to lay the hood flat, EE crouched under the hood to lift/push screens out of the way and ee felt pain in lower back thereafter. Pain did not go away, EE out on injury from 3/15/17 and sought medical attention on 3/17/17.

On 11/17, while operating eq. 2754 Loader in phase 2, EE said EE could feel the twinge in the shoulder. When EE woke up on 11/18 EE said it was more sensitive. As the day progressed on 11/18 at work, the repetitive motion of manually leveling the bucket sent sharp pains up the arm into the shoulder. EE contacted supervisor at 12 pm.

Employee operating 773 Haul Truck (1389) proceeded to drive down main haul road above primary plant, when they noticed another Haul Truck driving up. They immediately braked to a complete stop. Following behind employee was a 600 Komatsu loader (2799) operator. The Loader bucket hit the haul truck. Immediately after, employee felt pain to their back.

On 7/6 EE and Mansand crew was changing out conveyor 60 belt. EE claims the manual tugging & pulling of the belt might have caused lower back pain that was felt later that day. Pain worsened the next morning, but EE ignored it. Pain continued on for the next 2 wks. EE REPORTED INJURY on 7/20/15.

On 11/4/14 EE reported that on 10/30/14 he had a hard time lifting the cover for crusher 4 hydraulic controls that was covered with material. Several days later he noticed a lump by his belly button and went to see a doctor on 11/4/14. His doctor referred him to a specialist, on 11/17/14 diagnosis of hernia confirmed. No lost time on this injury as of yet.

While driving haul truck #1384, descending to a lower level of quarry haul road in phase 1 pit area, EE travelled over 2 depressions in the roadway causing him to bounce hard on his seat. Over the remainder of his work shift he began to feel pain to his lower back.

Employee, in the process of standing up, tripped on the wood that is used to keep screens off the ground. As he started to fall backwards, he caught himself before falling to the ground. In the process, he twisted his back

Miner was cutting off electrical conduit down from the batch plant, unistrut clamp was not secure and the conduit fell about 4 to 5 feet hitting employee on the hard hat. Upon impact, the hard hat suspension liner pressed into miners forehead, causing laceration. Miner received sutures on open wound.

Employee was walking around the shop truck in the shop, when his right knee buckled. Employee managed to catch himself and did not fall to the ground. The cause of knee buckling could not be established, employees knee was not struck by or against anything, nor did he step on anything or walk on uneven ground. Incident occurred 15 minutes into the start of the shift.

Employee was operating bench loader 2754 removing material close to a 30' face in phase 2. A rock, about 4' to 5' in diameter, slid down a 15' section and struck the edge of the loader bucket causing the equipment and operator to jerk. About an hour later the base plate broke off the seat and employee reported that his back was sore and left for the night.

Working on a driveshaft center support bearing race, EE tried to crack the race with a hammer & chisel. A small metal shard pierced through his rubber glove, causing a small laceration on left palm. On-site first aid administered. Weeks later EE felt a bump on his palm. On 5/10/13, EE sought medical attention, foreign body material removed. EE received sutures.

While descending on a fixed ladder from conveyor 54 down to the rooftop of Air Separator 1 (AS1), EE was unaware of the last 2 rungs that were missing (26" gap), as he continued down, he had to catch himself from falling onto the rooftop of AS1 and injured his right shoulder. Employee continued to work and thought the pain would go away. On 4/3/13 EE sought medical treatment.

While walking down C31 catwalk with a empty 1-gallon paint can in his left hand, about 10-15 feet from the head box, employee slipped and broke his fall by holding onto the handrail. EE did not feel any immediate pain. Later on that night employee felt pain to his lower back.

While operating 773 haul truck (equip# 1391) with a load, EE was driving down the Phase-2 haul road between the L-1 bench and H-3 bridge, the truck hit a bump in the haul road and EE tried to brace himself with his legs to cushion the bump.
